1.Detection of p53 and p21~(WAF1) in cholangiocarcinoma

Objective To clarify the clinical significance of p21 WAF1 and the relationship between it and p53. Methods Samples of human cholangiocarcinoma (HC) tissue and paired normal bile duct tissue adjacent to the tumor from 30 patients with HC were employed in this study. In situ hybridization (ISH) was used to detect the p53 and p21 mRNA expression. Immunohistochemistry (IHC) was applied to analyze p53 gene mutation and P21 protein expression. Results p21 WAF1 And p53 gene protein expression was detected in 36.7% (11/30) and 53.3% (16/30) of the carcinoma specimens, respectively, and none of the paired tissue by IHC. p53 Positivity was related to local lymph node metastasis. Comparing with non-lymph node metastatic group, p53 positivity in metastatic group was significantly higher. p53 Positivity of cholangiocarcinoma in clinical stage III was significantly higher than that in clinical stages I and II. The survival time was significantly shorter in patients with P53 protein expression than in those without. It was found that p53 expression was not associated with p21 WAF1 expression. Conclusions P53 Positivity may be correlated with tumor development but not tumor occurrence. The fact that p53 expression was not associated with p21 WAF1 expression indicates that p53-independent activation of p21 WAF1 may exist.

Objective To observe effect of Yaotui Litongsan on nerve-related behavior of autograft of nucleus pulposus to cavum epidurale in rats.Methods Fifty normal Wistar rats were randomly divided into five geoups:the sham operated group (A),injury model group (B),Yaotongning treat group (C),high dose and low dose group (Yaotui Litongsan treat,D and E),10 rats in each group.Autogenous nucleus pulposus was removed from the coccygeal vertebral and placed into the back of L5~6 nerve roots of left cavum epidurale,making the non-compressive model with transplanted autogenous nucleus pulposus.The mechanical withdraw threshold and thermal withdraw latency were determined.Results The significant mechanical hyperalgesia was showed in the rats of B,C,D,E groups postoperatively,but it was lighter in C,D,E groups than in B group (P

Objective: To explore the effect of CO 2 pneumoperitoneum in laparoscopic surgery on the cardiovascular function of the geriatric cholelithiasis.Methods:Sixty aged patients undergone laparoscopic surgery were selected.During various stage of CO 2 insufflation,the heart rate(HR),blood pressure,oxygen saturation ,electrocardiogram(ECG),and myocardial enzymes were dected,and the clinical values of these changes were analyzed.Results: when CO 2 pneumoperitoneum established ,the HR and blood pressure were increased obviously .The incidence of arrhythmia was 36% and the incidence of changes of ST segment was 30%,10 minutes after pneumoperitoneum eliminated,these changes were restored. The myocardial enzymes and the oxygon saturation of blood had no distinct changes. Conclusion: The procedure of CO 2 pneumoperitoneum caused temporany changes of the HR ,SBP/DSP and ECG.The laparoscopic surgery is effective and safe for geriatric cholelithiasis.It suggests that the dynamic monitoring of electrocardiogram is necessary,especially for those with abnormal heart function.
5.Low-power design of wireless ECG monitor

This paper introduces the low-power design of a portable wireless ECG monitor which adopts MC35 GPRS module.At first,it presents the hardware and software system.Secondly,according to the principle of low-power design,it describes the low-power design methods from the aspects of hardware and software respectively.Finally,this paper discusses how to improve this power management system.In practice,this design efficiently reduces the power-consuming,and prolongs the working time of the monitor.
6.The Experimental Research in the Effects of Ultrasonication on Degradation of PLGA Released DNA

Objective To identify the effective results of ultrasound in degradation of polymeric nanoparticles released DNA .Polymeric nanoparticles was made by dehydration of polyacetylglutamicacid (PLGA, polylactic-co-glycolic acid)solution. Method Green Fluorescent Protein (GFP) was enclosed by PLGA. Different kinds of ultrasound mode and different duct cycle and power ones were used to radiate PLGA solution for 90 s, 9 min, 20 min separately after the solution prepared for 2 hrs,then putted the solution on centrifugal machine at 13000 r/m. Using Choloroform to get rid of fat-soluble impurity,then applied nanodrop to survey the releasing rate of DNA. Finally the effect of cell expression were observed by fluorescent microscope. Results The amount of DNA released from PLGA in groups which were exposed to ultrasound were significantly different from the groups which were not exposed to ultrosound. The releasing amount of former groups had upper limitation. The releasing rate was increased with the increment of the irradiation time,frequency of ultrasound;The effect of the DNA releasing and PLGA degradation by continuous-wave irradiation was stronger than pulsed-wave ultrasound. Conclusion Ultrasound can promote the degradation of PLGA, and do help in DNA releasing and expression in vitro.

A case of a 3-year-old child suffered with severe crush injury to the right forearm and right hand on June, 2017. A comprehensive treatment was conducted with limb salvage, free flap repair and the repair of nerve, vessel and tendon for functional reconstructions followed by rehabilitation therapies. The function and appearance of the injured limb and hand recovered well 3 years after surgery.
8.CONGENITAL CHOLEDOCHAL CYST--REPORT OF 56 CASES

Objective.The aim of this paper is to describe the mechanism, clinical manifestation, diagnosis and surgical management of congenital choledochal cyst. Methods. From 1984 to 1997, 56 cases of congenital choledochal cyst were reviewed. Among them, 8 patients were male, 48 patients were female, the age ranges from 12 to 50 years old with an average of 26.3.Results.The main clinical manifestation includes jaundice, abdominal pain and abdominal mass. All of them were confirmed by ultrasonography, while 39 cases were performed ERCP and PTC with the same diagnosis. Fifty-one patients were performed cystectomy and hepatojejunostomy, two cases were performed cystjejunostomy because of diffused angioma and severe hemorrhage respectively, external drainage was performed in one case with emergent cholangitis, the other two cases were reported malignancy through biopsy and operation was abandoned.Conclusions. Ultrasound diagnostics is essential to accurately diagnose the cyst, preoperative ERCP is helpful for differentiating pancreatic duct from bile duct, while MRCP is a reliable method; cystectomy and cholangiojejunostomy is recommended, laparoscopic procedure is becoming more and more accepted.
9.Ultrasound enhances PLGA nanoparticle-mediated gene delivery in vivo

Objective To investigate the feasibility and the efficacy of ultrasound in promoting PLGA nanoparticle-mediated gene transfection in vivo.Methods Prostate cancer cell line PC-3 was used to generate xenografts in nude mice for gene transfection experiment in vivo.GFP plasmid was encapsulated in PLGA-based nanoparticles.Nanoparticles were injected into tumors locally.Two hours later,xenografts were exposed to ultrasound.Xenograft tissues were harvested in different time points to assess the efficiency of gene expression with regard to different parameters of ultrasound. Results PLGA nanoparticle-encapsulated GFP plasmids were readily transfected to PC-3 cells in vivo.A large number of GFP expressing cells were observed after exposed to ultrasound with 1.0 MHz 50% duty factor continuous wave.In comparison,ultrasound exposure with 40% duty factor pulse wave in vivo had low efficacy in terms of GFP expression.No animal death was noticed due to ultrasound exposure.Conclusions Ultrasound exposure can enhance the release of plasmid DNA content delivered by PLGA nanoparticles in vivo,local exposure to ultrasound wave would be used in conjunction with PLGA nanoparticle-mediated targeted delivery to the tissue or organ of interest.
10.OVERVIEW FOR THE DIAGNOSIS AND TREATMENT OF GALLBLADDER CARCINOMA

Objective. To improve the recognition of diagnosis and treatment of gallbladder cancer.Methods. Retrospective analysis of 52 cases of gallbladder carcinoma in our hospital from 1988 to 1998.Results. Preoperative diagnostic rate was 90.3%, of which 12 cases (23%) were early stage of carcinoma. The total operation resection rate was 55.8%, for which only 17.8% were advance stage of carcinoma.Conclution.The early diagnosis is the key factor of increasing treatment successful rate. The following are the symptoms that raise our special attention to carcinoma of gallbladder: (1) Age over 50 have recurrent cholecystitis and with past history of gallstone; (2) Congenital malformation of bile duct; (3) Local thickening and irregularity of gallbladder wall; (4) Polypoid lesion larger than 1cm inside gallbladder; (5) Atrophic gallbladder; (6) Intraluminal stone of gallbladder does not move when change in body position; (7) Regional lymph node enlargement.
