ObjectiveTo evaluate the clinical efficacy and safty of combined treatment telmisartan plus simvastatin on early diabetic nephropathy. Methods46 patients with early diabetic nephropathy were randomly divided into two groups. Control group ( n ＝ 23 ) received regular insulin therapy, captopril l0mg tid. Study group received telmisartan 80mg qd plus simvastatin 20mg qd on the basis of control group therapy. All patients received 16 weeks treatment. The clinical parameters were measured before and after therapy. ResultsAfter 16 weeks treatment,the urine MAU in both study group and control group were significantly decreased( all P ＜0. 01 ) ,however,the urine MAU in telmisartan plus simvastatin combination group decreased significantly than control group( P ＜ 0. 01 ). Improvement of lipid level in the study group better than the control group( P ＜0. 01 ). Kidney function,fasting blood glucose levels and HbAlc had no significant changes before and after treatment. ConclusionTelmisartan plus simvastatin combined treatment was better than the traditional therapy,which can delay the development of early diabetic nephropathy.

Objective To evaluate the expression of endothelial Per-ARNT-Sim domain protein-1(EPAS-1)/hypoxia-inducible factor-2?(HIF-2?) and its relationship with vascular endothelial growth factor (VEGF) in bladder transitional cell carcinoma (BTCC). Methods The expression of EPAS-1/HIF-2? and VEGF by immunohistochemical staining was examined in 60 cases of BTCC (Grade Ⅰ in 28 cases,Grade Ⅱ in 12,Grade Ⅲ in 20) and 8 subjects with normal urothelium. Of the 60 cases 29 had superficial (≤pT 1) bladder cancer and 31 had invasive (≥pT 2 bladder cancer).?2 test was used to assess the relationship between EPAS-1/HIF-2? and VEGF expression versus tumor grade and stage. Results The expressions of EPAS-1/HIF-2? and VEGF were negative in all normal bladder tissue but high in BTCC.Of the 60 cases,34 (56.6%) were positive and 26 ( 43.4%) were negative for EPAS1/HIF-2?.Four cases (14.3%) of Grade Ⅰ,11 (91.7%) of Grade Ⅱ and 19 (95.0%) of Grade Ⅲ were positive for EPAS-1/HIF-2?. Objective To identify the factors related to the influence of medical research papers. Methods The influence of 1146 research papers by the hospital in the period 1996-2003 and its relationship with such factors as the backgrounds of the authors and the quality of the papers were studied u-sing regression analysis. Results Nine factors, including the impact factor, the status of the paper (being or not being one based on fund-supported research or representing research achievements), and the status of the author (being or not being chosen for talent cultivation and undertaking or not undertaking research projects), were significantly correlated with the influence of the papers. Conclusion In order to enhance the influence of medical research papers, it is necessary for research' management departments not only to encourage research personnel to publish papers in journals with high impact factors but also to step up efforts to better personnel cultivation, degree education, and project and achievement management so as to set up a quality and distinguished research contingent.

Objective To study the chemical constituents in the seeds of Zizyphus jujuba var.spinosa.Methods The compounds were isolated by silica gel column chromatography and their structures were established by spectroscopic methods.Results A new keto-dammarane type saponin,jujuboside H(Ⅰ),along with three known compounds protojujuboside A(Ⅱ),spinosin(Ⅲ),and betulic acid(Ⅳ)were isolated.Conclusion Compound Ⅰ is a new compound named jujuboside H.

Objective To evaluate clinical effects of laparoscopy in acute abdominal emergency. Methods A retrospective analysis was made on clinical date of 146 cases of acute abdominal emergency from January 2001 to August 2005,including 86 cases of acute cholecystitis,45 cases of acute appendicitis,6 cases of closed abdominal trauma,5 cases of perforation of gastroduodenal ulcer,3 cases of pyosalpinx,and 1 case of adhesive intestinal obstruction. Results Out of the 146 cases,134 cases(91.8%) were successfully diagnosed and treated under laparcoscope,including 7 cases which could not be confirmatively diagnosed preoperatively.Conversions to open surgery were required in 12 cases(8.2%).No complications or fatal cases were noted.(Conclusions Laparoscopic) technique is safe,reliable,and micro-invasive in the diagnosis and treatment of acute abdominal emergency.
