1.Biomechanical study of atlas odontoid process replacement

[Objective] To investigate the stability and function of atlantoaxial segment after atlas odontoid process artificial joint replacement.[Method]Ten fresh adult human head and neck specimens were chosen for biomechanical models.The range of motion(ROM),neutral zone(NZ)and stiffness under intact state,post-decompression,post-replacement surgery and post-fatigue were measured respectively.[Result]After decompression,ROM,NZ and stiffness in flexion,extension,right and left lateral bending,and right and left axial rotation increased significantly(P0.05)than that in intact state group.[Conclusion]Artificial atlantoaxial joint in this study has been proved to have biomechanical significance in morphology and dynamic.It has advantages of stable replacement,simple operation,less injury and low wear characteristics.It provides a new choice for the atlantoaxial fusion of anterior or posterior approach.
2.Biomechanical comparison between artificial atlanto-odontoid joint replacament and Magerl transarticular screw fixation

[Objective]To determine the initial stability and function of a new artificial joint in a cadaveric cervical spine model by comparing it with a conventional method. [Methods]Twelve fresh human cadaveric cervical spines(C0~3) were randomly divided into 2 groups: group 1,resection of the odontoid with artificial atlanto-odontoid joint(AAOJ),and group 2,resection of the odontoid with Magerl atlas and axis by transarticular screw fixation(Magerl).For each specimen,the intact and resection of the odontoid underwent a flexibility test firstly,followed by the instrumented construct.Rotational angles of the C0~3 segment were measured to study the immediate stability and function of resection of the odontoid and AAOJ,compared with the intact and resection of the odontoid and Magerl.[Results]Compared to the intact state,resection of the odontoid and AAOJ resulted in a significant decrease in the range of motion(ROM) and neutral zone(NZ) during flexion,extension,and lateral bending(P0.05).Compared to the intact state,resection of the odontoid and Magerl resulted in a significant decrease in the range of motion(ROM) and neutral zone(NZ) during all 6 degrees of freedom(P0.05).[Conclusion]A new type of artificial atlanto-odontoid joint has been designed for correcting atlantoaxial instability resulted from C1、2 anterior decompression procedures.It can restore,to a great extent,the C1、2 axial rotation that is lost during current stabilization procedures.
3.Pharmacokinetics and Serum Fingerprints of Decoction for Puerperal Blood Stasis

OBJECTIVE:To study the pharmacokinetics and serum fingerprints of Decoction for Puerperal Blood Stasis at different time.METHODS:The content of Ferulaic acid(FA) was determined by HPLC.The pharmacokinetic study was carried out.The active constituent in the Decoction for Puerperal Blood Stasis was analyzed based on the serum fingerprints of FA.RESULTS:The distribution and metabolism of FA in rabbits after oral administration of Decoction for Puerperal Blood Stasis assummed two compartment model.The fingerprints between serum at different time and blank serum showed significant differences.CONCLUSION:The content change of FA monitored with serum fingerprints was in line with the rule of pharmacokinetics of FA in Decoction for Puerperal Blood Stasis.The fingerprints can be applied to clarify the pharmacokinetic characteristics,the pharmacological action and the action mechanism of Decoction for Puerperal Blood Stasis.
4.Separation and Preparation of 5 Phenylpropanoids from Cinnamomum Cassia by Semi-preparative HPLC and Their Immunosuppressive Activity Investigation

Objective:To simultaneously separate 5 phenylpropanoids from Cinnamomum cassia by semi-preparative HPLC, and explore their immunosuppressive activity. Methods:After extracted by ethanol, the ethyl acetate part of Cinnamomum cassia was isola-ted by semi-preparative HPLC. The separation was conducted on an Ultimate XB-C18 (250 mm × 10 mm, 5μm) semi-preparative chro-matographic column and the mobile phase was acetonitrile-water with gradient elution. The detection wavelength was 330 nm, and the flow rate was 2. 5 ml·min-1 . The sample volume was 0. 3 ml. MS and NMR spectroscopic analysis were used to determine the config-urations. A CCK-8 method was used to detect the immunosuppressive activity of phenylpropanoids. Results: Totally 5 phenylpro-panoids were separated from Cinnamomum cassia by the semi-preparative HPLC, and identified as erythro-guaiacylglycerol, (7R,8S)-syringoylglycerol, (7S,8S)-syringoylglycerol, 3-methoxyphenyl-acrylaldehyde and O-methoxy cinnamaldehyde. The inhibitory rates of T cells and B cells of the compound 3 was more than 20% at the concentration of 800μmol·L-1 . Conclusion:The method is conven-ient with good separation effect, which can simultaneously separate 5 phenylpropanoids from Cinnamomum cassia, and among them, the compound 3 shows immunosuppressive effect to some extent.
5.Anti-inflammatory, Analgesia and Antipyretic Effects of Chaige Qingre Granules

To investigate the anti-inflammatory, analgesia and antipyretic effects of Chaige Qingre granules. Meth-ods:Kunming mice were used to make auricle swelling model by dimthylphenanthrene, and the auricle swelling degree was measured to test the anti-inflammatory effect of Chaige Qingre granules. Mice were used to make pain models by acetic acid, and the body twis-ting of the mice was counted to observe the analgesia effect of the granules. The fever model in rats was induced by yeast, and the body temperature of the rats was measured to investigate the antipyretic effects of the granules. Results:Chaige Qingre granules could relieve the auricle swelling degree significantly, and the inhibition rate was 46. 3% in the high dose group. High and middle dose groups could reduce the body twisting frequency with the inhibition rate of 51. 1% and 61. 8%, respectively. Every Chaige Qingre granules group could lower the body temperature after one hour of the administration, and the temperature could be reduced 0. 56 degrees Celsius in the high dose group. Conclusion:Chaige Qingre granules can effectively inhibit auricle swelling degree, relieve pain and reduce heat.
6.SolitaireAB stent-assisted endovascular embolization of intracranial wide-necked aneurysms:a clinical ;follow-up study

Objective To investigate the mid-term effect of SolitaireAB stent-assisted interventional embolization with spring coils for the treatment of intracranial wide-necked aneurysms. Methods During the period from May 2009 to April 2013, a total of 49 patients with intracranial wide-necked aneurysm (49 aneurysms in total) received SolitaireAB stent-assisted interventional embolization treatment at authors’ hospital. In 41 patients, a total of 41 aneurysms were detected, of which ruptured aneurysm with bleeding was confirmed in 26 and un-ruptured aneurysm in 15. These 41 patients were followed up for 12-48 months. Based on modified Rankin scoring and DSA, CTA or MRA manifestations, the clinical results were evaluated. Results After the embolization treatment, re-bleeding of the aneurysm occurred in 2 cases, cerebral infarction in 3 cases, occlusion of the parent artery in one case and death in one case; the occurrence rate of complications was 14.2%. DSA, MRA or CTA performed at 12 months after the embolization treatment, showed that 32 aneurysms (78.0%) were completely obstructed, which was obviously higher than that observed on DSA performed immediately after the embolization procedure (21 aneurysms, 42.9%), the difference was statistically significant (P=0.02);residue of aneurismal neck was seen in 7 cases (17.1%) and partial occlusion in 2 cases (4.9%), which were much better than those observed on DSA that was performed immediately after the embolization procedure. Twenty-four aneurysms (58.5%) remained stable, showing no any change, and recurrence of aneurysm was observed in 4 cases (9.7%). At the last follow-up exam, the modified Rankin scoring showed that 0 point was seen in 18 cases (43.9%), one point in 10 cases (24.4%), 2 points in 5 cases (12.2%), 3 points in 4 cases (9.8%), 4 points in 2 cases (4.85%) and 5 points in 2 cases (4.85%). The self-care rate for daily activities was 80.5%, the prognosis was good. Conclusion For the treatment of intracranial wide-necked aneurysms, SolitaireAB stent-assisted interventional embolization with spring coils is safe and effective. This technique can improve the embolization rate and reduce the procedure-related complications.
7.The effect of long term home non-invasive positive pressure ventilation in patients with stable severe chronic obstructive pulmonary disease

Objective To evaluate the effect of long term home non-invasive positive pressure ventilation (HNIPPV ) in stable severe chronic obstructive pulmonary disease (COPD) .Methods Forty-two patients with stable severe COPD after hospital dis-charge were divided into 2 groups :The observation group (conventional treatment + HNIPPV ,22 patients) and control group (con-ventional treatment ,20 patients) .Parameters before and after one year follow-up observation were compared ,which includes arterial bloods gases ,lung function test ,6-min walking distance(6MWD) ,dyspnea grade ,scoring for emotional disorders ,the hospitalization rates .Results PaCO2 ,PaO2 ,6MWD ,dyspnea grade ,scoring for emotional disorders ,the hospitalization rates improved after one year in the observation group (P < 0 .05) .There were no significance of FVC and FEV1 between the two groups after one year . There were no patients who were dead or exited with other reason after one year in the two groups .Conclusion Long term HNIP-PV could decrease exacerbations ,respiratory failure and increase life quality ,this therapy is effective and safe for patients with sta-ble severe COPD .
8.Sphenopalatine ganglion stimulation with acupuncture for perennial allergic rhinitis:a non-randomized traditional Chinese acupuncture (verum acupuncture) controlled pilot trial

ObjectiveTo evaluate the efficacy of sphenopalatine ganglion stimulation with acupuncture for moderate-to-severe perennial allergic rhinitis.MethodsA total of 50 patients were recruited into a sphenopalatine ganglion stimulation group and a routine acupuncture group according to order of presentation, with 25 in each group. The sphenopalatine ganglion stimulation group received sphenopalatine ganglion stimulation with filiform needle, 1-2 sessions/week for 4 weeks. The routine acupuncture group received traditional acupuncture, withyingxiang(LI 20),yintang(GV29),fengchi(GB20),fengfu(GB16),zusanli(ST36) as the maln points, andyingxiang(LI 20),yintang(GV29),fengchi(GB20),fengfu(GB16),zusanli(ST36) as the adjunct points, 1-2 points from both the maln and adjunct points in each session, 2 sessions/week for 4 weeks. The nasal symptom score (2004 version), the total nasal symptom score (TNSS) and the total non-nasal symptom score (TNNSS) were used to evaluate symptom improvement. The Rhinoconjunctivitis Quality of Life Questionnalre (RQLQ) was used to assess the patients’ quality of life. The time to symptom alleviation, duration of symptom alleviation in every session and the recurrence duration during 1 month after the treatment were compared between the two groups.Results After the treatment, the score of the nasal symptom score (99.74 ± 31.89vs.196.83 ± 31.22;t=-4.912,P=0.001), TNSS (33.63 ± 12.37vs.71.82 ± 19.21;t=-3.463,P=0.003), TNNSS (33.63 ± 12.37vs.71.82 ± 19.21,t=-3.463,P=0.003) in the sphenopalatine ganglion stimulation were significant lower than those in the routine acupuncture group. Compared with the routine acupuncture group, the time to symptom alleviation was significant shorter (13.85 ± 4.21 minvs.45.63 ± 7.87 min;t=-1.763,P=0.008), while the duration of symptom alleviation was significant longer (37.92 ± 9.94 hvs.3.35 ± 1.23 h;t=7.637,P<0.01) after each session in the sphenopalatine ganglion stimulation group. Four weeks after the treatment, RQLQ score in the sphenopalatine ganglion stimulation group was significant lower than that in the routine acupuncture group (8.48 ± 3.71vs.37.68 ± 12.46;F=-7.312,P<0.01). The recurrence duration during 1 month after the treatment in the sphenopalatine ganglion stimulation group was significant longer than that in the routine acupuncture group (4.12 ± 2.15 dvs.23.53 ± 4.63 d;t=-8.879,P=0.003).ConclusionSphenopalatine Ganglion stimulation is superior to routine acupuncture in treatment of patients with moderate-to-severe perennial allergic rhinitis.
9.Clinical analysis of 33 cases of primary gastrointestinal malignant lymphoma

Objective To investigate the early diagnosis and treatment of primary gastrointestinal malignant lymphoma( PGIML). Methods The clinical materials of 33 cases with PGIML from 1997 to 2005 were retrospectively studied. Results The diagnosis rate of gastroenteroscopy was 44. 4% . The most common immune phenotype was B-cell lineage (90. 9% ). Of the 33 PGIML patients, 30 were treated with operation, 2 with H. poli elimination and 1 gave up treatment. Among the operation cases and operation plus chemotherapy cases 19/21 cases (90. 5% )and 6/9 cases (66. 7% ) survived more than 2 years respectively, the other 6 cases died. Conclusion It is hard to diagnose PGIML in early stage. The effectiveness of surgery on treating PGIML should be further confirmed by profound prospective randomized trials.
10.Application of quality control circle to reducing non-timely maintenance rate of medical equipment

Objective To apply quality control circle (QCC) to reducing non-timely maintenance rate of medical equipment.Methods QCC was established,and some measures were taken to observe its effect in decreasing non-timely maintenance rate of medical equipment.Results The non-timely maintenance rate fell from 12.1% to 4.5%,and 0.3 million Yuan was saved for the manpower cost.Conclusion QCC decreases the non-timely maintenance rate of medical equipment,increases the efficiency of the maintainer,shortens the downtime of the fault medical equipment,enhances hospital economic benefit and patient satisfaction and provides references for other hospitals.
