Chinese Journal of Practical Nursing 2016;32(12):896-900
Objective To establish a scientific, systemic objective and practical competency model for head nurses. Methods Based on the PPEB model, the competency model for head nurses was formulated through literature review, theoretical analysis, specialists interview and Delphi method. Results The head nurses′competency model consisted of professional competency, psychology competency, ethic competency and behavior competency, which included 10 second- level indicators and 33 third- level items. A total of 16 questionnaires were sent out in each round of consultation, and 15 and 16 questionnaires were retrieved separately. the experts′authority coefficient was 0.88 and the coordination coefficient was 0.131. Conclusions The enthusiam, authority and harmony of the specialists were good, which meant the competency model for head nurses were scientific and reliable. And it could provide references for head nurses′selection, training and performance appraisal.

Objective To establish a questionnaire to measure competence of head nurses and test its reliability and validity. Methods Based on the third-level items of the competency model built by Tanjie, the primary competency questionnaire was established combining literature review, team discussion and preliminary test. The reliability and validity of the questionnaire was tested in 236 head nurses. Results The questionnaire consisted of 4 dimensions and 33 items, and 4 common factors were extracted by principal components analysis, which contributed to 75.691%of the variance. The Cronbachα was 0.945 for the total questionnaire, and the content validity index was 0.960. Conclusions The questionnaire shows acceptable reliability and validity, and can be applied to the competence investigation of head nurses.

Objective To investigate the intra-and postoperative course of patients undergoing laparoscopic liver resections under intermittent total pedicle occlusion (IPO),hemihepatic vascular occlusion (HVO),and selective vascular occlusion(SVO).Methods Retrospective analysis the data of 41 cases of laparoscopic liver resection were conducted in three groups of patients under different occlusion methods, including 15 cases of intermittent total pedicle occlusion (IPO),12 cases of hemihepatic vascular occlusion (HVO) and 14 cases of selective vascular occlusion (SVO).Intraoperation blood loss, operation time, conversion to open operation, changes in postoperative liver function, hospital stays and complications were compared among the three methods. Results There was no operative death in any of the 41 patients. There was no conversion to open surgery.Generally, there was no significant difference among the three groups in blood loss, clamping time or operative time.Ten patients had postoperative complication and all were cured. The effect on liver function for Gro-HVO and Gro-SVO was significantly less severe than that for Gro-IPO (P < 0.05) after operation. Conclusions Both HVO and SVO are feasible and safe in laparoscopic hepatectomy(LH),and have advantage in reducing liver remnant ischemia injury and modality rate over IPO.HVO is easy to do for left lateral lobe or resection of the left half of the liver.SVO is suitable for right lobe resection.

Objective To explore the application of three-dimensional (3D) ultrasonography in screening the defects of fetal brain and skull. Methods Thirty-one cases of the fetal were detected in 3D ultrasonography. The ultrasonic data were compared with magnetic resonance imaging(MRI) and pathology. Results Among 31 cases according to 3D ultrasonography, there were 2 of anencephaly, 1 of exencephaly,2 of encephalocele,5 of holoprosencephaly,2 of arachnoidcyst, 1 of vein of Galen aneurysm,2 of agenesis of the corpus callosum, 1 of schizencephaly, 3 of Dandy-Walker syndrome, 7 of ventriculomegaly, and 5 of enlargement of cisterna magna.Compared with the results of MRI,the diagnostic accordance rate was 100% by 3D,it was 93.5% by 2D.Conclusions The structure of fetal brain and skull were viewed more clearly in 3D ultrasonography than 2D ultrasonography and the defects of fetal brain and skull can screened more exactly.

Objective To explore the atopic condition of children with stable asthma and their parents by detecting total IgE concentration in serum and collecting history. Methods Fifteen children with asthma in remission stage and 24 parents and 40 normal children were involved in this study. The concentrations of serum total IgE were measured with UniCAP Pharmacia system. The history including eczema and allergic rhinitis and asthma of children and parents were collected. Skin test with 10 kinds of inhalant allergens were taken in children with asthma. Results Of 15 children with asthma, 13 cases (86.7%)had eczema in infant stage and allergic rhinitis, 13 parents(86.7%)had allergic rhinitis or asthma. There was significant difference in the stable asthmatic children compared with parents and normal control group increased amounts of serum total lg IgE(F=68.42 P=0). There was significant difference in serum total lg IgE in patients group(2.43?0.73)kU/L compared with that in normal control group(0.72?0.54)kU/L (q=14.176 P0.05). Twelve children were positive in skin test, which were mainly dermatophagoides pteronyssinus and dermatophagoides farinae. Conclusions There is obvious congregate phenomena of atopy in the family of asthmatic children. The continuous high IgE concentration was associated with immune pathological mechanisms of atopic diseases.
