1.Research trends of cytokines and gene therapy for treating avascular necrosis of the femoral head

The treatment of avascular necrosis of the femoral head whose pathological procedure is femoral head avascularity and osteocyte necrosis induced by many etiological factors is one of the difficult subjects in orthopaedics. As a newly-developed technology, cytokines and gene therapy have been widely used in the research of avascular necrosis of the femoral head. Many cytokines and transduced genes have the effects of promoting vascular reconstruction and bone repairment so that it could recover the blood supply and repair the osteonecrosis of femoral heads. Most current researches remain in the experimental phase. Therefore, the research focus in the future comes down to the lengthening of gene transfection expression as well as the selection of vectors and tissue engineering bones that have low immunereaction and low toxicity.
2.Stage-I functional reconstruction of traumatic deviated nose deformity with chronic rhino-sinusitis

OBJECTIVE To explore the feasibility, surgical method, surgical effect and postoperative complications of stage-I functional reconstruction of traumatic deviated nose deformityaccompanied with chronic rhino-sinusitis.METHODS 37 patients with traumatic deviated nose deformity accompanied with chronic rhino-sinusitis were selected in this study.Methods of stage-I reconstruction included endoscopic nasal septoplasty, functional endoscopic sinsus surgery(FESS), turbinectomy, rhinoplasty and so on.The patients were followed up for one month after operation.The shape of the nose was observed and the nasal bias was measured also.At 3 months after operation, the therapeutic effect about the chronic nasosinusitis was estimated by endoscopic check and patients' subjective symptom.The data obtained preoperatively and postoperatively were treated with statistical methods.RESULTS 37 patients with traumatic deviated nose deformity were selected in this study.The postoperative bias was 2.04 ?0.97 mm and preoperative bias was(6.64?2.21) mm.A t-test revealed the significant difference between pre-and postoperative bias(t=6.374, P

7.Application of egg-yolk-antibody to prevent recolonization of Streptococcus mutans

Objective:To investigate the effect of local application of the water soluble protein fraction,containing specific egg yolk antibody,on the recolonization of Streptococcus mutans.Methods:Whole cells of inactivated streptococcus mutans were used as antigen to immunize bens,then water soluble protein fraction(WSF) was extracted from the eggs. Mouth wash containing 0.1 mg/ml of WSF was prepared and administered to 8 volunteers.The mouth wash was used once every two days for 2 weeks.Vehicle solution was used in other 6 volunteers as the control.S.mutans in saliva was monitored for 100 days.Results:Before using the mouth washes,S.mutans level in saliva of the volunteers was 36.4%.S.mutans was removed by hibitane,it kept less than 3% in 100 days in the tested individuals,while 23%~37% in the controls.Conclusion:The WSF containing specific egg yolk antibody can effectively prevent the recolonization of streptococcus mutans.
8.Clinical study of combination treatment with Clofazimine and others for patients with multidrug-resistant tuberculosis

Objective To evaluate the clinical efficacy and safety of combination treatment with clofazimine ( Cfz ) and other antituberculosis drugs for patients with multidrug-resistant tuberculosis(MDR-TB).Methods 32 cases of MDR-TB patients were treated with combination regimens that included clofaziminefrom October 2011 to September 2016 in our hospitol,according to the history of drug use and drug susceptibility test results using individualized chemotherapy,the starting dose of clofazimine was 0.1 g/day,oral,some patients with adverse reactions and tolerance adjusted to 0.05 g/day,treatment for the last 12 months for three consecutive sputum mycobacterium tuberculosis culture and sputum smear acid-fast bacilli were cured,observe the clinical efficacy and safety.Results After treatment with the combined regimen,56.2%(18/32) of patients were cured,43.8%(14/32) of patients failed treatment, there was no statistically significant difference in the number of drug-resistant patients before and after CFZ treatment,there was no statistically significant difference between the time of failure and the time of CFZ treatment,after taking CFZ combined with anti-tuberculosis program,the number of drug users was statistically significant of patients cured and failed (P<0.05).The average time of sputum culture inversion was 16w.90.6% (29/32) of patients with adverse reactions,mainly including skin color change,ichthyosis and gastrointestinal tract and other adverse reactions, through dose adjustment and symptomatic treatment to continue treatment.The average duration of treatment with clofazimine was about 13 months.ConclusionClofazimine was welltolerated,combination treatment with Clofazimine and others for patients with MDR-TBhave better efficacy .
9.Application of barbed absorbable suture line in retroperitoneoscopic partial nephrectomy

Objective To investigate the safety and feasibility of barbed absorbable suture line in retroperitoneoscopic partial nephrectomy. Methods 45 patients who were suitable for retroperitoneoscopic partial nephrectomy from February 2013 to February 2014 were collected and randomly divided into V-Loc group and the control group. 23 cases of V-Loc barbed absorbable suture line were chosen by suture kidney in V-Loc group.There were 22 cases of coated vicryl plus antibacterial suture and Hem-o-lok clip in the control group. The differences of operative time, blood loss, warm ischemia time, suture time, hospital stay and suture material costs were compared respectively between the two groups. Results All 45 cases of retroperitoneoscopic partial nephrectomy were successfully performed, without conversion to open surgery and important intraoperative complications. Postoperative pathology report showed that there were 37 cases with renal clear cell carcinoma, 3 cases with papillary renal cell carcinoma, 2 cases with chromophobe renal cell carcinoma with negative surgical margin, 3 cases with renal angiomyolipomas. In the V-Loc group and the control group, the average operative time were (90.8±9.6) and (96.9±9.1) min (P>0.05), the mean blood loss were (80.4±24.2) and (99.5±24.8) ml (P>0.05), the mean warm ischemia time were (21.5±3.4) and (25.3±1.9) min (P<0.05), suture time were (10.8±1.7) and (16.8±3.1) min (P<0.05), respectively, postoperative hospital stay were (5.7±1.7) and (7.1±1.9) d (P>0.05), the average cost of renal parenchymal suture material were (1 215.5±101.4) and (2 073.5±301.8) yuan (P<0.05). The warm ischemia time,suture time and suture material costs in the V-Loc group were significantly lower than those in the control group (all P<0.05). Conclusion The application of barbed absorbable suture line in retroperitoneoscopic partial nephrectomy can shorten warm ischemia time and suture time, and save hospitalization costs, with favorable safety and feasibility.
10.Induction and culturing of dendritic cells derived from human peripheral blood monocytes in vitro

Objective To generate high purity and maturity DC from human peripheral blood monocytes in vitro.Methods PBMC were isolated directly from whole blood by density gradient centrifugation and purified by collecting the attached cell,DC were then generated by induction and culturing PBMC for five days with RPMI1640 medium containing rhGM-CSF and rhIL-4 in vitro,and under the condition of 37 ℃,5% CO2.On the fifth day,rhTNF-α was added into DCs cultures,which were then incubated for three additional days.The morphology was monitored by light microscopy,and the phenotypes were determined by FCM.Results After eight days of culture,the cells developed typical and significant dendritic morphology and plenty of cells expressed CD1a, CD80 and CD83,features of DC.Including(78.07±9.43)%CD1a,(60.11±20.50)% CD80 and(46.82±14.15)% CD83 were expressed.About (3.12±1.30)x106 DC cells were derived from 40ml human peripheral blood.Conclusion The way to generate DCs is simple and easy.The DCs produced by this method acquired high purity and maturity antigenic characteristics of DCs.
