1.Accumulation of lipid and the proliferation of pancreatic islet beta cells

Objective To explore whether an excessive accumulation of lipids is involved in the suppression of β cell proliferation and to determine the role of glucose in this experiment. Methods The proliferation of the Ins-1E cells was studied by 3H-thymidine incorporation. The accumulation of fatty acids in beta cells was studied by 3H-palmitate incorporation. Using Taqman real-time RT-PCR, we measured the expression of carnitine palmitoyltransferase-1 (CPT-1) gene. Results As compared with 3.3 mmol/L glucose, the 6.6 mmol/L and 11 mmol/L glucose increased the cell proliferation by 2.27 and 3.04 folds (P<0.05), and the supplemented 0.4 mmol/L palmitate blunted the glucose-induced cell proliferation (1.1 and 0.86 fold, at 6.6 and 11 mmol/L glucose, respectively). With increasing glucose levels, the incorporation of palmitate was augmented,which ranged from 15% to 92%. Wy 14 643 and Triacsin C alleviated the palmitate-induced inhibition of cell proliferation and reduced the incorporation of palmitate. Glucose did not change the expression of CPT-1 gene, but inhibited the palmitate-induced expression in a dose dependent manner. Conclusions The FA-stimulated CPT-1 gene expression is inhibited by glucose, which increases the FA accumulation in β cells and inhibits their proliferation. This could be underlying the mechanism of glucose-dependent lipotoxicity.

Objective To investigate the renoprotective effect of erythropoietin(EPO) in streptozotocin-induced diabetic rats and to explore the possible mechanism.Methods The SD rats were randomly divided into there groups: normal control rats, diabetic, diabetic treated with EPO(NC, DM, DE groups).The rats were sacrificed after 8 weeks treatment.Renal morphology was observed by light microscopy.The expression of erythropoietin receptor(EPOR) in kidney was detected by immunofluorescence and Western blotting.The expression of p47phox, transforming growthfactor (TGF)β1andfibronectin (FN)proteininkidneywasdetectedby immunohistochemistry and Western blotting.The activity of antioxidants including total antioxidant capacity (T-AOC), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and the level of malondialdehyde(MDA) in kidney were also measured.Results EPO treatment notably attenuated renal pathologic and functional changes.The expression of EPOR was found in kidney,but there was no difference among groups(P>0.05).Compared with normal rats, diabetic rats showed an elevated expression of p47phox, TGF-β1, FN proteins and MDA levels in kidney as well as reduced activities of SOD, GSH-Px and T-AOC (all P<0.01).Compared with diabetic rats, EPO could decrease the protein expression of p47phox,TGF-β1and FN in kidney (all P<0.05).Meanwhile, elevated MDA level in the kidney was decreased as well as decreased SOD, GSH-Px,T-AOC activities were significantly remitted in DE group(all P<0.01).Conclusion EPO can amelioraterenaldamagevia theinhibition of oxidativestressandTGF-β1andFNprotein expression in streptozotocin-induced diabetic rats.
4.Effects of nitric oxide synthase inhibitor in two-week oral treatment on hyperdynamic circulatory state in cirrhotic rats

To investigate the effects of low dosage of nitric oxide synthase (NOS) inhibitor Nc-nitro -L-arginine methyl ester ( L-NAME) in two-week treatment on the hyperdynamic circulatory state in rats with cirrhosis. METHODS: Cirrhosis model was induced in male SD rats by injection of 60 % CCL4 oily solution subcutaneously. Cirrhotic rats were treated with L-NAME ( 0.5 mg·kg-1·d-1) by gavage for two weeks. Mean arterial pressure ( AP ), portal pressure(PP), cardiac output ( CO ), cardiac index ( CI ), splanchnic vascular resistance ( SVR ), splanchnic blood flow(SBF) and serum nitrite levels were determined in L-NAME-treated, L-NAME-untreated cirrhotic rats and controls by using 57Co-labled microsphere technique and a fluorometric assay, respectively. RESULTS: Untreated cirrhotic rats had significantly lower MAP, SVR and higher PP, CO, CI, SBF and nitrite concentration than those of the controls (all,P< 0.01 ). In treated cirrhotic rats, L-NAME significantly attenuated the increase of CO, CI, SBF, nitrite concentration and the decrease of MAP and SVR. In treated cirrhotic rats, L-NAME induced a marked decrease of nitrite concentration than untreated cirrhotic rats[(1.471±0.907)μmol/L vs (4.204±1.253) μmol/L, P<0.01]. CONCLUSION: The endogenous NO may play an important role in the changes of hemodynamics pattern in cirrhosis, and hyperdynamic circulatory state in rats with cirrhosis can be ameliorated by oral two-week administration of lower dose of L-NAME.
5.Clinical analysis of renal cell carcinoma with regional lymph node metastasis

Objective To discuss the characteristics of renal cell carcinoma with regional lymph node metastasis at diagnosis. Methods The data of 19 patients diagnosed with renal cell carcinoma with regional lymph node metastases at diagnosis from January 2004 to December 2008 were reviewed.The median age was 57 years (29-77).The study group included 15 males and four females.The primary tumor was located in the left kidney in 12 patients and fight in seven patients.The median maximam diameter of retroperitoneal lymph nodes was 2.8 cm(1.5-5.0).The lymph nodes in four patients were not detected by the preoperative image examination,but were confirmed by intraoperative exploration.Eleven cases had enlarged retroperitoneal lymph nodes resected and eight had regional lymph nodes dissected. Results The patients with regional lymph node metastases at diagnosis of renal celI carcinoma accounted for 1.6% (19/1213) of the total renal cell carcinoma cases.With a median follow-up of 34 months,six patients were survival without progression,and seven were survival with progression.giving a 5-year survival rate of 68.4%.The survival and recurrence rates after surgery were not significantly different by Fisher test(P=0.644 and 0.319 respectively) between the patients who underwent retroperitoneal regional lymph node dissection and those who underwent enlarged lymph node resection. Condmiom Renal cell carcinoma with regional lymph node metastasis at diagnosis is uncommon.Some patients may achieve long-term tumor-free survival through regional lymph node dissection or enlarged Iymph nodes resection.
6.Effects of erythropoietin on renal tubular cells apoptosis induced by high glucose

Objective To investigate whether erythropoietin (EPO) can inhibit the proapoptotic effect of high glucose on rat proximal tubular epithelial cells, and the possible mechanisms in which EPO exerts its anti-apoptotic role. Methods Rat proximal tubular epithelial cells (NRK-52E) were divided into 5 groups: normal control group, osmolarity control group, high glucose group, high glucose with EPO (50 U/ml) group and high glucose with EPO (100 U/ml) group. The expression of EPO receptor (EPOR) in NRK-52E cells was examined by immunocytochemistry. The effect of high glucose on the expression of EPOR was detected by Western blotting. The rate of apoptosis was evaluated by flow cytometry Annexin V-FITC/PI double stains. The intracellular ROS was detected using fluorescent probe CM-H2DCFDA. The expression of bcl-2, bax and caspase-3 mRNA were examined by RT-PCR. Results The expression of EPOR was demonstrated in NRK-52E cells, and high glucose could up-regulate the expression of EPOR. High glucose could induce oxidative stress in NRK-52E cells, and up-regulate the mRNA expression of bax and caspase-3, down-regulate the mRNA expression of bcl-2. These effects of high glucose on NRK-52E cells could be reversed by EPO. Conclusion EPO inhibits NRK-52E cells apoptosis induced by high glucose through attenuating oxidative stress,up-regulating theexpression of bcl-2 mRNA and down-regulating the expression of bax and caspase-3 mRNA, which may be mediated by EPOR.
7.High Performance Liquid Chromatogram-Mass Spectrometry for Helicobacter pylori Infection-associated Differentially Expressed Proteins in Urine with Relative Molecular Mass More than 10 kDa

Background:Helicobacter pylori(Hp)is an important pathogen for peptic ulcer and gastric cancer,and is reportedly associated with a variety of extragastrointestinal diseases. However,there is no body fluid detection technique for Hp infection in clinical practice. Aims:To identify Hp infection-associated differentially expressed proteins in urine with relative molecular mass more than 10 kDa and provide potential biomarkers for diagnosis of Hp infection through body fluid detection. Methods:Midstream urine was collected from volunteers in the morning,and 13 C-urea breath test was performed to determine Hp infection. Each of 15 Hp-negative and 15 Hp-positive urine samples were mixed respectively for protein extraction. Spectra data were acquired by high performance liquid chromatogram-mass spectrometry,and label-free technology was used for relative quantitative analysis. The other 26 urine samples(15 Hp-negative and 11 Hp-positive) were used for validation by full scan. IPA software was employed for bioinformatics analysis. Results:A total of 475 urinary proteins were detected by label-free quantitative analysis and 42 differentially expressed proteins were identified. Finally,11 significantly up-regulated differentially expressed proteins were confirmed by external scanning validation. Bioinformatics analysis revealed the molecular functions,biological pathways,and related diseases of these differentially expressed proteins. Conclusions:These 11 differentially expressed proteins more than 10 kDa identified in urine might be potential biomarkers for diagnosis of Hp infection and provide molecular evidence for the correlation of Hp infection with extragastrointestinal diseases.
8.Observation of intravesical chemotherapy effect in papillary urothelial neoplasm of low malignant potential urinary bladder after transurethral resection

Objective To observe the effect of intravesical chemotherapy in papillary urothelial neoplasm of low malignant potential (PUNLMP) urinary bladder after transurethral resection of bladder tumor (TUR-Bt). Methods 44 cases of PUNLMP confirmed by pathological examination from Jan 2004 to Dec 2013 were retrospectively analyzed. Clinical manifestation: 24 asymptomatic, 19 gross hematuria and 1 abdominal discomfort. All cases were treated by TUR-Bt and immediately administered by intravesical instillation of 40 mg HCPT once, then 32 cases (A group) underwent additional intravesical instillation of HCPT once, and 12 cases (B group) underwent instillation qw for 8 times. Results PUNLMP cases accouted for 2.28 %(44/1 926) of bladder neoplasms admitted during the same period. There were 32 males and 12 females. 39 patients had multiple tumors, and 5 had single. All of cases were followed up with the medial follow-up time of 82 months (16-136 months), only one case in B group recurred after 14 months, and postoperatively without pathological progression. The difference of recurrence rates between two groups had no statistical significance (P=0.273). Conclusions PUNLMP has low incidence rate and is more common among middle-aging males. Doctors should consider PUNLMP when patients with bladder neoplasm are asymptomatically detected. Immediate instillation combined with additional one postoperatively could reduce the recurrence rate of PUNLMP obviously with satisfying prognosis. However, maintenance instillation does not enhance effect.
9.Accuracy of Corvis ST for the measurement of central corneal thickness and intraocular pressure in myopia

Background Corvis ST corneal biomechanical analyzer (Corvis ST) can offer corneal biomechanical parameters,intraocular pressure (IOP) and central corneal thickness (CCT),and measured IOP value was corrected based on CCT and biomechanical factors.Corvis ST is applied abroad,but the study on its accuracy is few in China.Objective This diagnostic trial was to evaluate the accuracy of Corvis ST for CCT and IOP measurement in myopic population.Methods Fifty-six eyes from 56 myopic patients were prospective recruited in Visual Science and Optometry Center of Guangxi from November to December in 2012.IOP was measured by using Corvis ST and Goldmann applanation tonometer (GAT),and CCT was measured by Corvis ST and A type ultrasonic pachymetry.The CCT difference between Corvis ST and A type ultrasonic pachymetry as well as IOP between Corvis ST and GAT were compared by using paired-t test,and agreements of measured outcomes were analyzed by Bland-Altman method.This study was approved by the Ethic Committee of People's Hospital of Guangxi and written informed consent was obtained from all subjects.Results The CCT from Corvis ST was (539.82± 19.79) μm,which was significantly higher than (535.34± 19.41) μm from A type ultrasonic pachymetry (t =4.19,P<0.001).Bland-Altman analysis revealed that the CCT measured by Corvis ST was 4.5 μm higher than that of A type ultrasonic pachymetry,with the 95% limit of agreement ranged from-11.2 to 20.2 μm,and 7.1% (4/56) of points were located at the outside of the 95% confidence interval.The IOP measured by Corvis ST and GAT were (15.75±1.60) mmHg and (16.23 ±2.40) mmHg,respectively,showing statistically significant difference between the two methods (t=2.15,P =0.04).Bland-Altman analysis revealed that the IOP measurement of Corvis ST was 0.5 mmHg lower than that of GAT,with the 95% limit of agreement ranged from-3.8 to 2.8 mmHg,and 3.57% (2/56) of points were located at the outside of the 95% confidence interval.Conclusions CCT obtained by Corvis ST is higher than that by A type ultrasonic pachymetry with poor agreement between these two outcomes,and the two methods cannot replace each other clinically in myopic eyes.IOP value from Corvis ST is slightly lower than that from GAT,showing a good agreement between these two outcomes.The IOP value of Corvis ST shows satisfactory accuracy.
10.An experiment study of the vascular pedicled periosteum transfer for the treatment of femoral head necrosis in young dogs

Objective To explore an effective method for establishing femoral head necrosis models in young dog and to evaluate osteogenesis of vascular pedicled periosteum transplanted in the repairment offemoral head necrosis of young dogs.Methods Animal models of femoral head necrosis were established by liguting and destroying femoral neck artery circles as well as freezing femoral heads with liquid nitrogen in 29 dogs.At 4 weeks, the animal models of femoral head necrosis were evaluated through X-ray, MRI, ECT,and 2 dogs were randomly executed and detectd by histology examination, then 29 dogs were divided randomly into experimental group(n = 12), control group(n = 9 )and blank group(n = 8).Dogs in the experimental group were treated with the vascular pedicled periosteum from isolateral great trochanter, dogs in the control group were treated with the vascular pedicled bone from isolateral great trochanter, and the blank group were without any treatment.4,8, 12 weeks postoperation,these femoral heads were examined morphologically, radiologieally(X-ray, MRI, ECT) and histologically.All animals were executed and detectd by histology at 12 weeks to observe the repairs of necrotic femoral heads at different periods.Results Twelve weeks postoperation, dogs femoral head figurations in the experimental group were normal, MRI signals were odds in femoral head, ECT studies shows decreased radionuclide compared with contralateral femoral head;capillary vessel and osteoblast multiplication and new traboeulacs were matured in histology examination.Dogs femoral head figurations in the contral group were irregular, MRI revealed high-low signals intermix in femoral head, ECT revealed obviously decreased radionuclide, osteoblast actived, new trabeculac and lipocyte were observed in histology examination.In blank group, femoral head figurations were distorted and sunk, while MRI signals were low, ECT showed no radionuclide uptaking.Collapsed trabeculas and many vacant bone lacuna were observed in HE staining.Dogs femoral head figurations in the blank group taken on distortion andsunk, MRI signals were low, without radionuclide revealed uptake, trabeculac collapse, many bone lacuna were vacant.At the same time, radionuclide counts of vascular pedicled periosteums/vascular pedicled bones in ECT were significantly different between experimental group and control group(P < 0.01 ).Conclusion The vascular pedicled periosteum has higher osteogenesis capability compared with the vascular pedicled bone and can effectively repair the femoral head necrosis of young dogs by this means.
