1.Preparation and evaluation of biodegradable molecularly imprinted polymers

BACKGROUND:The cross-linking reagent is the main unit to support the skeleton of molecularly imprinted polymers, which is closely related to the bio-friendly adaption of polymers. The biocompatible and biodegradable capacities of common crosslinking agents are unclear.
OBJECTIVE:To prepare a novel biodegradable molecularly imprinted polymer and to evaluate the adsorption and biodegradability properties of the polymer.
METHODS:The biodegradable molecularly imprinted polymer was synthesized with the acrylated polyε-caprolactone as the cross-linking reagent under the UV polymerization. The adsorption of the polymer was evaluated by adsorption isotherm, Scatchard analysis and kinetic curves. The biodegradation of the polymer was observed in the simulated physiological environment system in vitro.
RESULTS AND CONCLUSION:Both the molecularly imprinted polymer and non-molecularly imprinted polymer showed an adsorption effect on theophyl ine, the template. But the adsorption amount of the molecularly imprinted polymer was significantly more than that of the non-molecularly imprinted polymer. The drug loading and encapsulation efficiency of the molecularly imprinted polymer were 1.54%and 12.48%, respectively. The degradation rates of molecularly imprinted polymer and poly(ε-caprolactone) diol were 6.60%and 1.33%, respectively, within the observation time in vitro. The obtained molecularly imprinted polymer not only exhibited specific adsorption to certain molecules but also showed good biodegradable properties in the simulated physiological environment system, which is necessary to be a potent drug carrier.
2.Research Progress of PNI and CONUT in Breast Cancer

Breast cancer has overtaken lung cancer as the most common malignancy in women. Although the early diagnostic rate has continuously improved, recurrence and metastasis remain a problem to be solved. Therefore, scientists should search for effective prognostic markers for breast cancer patients and adopt individualized programs for different patients. Studies have shown that cancer prognosis, to a certain extent, is related to the nutrition inflammation index. Preoperative prognostic nutritional index (PNI) and controlled nutritional status (CONUT) are indicators that comprehensively reflect the nutritional level and inflammatory state of patients, respectively. Different from other cancers, the incidence of breast cancer is related to nutritional status, and an extremely high or low score is not conducive to the prognosis of breast cancer patients. This paper reviews the research progress of PNI and CONUT in breast cancer.
3.Correlation between demand for transitional care service and readiness for hospital discharge in patients with day surgery

Objective To explore the demand for transitional care service and readiness for hospital discharge in patients with day surgery and analyze their correlation. Methods From October to December 2016, a total of 288 patients undergoing day surgery of Day Surgery Ward in the West China Hospital, Sichuan University, were investigated with questionnaire. Survey instrument included the general information questionnaire, Transitional Care Service Demand Questionnaire and Readiness for Hospital Discharge Scale (RHDS). The dimension score and total score were described with a ((x)±s) deviation. Pearson correlation was used to analyze the correlation between the score of demand for transitional care service and readiness for hospital discharge of patients. Results The total score of demand for transitional care service of patients with day surgery was (2.88±0.78) with a high level and (2.93±0.81) for the highest score in the dimension of wound self observation and care. The score of readiness for hospital discharge was (80.44±13.06) in patients with a high level. The total score of readiness for hospital discharge had a negative correlation with the total score of demand for transitional care service (r=-0.424, P< 0.05). Conclusions Patients with day surgery are with a high demand for transitional care service. Improving the readiness for hospital discharge can reduce demand for transitional care service. We should adopt positive measures to improve patients' self-care agency during hospitalization, draw up transitional care scheme according to different diseases and provide focused transitional care service.
4.Effects of intensive self-care intervention in patients undergoing day self-retaining laryngoscope vocal polyp resection

Objective To explore the effects of intensive self-care intervention on self-care agency, reactive mode, psychology and voice functional recovery in patients undergoing self-retaining laryngoscope CO2 laser vocal polyp resection after leaving hospital. Methods From January 2016 to June 2017, 75 patients with selective self-retaining laryngoscope CO2laser vocal polyp resection were selected. From January to September 2016, a total of 37 patients with conventional nursing were selected as control group. From October 2016 to June 2017, a total of 38 patients accepting intensive self-care intervention were chosen as observation group. The self-care agency, reactive mode and voice functional recovery of patients in two groups were compared at different times. Results The score of self-care agency of two groups all increased one and three months after leaving hospital compared with that on admission (P< 0.05). The score of self-care agency of observation group one and three months after leaving hospital were (125.32±6.75) and (122.46±8.24) significantly higher than those [(117.69±12.47) and (115.38±10.25)]of control group (P< 0.05). The score of positive coping style of observation group one and three months after leaving hospital were (23.87±1.76) and (24.00±1.83) significantly higher than those [(20.13±1.20) and (20.08±1.35)]of control group (P< 0.05). The fine rate of voice functional recovery in observation group one month after leaving hospital was 78.95% higher than that (51.35%) in control group with a significant difference (P<0.05). However, there was no statistically significant difference in the fine rate of voice functional recovery between two groups after 3 months leaving hospital (P>0.05). Conclusions Intensive self-care intervention can improve the self-care agency, positive coping style and promote voice functional recovery in patients undergoing self-retaining laryngoscope CO2laser vocal polyp resection.

Objective:To explore the progress and prospect of evaluation methods for infertility patients.Methods:From September 2017 to December 2020,199 cases of infertility patients who have accepted mini-hysteroscopy examination in the Department of Gynecological Endocrinology,Beijing Obstetrics and Gynecology Hospital,Capital Medical University.The patients who had primary ovary insufficiency,tubal infertility were excluded;patients with hysteroscopy contradiction,such as acute or sub-acute vaginal tract inflammation,en-dometrium lesion or endometrium carcinoma,severe cardiovascular,liver or kidney insufficiency were excluded.40 patients were excluded and 15 patients lost follow up,144 patients were eventually included in the analysis.The patient's age,body mass index,infertility etiology,preoperative and postoperative stimulate ovulation cycle,pregnancy state and pregnancy time were recorded.The effect of mini-hysteroscopy on ovulation induction cycle was analyzed in pregnant patients.Results:The average age of 144 patients was 32.10±4.67 years old.96 (66.67%) patients were found to have intrauterine abnormalities,while 48 (33.33%) patients were found to have normal intrauterine cavity.48 cases of pregnancy,accounting for 33.33%;the median and interquartile ranges[M (Q1,Q3)]of ovulation induction cycles before and after surgery were respectively[3 (0,5)],[2 (1,3)],the difference was statistically significant(P < 0.05).Among the 48 pregnant patients,32 of them had no structural lesions in hysteroscopy,the ovula-tion induction cycle of these patients before and after hysteroscopy were respectively[5 (0,9)],[1 (0,3)],the difference was statistically significant (P < 0.05).Conclusion:Uterine cavity evaluation is important for the assessments of fertility.As an advanced diagnosis instrument,mini-hysteroscopy can not only timely detect the intrauterine abnormalities of patients,but also with great application value in shorten the ovulation cycle in infertility patients and increase the pregnancy rate.
6.Effect of collaborative care modelbased on family-community-professional team on the oral health-related quality of life in older adults

Objective:To explore the effects of a collaborative care model based on family-community- professional team on the oral health-related quality of life in older adults, and in order to provide reference basis for oral health management of the elderly.Methods:Using one-group pretest-posttest design study, 110 cases of older adults who participated in health checkups at the Yuyang Community Health Center in Yichang City from May 1 to June 30, 2023 were selected by convenience sampling method as study subjects. A oral health intervention program was constructed through literature review, Delphi expert consultation, and pilot trials and implemented for 6 months to compare the differences in oral health self-efficacy level, oral health-related quality of life before and after the intervention of the study subjects.Results:Finally, 108 study subjects completed the intervention including 49 males, 59 females, and 58 were 60-69 years old, 45 were 70-79 years old, and 5 were aged 80 and over; the total oral health self-efficacy score of older adults after the intervention was (59.19 ± 3.43) points, which was higher than (55.05 ± 5.10) points before the intervention, and the difference was statistically significant ( t = - 7.69, P<0.05); the total score of oral health-related quality of life after the intervention was (46.11 ± 5.14) points, which was higher than that before the intervention (41.39 ± 4.06), and the difference was statistically significant ( t = -7.02, P<0.05). Conclusions:The collaborative care model based on family-community- professional team can improve the level of oral health self-efficacy and oral health-related quality of life of the elderly, and provide a reference point for oral health management in the elderly.
7.Influence of comprehensive incubational measures on the perioperative treatment of extensively burned patients who underwent escharectomy and skin grafting

Objective:To explore the influence of standardized and comprehensive incubational measures on perioperative treatment of extensively burned patients who underwent escharectomy and skin grafting.Methods:From January 2017 to November 2018, 50 patients with extensive burn who underwent escharectomy and skin grafting in the First Affiliated Hospital of Air Force Medical University and met the inclusion criteria of this study, were recruited in this retrospective cohort study. According to the incubational measures at that time, 20 patients (14 males and 6 females, aged (33.5±5.2) years) who received routine incubation during the perioperative period from January to October 2017 were set as routine incubation group, and 30 patients (23 males and 7 females, aged (35.8±1.4) years) who received standardized comprehensive incubational measures during the perioperative period from November 2017 to November 2018 were set as comprehensive incubation group. Their body temperature was controlled mainly in 4 stages: preoperative preparation and transfer from intensive care unit (ICU) to operating room, preoperative preparation in operating room, intraoperative operating room management, as well as postoperative transfer from operating room to ICU. The initial body temperature in operating room and intraoperative hypothermia duration, intraoperative blood loss, postoperative recovery time, postoperative chill, blister, and ulcer, and wound healing rate on post operation day (POD) 10 were recorded and calculated. Data were statistically analyzed with two independent samples t test and chi-square test. Results:(1) The initial body temperature in operating room of patients in comprehensive incubation group was (36.3±0.4) ℃, which was significantly higher than (35.6±0.4)℃ in routine incubation group, t=6.658, P<0.01; the intraoperative duration of hypothermia was (205±38) min, which was significantly shorter than (234±42) min in routine incubation group, t=2.564, P<0.05. (2) The intraoperative blood loss of patients in comprehensive incubation group was (323±114) mL, which was significantly less than (490±162) mL in routine incubation group, t=4.272, P<0.01; the postoperative recovery time was (36±8) min, which was significantly shorter than (49±17) min in routine incubation group, t=3.229, P<0.01. (3) The incidence of postoperative chill of patients in comprehensive incubation group was significantly lower than that in routine incubation group ( χ2=28.626, P<0.01). The incidences of postoperative blister and ulcer of patients between the 2 groups were close. (4) On POD 10, the wound healing rate of patients in comprehensive incubation group was (78.08±0.06)%, which was significantly higher than (71.03±0.08)% in routine incubation group, t=3.694, P<0.01. Conclusions:The standardized and comprehensive incubational measures can effectively improve the initial body temperature of patients entering the operating room, shorten the intraoperative duration of hypothermia, reduce the amount of blood loss and postoperative complications, as well as shorten the postoperative recovery time, thus improve the wound healing rate.
8.Chemical Components Distribution and Transcriptome Analysis of Different Tissues from Codonopsis pilosula

ObjectiveThe transcriptome characteristics of different tissues of Codonopsis pilosula were analyzed to illustrate the genetic basis of the accumulation of active ingredients in the root of C. pilosula, and to provide theoretical basis for its high-quality production and cultivation. MethodDifferent tissues of C. pilosula at flowering stage were selected as experimental materials, and the contents of tangshenoside Ⅰ, lobetyolin and atractylenolide Ⅲ were detected by high performance liquid chromatography(HPLC). RNA-Seq was used to perform transcriptome sequencing of different tissues, and the differentially expressed genes were screened and analyzed by Gene Ontology(GO) and Kyoto Gene and Encyclopedia of Genes and Genomes(KEGG) enrichment analysis, in order to explore the characteristics of active compound distribution and the transcriptional profiles. ResultThe contents of polysaccharides and tangshenoside Ⅰ in the root of C. pilosula were significantly higher than those in other tissues. The transcriptional profiles of the root were significantly different from those of stem, leaf and flower. Cluster analysis, GO and KEGG enrichment analysis of differential gene expression showed that the differential expression genes were mainly enriched in flavonoid and phenylpropanoid biosynthesis, sucrose-starch metabolism, plant hormone signal transduction, plant-pathogen interaction, mitogen-activated protein kinase(MAPK) cascade signal transduction, Adenosine triphosphate(ATP)-binding cassette(ABC) transporter and other pathways. The expression of genes related to biosynthesis of phenylpropanoid compounds were significantly up-regulated in the roots and flowers, and ABC transporter proteins were mostly highly expressed in the flowers. The expression of key enzyme genes for polysaccharide synthesis, such as sucrose:sucrose 1-fructosyltransferase(1-SST) and fructan 1-exohydrolase(1-Feh), were significantly up-regulated in the roots, and a large number of stress-responsive genes closely related to the accumulation of secondary metabolites were significantly up-regulated in the roots. ConclusionThe active compound content and transcriptional profiles in C. pilosula roots were significantly different from those in stem, leaf, flower and other tissues, showing tissue specificity. Meanwhile, the genes related to stress response and biosynthesis of active compound, such as fructan and phenylpropanoid compounds, were up-regulated in roots of C. pilosula.
9.Design and clinical application of specialized protective cap for patients with alopecia after autologous hair transplantation

Objective:To investigate the design of specialized protective cap for patients with alopecia after autologous hair transplantation and its application value in nursing care after autologous hair transplantation.Methods:The author designed a kind of specialized protective cap for patients with alopecia after autologous hair transplantation with elastic gauze, fiber, silica gel, and other materials. It was divided into two parts, the front piece was mainly used to protect the hair receiving site, and the back piece was mainly used for pressure hemostasis at the hair donor site. From February 2017 to January 2019, 81 patients with alopecia and had autologous hair transplantation in the First Affiliated Hospital of Air Force Military Medical University, who met the inclusion criteria, were enrolled in this prospective controlled study. According to the tail number of admission number of each patient, 43 patients with odd numbers were recruited in protective cap group (38 males and 5 females, aged 23 to 52 years) and 38 patients with even numbers were recruited in convention group (34 males and 4 females, aged 22 to 55 years). After hair transplantation surgery, patients in the two groups received routine postoperative education. Patients in the conventional group were treated with conventional dressing after surgery. On this basis, patients in protective cap group wore the specialized protective caps for at least 1 week continuously except for necessary dressing change, wound clean, and dressing remove. The follow-ups was performed by responsible doctors and nurses at clinic. The postoperative hemorrhage at the hair donor site on post surgery day (PSD) 3 and swelling of scalp at the surgical site on PSD 7, the folliculitis at the hair receiving site and survival condition of transplanted hair follicle at the receiving site, and satisfaction score within 3 months after surgery were observed and recorded. Data were statistically analyzed with two independent sample t test, chi-square test, and Fisher′s exact probability test. Results:(1) On PSD 3, one patient in protective cap group had hemorrhage at the hair donor site, which was significantly less than 8 patients in convention group ( P<0.05). (2) On PSD 7, 4 patients in protective cap group had swelling of scalp at the surgical site, which was significantly less than 11 patients in convention group ( χ2=5.160, P<0.05). (3) Within 3 months after surgery, 0 patient in protective cap group had folliculitis at the hair receiving site, which was less than 3 patients in convention group. (4) In 3 months after surgery, the survival number of hair follicle in each 100 transplanted hair follicles at the hair receiving site of patients in protective cap group was 94.9±2.8, which was significantly more than 91.1±4.7 in convention group ( t=4.354, P<0.01). (5) The patients′ satisfaction score in protective cap group was (14.2±2.6) points, which was significantly higher than (12.1±3.0) points in convention group ( t=3.338, P<0.01). Conclusions:After autologous hair transplantation, the specialized protective cap can reduce postoperative hemorrhage at the hair donor site, swelling of scalp at the surgical site, as well as improve the survival rate of transplanted hair follicles at the hair receiving site and score of patient satisfaction.
10.Multicenter study on the etiology characteristics of neonatal purulent meningitis

Objective:To study the distribution and antibiotics resistance of the main pathogens of neonatal purulent meningitis in different regions of China.Methods:A retrospective descriptive clinical epidemiological study was conducted in children with neonatal purulent meningitis which admitted to 18 tertiary hospitals in different regions of China between January 2015 to December 2019. The test results of blood and cerebrospinal fluid, and drug sensitivity test results of the main pathogens were collected. The distributions of pathogenic bacteria in children with neonatal purulent meningitis in preterm and term infants, early and late onset infants, in Zhejiang Province and other regions outside Zhejiang Province, and in Wenzhou region and other regions of Zhejiang Province were analyzed. The chi-square test was used for statistical analysis.Results:A total of 210 neonatal purulent meningitis cases were collected. The common pathogens were Escherichia coli ( E. coli)(41.4%(87/210)) and Streptococcus agalactiae ( S. agalactiae)(27.1%(57/210)). The proportion of Gram-negative bacteria in preterm infants (77.6%(45/58)) with neonatal purulent meningitis was higher than that in term infants (47.4%(72/152)), and the difference was statistically significant ( χ2=15.54, P=0.001). There were no significant differences in the constituent ratios of E. coli (36.5%(31/85) vs 44.8%(56/125)) and S. agalactiae (24.7%(21/85) vs 28.8%(36/125)) between early onset and late onset cases (both P>0.05). The most common pathogen was E. coli in different regions, with 46.7%(64/137) in Zhejiang Province and 31.5%(23/73) in other regions outside Zhejiang Province. In Zhejiang Province, S. agalactiae was detected in 49 out of 137 cases (35.8%), which was significantly higher than other regions outside Zhejiang Province (11.0%(8/73)). The proportions of Klebsiella pneumoniae, and coagulase-negative Staphylococcus in other regions outside Zhejiang Province (17.8%(13/73) and 16.4%(12/73)) were both higher than those in Zhejiang Province (2.9%(4/137) and 5.1%(7/137)). The differences were all statistically significant ( χ2=14.82, 12.26 and 7.43, respectively, all P<0.05). The proportion of Gram-positive bacteria in Wenzhou City (60.8%(31/51)) was higher than that in other regions in Zhejiang Province (38.4%(33/86)), and the difference was statistically significant ( χ2=6.46, P=0.011). E. coli was sensitive to meropenem (0/45), and 74.4%(32/43) of them were resistant to ampicillin. E. coli had different degrees of resistance to other common cephalosporins, among which, cefotaxime had the highest resistance rate of 41.8%(23/55), followed by ceftriaxone (32.4%(23/71)). S. agalactiae was sensitive to penicillin, vancomycin and linezolid. Conclusions:The composition ratios of pathogenic bacteria of neonatal purulent meningitis are different in different regions of China. The most common pathogen is E. coli, which is sensitive to meropenem, while it has different degrees of resistance to other common cephalosporins, especially to cefotaxime.
