1.The Evaluation of the Quality of Life of Prelingually Deaf Adolescents with Cochlear Implants

Objective To evaluate the family and individual quality of life for the prelingually deaf adolescents with cochlear implants and compare the influence of preoperative communication mode.Methods A retrospective study was conducted in 21 prelingually profoundly deaf adolescents divided into the nonverbal group and oral group according to preoperative communication modes.The scores of the children using hearing implants quality of life (CuHI Qol) and nijmegen cochlear implant questionnaire (NCIQ) were collected and compared statistically between the two groups.Results The postoperative scores of CuHI Qol (51.71±8.9) in 21 patients were significantly higher than those before operation (42.14±6.04,P<0.001), and no significant differences were observed in the family influence between the two groups while the scores of oral group were significantly higher than the nonverbal group in parents' expectation and quality of life(P<0.05).The NCIQ scores of the oral group were significantly higher than nonverbal group(P<0.05) in the following subdomains : advanced sound perception, speech production, social interactions and there was no statistical difference in basic sound perception, self-esteem and activities.Conclusion The quality of life of prelingually deafened adolescents with cochlear implants can be improved obviously and patients with oral communication mode before operation have better quality of life.
2.The Significance of Changes in Plasma Calcitonin Gene-related Peptide and Endothelin in Coronary Syndrome

AIM To investigate the significance of CGRP and ET in the coronary syndrome and the significance of their changes in the incidence of the acute coronary syndrome.Methods Group with UA group ( n =30), AMI group ( n =28), control group ( n =25) were established.In control group blood sample take on an empty stomach in the morning ,while group with AMI on hospitalization and after one week, group with UA at angina pectoris attack and after remission. The concentration of plasm CGRP and ET were measured .Results The plasm CGRP concentration in group with AMI was higher than that in group with UA and that in control group ,the plasm ET elevated markedly, while the rate of CGRP/ET decreased; Compared with that in the remission period of the acute coronary syndrome ,The plasm CGRP elevated, the plasm ET elevated markedly, while the rate of CGRP/ET decreased.Among defferent acute coronary syndrome ,the plasm CRGP and ET concentrantion was highest in AMI group,higher in group with accelerated UA, high in group with initial attack UA;the rate of CGRP/ET was highest in group with initial UA,higher in group with accelerated UA,high in group with AMI.Conclusions The changes of plasm CGRP and ET concentration are well correlated with the extent of coronary lesion, the level of plasm CGRP and ET and the imbanlance of thier receptor play an important role in the pathogenesis and development of acute coronary syndrome. 3.The Effects of Cochlear Implantation on Children with Large vestibular Aqueduct Syndrome

Objective The purpose of this study is to compare the most comfortable levels and neural re-sponse imaging threshold between children of cochlear implants with large vestibular aqueduct syndrome and normal inner ears. Methods Thirty-eight implanted children participated in this study and were divided into two groups. Group A consisted of 32 patients with normal inner ears and group B 6 patients with radiographically proven large vestibular aqueduct syndrome. All of the patients were the recipients of the cochlear implants by Advanced Bionics. The initial time for the first programming session was approximately one month after surgery. The psychophysical tests included M- level tests. Programming techniques used in the test were suitable for the age of patients. Sound-Wave 1.4 software was used to test tNRI. Results Using the rank sum test, M-level and tNRI of electrode 3,7,11 and 15 did not differ significantly between group A and B(P>0. 05). Conclusion The parameters of mapping are not significantly different between the implanted children with large vestibular aqueduct syndrome and normal inner ear. The management and procedures of mapping used to the subjects with normal inner ear can be applied to the children with large vestibular aqueduct syndrome.
4.Effects of IL-1 ? on the Biological Activity of Human Periodontal Ligament Cells in Vitro

Objective To explore the effects of interleukin- 1?(IL-1?)on the biological activity of human periodontal ligament(PDL) cells in vitro. Methods Human PDL cells were cultured in DMEM medium containing IL-1?(0.1,0.5,1,5 and 10ng/ml) for 1,2,3,4 and 5 days, respectively. The proliferation of PDL cells was measured by MTT assay at the first, second, third, fourth and fifth days after IL-1? treatment, respectively. Fibronectin level in the medium was determined by ELISA at the fourth days after 10ng/ml IL-1? treatment, and alkaline phosphatase(ALP) activity was measured by enzyme kinetic method. Retrospective analysis of two cases of advanced carcinoma of the middle ear. Two patients underwent operation and radiotherapy. A case developed extensive necrosis in ear and neck, which finally led to lethal hemorrhage. Multiple relapse with cranial fossa invasion and extensive necrosis was found in the other case.

6.Multislice spiral CT virtual endoscopy: the clinical application in the diagnosis of vascular diseases

Objective To evaluate the display ability and the clinical application of multislice spiral CT virtual endoscopy (CTVE) in the diagnosis of vascular diseases. Methods Fifty-six patients were examined by CT virtual endoscopy.32 patients also had DSA for comparison.18 patients were confirmed by operation (3 patients also had DSA).9 patients with pulmonary embolism were confirmed by other examinations and treatments.Results 56 patients showed successful CT virtual endoscopy images.CT virtual endoscopy images could clearly show the inner wall and inner structures of the vessels,mural thrombus,calcification form,stenosis of the vessels,the true and false lumina of aortic dissections,the lesion of false aneurysm,and the anatomic relationship between the lesion and surrounding structures of the normal vessels.Out of the 32 patients with vessel abnormalities on DSA,24 patients (75%) showed the lesions on CTVE.17 out of 18 patients with operation showed the lesions on CTVE.All 9 patients with pulmonary embolism showed the abnormalities on CTVE.Chi-square test:? 2=5.2809,P=0.071 patients showed the vascular diseases on CTVE in 56 patients (84%).Conclusion MSCT virtual endoscopy could provide more information of the vessel by displaying the inner wall and allowing users to map the vessels in a display.It is helpful in correct diagnosis and guiding surgery.
7.Coronary artery stenosis: comparison study of CT angiography with 16-detector rows scanner and convention angiography

Objective To evaluate the accuracy of 16-detector row computed tomography coronary angiography in detection of hemodynamically relevant stenosis(≥50%) of the coronary artery. Methods Twenty-six patients undergoing both multi-detector row CT and conventional coronary angiography were included in this retrospective study. CT scan were obtained with collimation of 16 and rotation time of 500 ms. Retrospective electrocardiographic(ECG) gating was used for image reconstrution. The stenosis degree was detected with axial scanning and three-dimensional reformation. Results With the results of conventional coronary angiography as golden standard, the sensitivity, specificity, accuracy, positive predicted value and negative predicted value of the CT angiography in detection of hemodynamically relevant stenosis(≥50%) were 90.0%, 95.5%, 94.7%, 77.6%, and 98.2% respectively. Conclusion Sixteen-detector row computed tomography coronary angiography has high clinical value with evaluating hemodynamically relevant stenosis(≥50%) of coronary artery,and could be a significant guide for diagnosis and therapy of coronary heart disease.
8.Clinical Value of HRCT in Staging Lung Interstitial Disease in Connective Tissue Diseases

Objective To study the clinical value of HRCT in staging of lung interstitial disease(LID) in connective tissue disease(CTD).Methods 222 patients with CTD confirmed clinically underwent HRCT scan.The staging of LID according to the HRCT features of LID were done and the therapeutic effect was compared between each stage.Results In 222 cases ,64 cases were negative on HRCT as stage 0,158 cases had disseminated LID in different degree,including stage Ⅰ in 107,stage Ⅱ in 36 and stage Ⅲ in 15.There were significant statistically between the therapeutic effect and staging of LID.Conclusion The staging of CTD with LID by HRCT is helpful for judging the extent of LID and clinical treatment.
9.Evaluation of the coronary collateral circulation with 64-sUce spiral CT: a comparative study with coronary angiography

Objective To evaluate the ability of 64-slice spiral CT(64-MSCT) in demonstrating the coronary collateral circulation(CCC).Methods Patients undergoing 64- MSCT coronary angiography with relatively good image quality from August 2005 to April 2007 were entered into the study according to following requirements: patients underwent a traditional coronary artery angiography (CAG) after the coronary CT imaging; the CAG showed there was stenosis greater than 90% in at least one major coronary artery.The CCCs were assessed in CAG by the Rentrop grading, and the CAG served as the gold standard to determine the accuracy of 64-MSCT angiography in detecting collateral circulations.All CCCs were grouped angiographieally into three groups of CC0, CC1 ,and CC2 by Werner collateral collection(CC) grading.In the 64-MSCT coronary angiography imaging, a new grading(MSCT-CC) was used to evaluate CCCs.The consistency between these two grading standards was analyzed by Kappa test.Resalts A total of 97 patients were entered into the study, among whom at least one CCC was found by CAG in 37 patients and at least one CCC was detected by 64-MSCT angiography.Taken the CAG results as the reference standard, the sensitivity and specificity of the 64-MSCT angiography in detecting patients with the CCCs were 89.2% (33/37) and 93.8% (60/64) respectively.The sensitivity of the 64-MSCT angiography detecting the CCCs was 75.8% (47/62). All the CCCs detected by CAG and 64-MSCT angiography were grouped into three groups according to Werner grading: 4 in CC0 group, 24 in CC1 group and 19 in CC2 group.The CCCs were grouped into three groups according to MSCT-CC: 7 in MSCT-CC0 group, 22 in MSCT-CC1 group and 18 in MSCT-CC3 group.The consistency of the two grading criteria was relatively high (Kappa = 0.857, P < 0.01).Conclusion 64-MSCT angiography can accurately evaluate the coronary collateral circulation.
10.Clinical Application in Diagnosis of Pulmonary Embolism by Multiple Spiral CT Angiography

Objective To evaluate the effectiveness of multiple spiral CT angiography in diagnosis of pulmonary embolism.Methods 16 patients clinically suspected of pulmonary embolism underwent multiple spiral CT angiography.Emboli were found in the 8 patients of all on multiple spiral CT angiography.5 of the patients were reexamined with this method 20 to 30 days after thrombolysis. Results In 8 patients with pulmonary embolism,43 vessels were involved with pulmonary embolism.The presence of pulmonary embolism on CT was seen as a total occlusion,partial filling defect,mural filling defect et al;dilated of pulmonary artery trunk;all signs with deceleration of pulmonary blood or “mosaic sign”. 5 patients were reexamined with this method.The emboli dissoved partly or entirely.Conclusion Multiple spiral CT angiography is a noninvasive and effective method for examining pulmonary embolism.
