1.Comparison of the effect of gatifloxacin and levofloxacin for treating acute bacterial infection

Objective To compare the effect of gatifloxacin and levofloxacin for treating acute bacterial infection.Methods According to the digital table,228 patients with respiratory or urinary tract infections were randomly divided into group A and group B,114 cases in each group.Group A was given gatifloxacin 200mg,adding 5% glucose injection 100ml intravenous drip injection twice daily; Group B was treated with levofloxacin 200mg,by adding 5% glucose injection 100ml intravenous infusion twice daily.The treatment course lasted 7-14 days.The clincal effect,bacterial clearance rate and adverse reactions of the two groups were observed.Results The cure rate and total effective rate of group A was 68.4%,93.0%,respectively,which of group B was 66.7%,90.3%,respectively,the differences were statistically significant (x2 =0.184,0.213,all P >0.05).The bacterial clearance rate and bacterial drug sensitivity test sensitivity was 89.4%,78.8%,respectively,which of group B was 77.2%,70.3 %,respectively,the differences were all statistically significant (x2 =4.812,4.236,all P < 0.05).The incidence rate of adverse reactions of group A was 7.0%,which of group B was 6.1%,there was no statistically significant difference (x2 =0.672,P >0.05).Conclusion Gatifloxacin has better effect and lower adverse reactions in the treatment of acute bacterial respiratory or urinary tract infections,and there is no significant difference compared with levofloxacin treatment,and it is worthy of clinical application.
2.An experimental study of bone defects repaired with bone morphogenetic protein using polylactic acid as carrier

Objective To study the feasibility of polylactic acid(PLA) as carrier of bone morphogenetic protein(BMP), and observe its results of inducing osteogenesis. Methods A 12 mm bone defect experimental model in the left ulna of rabbits was made, the animals were divided into three groups at random, bovine BMP 10 mg with PLA carrier was implanted into the defect area in the experimental group, bovine BMP 10 mg with bovine cancellous bone matrix carrier was implanted in the control group, and nothing was implanted in the blank group. The osteogenesis of the bone in the defect area was observed by regular roentgenography, the histological changes of the bone defect tissue in 4th, 8th, 12th week after operation were studied and the new bone formation was measured by image analysis. Results The results revealed that the bone defect in the experimental group had excellent repair than that seen in the control and blank group, both bone union and the period of bone mature were earlier than that seen in the control and blank groups. There was fibration in the blank group at 12th week. Conclusion The result indicated that PLA is a material able to be used as a carrier of BMP in inducing osteogenesis, it has a better effect of inducing osteogenesis than xenogenous cancellous bone matrix. Compared with other carriers, PLA has the following advantages: 1)It releases BMP gradually during degradation, so the BMP concentration in bone defect can be maintained effectively. 2)It is a material with some rigidity, so is able to exert flexible fixation of the fractures. 3)It can be shaped to adjust itself for the individual fractures. 4)It has a good histocompatibility and an adjustable property. However, the relationship between the molecule weight, aperture of PLA and the best inducing osteogenesis need a further study.
3.Arthroscopic reconstruction of posterior cruciate ligament with semit endinosus tendon and gracilis tendon

Objective To evaluate the short-term results o f reconstruction of PCL(posterior cruciate liga-ment )with quadruple-stranded semitendi nosus tendon and bi-stranded gracilis tendon and to discuss the key point s of the surgery and postoperative rehab ilitation.Methods 12cases of PCL ruptures were reconstructed arthroscopically with quadruple-stranded semitendi nosus tendon and bidruple-stranded gracilis tendon.Results No patients had symptoms of knee instability.The sa g sign and posterior drawer test were negative in all.According to Lysholm knee function scale,10patients were rated as excellent and 2as good 1year after operation,with an average Lysholm score of 85?4.1.Conclusion Arthroscopic reconstruction of posterior cruciate ligament with quadr uple-stranded semitendinosus tendon and bi-stran ded gracilis tendon can result in lim ited invasion,strong graft and satisfactory clinical results.[
4.Protection by Jiawei Buyang Huanwu Decoction from the Harm of Infrasound (16 Hz,130 dB)in Mice

ObjectiveTo study the effects of Jiawei Buyang Huanwu Decoction on learning and memory abilities of mice exposed to 130 dB infrasound (16 Hz). Methods80 BALB/c mice were divided into blank-control group, infrasound-control group, infrasound and drug-treated group (it was divided into 3 sub-groups according to dose). Learning and memory abilities were measured 14 d later. ResultsCompared with blank-control group, learning and memory abilities decreased (P<0.05) in infrasound-control group. Compared with infrasound-control group, learning and memory abilities increased(P<0.01) in infrasound and drug-treated group. Conclusion16 Hz,130 dB infrasound could damage learning and memory abilities. Jiawei Buyang Huanwu Decoction could protect mice from learning and memory harm exposed to the infrasound.

7.Effect of different cyclic stretching strengths on expressions of phospholipase A2 and cyclooxygenase in human tenocytes in vitro

ObjectiveTo investigate the effect of different cyclic strengths on expressions of phospholipase A2 (PLA2) and cyclooxygenase (COX) in human tenocytes.MethodsHuman tenocytes were uniaxially stretched with different stretching intensity (4%, 8% and 12%) under 0.5 Hz for four hours.Non-stretched tenocytes were applied to the control group.The expressions of cytosolic PLA2(cPLA2), COX1 and COX2 were measured by Western blot and RT-PCR.The secretion of secretory PLA2 (sPLA2) was measured by ELISA.Results The mRNA expressions of cPLA2, COX1 and COX2 in control group, 4%, 8% and 12% stretch groups showed an increase trend.But protein expressions of cPLA2 and COX1 in 4% stretch group were increased insignificantly compared with the control group (P > 0.05).Protein expressions of cPLA2 and COX1 in 8% and 12% stretch groups were increased more significantly compared with the control group (P < 0.01).The COX2 expression in 4%,8% and 12% stretch groups showed statistical difference compared with that in the control group (P <0.01) and the difference increased with stretch intensity.There was no different expression of sPLA2 between 4% stretch group and control group (P = 0.260).However, expression of sPLA2 was increased markedly in 8% and 12% stretch groups (P < 0.01).ConclusionsThe expressions of human tendnocytes PLA2, COX1 and COX2 in vitro are positively correlated with stretch intensity.PLA2/COX system may be a new molecule target in clinical treatment of tendinopathy.
8.Minimally invasive ankle arthrodesis with percutaneous cannulated screws

ObjectiveTo evaluate the clinical results of minimally invasive ankle arthrodesis with percutaneous cannulated screws.MethodsBetween April 2005 and October 2010, 12 patients with the ankle arthrodesis for unilateral severe arthritis (Kellgren-Lawrence class Ⅲ) were prospectively analyzed, including 2 cases of rheumatoid arthritis, 8 cases of post-traumatic arthritis, and 2 cases of osteoarthritis. There were 7 males and 5 females with an average age of 42.0 years(range, 25-7 1). The average disease duration was 7.3 years (range, 1-21). The anterior median incision of 3.0-5.0 cm was made to explore the ankle joint.The cartilage of tibial-talus joint was completely debrided. Two guide pins were inserted from posterosuperior to anteroinferior, and cannulated screws were implanted to fix ankle joint. All patients were physically examined with an extended protocol of questionnaires and the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle and Hindfoot Scales. Ankle fusion in all patients was evaluated by clinical examination, and conventional radiography including anterior-posterior, lateral and mortise views of the ankle. ResultsAll 12 patients were followed up postoperatively for an average of 21.5 months(range, 6-55), and were proved be bone union by clinical examination and radiology with a mean of 13.5 weeks (range, 9-21). The AOFAS rating scale improved from a mean of 42.8±8.6 points at pre-operation to a mean of 66.6±5.4 points at post-operation 6 months,showing significant difference(t=-3.075, P=O.012), and to a mean of 72.3±4.6 at the final follow-up, also showing significant difference with pre-operation (t=-8.595, P=-0.006). There was no infection,local skin necrosis, screw loosing, and so on. ConclusionThe minimally invasive ankle arthrodesis with percutaneous cannulated scews is a recommend procedure, with mini-invasion, short surgery time, high fusion rate, good clinical outcomes, few complications.
9.Impact of sulfentanyl on ropivacaine epidural block during abdominal panhysterectomy

Objective To explore the impact of sulfentanyl on sufentanil epidural block during abdominal panhysterectomy. Methods 90 patients scheduled for panhysterectomy were randomly divided into three groups. Tthe control group received epidural administration of 1% ropivacaine of 0.2 mL/kg after 2% idocaine of 3 mL , while the study group 1 received 10μg sufentanil and the study group 2 received 20μg sufentanil in addition to the medications used in the control group. The anesthetic effect, changes in vital signs, and incidence of adverse reactions were compared among the three groups. Results In group S1 and group S2, the onset of epidural anesthesia was faster , time to the highest plane of sensory blockade and time to degree 3 in the Bromag scores were faster , duration of sensory blockade was longer , and OAA/S score was better , as compared with group D , with significant statistical significances (P<0.01);and the effect was better in group S2 than in group S1. There was no difference among the three groups in adverse reactions. MAP , HR and SPO2 were lower in groups S1 and S2 than in group D during the procedure, with a statistical difference (P<0.05). Conclusions Proper dose of sufentanil plays a positive role in ropivacaine epidural block during panhysterectomy , not only increases the onset of anesthesia, but also makes the anesthestic effect better, and has higher safety It is worth popularizing clinically.
10.Implantation of antibiotic calcium sulfate carriers in two-stage revision for periprosthetic knee joint infection

Objective:To investigate the feasibility of replacing the femoral prosthesis and implanting antibiotic calcium sulfate carriers in a two-stage revision for periprosthetic infection following total knee arthroplasty (TKA).Methods:Between May 2017 and January 2020, 35 patients were admitted to Department of Orthopaedic Surgery, The First Affiliated Hospital to Zhengzhou University for periprosthetic infection after TKA. They were 12 males and 23 females, aged from 49 to 84 years (average, 67.9 years). The two-stage revision for periprosthetic infection was performed for all of them and replacement of femoral prosthesis and implantation of antibiotic calcium sulfate carriers were carried out in stage-one revision. Recorded were postoperative culture of micro-organisms, white blood cell count (WBC), erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) before stage-one and stage-two revisions; the Hospital for Special Surgery (HSS) knee score, range of motion (ROM) and American Knee Society Score (KSS) were compared between preoperation and the last follow-up.Results:Postoperative negative culture was found in 22 cases (62.9%), and positive one in 13 cases (37.1%) of which 4 were caused by Staphylococcus aureus, 2 by Staphylococcus epidermidis, 2 by Candida glabrata, 2 by Candida parapsilosis, one by Candida albicans, one by Mycobacterium tuberculosis and one by Escherichia coli. WBC, ESR and CRP decreased on average from 13.67×10 9/L, 49.71 mm/h and 45.13 mg/L before stage-one revision to 6.44×10 9/L, 18.79 mm/h and 7.82 mg/L before stage-two revision. All patients were followed up for an average of 22.4 months (from 8 to 41 months). At the last follow-up, ROM, HSS and KSS were significantly increased from preoperative 73.2°±15.9°, 59.5±14.6 and 36.1±6.0 to 105.6°±13.2°, 84.3±10.0 and 86.1±5.6, respectively ( P<0.05). None of the patients showed any sign of re-infection at the last follow-up. Conclusion:For patients with periprosthetic infection following total knee arthroplasty, replacing femoral prosthesis and implantation of antibiotic calcium sulfate carriers can well control infection, facilitating recovery of range of motion and function after surgery.
