Objective To evaluate clinical effect of mild hypothermia on patients with severe brain injury.Methods 40 patients with severe brain injury were randomly divided into hypothermic group(n=20) and control group(n=20).Their excellence rates cvere compared.Results The total excellence rate of treatment group was 80% and that of control group was 50%,the former was significantly higher than the latter.Conclusion Mild hypothermia can improve the outcome of patients with severe brain injury.

Objective To explore the relationship among the changes of five coagulation indexes and clinical stage,metastasis in pre-operative patients with gastric carcinoma,and to investigate the coagulation in patients with different clinical stages and its clinical significance.Methods Eight hundred and sixty-eight gastric carcinoma patients( carcinoma group) who were firstly diagnosed and 213 patients ( control group) who were diagnosed as inguinal hernia in the same hospital during the same period were enrolled.Levels of rothrombin time (PT),activated partial thromboplastin rime (APTT),fibrinogen (FBG),plasma thrombin time (TT) and platelet count (PLT)were observed and compared in these two groups.Results (1)Compared with health control group,PT( [ 11.51 ± 1.21 ] s vs [ 11.27 ± 0.91 ] s,t =- 3.226,P ＜ 0.01 ),FBG( [ 3010.0 ±470.4] mg/L vs [ 2307.1 ± 651.8 ] mg/L,t =- 4.240,P ＜ 0.01 ) and PLT ( 238.57 ± 78.50 ) x 109/L vs (206.79 ±61.01 ) x 109/L,t =-5.514,P ＜0.01 )were significantly increased in gastric carcinoma group.(2)PT,FBG and PLT were significantly different between control group and different clinical stages of gastric carcinoma group ( P ＜ 0.05 or P ＜ 0.01 ).The levels of APTF,TT exhibited no significant difference between control group and different clinical stages of gastric carcinoma group ( P ＞ 0.05 ).( 3 ) Compared with gastric carcinoma without metastasis group,preoperative blood PT extended significantly ([ 11.41 ± 0.99] s vs [ 11.57 ± 1.32 ]s,t =- 2.095,P ＜ 0.05 ),FBG concentration increased significantly ( [ 2639.1 ± 748.6 ] mg/Lvs [ 3233.2 ± 591.60 ] mg/L,t =- 2.307,P ＜ 0.05 ) and PLT count increased significantly ( [ 224.02 ±76.94] x 109/L vs [ 247.32 ± 78.37 ] x 109/L,t =- 4.299,P ＜ 0.01 ) in gastric carcinoma with metastasis group.(4) Correlation analysis on four coagulation and PLT count with clinical stage:PT ( r =0.071,P ＜ 0.05 ),FBG ( r =0.271,P ＜ 0.01 ) and PLT( r =0.166,P ＜ 0.01 ) were positively correlated with clinical stage of gastric carcinoma and TT( r =- 0.077,P ＜ 0.05 ) was negatively correlated with clinical stage,among the total,the correlation coefficient of FBS with clinical stage was maximum.Conclusion Most patients with gastric cancer present state of blood coagulation and bleeding tendency,particularly in advanced gastric cancer,and with increased clinical stage of gastric cancer,some coagulation indicators showing a rising trend.Detection of coagulation index indirectly indicates the incidence,invasion and metastasis of gastric carcinoma.

OBJECTIVETo investigate the effects of different acupoint combination on prevention and treatment of stress gastric ulcer and to search for a better acupoint combination.

METHODSThe theoretical basis and experimental research on treatment of stomach diseases with the combination of he-(sea) points and front-mu points as well as back-shu points and front-mu points are analyzed in this article.

RESULTSThe combination of he-sea points and front-mu points is suitable to be applied on acute diseases, febrile diseases and diseases of fu organs. While the combination of back-shu points and front-mu points is more applicable on chronic gastric diseases.

CONCLUSIONIt is proved that the combination of he-sea point (Zusanli ST 36) and front-mu point (Zhongwan CV 12) has reliable effect on prevention and treatment of stress gastric ulcer.

This article reviewed occurrence of ADR of clinical external use of 50 Chinese medicinal herbs, such as Verbascum Thapsus, Ranunculus Sceleratus, and Euphorbia Esula. It summarized from foundation, property, efficacy, clinical manifestation, and classification of ADR, and discussed early warning ways of ADR from TCM external use, with a purpose to provide references for TCM reasonable medication.

Objective To analyse CT appearances of malignant neurilemoma in the thorax and abdomen,in order to improve the diagnostic accuracy of this disorder. Methods There were 9 cases(5 cases in thorax and 4 cases in abdomen) with malignant neurilemoma proved by pathology (7 cases by surgery and 2 cases by CT guided percutaneous puncture biopsy). CT both plain and enhanced scans were performed in all cases. Results Tumors were located in the mediastinum in 5, in abdominal wall and retroperitoneum in 2 respectively .7 cases were isolated masses,2cases were disseminated masses,in company with pleuritic fluid was in 4 and with osteogenic metastasis of vertabral body in one case.The isolated masses appeared as round or ellipse with low density in the centre,the CT value was 5~20 HU,6 cases had complete capsule.The disseminated masses in 2 cases appeared as homogeneous density.On contrast-enhanced scan,the solid parts of masses were enhanced in different degree. Conclusion The isolated thoracic and abdominal malignant neurilemoma is of certain CT characteristics, but the disseminated one is not.

Objective To explore the inhibitory effects of the vector on the growth of mammary adenocarcinoma cell MCF-7 and human telomerase reverse transcriptase(hTERT) gene expression in MCF-7 cells,by the way of the construction of the RNA interference(RNAi) expression vector of hTERT gene.Methods An interference target sequence TGT TCA GCG TGC TCA ACT A,which aimed at hTERT gene,was designed and the small interfering RNA(siRNA) expression plasmid pGenesil-hTERT was constructed by means of recombination.The recombinant plasmid was transfected into the MCF-7 cells by LipofectamineTM 2000 after the identification of the recombinant expression vector pGenesil-hTERT in the light of electrophoresis after digestion and DNA sequencing,then RT-PCR analysis was used to detect the gene expression,and Western blotting analysis was used herewith to detect the protein expression of hTERT,and the growth of MCF-7 cells was tested by MTT assay.Results The results of analysis and the sequencing identification confirmed that the target sequence was inserted into the predicted site precisely,and the recombining plasmid was successfully constructed.RT-PCR and Western blotting analysis showed that hTERT gene expression and protein expression of pGenesil-hTERT group were obviously decreased in MCF-7 cells,and the growth of MCF-7 cells of pGenesil-hTERT group was also inhibited.Conclusion The endogenous hairpin siRNA produced by DNA plasmid is capable of mediating hTERT gene inhibition in MCF-7 cells effectively,and furthermore inhibiting the cells growth and decreasing the speed of cells proliferation.This research may act as the experiment evidence to the gene therapy of cancer.

Objective To evaluate the feasibility and advantages of transvaginal hysterectomy (TVH) for enlarged uterus without prolapse. Methods Six hundred and ninety-six patients underwent TVH for gynecological disorders from Sept, 1998 to Dec, 2002. Meanwhile, the method of operation, complications of surgery, operation time, estimated operative blood loss, postoperative recovery and hospital stay days were retrospectively analyzed. Results The operation was successful in 693 out of 696 cases (99.6%). The mean operative time was 49.3 minutes (11-125 minutes). The estimated blood loss was 69.7ml (15-450ml). Operative injuries were found in 3 patients (0.4%). Complications attributable to surgery were found in 6 cases (1%). The mean time of subsidence of fever was 1.2 days (0-5 days) and the time of passing flatus was 22.6 hours (8-32 hours). The average hospital stay was 5.3 days (4-15 days). Conclusion TVH can be performed safely for most of non-prolapsed large uterus. Because it has the advantages of less bleeding, short operating time, rapid recovery and less cost compared to TAH and LAVH, it is our opinion it should be the first option for enlarged uterus.

Objective To evaluate the advantages and feasibility of transvaginal excision of ovarian tumor. Methods A series of 62 patients with ovarian benign tumors had the tumors excised transvaginally from Dec 2001 to Oct 2003. The indications of transvaginal surgery, the method of operation, the complications following the operation, the overall operation time, the estimated blood loss during operation, the postoperative complications, and hospital stay days were analyzed retrospectively. Results Transvaginal ovarian tumor excision was successfully completed with neither the aid of laparoscopy or shift to laparotomy. No intra-abdominal spillage of cystic contents occurred. The mean operation time was 39.4 minutes. The estimated blood loss was 37.5ml. No postoperative complications occurred. The mean length of a low fever was 1.3 days, and the time of recovery of bowel movement was 20.1 hours. The average hospital stay was 4.2 days. Conclusion For free benign ovarian tumors, the transvaginal route should be considered to minimize trauma. There would be no skin incision, and it could be carried out in patients who had relatively large ovarian tumors with no intra-abdominal spillage. There would be less blood loss, shorter operation time, shorter hospital stay and lower cost.

Objective To evaluate the feasibility and advantages of the excision of uterine fibroids by vaginal myomectomy. Methods A series of 58 patients with uterine fibroids were managed with myomectomy from May 2001 to Dec 2003. Meanwhile, the indications of vaginal surgery, the methods of operation, the complications of surgery, the overall operating time, the estimated operative blood loss, the postoperative recovery and hospital stay were explored. Results Myomectomy were successfully completed vaginally without shifting laparotomy in 57 patients. The successful rate was 98 3%. The mean operative time was 48 16 minutes. The estimated blood loss was 70 58ml. The postoperative complication was occurred in one patient with uterine infection. The mean length of lower fever was 1 8 days and the bower movement was 1 3 days. The average hospital stay was 4 9 days. Conclusions For uterine fibroids, the vaginal myomectomy is a good choice when the indication of operation are managed properly. It can be used in patients who have numerous, relatively large, and intramural fibroids with micro invasive surgery, little interfering with abdominal organs, less blood loss, less operating time, less cost and less hospital stay in comparison with abdominal myomectomy or laparoscopic myomectomy
