Neovascular glaucoma ( NVG ) is a kind of intractable eye disease with complex etiology, strong destruction and poor effect on treatment. Extensive retinal ischemia and hypoxia is the main etiology, and the key of treatment is early diagnosis, active prevention and taking effective measures to prevent the production of vascular endothelial growth factor. According to the related literature over recent years, the authors will discuss pros and cons for medical, surgical and combined treatment in this review.

Objective To explore the feasibility of dual-source CT virtual scan replace conventional scan in the CT peritoneography. Methods Prospective enrolled 29 cases who accepted peritoneal dialysis (PD) treatment and were highly suspected with PD-related complications. The patients were firstly underwent CT conventional in the fasting state by applied the dual-source CT (single energy pattern), and then 2 L dialysate which mixed with contrast medium was infused into peritoneal cavity, and the CT scan was performed again after 30 min (dual-energy pattern). Scaning data were processed by virtual post-processing software to obtain virtual CT scan imaging. Subjective evaluation (including lesions and image quality score) and objective evaluation (including the CT value, noise and signal to noise ratio) were performed on the conventional scan and virtual scan, and recorded the radiation dose. Wilcoxon test was used to analyse the differences of lesions revealing and image quality score of conventional scan imaging and virtual scan imaging, and paired t test was applied to compare the differences of radiation dose and the other quality objective evaluation index. Results There was no significant difference between conventional CT scan and virtual CT scan in the cyst, calcification, calculus, hematoma and catheterization (P>0.05). The image quality score of conventional CT scan and virtual CT scan was 5.0±0.0 and 4.9±0.4, and there was no significant difference (Z=-1.6,P=0.10). There was no statistically significant difference in CT value of muscle, kidney, subcutaneous fat, urine in bladder and intraperitoneal fluid between the two groups (P>0.05), and the CT value of liver in the virtual CT scan was higher than that in the conventional CT scan (P<0.05), but the value of bone in the virtual CT scan was lower than that in the conventional CT (P<0.05). The noise of conventional CT was significant higher than that of invirtual CT, but the SNR was significant lower than virtual CT scan (P<0.05). The effective radiation dose in the conventional CT scan and virtual CT scan were(9.6±1.4)and(15.9±4.9)mSv, and showed significant difference. Furthermore, the effective radiation dose decreased by 39.6%(6.3/15.9). Conclusion Dual-energy CT scan can replace conventional CT scan in the CT peritoneography, which can guarantee the accuracy of diagnosis and reduce radiation dose.

Objective To detect hepatitis C virus core antigen in 7721 cells transfected with HCV cDNA by immunohistochemistry method with human single chain Fv antibody(scFv). Methods The recombinant phages were panned by core antigen which was coated in a microtiter plate. After three rounds of biopanning, 48 clones were identified specific to core antigen. The affinity and specificity of scFv were evaluated by ELISA and immunohistochemistry. Results ScFv-core DNA digestion and sequence data showed that the scFv gene was composed of 774bp. Conclusion Human single chain Fv antibody against HCV core antigen has a specific combining capacity with hepatitis C virus core antigen.

OBJECTIVE:To develop a method for concentration determination of voriconazole in human plasma and apply it in the clinic. METHODS:UPLC-MS/MS method was adopted. Using ketoconazole as internal standard,the determination was per-formed on Shim-pack VP-ODS column with mobile phase consisted of water(containing 1‰ formic acid and 2 mmol/L ammonium acetate)-acetonitrile(gradient elution)at flow rate of 0.3 ml/min and column temperature of 40℃.The electrospray ion source,pos-itive ionizing pattern and multiple reaction monitoring were used;the mass transition ion-pairs of voriconazole and internal standard were m/z 351.2→282.2 and m/z 532.1→490.2. RESULTS:The linear range of voriconazole were 1-10 000 ng/ml (r=0.999 5,n=5),and the limit of quantitation was 1 ng/ml;RSDs of inter-day and intra-day were all lower than 10%;method recovery was higher than 90%(RSD<8%),and extraction recovery was higher than 70%(RSD<8%). The plasma concentrations of voriconazole in 10 patients with invasive fungal infection determined by this method were 507.33-7 011.24 ng/ml,and those of 3 patients were outside the recommended treatment concentration range. CONCLUSIONS:The established method is fast,accurate and sensitive,and can be applied for the therapeutic drug monitoring of voriconazole.

As one of the first resident standardized training bases, department of ophthalmology of Shanghai Ruijin Hospital participated in this reform process from 2010. Relevant rules and regulations (training management system , training scheme implementation system and evaluation system ) were strictly obeyed. When new problems emerged, under the guidance of department in charge, a series of regimens were formulated and improved gradually by Ophthalmology Professional Committee of Shanghai Resident Standardized Training Department. Based on reviewing and summarizing the work in the last 3 years, some thoughts and suggestions on the resident standardized training in future were put forward ,in-cluding how to better solve the“heavily used, lightly cultured” problem, the“disregarding medical ethics establishment”problem, the“disregarding assessment of teachers”problem and the“disregard-ing obtaining employment”problem.

Objective To investigate the effect of 3 concentrations of sevoflurane pretreatment on acute lung injury induced by endotoxin in rats. Methods Thirty-six male SD rats were randomly divided into 6 groups:a control group(Group A), a sevoflurane group(Group B), a 3.6% sevoflurane pretreatment group(Group C), a 2.4% Sevoflurane pretreatment group(Group D), a 1.2% sevo sevoflurane pretreatment group(Group E), and lipopolysaccharide (Group F).The rats were killed 6 h after lipopolysaccharide (LPS) or saline injection.Histological examinations of the lung tissues were performed with light microscope. Lung wet/dry weight (W/D) ratio, myeloperoxidase (MPO) activity, and intercellular adhesion molecule 1 (ICAM-1) mRNA expression were assayed. Results Introvenous LPS significantly aggravated lung tissue damage,increased lung W/D ratio, MPO activity, and lung ICAM-1 mRNA expression compared with the control group(P＜0.05). Precondition with 2.4% sevoflurane significantly attenuated the above mentioned changes induced by LPS (P＜0.05). The 3.6% LPS (Group C) significantly attenuated lung tissue damage and decreased MPO activity,lung ICAM-1 mRNA expression compared with the Group F (P＜0.05),but no significant change in lung W/D ratio was seen (P＞0.05). MPO activity was significantly decreased in Group E (P＜0.05), but lung W/D ratio and lung ICAM- 1 mRNA expression had no significant changes (P＞0.05).Conclusion Precondition with 2.4% sevoflurane can reduce LPS induced lung injury in rats. Decreased expression of ICAM-1 and less accumulation of neutrophils were participated in its mechanism.

Decreasing trend has occurred in incidence of colorectal cancer in developed countries. A shift from left to right in location of colorectal cancer has been recognized,which may be associated with colonoscopy screening,aging population,diet structure modification,increased incidence of cancer and diabe-tes. Factors including sex,race and education may also play a role to some extent. Right-sided intestinal cancers have higher proportion of poor differentiation,terminal stage and mucinous component. Complications and second primary intestinal cancer are more common in right-sided intestinal cancers. Left-sided intestinal cancers tend to be well differentiated and at relatively early stage at diagnosis. With respect to molecular mecha-nism,right-sided cancers are associated with mismatch repair system,while left-sided cancers are related to p53 mutation. Based on the differences in clinicopathology and genetics,it′s implied that left-sided and right-sided intestinal cancers may belong to two different kind of disease. It′s suggested that attentions should be paid differently according to their respective characteristics in clinical practice and trials.
