1.The effect of relaxation training with listening to music on negative emotions of patients with maintenance hemodialysis

Objective To explore the effect of relaxation training with listening to music on negative emotions of hemodialysis patients.Methods 70 hemodialysis patients were divided into the intervention group and the control group by random number table,and each group had 35 cases.The control group was given routine care,health education and routine discharge guidance during dialysis by the responsible nurses.The intervention group had relaxation training with listening to music on the basis of routine care.Before the intervention and three months after the intervention the Hamilton Rating Scale for Depression was used as an assessment tool to evaluate negative emotion of patients.Results Before intervention,the HAMD scores of the two groups showed no significant differences.3 months after intervention,HAMD scores had significant difference between two groups.Conclusions Relaxation training with listening to music can significantly improve patients' negative emotions,and is worthy of clinical application.
2.Effect of inhibiting the phosphorylation of signal transducer and activator of transcription-3 on Th2 cell-mediated airway inflammation and airway remodeling in a mouse model of asthma

Objective To study the effect of inhibiting the phosphorylation of signal transducer and activator of transcription-3 (STAT3) on Th2 cell-mediated airway inflammation and airway remodeling,and to explore the role of STAT3 in the pathophysiology of bronchial asthma.Methods Forty Balb/c mice were randomly divided into control group(n =10),asthma group(n =10),AG490 by intraperitoneal group (n =10),and AG490 by inhalation group (n =10).The mice were sensitized with ovalbumin to establish the asthmatic model.The histological changes were evaluated by means of HE staining,while total broalchial wall thickness(Wat) and smooth muscle thickness(Wam) were measured by using image analysis system.The percentages of collagen deposition were detected by way of Masson's trichrome staining; the bronchoalveolar lavage fluid (BALF) were collected,the total cell and the cell differentials were counted,the levels of IL-4,IL-5 in BALF were measured by enzyme-linked immunosorbent assay; the lung tissue extracts were analyzed for phosphorylation of STAT3 (p-STAT3) by Western blot.The SPSS 13.0 software was used to analyze the data.Results 1.The histological changes by HE staining showed that less inflammatory cells infiltration in airway and around the pulmonary vascular in AG490 administration groups compared with asthmatic group.Wat,Wam and the percentages of collagen deposition in AG490 administration groups was significantly lower than that in asthmatic group(F =49.5,41.7,58.2,all P < 0.05).2.The level of p-STAT3 in the lung of AG490 administration groups were significantly lower than those in asthmatic group(F =34.17,P < 0.05).3.The total cells and eosinophils amounts in BALF of AG490 administration groups were significantly lower than those in asthmatic group (F =42.5,64.7,all P < 0.05).The levels of IL-4,IL-5 in BALF of AG490 administration groups were significantly lower than those in asthmatic group,respectively (F =39.2,75.1,all P < 0.05).Conclusions STAT3 signaling pathway is pivotal in Th2 cell-mediated airway inflammation and airway remodeling in asthmatic models,and AG490 can ameliorate airway inflammation and airway remodeling efficiently by inhibiting the phosphorylation of STAT3,and targeting this signaling pathway may be a novel therapy for asthma.
3.Effects of active cycle of breathing techniques on the respiratory function and quality of life in patients undergoing mechanical ventilation

Objective To study the effect of active cycle of breathing techniques ( ACBT ) on the respiratory function and quality of life in patieats undergoing mechanical ventilation .Methods A total of 65 patients with respiratory failure treated by mechanical ventilation were randomly divided into control group ( n=30) and ACBT group ( n=35) .The patients in ACBT group were treated by ACBT , and the patients in the control group were treated by conventional nursing care .Patients'vital signs , respiratory function and quality of life were compared.The level of physical recovery and quality of life were measured by the scale of the Barthel index and the activities of daily living (ADL), respectively.Results In the ACBT group, the mean time of mechanical ventilation was (24.0 ±8.9) h and the success rate of ventilator weaning was 77.1%, which were better than those of (75.0 ±6.7) h and 53.3%in the control group.There were significant differences between the groups (t=2.73,6.92, respectively;P<0.05).At the first day after ventilation, there was no significant difference in the score of ADL between the groups (P>0.05).But the score of ADL was significantly different at the third day (t=2.85,P<0.05).30.0% patients in the control group and 57.1% patients in the ACBT group had a good viability, the difference was significant (χ2 =6.47,P <0.05).Conclusions The active cycle of breathing techniques was useful in shortening the time of mechanical ventilation and can improve patients 'quality of life.
4.Expression of Hsa_circ_0003928 in diabetic foot ulcer and its relationship with severity and prognosis

Objective To investigate the expression of serum Hsa_circ_0003928 in patients with diabetic foot ulcer(DFU)and its relationship with the severity and prognosis of disease.Methods 113 DFU patients were selected as the study subjects.According to the severity of infection,19 cases were classified into level 1(no infection),40 cases at level 2(mild infection),20 cases at level 3(moderate infection),and 34 cases at level 4(severe infection).According to the prognosis of DFU patients,they were divided into good prognosis group(GP,n=63)and poor prognosis group(PP,n=50).The baseline data and levels of IL-6,C-RP and Hsa_circ_0003928 were compared among the four groups.Logistic regression was used to analyze the risk factors of poor prognosis in patients with DFU.The receiver operating characteristic(ROC)curve was used to analyze the value ofHsa_circ_0003928,C-RP and IL-6 in predicting the poor prognosis in DFU patients.Results The DFU duration,infection grade 3~4,serum creatinine,uric acid,BUN,C-RP,IL-6 and Hsa_circ_0003928 levels in PP group were significantly higher than those in GP group(P<0.05 or P<0.01).Grade 3~4 DFU patients had higher Hsa_circ_0003928 expression than grade 1~2(P<0.01).Logistic regression analysis showed that long duration of DFU,infection grade 3~4,higher levels of BUN,C-RP,IL-6 and Hsa_circ_0003928 were risk factors for poor prognosis in DFU patients.ROC curve showed that Hsa_circ_0003928 had the greatest AUC(0.882,95%CI 0.819~0.942)in predicting poor prognosis in DFU patients,with sensitivity 87.5%and specificity 85.6%,respectively.Conclusion Elevated Hsa_circ_0003928 is associated with DFU severity and poor prognosis,which has certain predictive value for the prognosis of DFU patients.
5.Establishment and application of multidisciplinary chain management model based on information technology in surgical patient handover

Objective? To explore the clinical application of information technology in the multidisciplinary chain management of surgical patient handover, in order to reduce the safety hazard in the process of surgical patient handover. Methods? In the contemporary controlled study, 300 patients were selected as the control group from February to July 2017 by cluster sampling, and 303 patients were selected as experimental group from February to July 2018. Traditional sectional handover method was adopted in the control group, to managed the patients' handover from each section. In the experimental group, based on the information communication platform between the surgery room and relevant clinical departments, by clinical electronic documents chain transfer method, quality inspection and supervision information feedback technology, to achieve shared decision and improvement of patients handover problems, we built the multidisciplinary management pattern. The two groups were compared for statistical difference from six aspects:the rate of acceptance specification, the accuracy of identity verification method, the rate of operation labeling verification, the accuracy of inventory of items, the evaluation of pipeline patency, and the evaluation of skin condition at the compression site. Results? The rate of standard delivery of surgical patient handover increased from 49.33% to 92.08% in the two groups. The experimental group was better than the control group in the rate of correct delivery of handover from the following five aspects: identity recognition, indication of surgical site, inventory of articles carried, unobstructed pipeline and skin state of the pressure site with statistical significance (P< 0.01). Conclusions? The multidisciplinary chain management model of surgical patient handover supported by information technology is effective in clinical application, which can significantly reduce the risk of hazard in patient handover and is worthy of clinical promotion.
