1.Research advances in the management of war wound/trauma and its perspective

Objective To propose the orientation,goals and some key issues for future research in the field of war wound/trauma through reviewing the updated literature.Methods The latest literature in the field of war wound/trauma were reviewed.Results The notion "medicine always be with soldiers" had been raised by America and the Europe military circles,with special emphasis on emergency techniques and equipments,which were used in the front of battle field.This forwarded the medical aids as quick as possible and the formation of medical rescue system without crack.Meanwhile,much attention had been paid to the pathogenesis and prevention of post-traumatic complications,wounding characteristics and mechanisms of high-tech new weapons,as well as repair and regeneration of injured tissues.In China,a great advances had been made in the filed of emergency rescue,shock resuscitation,complication treatment and wound repair.Conclusions During the coming "twelfth five-year program",the research standpoint is to meet the needs of Chinese troops to perform better in modern wars and non-military actions,and the aim is to significantly reduce the rate of mortality and disability of trauma as well as to improve the whole army's health.To fulfill this aim,a lot of work should be done to enhance the trauma emergency ability at the battle front,the organization and remedy ability at conditions of combined operations of armed forces,and the translational medical research for rescue of trauma,so as to unceasingly increase the medical service ability of our army to cope with secure threaten and to accomplish multiple military missions.
2.Stress Disorder Assessing with the MMPI- PK Scale

Objective: To investigate psychopathology and diagnostic efficiency with MMPI- PK scale for patients with stress disorder(SD). Methods: MMPI- 2s were administered to forty- three in patients with SD and forty- four well- adjusted trauma survivors as controls. Results: Patients with SD obtained significantly higher PK scores than controls. Gender, types of traumatic events and types of diagnoses for patients with SD did not substantially influence PK scores. When an optimal cut- off point of 17 was utilized, sensitivity and specificity obtained by PK assessment are respectively 94.9% and 66.7%. Conclusion: Patients with SD and trauma survivors show significantly different psychopathology. PK scale was sen- sitive to the assessment of posttraumatic stress symptoms though it produced more false- positive than false- negative errors.
3.The relationship between polymorphism in XRCC1 and TNM staging and lymph node metastasis of esophageal squamous cell carcinoma

Objective To assess the relationship between polymorphism of XRCC1 Arg194Trp Arg399Gln and ESCC TNM stage and regional lymph node metastasis. Methods PCR and PCR-RFLP were used to detect polymorphism of XRCC1 Arg194Trp and Arg399Gin. The results were analyzed and compared to TNM staging and regional lymph node metastasis. Results The XRCC1 194Arg/Trp and 399Arg/Arg wildtype in ESCC with metastasis is 32.3%, which is higher than 194Arg/Arg (20.0%, P=0.01). There was no remarkable difference between IIb-IV stage ESCC with lymph node metastasis and 0-IIa stage without metastasis. Conclusion The ESCC patients with 194 variability and 399 wildtype had high metastasis rate, XRCC1 Arg194Trp and Arg399Gln had relationship with ESCC TNM stage and lymph node metastasis. Both of them affected to prognosis of ESCC.
4.Experience in the surgical treatment of 16 cases of cardiac rupture caused by blunt chest trauma

ObjectiveTo retrospectively analyze the experience in the treatment of cardiac rupture caused by blunt chest trauma and to explore the approaches and methods to improve patient survival.Methods From 2004 to 2010,16 cases of closed chest trauma caused by cardiac rupture,who were treated in our hospital,were treated by heart repair using median sternotomy incision approach.Results Except for 1 patient occurred cardiac arrest before the establishment of CPB,who was failed for cardiopulmonary resuscitation after surgical repair,the remaining 15 patients were all successfully treated.Conclusion The diagnosis of cardiac rupture should be made as soon as possible once it occurs.Timely and decisive rescue and emergency surgery should be given.Heart repair can greatly improve the patient's survival.
5.Randomized controlled study of effects of leonurus heterophyllus injection and oxytocin on postpartum hemorrhage

Objective To investigate the effects of leonurus heterophyllus injection and oxytocin and their combination applica-tion in treating postpartum hemorrhage .Methods Sixty cases of cesarean section due to the medical factors or the social factors and 54 cases of vaginal delivery were randomly divided into 3 groups by the blind method ,i .e .the leonurus heterophyllus injection group ,the oxytocin group and the leonurus heterophyllus injection plus oxytocin group .Each group was treated by the specific mode .The amounts of intraoperative bleeding and at postpartum 48 h bleeding were recorded ,and the time of the third stage of la-bor was recorded .One way Anova was used to analyze the obtained data .Results The intraoperative bleeding amounts were (1 014 .75 ± 159 .10) mL in the leonurus heterophyllus injection group and the cesarean section group ,(433 .88 ± 75 .34) mL in the leonurus heterophyllus injection plus oxytocin group and (562 .30 ± 102 .00) mL in the oxytocin group ,the difference among the groups were statistically significant(F=67 .48 ,P<0 .01) .The post hoc LSD test showed that under P<0 .05 ,LSD=109 .58 ,indi-cating the significant differences between the two groups ;in the vaginal delivery group ,no statistical difference in the total bleeding amounts ,postpartum 2 h and 2-6 h bleeding amounts had no statistical differences among 3 groups ,but the other time periods of detection had difference ,in which ,compared with the oxytocin group ,postpartum 24 h bleeding amount in the leonurus heterophyl-lus injection group was relatively less ;in the cesarean section group ,the third stage of labor had no statistical difference among 3 kinds of treatment group .Conclusion Using leonurus heterophyllus injection after vaginal delivery can reach the similar effect as oxytocin .However ,leonurus heterophyllus injection is not recommended to be exclusively used in cesarean section .
6.Preparation of lipsomal Clodronate and its inducing effect on apoptosis of peritoneal macrophages in rats after severe acute pancreatitis

Objective To explore the preparation of liposomal clodronate and investigate its inducing effects on the apoptosis of peritoneal macrophages in rats after severe acute pancreatitis (SAP).Methods Liposomal clodronate was prepared by means of thin film. SAP rat model was established by retrograde injection of 5% sodium taurocholate into the pancreatic duct. The peritoneal macrophages were obtained from SAP rats. After exposure to different doses of liposomal clodronate (50, 100,150 μl), the PM proliferation was determined by MTT colourimetry. The apoptosis of PM was measured by flow cytometry and agarose gel electrophoresis, respectively. Results The prepared liposomal clodronate had a suitable encapsulation efficiency of clodronate (5.8%) with an average size of 200 nm. The spherical shape of liposome was confirmed by transmission electron microscope. Exposed to liposomal clodronate of different doses resulted in a obvious growth depression (P<0.01). The apoptotic rate of the PM was (10.32±0.34) %, (18.16±0.49)% and (29.87±0.35)% in three different dose groups and the difference was marked (P<0.01). 1.2% of agarose gel electrophoresis of DNA extracted from apoptotic macrophages induced by liposomal clodronate showed clearer and characteristic ladder following the liposomal clodronate concentration. Conclusion Liposomal clodronate has a definite effect on peritoneal macrophages in SAP rats.
7.The effect of scan duration on ALFF of resting-state fMRI

Objective To examine the reliability and similarity of amplitude of low-frequency fluctuation (ALFF)for different scan duration.Methods 10 min 10 s resting state fMRI data acquired from 13 healthy volunteers separately were divided into 10 pieces of the same length.The reliability and similarity of ALFF were assessed for each scan length (1-10 min).Results Spatial maps of ALFF steadily appeared in each time period (1-10 min);but during the time evolution,both spatial distribution and extent of ALFF increased gradually.At only one minute,there were detectable statistical differences within the group.When the time ran-ging from 5 to 10 min,relatively stable ALFF results in the group achieved.Conclusion ALFF is a relatively stable index,it can be easily affected by scan duration.In the case of 1-5 min,it should be carefully analyzed and interpretated particularly.
8.Changes of intermediate monocytes in peripheral blood of patients with preeclampsia and their significance

ObjectiveTo explore the role of peripheral blood intermediate monocytes in the pathogenesis of preeclampsia.MethodsFifty-two patients with established preeclampsia in Binhai County People's Hospital from October 2014 to October 2015, 42 healthy pregnant women and 42 healthy non-pregnant women were enrolled in this study. The percentage of intermediate monocyte subsets, ratio of positive cells and mean fluorescence intensity (MFI) of Toll-like receptor (TLR) 2, TLR4, CD64, and triggering receptor expressed on myeloid cell-1(TREM-1), and MFI of intracellular tumor necrosis factor-α (TNF-α) and interleukin (IL)-6 were evaluated by flow cytometry. The concentrations of IL-6, IL-8, IL-1β, IL-12P70 and TNF-α in serum were analyzed using Luminex liquid phase chip technology. Independent two samplest-test, analysis of variance, Mann-WhitneyU test, Kruskal-Wallis test and Pearson correlation analysis were used for statistical analysis.ResultsPercentage of intermediate monocytes was higher in preeclampsia patients [10.4%(5.3%-19.9%)]than in healthy pregnant women [6.6%(4.9%-7.8%)], and both were higher than in non-pregnant women [3.8%(2.4%-5.0)%](allP<0.05). The ratio of TLR4 and CD64 positive intermediate monocytes [(60.1±12.5)%vs (24.9±8.8)%; (85.3±5.4)% vs (67.4±7.5)%](t were 15.416 and 13.437, bothP<0.05), and MFI of TLR4 (50.3±10.2 vs 26.8±8.6), TREM-1(35.6±4.1 vs 28.6±4.7) and CD64 (39.8±5.2 vs 28.9±4.8) (t were 11.898, 7.707 and 10.454, allP<0.05) were higher in preeclampsia patients than in healthy pregnant women. MFI of intracellular IL-6 (32.3±4.7 vs 28.6±3.5) and TNF-α (44.6±6.3 vs 36.7±8.3) in intermediate monocytes of preeclampsia patients was also significantly higher than that of healthy pregnant women (t were 4.239 and 5.245, bothP<0.05). Serum concentrations of IL-6, IL-8 and TNF-α were higher in preeclamptic patients than in healthy pregnant women and non-pregnant women (allP<0.05). Furthermore, a positive correlation was found between the percentage of intermediate monocytes and the serum levels of IL-6 and TNF-α in preeclamptic patients (r were 0.397 and 0.347, bothP<0.05).ConclusionsMonocyte subpopulations from preeclamptic patients are abnormally skewed toward intermediate monocytes which have high expressions of TLR4, TREM-1 and CD64, and secret more proinflammatory cytokines such as IL-6 and TNF-α. Therefore, intermediate monocytes are specifically altered in preeclamptic patients and may play a role in the pathophysiology of preeclampsia.
9.CysC specific peptides: bioinformatics analysis and mass spectrometry verification

Objective To analyze the specific peptide of cystatin C (CysC) and its characteristics by bioinformatics technology,and verify the predicted results by mass spectrometry.Methods Online software was applied to analyze the physicochemical properties and homology of CysC peptides hydrolyzed by trypsin and predict the associated parameters of ionized fragmentation of specific peptide by mass spectrometry.Precursor ion scan and product ion scan were conducted on the samples of synthetic specific peptide.The recombinant human CysC and serum samples were analyzed by mass spectrometry after trypsin digestion.The results of analysis were compared with the outcomes predicted by bioinformatics.Results T3 (ALDFAVGEYNK) was considered as the specific peptide of CysC by software analysis.When selecting[M + 2H] 2 + for product ion scan,almost all the y and b ions of fragmentation were observed using tandem mass spectrometry (MS/MS),showing consistency with Skyline predictions.Moreover,both the peptides from the human recombinant CysC and serum sample following the trypsin digestion were eluted at the same time with the isotope-labeled T3 * under the fixed conditions.Conclusion Bioinformatics technology could be available for picking out the specific peptides of target protein quickly and efficiently and predicting the ionized fragmentation precisely by mass spectrometry scanning.
10.KAP-1 knock down by shRNA imparts sphere-forming activity of Panc-1 cells

Objective To investigate whether KAP-1 could induce CSC-self renewal in pancreatic cancer cell lines.Methods KAP-1 expression was examined in 14 cases of pancreatic cancer with immunohistochemistry.KAP-1 shRNA amplified by PCR was inserted into pGC-LV in vector,and then identified by restriction endonuclease digestion and nucleotide sequencing.The lentiviral vector pGC-shRNA-KAP-1 was co-transfected with pHelper 1.0 and pHelper 2.0 packaging plasmids into HEK 293T cells,and the lentivirus was collected and virus titer was measured.The expressions of KAP-1 and vimentin were detected by Western blot and RT-qPCR when human pancreatic cancer cell line Panc-1 was infected by the lentivirus.Sphere forming assay was conducted to assess the capacity of CSC or CSC-like cell self-renewal in this study.Results The KAP-1 expression level in cancerous tissues on immunohistochemistry was significantly higher than in the corresponding normal tissues (P=0.002).After infected by lentivirus,the expressions of KAP-1 and vimentin were knocked down,which could be detected by Wesren blot and RT-qPCR.Compared to Panc-1-GFP (NC) cells,the outcomes suggested that knocking down the expression of KAP-1 could decrease the formation of pancreatospheres,which further suggested the capacity of CSC-self-renewal in primary and secondary pancreatospheres of Panc-1-shRNA-KAP-1 and NC cells.Conclusions KAP-1 expression in pancreatic cancer tissues has been identified.The lentiviral vector for shRNAs targeting KAP-1 was constructed successfully.The formation of pancreatospheres decreased by knocking down the KAP 1 gene.KAP-1 is involved in the regulation of CSC phenotype.The mechanism is probably related to the upregulated expression of the EMT marker vimentin.
