1.Investigation of psychological health in middle-school students of Zhuang nationality

BACKGROUND: The middle-school stage is the key period of personality figuring and taking shape.The mentation in this period is filled with a lot of special psychological contradictions and conflicts. Long-term psychological worry will affect the formation of perfect personality.OBJECTIVE: To understand the psychological health status in the middle-school students of Zhuang nationality and the main factors of influence.DESIGN: Randomized cluster sampling and questionnaire investigation based on middle-school students.SETTING: Department of Preventive Medicine, Youjiang Medical College of Nationalities.PARTICIPANTS: 2 000 middle-school students of 34 classes from 3 middle schools in Baise City and Longlin County were selected to attend the investigation with randomized cluster sampling method in November 2003.METHODS: The psychological health statuses of middle-school students were evaluated with symptom checklist (SCL).The list contained 90 items,including 9 factors of psychological symptom such as somatization,compulsion,sensitivity to human relations,anxiety,depression,hostility,dread,paranoia and psychosis.The grading system from 1 to 5 grades was used and the standard of the adult norm of the whole country was refered to:Factor score<1.8 was normal;Factor score from 1.8 to 2.0 was mild abnormality;Factor score from 2.1 to 3.0 was moderate abnormality;Factor score>3.0 was severe abnormality. Instructive words were unified.The investigative method and behavior were normalized.The questionnaires were filled in without recording the names and taken back from the spot and then compared with the norm of the whole country.MAIN OUTCOME MEASURES: The evaluated result of psychological health in middle-school students.RESULTS: 2 000 questionnaires were provided and 1 855 were valid after return with the effective rate 92.75%. ①The average scores of each factor of SCL were all higher in the middle-school students of Zhuang nationality than in the norm of the whole country. The significance of difference existed(P=0.000).②The scores of somatization,depression,anxiety,dread and psychosis were all higher in the schoolgirls than in the schoolboys.The significance of difference existed (P=0.000).③Except hostility factor,the scores of other factors were higher in the senior middle-school students than in the junior middle-school students.The difference was statistically significant(P=0.000).④The arranged sequences(from bigness to smallness) of each factor that affected psychological health of middle-school students of Zhuang nationality in Baise were depression,somatization,compulsion,anxiety,sensitivity to human relations,psychosis,paranoia,hostility and dread.The sensitivity to human relations ranked 4th and the anxiety ranked 6th in the schoolboys,but the orders of the 2 factors were just the opposite in the schoolgirls and the orders of other factors were the same in both the schoolboys and the schoolgirls.The first 3 factors all were:depression,somatization, compulsion.CONCLUNSION: The psychological health status was lower in the middle-school students of Zhuang nationality than in the norm of the whole country and the first 3 factors that affected their psychological health were:depression,somatization and compulsion. The levels of psychological health on somatization,depression,anxiety,dread and psychosis were lower in the schoolgirls than in the schoolboys. The psychological problems were more serious in the senior middle-school students than in the junior middleschool students. The levels of psychological health in middle-school students of Zhuang nationality were relatively lower and the psychological education and intervention study should be provided aiming at psychological characteristics of different population.
2.The clinico-pathological characteristics of nodular goiter with lymphocytes infiltration

Objective To investigate the clinico-pathological characteristics of nodular goiter (NG) with lymphocytes infiltration and its relationship with Hashimoto′s disease. Methods Clinical and histologic data were collected and reviewed for 1121 cases of nodular goiter and 55 cases of thyroid adenoma, among which 83 cases had lymphocytes infiltration. These 83 cases of NG with lymphocytes infiltration were observed histologically in detail, followed up and assessed for the correlation of thyroid function to histologic findings. Results The overall incidence of lymphocytic infiltration in non-toxic nodular goiter was 7.4% (83/1121). Most of them were women of middle age. All had lymphocytes infiltration, follicule atrophy and oxiphilic epithelia in the background of nodular goiter. Seventy-six percent underwent cystic degeneration or had fibrous scar. Nineteen percent with lymphocytes infiltration in NG developed post-operative hypothyroidism. Six cases (7%) had high levels of thyroid auto-antibodies. None of the 55 cases of thyroid adenoma had overt lymphocytes infiltration. Conclusion Nodular goiter with lymphocytes infiltration and Hashimoto′s disease shares similar morphology and clinical presentation in some degree. It seems that NG with lymphocytes infiltration is a kind of inflammatory hyperplasia relating to autoimmunity and perhaps the beginning of some cases of Hashimoto′s disease.
3.Tumors with poroid features: a clinicopathologic analysis of 45 cases.

OBJECTIVETo investigate the clinicopathological features and the differential diagnosis of poroma and porocarcinoma.
METHODSHistopathological characteristics and clinical data of 35 cases of poroma and 10 cases of porocarcinoma were analyzed retrospectively.
RESULTSThe average age of 35 patients of poroma was 48 years. The average age of 10 patients of porocarcinoma was 65 years. Both poroma and porocarcinoma occured most frequently on the scalp and face,as well as the extremities. Histologically, cases of poroma were divided into three subtypes, including classic poroma (23 cases), hidroacanthoma simplex (3 cases) and dermal duct tumor (9 cases). Residual foci of benign poroma were found in all cases of porocarcinoma, most of which were classic poroma. The malignant components showed severe dysplasia and/or stromal infiltration.
CONCLUSIONThe diagnosis of poroma and porocarcinoma is mainly based on the microscopic characteristics. An invasive architectural pattern and/or significant cytologic pleomorphism are the most important clues for the diagnosis of porocarcinoma. Neither focal mitotic activity nor the presence of necrosis was the diagnostic feature of porocarcinoma. Malignant transformation can occur in some cases of long existing poroma with recent, rapid tumor enlargement.

4.Effect of Integrative Nursing Intervention on Chronic Heart Failure of Elderly Patients

ObjectiveTo investigate the effect of integrative nursing intervention on chronic heart failure of elderly patients. Methods80 elderly patients with chronic heart failure were divided into integrative nursing intervention group (n=40) and the control group (n=40). All the patients were treated with routine treatment and nursing. The patients of integrative nursing intervention group were given prescient nursing, mental nursing and exercise rehabilitation. ResultsThe total effective incidence of integrative nursing intervention group was more than that of the control group (P<0.05), and the 6 minutes walk test was longer (P<0.05).ConclusionIntegrative nursing intervention is benefit to the recovery of the elderly patients with chronic heart failure.
5.Clinicopathologic analysis of spiradenoma: 25 case reports

Objective To analyze clinicopathologic features and malignant transformation of spiradenoma (SA).Methods Clinicopathologic data on 25 cases of SA were retrospectively analyzed.An immunohistochemical study was conducted on 5 of the 25 patients.Clinicopathologic features of SA were summarized based on clinicopathologic data,immunohistochemical results and relevant literature.Results The average age of these patients was 40 years.Lesions were acquired and solitary in 24 cases (96%,24/25),and occurred most frequently in the extremities.The diameter of tumors was less than 2 cm in 20 cases (87%,20/23).Twenty-three cases were benign SA with various histological manifestations,including 15 cases (65%,15/23) of typical SA,8 cases (35%,8/23) of cylindroma.VarTing numbers of mitotic figures were observed in different cases.The immunohistochemical study showed epithelial and myoepithelial differentiation.Two cases of SA experienced malignant transformation into poorlydifferentiated basal cell adenocarcinoma and well-differentiated adenocarcinoma respectively.Lymphocytes decreased significantly in number or disappeared in the malignant area,and both the malignant tumors exceeded 2 cm in diameter.Conclusions SA is a kind of neoplasm with distinct histological characteristics and nonspecific clinical manifestations.Most SA cases have a benign clinical course,but malignant transformation should be considered in some cases with longterm clinical course and large size.
6.Isolation and culture of rat limbal stem cells and research on trans differation capacity

Objective To isolate and culture the rat limbal stem cells ( LSCs) in vitro,and investigate briefly their capacity for transdif-ferentiation into neural stem cells ( NSCs) with cytokine EGF,bFGF and RA.Methods LSCs derived from rats were cultured and identified by immunohistochemistry in vitro.LSCs were induced to differentiate into NSCs in the presence of EGF (20 ng/mL) ,bFGF (10 ng/mL) and with or without of RA(group 1 or group 2)(25 ng/mL) for seven days.Cultures without factors were used as control group.Then the neural marker Nestin of the coultured cells were measured by immunohistology staining.Furthermore,the positive cell rate was counted under micro-scope between the 2 groups and analyzed by statistical software.Results It showed that P63 was positive in LSCs.Nestin in both of the dif-ferentiation groups was positive at the rate of (77.01 ±6.32)%and (84.01 ±5.43)%respectively,of which the second group was higher than the first one (P<0.05).However,it was negative in the control group.A band of Nestin protein from cells was detected by western blot assay.Conclusion LSCs are successfully isolated and cultured in vitro.LSCs could be induced to differentiate into NSCs in the presence of EGF and bFGF.Moreover,the differentiation capability is enhanced in the condition of RA.

9.Factors influencing success of external cephalic version and their clinical significance

Objective:To analyze the factors influencing the success rate of external cephalic version (ECV) and to create a preoperative scoring scale for stratified management of pregnant women who were preparing for ECV.Methods:This prospective study was conducted on singleton pregnant women who underwent ECV without anesthesia in Fujian Maternity and Child Health Hospital from January 1, 2017, to December 31, 2019. Univariate (two independent samples t-test, Mann-Whitney U test, and Chi-square test) and multivariate logistic regression were used to screen the clinical characteristics affecting the success of ECV, and receiver operating characteristic (ROC) curve was used to determine the cut-off value and convert quantitative variables into dichotomous variables. The independent variables were scored according to the regression coefficient in multivariate logistic regression analysis, and then a preoperative scoring scale was created. The ROC curve was used to calculate the cut-off value for the scoring scale. The subjects were divided into low and high score groups according to the cut-off value. The area under the ROC curve was used for evaluating the effectiveness of the scale in predicting the success of ECV. The success rate of ECV, difficulty of the operation and mode of delivery were compared between the two groups. Results:A total of 1 338 pregnant women met the inclusion criteria during the study period. After the exclusion of 885 women, 165 refused ECV in favor of direct cesarean section, 27 spontaneously converted to cephalic position before ECV, 261 who voluntarily accepted ECV were finally enrolled. ECV succeeded in 202 cases and failed in 59. (1) Favorable factors for ECV without anesthesia were the distance between the fetal breech and ischial spine <-3.5 cm ( OR=0.177, 95% CI: 0.071-0.438, P=0.009), the sum of the fundal height and the station of the fetal breech based on the ischial spine <30.25 cm ( OR=0.225, 95% CI: 0.094-0.537, P=0.001), amniotic fluid index ≥12 cm ( OR=0.399, 95% CI: 0.164-0.969, P=0.042), the surgeon's ability to hold the fetal head or breech with one hand ( OR=0.241, 95% CI: 0.098-0.589, P=0.002; OR=0.219, 95% CI: 0.087-0.546, P=0.001), and the fetal head located on the right or left upper abdomen of the mother ( OR=0.184, 95% CI: 0.059-0.568, P=0.003; OR=0.253, 95% CI: 0.084-0.760, P=0.014). (2) The area under the ROC curve of the preoperative score for predicting the success of ECV was 0.881 (95% CI: 0.821-0.941) and the cut-off value was 5.5. The subjects were divided into low (0-5 scores) and high (6-11 scores) score groups and the area under the ROC curve for predicting the success of ECV by grouping was 0.843 (95% CI: 0.774-0.912). Compared with the low score group, the high score group had a shorter ECV duration [2.0 min (0.5-10.0 min) vs 10.0 min (0.9-25.8 min), Z=-6.83, P<0.001], less attempts [1.0 times (1.0-4.0 times) vs 3.0 times (1.0-5.0 times), Z=-8.41, P<0.001], higher success rate [92.7% (190/205) vs 21.4% (12/56), χ2=127.64, P<0.001], higher rate of vaginal birth [75.4% (147/195) vs 18.5% (10/54)] and lower cesarean section rate [24.6% (48/195) vs 81.5% (44/54)] ( χ2=58.70, P<0.001). Conclusions:Preoperative scoring based on the factors influencing the success rate of ECV (the distance between the fetal breech and ischial spine, the sum of the fundal height and the station of the fetal breech based on the ischial spine <30.25 cm, amniotic fluid index ≥12 cm, the surgeon's ability to hold the fetal head or breech with one hand, and the fetal head locating on the right or left upper abdomen of the mother) is conducive to the individualized evaluation of the difficulty and the success rate of ECV as well as the success rate of vaginal delivery after ECV, which can provide a reference for clinical stratified management of ECV patients.
10.Effect of Huashi Runzao Prescription on Structure and Function of Submandibular Gland in Sjögren's Syndrome Model Mice

ObjectiveTo investigate the effects of Huashi Runzao prescription (HRP) on the histopathological injury and function of submandibular gland in naive non-obese diabetic (NOD/Ltj) mouse model of Sjögren's syndrome (SS) and its regulatory effect on aquaporin 5 (AQP5) expression in submandibular gland cells. MethodThe SS model was induced in NOD/Ltj mice. The NOD/Ltj female mice aged nine weeks were selected and randomly assigned into model group,HRP group (7.15 g·kg-1·d-1),and hydroxychloroquine (HCQ) group (1.30 g·kg-1·d-1), and female BALB/c mice in the same age were selected and assigned into the normal group, with six mice in each group. Drug intervention lasted eight weeks. The water consumption and salivary flow rate (SFR) of each group were recorded. The pathological staining results of the submandibular gland of mice in each group were observed and scored. AQP5 expression was determined by immunohistochemistry (IHC) and Western blot. ResultCompared with the normal group, the model group showed increased water consumption (P<0.05) and reduced SFR (P<0.05). Compared with the model group, the HRP group showed decreased water consumption (P<0.05) and increased SFR (P<0.05), and the HCQ group showed increased SFR (P<0.05). In terms of histopathological results of the submandibular gland,compared with the normal group,the model group showed increased pathological score, number of lymphocyte infiltration foci,and percentage of lymphatic infiltration area (P<0.05). Compared with the model group, the HRP group showed reduced pathological scores and number of lymphocyte infiltration foci (P<0.05), and the HRP group and the HCQ group showed reduced percentage of lymphatic infiltration area(P<0.05). The results of IHC and Western blot showed that compared with the normal group,the model group showed down-regulated expression level of AQP5 protein (P<0.05), and compared with the model group and the HCQ group,the HRP group showed up-regulated expression level of AQP5 protein (P<0.05). ConclusionHRP can improve the secretion function of submandibular gland acinous cells and glandular structure injury in SS model mice, and its mechanism may be related to the up-regulation of AQP5 protein expression level in submandibular gland cells.
