1. Preparation of andrographolide derivative ISA-loaded bile salt-phosphatidy-I choline-mixed micelles and pharmacokinetics evaluation in rats

OBJECTIVE: To prepare ISA-loaded bile salt-phosphatidylcholine-mixed micelles (ISA-BS/PC-MM) and study the release characteristics in vitro and pharmacokinetics in rats. METHODS : ISA-BS/PC-MM were prepared by film dispersion method. The formulation was optimized by the central composite design-response surface method. The release behaviors in vitro of the micelles were studied by dialysis method and its pharmacokinetic characteristics were studied by intrastric administration in rats. RESULTS: The drug concentration, entrapment efficiency, drug loading and average diameter of the optimized ISA-BS/PC-MM were 0.87 mg · mL-1, 86.34%, 4.87% and 148.3 nm, respectively. The release characteristics of ISA-BS/PC-MM were well fitted with Rither-Pep-pas equation. The pharmacokinetic data of ISA and I-SA-BS/PC-MM were in accord with two-compartment model. Compared with the original drug, micelles increased the absorption rate constant, shortened tmax, prolonged the elimination half-life and MRT, reduced the volume of distribution and clearance rate, and increased AUC. CONCLUSION ISA-loaded bile salt-phosphatidy-lcholine-mixed micelles can increase drug solubility and enhance bioavailability.
2.Clinical effect observation on liver-regulating and blood-tonifying acupuncture for cervical spondylosis of vertebral artery type

Objective:To observe the clinical effect of liver-regulating and blood-tonifying acupuncture in the treatment of cervical spondylosis of vertebral artery type. Methods:A total of 60 cases meeting the inclusion criteria were randomized into an observation group and a control group, 30 cases in each group. Cases in the observation group received liver-regulating and blood-tonifying acupuncture, in which Jiaji (EX-B 2) points of C3-C7, Ganshu (BL 18) and Geshu (BL 17) were used; cases in the control group received routine acupuncture treatment in which jiaji (EX-B2) points of C3-C7 were used. Both groups received 5 times of treatment in a week with a 2-day rest, symptoms evaluation was done after 4 weeks of treatment, and then therapeutical effect was evaluated. Results:The total effective rate was 90.0% in the observation group, 73.3% in the control group, and inter-group comparison showed a statistical significance (P<0.05). After treatment, the symptoms and signs scores in both groups all substantially dropped, showing statistical significances (both P<0.05); inter-group comparison showed that the improvements in subscales of dizziness, pressing pain along vertebrae and revolve-cervix test in the observation group were superior than those in the control group, showing statistical significances (all P<0.05), while the inter-group comparisons of subscales of headache and vomiting and nause didn't show any statistical significances after treatment (both P>0.05). Conclusion: Liver-regulating and blood-tonifying acupuncture is effective in treating cervical spondylosis of vertebral artery type, and it can significantly improve patients' symptoms and signs.
3.Clinical study on the complications of transvaginal hysterectomy:analysis for 1990 cases

Objective To analyze incidence,pathogenesis and treatment of the complications of transvaginal hysterectomy(TVH)collected during past six years,and to discuss the possible methods of prevention.Methods The patients involved in present study were admitted and underwent the transvaginal hysterectomy in the General Hospital of PLA from Jan.2000 to Jun.2006.The data of complications occurred at intra-or post-operation were retrospectively analyzed.Results Among all of the 1990 cases with TVH,complications occurred in 75 cases,the incidence of complication was 3.77%,including 21 cases of bladder injury(1.06%),5 cases of ureter trauma(0.25%),6 cases of rectal injury(0.3%),30 cases of vaginal stump inflammation(1.51%),8 cases of vaginal stump haematoma(0.4%),2 cases of intra-abdominal hemorrhage(0.1%),2 cases of vaginal narrowness(0.1%)and 1 case of intestinal obstruction(0.05%).The bladder injury and vaginal stump inflammation almost toke the two third of total cases.Some complications were got correction by surgery appropriately during or after the operation when injuries were found.Some cases were managed by conservative treatment.All of the patients with complications were healed without any sequelae.The organ injuries occurred more easily in the cases with pelvic adhesion or less movable large uteri,such as the cases with the history of pelvic operation,the endometriosis or the lower segment myoma.Conclusions The organs adjoined to uterus were easily damaged by TVH during the performance of operation,and the corresponding complications are common after the operations.The operator should familiar with the pelvic anatomy,master the skilled surgical techniques and suitably control the indications of the TVH.With the improvement of techniques in TVH and the research of new instrument applied in this procedure,the incidence of complications in TVH would be further reduced.
4.Expression of lysyl oxidase in the pelvic floor tissues of women with stress urinary incontinence

Objective To detect the expression of lysyl oxidase (LOX) in pelvic floor tissues and the serum copper ions level of the women with stress urinary incontinence (SUI) for exploring the role of LOX in SUI pathogenesis. Methods The sera of 25 patients with SUI were collected before the operation. The content of serum copper was measured by the technique of atomic absorption spectrum. Meanwhile, 25 patients with similar age but suffering from benign gynecological diseases and undergone total hysterectomy were enrolled as control. Biopsy specimens were obtained from the uterine cardinal sacral ligament of the patients with and without SUI during the operation. The histological structure of pelvic collagen fiber was examined with routine HE staining and Van Gieson (VG) staining. Immunohistochemistry was used to determine the location of LOX. Protein expression of the LOX in pelvic floor tissue was analyzed by Western blotting. Results There was no statistical difference in serum copper content between SUI group and the control group. In the HE and VG staining sections, collagen fiber density of pelvic tissue in SUI group was decreased and fibers were arranged disorderly. Immunohistochemistry showed that LOX was mainly expressed in the cytoplasm of fibroblast. The number of positive cells in pelvic tissues of the patients with SUI was smaller than that in the control group, and the positive signal in SUI group was weaker than that in the control group. The reproductive toxicity of environmental endocrine disruptors has attracted substantial attention from researchers in recent years. Bisphenol A (BPA) is among the most prominent environmental estrogens worldwide, demonstrated to be related with the impairment of male reproductive function as well as other health problems, such as diabetes, obesity, cardiovascular diseases, and cancer. BPA acts primarily by mimicking antiandrogenic and estrogenic effects, disturbing the hypothalamic-pituitary-testicular axis and modulating gene expressions and enzyme activities in the hormone biosynthesis affecting steroids or its receptors. BPA is also involved in DNA methylation and the effects of epigenetics, resulting in dyszoospermia, oligoasthenoteratospermia/azoospermia and/or infertility in males. This review addresses the effects of BPA on male reproductive function, focusing on the mechanisms of its toxicity on spermatogenesis, semen quality, and the reproductive system.

6.Combined determination of serum C-reactive protein,lipase,interleukin-1β,and intercellular adhesion molecule 1 in early identification of severity of acute pancreatitis

Objective Toevaluatethevalueofcombineddeterminationofvariousindicesintheclinicaldiagnosisofacutepancreatitis (AP)through dynamic measurements of the levels of peripheral serum C-reactive protein (CRP),lipase (LPS),interleukin-1β(IL-1β),andintercellularadhesionmolecule1(ICAM-1),aswellaschangesintheirlevels,inpatientswithAP.Methods Atotalof86 patients with AP who were treated in Fourth Affiliated Hospital of China Medical University from January 2010 to December 2012 were select-ed,among whom there were 39 patients with severe acute pancreatitis (SAP)and 47 patients with mild acute pancreatitis (MAP).Serum sam-ples were collected from these patients on days 1,3,5,and 7 of admission.Twelve healthy subjects were selected as controls.Enzyme-linked immunosorbent assay was applied to measure the levels of CRP,IL-1β,and ICAM-1 ,and the dry slides method was applied to measure the concentration of serum LPS.The t test and χ2 test were applied for comparison of continuous data and categorical data between groups, respectively.Results Onday1ofadmission,thepatientsinMAPgrouphadsignificantlyhigherlevelsofCRP,IL-1β,andLPSthan those in control group (t=-74.126,-60.135,and -364.153,respectively;all P<0.001);on day 1 of admission,the patients in SAP group had significantly higher levels of CRP,ICAM-1,IL-1β,and LPS than those in control group (t=-121.355,-25.728,-89.422,and -415.840,respectively;all P<0.001).The peak concentration of each index appeared on day 1 or 3 of admission,and decreased progressively in the course of therapy.The patients in SAP group had significantly higher levels of serum CRP,LPS,IL-1β,and ICAM-1thanthoseinMAPgroup(allP<0.05).Conclusion CombineddeterminationofserumCRP,LPS,IL-1β,andICAM-1 levels has a certain clinical value in early identification of severity of AP.
7.Effect of embryonic stem cells-derived neural stem cells on macrophage in vitro

Objective:To explore the influence of embryonic stem cells-derived neural stem cells on the proliferation and secretion cytokines of bone marrow-derived macrophages. Methods:Mouse bone marrow derived macrophages were isolated and cultured in L929 medium. After macrophages were treated with NSCs supernatant for 3 days,SRB method was used to detect the proliferation of macrophages. The phagocytosis of macrophages were detected by incubating with RFP-Beads for 1 h. Meanwhile,the expression of TNF-α and IL-1β were detected by ELISA. Results:NSCs were successfully induced from ESC. In control group and NSC group, the proliferation rate of macrophages were 100 % and (126. 29 ± 5. 41)%,the phagocytosis rate were (70. 23 ± 2. 57)% and (90. 32 ± 8. 49)%. Compared to the control group,the levels of IL-6,IL-1βin macrophage treated with NSCs decreased (P<0. 05). Conclusion:ESC-derived NSCs can promote the proliferation and phagocytosis of bone marrow-derived macrophage, and suppress the secretion of pro-inflammatory cytokines.

Chronic kidney disease(CKD) is a major global health issue that leads to end-stage renal disease which untreated.The use of stem cell therapy provides a new perspective in this area.Reviewing the experimental studies of stem cell therapy on CKD,many different,and even contradictory reports in this area were found.In this paper,the recent reports,and present several key issues of experimental stem cell treatment on CKD were reviewed,including the source of stem cell,the sort of experimental animal,the time of treatment and other experimental details.Hoping these may lead more understanding in this area.
9.Analysis of poor vision conditions and risk factors of myopia in primary school He'xi district of Sanya

AIM:To evaluate the poor vision condition and analyze the myopia etiological factor in primary school He'xi district of Sanya.
METHODS: A cohort of 1 218 subjects were recruited randomly from grade 1~6 of two primary schools. Visual activity test, dioptroscopy and risk factor questionnaire were evaluated.
RESULTS: The prevalence of poor vision was 29. 55%. The incidence of myopia increased with grade, and was significantly higher in girls than that in boys. The outdoor activity time of myopic pupil ( 7. 25 ± 5. 35h/wk ) was significantly lower than emmetropic pupil (11. 86±6. 65h/wk) ( P < 0. 05 ). The usage of electronic product ( TV, computater and cell phone) time of myopic pupil (13. 36±8. 35h/wk) was significantly higher than emmetropic pupil (7. 75±5. 83h/wk) (P<0. 05).
CONCLUSION: The increase of myopia incidence is closely related with sparing outdoor activity and excess usage of electronic product.
