1.Nursing care of 12 patients with severe idiopathic scoliosis treated by halo-pelvic traction preoperatively

This paper reports the nursing care of 12 patients with idiopathic scoliosis treated with halo-pelvic traction preoperatively which focused on breathing training, traction frame management. One patient suffered from temporary brachial plexus injury and four cases suffered from superior mesenteric artery syndrome. With 14-21 day's traction and nursing care, the correction rate of Cobb angle was 35%-50%,the forced vital capacity was improved by 25%,and all the patients received orthomorphia surgery in time. It is suggested that the patients with severe idiopathic scoliosis treated by halo-pelvic traction could take out-of-bed activity freely. It could not only relieve pain and reduce mental pressure, but also improve the safety of orthomorphia surgery.
2.Application of fast-track rehabilitation concept in perioperative nursing of minimally invasive hip replacement

Objective To evaluate the perioperative rehabilitation treatment effects of the patients receiving minimally invasive hip replacement under the guidance of fast-track rehabilitation surgery concept. Methods Seventy-two pa-tients with femoral head aseptic necrosis and femoral neck fractures treated in our hospital were selected and divided into the FTS nursing group and the traditional nursing group according to the hospital admission time, with 36 patients in each group. The traditional nursing group followed the traditional clinical pathway and the FTS group followed the clinical pathway with FTS run through artificial hip replacement for the purpose of treatment and optimized nursing. The nursing and treatment effects of the two groups were analyzed retrospectively. Results The FTS group had better nursing satisfaction than the traditional nursing group (P<0.05), and shorter hospital stay and lower hospitalization cost than the traditional group (P<0.05). In terms of postoperative long-term rehabilitation effect, the two groups were not significantly different in the ADL scores in the 3rd and 6th month(t=-1.467, P=0.147;t=-1.599, P=0.114), but in terms of postoperative short-term rehabilitation effect, the FTS group had higher ADL scores than the traditional nursing group in the 2nd week and 1st month, with statistically significant differences (t=-2.307, P=0.024; t=-2.451, P=0.017). Conclusion In the clinical pathway of artificial hip replacement, the application of fast-track rehabilitation concept can effectively prevent the occurrence of complications, shorten hospital stay, reduce hospitalization cost, improve nursing satisfaction, accelerate functional recovery of the patients receiving artificial hip replacement and improve the quality of life.
3.Effectiveness of a home-based exercise program among patients with lower limb spasticity post-stroke: A randomized controlled trial

Purpose:
To evaluate the effectiveness of advanced practice nurse–guided home-based rehabilitation exercise program (HREPro) among patients with lower limb spasticity post-stroke.
Methods:
This randomized controlled study recruited 121 patients with lower limb spasticity post-stroke. Intervention (n = 59) and control (n = 62) groups underwent 12-month HREPro and conventional rehabilitation, respectively, after discharge. The Fugl–Meyer assessment of spasticity measurement, modified Ashworth scale of motor function, 10-Meter Walk Test of walking ability, and Barthel index of activities of daily living (ADL) were evaluated at 0, 3, 6, and 12 months after discharge.
Results:
Significant differences were found in spasticity degree, motor function, walking ability, and ADL at 6 and 12 months after discharge between the control and intervention groups. Lower limb spasticity and ADL in the intervention group were significantly improved.
Conclusion
HREPro is effective for rehabilitation of patients with lower limb spasticity post-stroke and has favorable home application.

5.A study of nursing model: establishment of green channel and independent of orthopedics unit for elderly patients with hip fracture

ObjectiveTo establish a green passage for elderly patients with hip fracture, and explore the feasibility and nursing mode of establishing an independent orthopedics unit for elderly patients with hip fracture. Methods? From January 2017 to June 2018, a total of 391 cases of elderly patients with hip fracture in the Second Affiliated Hospital of Wenzhou Medical University were selected. The patients were divided into two groups according to their admission to the ward. The 213 patients in the elderly hip fracture independent orthopedics unit group were admitted to the emergency green channel; 178 patients in the general orthopedics ward were given routine treatment and nursing. The time interval from admission to operation and the prognosis were compared between the two groups. Results? 77.46% of the elderly patients in the independent orthopedics unit group received operation within 36 hours after admission, while only 29.21% in the general orthopedics ward group, with statistical significance (P< 0.01). Rehabilitation and discharge rate of independent orthopaedic unit group was 89.20% and 77.53% in control group within one week after operation. The difference was statistically significant (P=0.009). Conclusions? The establishment of independent orthopedics unit with green channel in geriatric orthopedics reduces the waiting time, bed rest and hospitalization time of patients, and saves medical and nursing resources.
6.Correlation of nurses' comfort with touch, job stressors and job burnout in postpartum nurses with the second-child

Objective? To explore the correlation among nurses' comfort with touch, job stressors and job burnout in postpartum nurses with the second-child,and to provide evidence for nursing managers to formulate and implement humanized and scientific management plans. Methods? By convenience sampling, postpartum nurses with the second-child back to work in 6 Class Ⅲ Grade A hospitals in Wenzhou from February 2017 to July 2018 were selected as the research objects. Self-designed General Information Questionnaire, Nurses' Comfort with Touch Scale(NCTS),Chinese Nurses' Work-Stressor Scale(CNSS), Chinese version of Maslach Burnout Inventory General Survey(MBI-GS) were used in the study. Totally 125 questionnaires were distributed and 119 valid ones were retrieved, yielding an effective recovery rate of 95.2%. Results? The 119 postpartum nurses back to work achieved a total score of (43.56±6.09) in NCTS, (535.58±245.96) in CNSS, (25.87±10.43)in the "job burnout" dimension in Chinese version of MBI-GS, (8.96±6.03) in"depersonalization" and (30.12±8.67)in "personal sense of achievement". Pearson correlation analysis indicated that, the returning postpartum second-child nurses' scores of "emotional burnout", "depersonalization" and"work stressors" were negatively correlated with the score of "comfort with touch"(P< 0.05), the "personal sense of achievements" was positively correlated with "comfort with touch" (P<0.01). Multivariate regression analysis showed that emotional burnout, nurses' job stressors, whether receive special care from departments, and income satisfaction were the influencing factors of nurses' touch comfort. Conclusions? Touch comfort is related to job stress and job burnout in the postpartum second-child nurses back to work. Nursing managers should formulate and implement corresponding management training programs according to the characteristics of them, and give more care and support to improve their work enthusiasm and work efficiency.
