1.Isoflurane increase renal artial smooth muscle contraction may through phosphorylated ERK1/2 pathway

Aim This study was to investigate the effect of isoflurane on contracted renal arterial smooth muscle and to determine whether the MAPK pathway is involved in the intracellular signaling. Methods ①Rabbit renal arterial strips were treated with sapoin to make the sarcolemma permeability. And the calcium stored in reticulum (SR) was released using caffeine.Stead state tense was obtained when the strips were soaked in submaximal calcium concentration buffer.Then the submaximal calcium concentration buffer with various concentration isoflurane was admininstered,the change of tensions was detected.②primary renal arterial smooth muscle cells were cultured and treated with different concentration of isoflurane for different duration,and the levels of phosphorylated ERK1/2 in different groups were detected with western blot.Result Isoflurane all caused rabbit renal artery strips further contraction after they have reached equilibrium,and the tension altitude was isoflurane concentration dependent. Isoflurane upregulates ERK1/2 (p44/42) phosphorylation in cultured renal arterial smooth muscle cells in a concentration and time depend way.Conclusion Isoflurane can upregulates the tension of contracted renal arterial smooth muscle. And it may involve the activation of p44/42 MAPK pathway.

Transnasal endoscopical biliary catheterization was done successfully in 44 cases,23 cases with complete data were reported.These cases consisted of decompression and drainage of benign and malignant biliary obstruction (12 cases),dissolution and irrigation treatment of remnants of ascaris in the biliary tract (5 cases),dissolution of gallstone (2 cases),irrigation for biliary calculi (2 cases),hemos-tasis after endoscopic sphincterotomy (EST,1 case),and the examination and culture of the bile ( 1 case).The results were satisfactory without complication.Problems concerning EST was discussed,and endoscopic retrograde biliary drainage (ERBD) and percutaneous transhepatic biliary drainage (PTBD) were compared.

BACKGROUND:. Growth factor starts a new epoch of the research and application of wound healing, and the effect of the combination of Chinese tradition medicine and biomedicine in wound healing is still in the discussion.OBJECTIVE: To observe the effect in promoting wound healing of rats with bletilla colloid, a Chinese herbal medicine, as a carrier of exogenous high molecular weight nerve growth factor (HMW-NGF).DESIGN: The single sample observation was adopted in the identification of activities of HMW-NGF; The randomized control experiment with animal as subjects was adopted in the observation of the effect of the HMW-NGF mixed with the bletilla colloid as a carrier in promoting wound healing.SETTING: Department of Orthopaedics, Shenzhen Buji People's Hospital,and Department of Orthopaedics, Tongji Hospital Affiliated to Huazhong University of Science and Technology.MATERIALS: Forty SD rats aged 90-120 days of either gender were employed, with the body mass of 220-280 g; The freeze dried preparation of cattle seminal fluid with HMW-NGF with 1mg each.METHODS: From September 2001 to December 2004, the experiment was carried out in the Department of Orthopaedics in Shenzhen Buji People's Hospital and Central Laboratory of Department of Orthopaedics in Tongji Hospital Affiliated to Huazhong University of Science and Technolcolloid, HMW-NGF, HMW-NGF+ bletilla colloid were added into the serum-free medium (SFM), and the influences of them on the dorsal root lected and divided into 4 groups with 10 rats in each group. The rats of normal control group had no medicine on the wound. In the bletilla colloid group, the HMW-NGF group and the HMW-NGF+bletilla colloid group, incision size 2 cm was made at the back of each rat, and bletilla colloid,HMW-NGF, HMW-NGF+ bletilla colloid were applied respectively on the wound, once per day. The wound area was measured at day 3 and 10 after treatment, and the wound area was calculated by the image analysator. The category and the quantity of the emigrated cells were observed with light microscope, and the healing time of the wound was observed.tion under light microscope.the DRG of chick embryo was cultured, the reaction of evection growth around the ganglion in the HMW-NGF, HMW-NGF+ bletilla colloid with the dilution desity of 1:106-1: 108 was most significant, which was not seen in healing with HMW-NGF + bletilla colloid was significantly increased as compared with that in normal control group, the bletilla colloid group and the The healing time of the wound of the normal control group, the bletilla colloid group and the HMW-NGF group and the HMW-NGF+ bletilla colloid group were (19.5±0.7), (17.3±0.6), ( 16.6±0.7 )and ( 14.9±0.4 ) days respecvation of cut sheets 3 days after treatment: The quantity of polymorphonuclear leucocytes, histoleueocyteand fibroblast in the bletilla colloid group,the HMW-NGF group and the HMW-NGF+ bletilla colloid group were significantly larger than that in the normal control group, and the micrangium could be seen in the HMW-NGF group and the HMW-NGF+ bletilla colloid group. The observation of cut sheets 10 days after treatment: The granulation tissue in the HMW-NGF group and the HMW-NGF+ bletilla colloid group was compacter and had more micrangium as compared with the normal control group and the bletilla colloid group.CONCLUSION: HMW-NGF+ bletilla colloid has significant effect on promoting the healing of the wound at the earlier and later period, and the effect is better than that of bletilla colloid or HMW-NGF.

Objective To explore the extent and severity of cognitive dysfunction in patients with schizophrenia and in order to offer more credible evidence to the treatment of schizophrenia. 5.Long-term clinical observation of anterior allogenic iliac graft in treatment of thoracolumbar fractures

Objective To evaluate the long-term clinical efficacy of iliac bone block allograft in anterior reconstruction of thoracolumbar fractures.Methods A follow-up study was carried out on 93 patients with thoracolumbar fractures treated by anterior decompression and interbody fusion with iliac bone block allograft from February 2004 to March 2007.The radiographic and clinical outcomes were retrospectively assessed.Results Sixty-six patients were followed up for 60-97 months (mean,78 months).The effective interbody fusion rate was 100%,with the fusion time of 6-12 months (mean,10 months).The Cobb' s angle corrected from pre-operative (21.6 ± 8.3)° to post-operative (5.8 ±5.2)°,but when the infusion became effective,the angle showed average loss of 2.4°.All the 66 patients were associated with various degrees of neurological deficiency,but the patients with incomplete nerve dysfunction obtained different degree of recovery.Conclusions Iliac bone block allograft is effective in reconstruction of spinal anterior-middle column stability with the aid of internal fixation instruments.Thereby,it may be a potential alternative to autograft in the treatment of thoracolumbar fractures and can attain early and late stage persistent spinal stability.
6.The use of three kinds internal fixators in improving spinal stability postoperatively

Aim To evaluate the clinical value of two cervical spinallocking plates, Orion and Zephir, and a new anterior cervical instrumentalsystem combining an intradiscal cage with an integrated plate(PCB) afteranterior cervical decompression surgery. Methods 47 patients were followedup for 7 - 26 months with an average of 15 months after surgery. Of them, 30were treated with Orion, 1 1 with Zephir and another 6 with PCB. Results 26cases with preoperative intervertebral space constriction and 18 cases withchanges of cervical physiological protrusion all achieved satisfying effect ofrebuilding intervertebral space height after operation. The cases with obviousintervertebral space height loss were not seen after follow up. The physio-logical curvature corrected by operation was well kept. Neurological functionwas improved in 43 cases on different extent after operation. The other 4cases with no obvious recovery of neurological function were all that withcomplete paraplegia after trauma. Conclusion PCB is suitable for treatmentof the acute cervical protrusion or the cervical spondylotic myelopathy at onelevel. Zephir and Orion are suitable to all kinds of cervical vertebra lesions atmore than one level and complex lesion at one level, which helps the re-covery of cervical physiological curvature and height of disc space.
7.Protective effect of pulsed magnetic field against cerebral ischemia-reperfusion injury in rats

Objective To examine the neuroprotective effect of pulsed magnetic field in a animal focal cere-bral ischemica-reperfusion injury model. Methods Forty-eight Sprague-Dawley (SD) rats were randomized into 3 groups, a sham-operation group, a model group and a pulsed magnetic field group, with rats 16 in each group. Mid-dle cerebral artery occlusion (MCAO) method was employed to establish the focal cerebral ischemia-reperfusion inju-ry model in rats of model group and pulsed magnetic field group. Rats in sham-operation group was subject to the same operation procedure but not underwent ischemia-reperfusion. The infarction volume, histopathological damage and expressions of IGF-1 in ischemic brain tissue were investigated to evaluate the effect of pulsed magnetic field. Results The infarction volume was reduced, histopathological damage alleviated and expressions of IGF-1 in ische-mic brain tissue elevated in the pulsed magnetic field group as compared against the model group (P<0.05). Con-clusions Pulsed magnetic field might provide neuro-protection against cerebral ischemia-reperfusion injury.
8.Different surgical approaches for treatment of fracture and dislocation of the lower cervical spine

Objective To investigate outcomes of different surgical approaches for treating cases of fracture and dislocation of the lower cervical spine.Methods The study involved 26 cases of fracture and dislocation of the lower cervical spine treated surgically from December 2002 to January 2012,including 19 males and 7 females with age ranging from 27 to 62 years (average 39 years).According to the AO classification,there were 12 cases of type B3.1,three of type B3.2,two of type C2.1,three of type C3.1,and six of type C3.2.Preoperative spinal cord function graded by Frankel criteria was six cases of grade A,five of grade B,seven of grade C,six of grade D,and two of grade E.Conventional skull traction was done for all patients before operation.Vertebral cannal decompression and interbody fusion through anterior,posterior or anterior-posterior approaches were determined according to type of fracture dislocation and severity of spinal cord injury.Radiography was performed regularly after operation to review the correction of dislocation,restoration of vertebral height,and interbody fusion.Spinal cord function was also evaluated postoperatively.Results No large blood vessel injury or aggravation of spinal cord injury occurred intraoperatively.There were no complications of incision infection,leakage of cerebrospinal fluid,herniation of bone graft or implant breakage postoperatively.All cases obtained successful correction of fracture and dislocation of the lower cervical spine as well as the recovery of cervical sequence,physiological curvature,and vertebral height in the 12 to 24 months of follow-up (average 16 months).Bony fusion was obtained for all cases at postoperative 3-6 months (average 3.5 months).Spinal function evaluated by Frankel criteria at the latest follow-up showed was grade A in six cases,grade B in three,grade C in five,grade D in five and grade E in seven,with different degree of improvement for all cases.Conclusions Operative approaches should be selected according to the specific status of fracture and dislocation of the lower cervical spine.Anterior approach can be performed for vertebral or intervertebral disc injury straightly and the procedure handles cervical instability immediately.Posterior surgical approach can be used to settle dislocation and interlocking of the articular process directly,but the intervertebral disc injury should be ruled out simultaneously in order to avoid further injury of spinal cord during the reduction process.Combined anterior and posterior surgical approach can be applied to treat fracture and dislocation of lower cervical spine and intervertebral disc injury concurrently but has high risk and large operation wound.
9.Preliminary clinical observation of new allograft posterior lumbar fusion cage of anatomical shape

Objective To explore preliminary clinical results of a new anatomical shape allogeneic bone posterior lumbar fusion cage. Methods Follow-up patients used the allogeneic bone posterior lumbar fusion cage and use imaging methods and clinical score (VAS, ODI) to evaluate the patients′ clinical efficacy. Results 14 patients were followed up for 6 months or more , with an average follow-up time of 9.7 months , mean preoperative VAS 6.8 ± 1.1, ODI 32.7 ± 4.5. The mean preoperative disc height was (9.7 ± 2.0) mm and the average intervertebral height of 3 days post operation was (13.2 ± 1.7) mm. All patients got bony fusion in 6 months post operation, pain and function scores improved significantly compared with the pre-operation: VAS 2.4 ± 0.8 (P =0.000), ODI 9.8 ± 2.5 (P = 0.000), the average intervertebral height was (13.1 ± 1.7) mm (P = 0.000). The average was VAS 2.1 ± 0.1 (P = 0.000), average ODI was 8.9 ± 0.9 (P = 0.000) at last follow up. Average of intervertebral height was (13.0 ± 1.8) mm, no significant difference compared to three days after surgery (P=0.831). No serious complications and deep surgical site infection was observed. All implants were found no fragmentation, shift, cutting boards, and obviously sinking. Conclusion The anatomical shape allogeneic bone posterior lumbar fusion device is suitable for posterior lumbar interbody fusion , advantages of high fusion rate , satisfactory clinical results in the initial clinical trials , but its long-term efficacy requires further observation.
10.Efficacy of pulsed dual-wavelength 595 and 1064 nm laser in treatment of ulcerated infantile hemangioma

Objective To observe the efficacy of pulsed dual-wavelength 595 and 1064 nm laser in treating ulcerated infantile hemangioma.Methods Lesions from 40 patients were treated with the laser.The distribution of lesion sites was in buttuck and perianal area (13 patients),vulvar lips (7),groin and inside thigh (5),scrotum (4),neck (3),scalp (2),lip (2),trunk and limbs (4).All lesions including ulcer and non-ulcer were conducted with 595 nm plused dye leaser at the first therapy.Later,595 nm pulsed dye leaser was continued or performed with Group 1 or Group 2 parameter of 595 and 1064 nm multiplex mode until the treatment was finished according to the thickness of the lesion.The frequency of treatment was one to six times.The interval of the treatment was four weeks.Results The ulcer from 39 lesions healed gradually in 1 to 2 weeks (average 9.5 days) after the first treatment.These ulcer lesions were healed four weeks after the first time of therapy.The recovery rate of ulcerated lesion was 97.5 %.For the whole lesion,after 1 to 6 treatments,13 lesions showed excellent result,19 lesions showed good result,and the effective rate was 80 %.Pain relieved 2 to 7days (average 3.5 days) after the first laser therapy in 35 patients who accompanied with pain.Pain score decreased from 1.875 to 0.125 before and after treatment,with significant difference(P<0.05).The tolerance of the treatment was good and no side effect was observed.Conclusions 595and 1064 nm dual-wavelength laser is a safe and effective tool for treating ulcerated infantile hemangioma.
