1.Effect of ultra-early stent-assisted coil embolization on ruptured intracranial aneurysms

Objective To investigate the therapeutic effect of the ultra-early stent-assisted coil embolization of the ruptured intracranial aneurysms. Methods The clinical data of 13 patients with ruptured intracranial aneurysm treated by ultra-early stent-assisted coil embolization were analyzed retrospectively. The preoperative Hunt-Hess gradeⅠ-Ⅱ was in 7 cases,gradeⅢ was in 4 cases,and grade Ⅳ was in 2 cases. The patients were treated with stent-assisted coil embolization under the general anesthesia with endotracheal intubation within 24 h of aneurysm rupture. The postoperative embolization was assessed according to the Raymond grading standard. The postoperative complications and the assessment of the follow-up results from 1 to 6 months after procedure according to the modified Rankin scale (mRS ) scores were observed. Results All 11 patients recovered well,1 case had postoperative hemiplegia,1 case had postoperative bleeding,and none of them died. During the follow-up period,no patients had rebleeding, 1 had recurrence,and DSA revealed that the patient was embolized completely at 2 months after reembolization. Conclusion The method of ultra-early stent-assisted coil embolization of ruptured intracranial aneurysms is feasible. It may improve the cure rate of the ruptured aneurysms and improve the prognosis of patients.
2.Aseptic Surgical Incision Infection:A Clinical Analysis

OBJECTIVE To study and analyze the risk factors of surgical incision infection. METHODS All of 17 044 aseptic surgery patients from 2004 to 2006 were investigated. RESULTS Totally 36(0.21%) cases occurred infection with class Ⅰ surgical site.The incision infection ratio was 0.24%,0.22% and 0.18% in 2004,2005,and 2006,respectively.The operation time more than three hours occurred in 17 patients,two and three hours were in 12 cases,and less than two hours in the seven cases,accounted for 47.23%,33.33% and 19.44%,respectively. CONCLUSIONS The risk factors of incision infection after aseptic were as age,operation times,seasons,perioperative administration and underlying diseases.In order suroery to prevent the incition infection in aseptic surgery,we should pay more attention to management and education among medical staff.
3.Determination and Impurity Profile of Related Substances of Metronidazole by HPLC

Objective To establish HPLC determination method and impurity profile of the related substances in metronidazole.Methods A Welch Ultimate(R)XB-C1s (4.6 mm× 250 mm,5 μm)was used with a mobile phase consisting of methanol-1.36 g· L-1 solution of potassium dihydrogen phosphate (20∶ 80).The detection wavelength was 315 nm and the flow rate was 1 mL· min-1.Its related substances were determined by principal component self-contrast method.Results Good separation of metronidazole and the impurities could be achieved.Twenty batches of samples in the past six years were determined which meet quality standards.The study of impurity profiles could effectively monitor the synthetic process and the change of impurities in metronidazole.Conclusion The method is simple,quick and sensitive,which can be used to control the related substances in metronidazole.Meanwhile,the impurity profiles ensure the quality stability of metronidazole.
4.Correlation between heart rate variability and arteriosclerosis in patients with essential hypertension

Objective: To study correlation between heart rate variability (HRV) and arteriosclerosis in patients with essential hypertension (EH).Methods: A total of 139 EH patients from our hospital were enrolled as EH group.According to carotid-femoral pulse wave velocity (PWV), they were divided into normal subgroup (n=65, PWV<9 m/s) and abnormal subgroup (n=74, PWV≥9 m/s).Another 60 healthy adults undergoing physical examination in our hospital during the same period were selected as healthy control group.HRV indexes, PWV, carotid intima-media thickness (IMT) and number of carotid plaques were compared among all groups.Correlation among above indexes were analyzed in EH patients.Results: Compared with healthy control group, there was significant reduction in standard deviation of normal to normal RR intervals in all 5 min segments of the entire recording[SDANN, (142.3±21.4)ms vs.(119.3±16.5)ms], significant rise in low frequency[LF, (341.7±37.9)Hz vs.(695.4±43.3) Hz], high frequency[HF, (519.1±33.6) Hz vs.(636.8±39.5)Hz], LF/HF[(0.6±0.3) vs.(1.0±0.2)], PWV[(7.44±0.82) m/s vs.(11.13±0.65) m/s]and IMT[(1.03±0.24)mm vs.(1.74±0.41)mm]in EH group, P<0.01 all;in EH group, compared with normal PWV subgroup, there was significant reduction in SDANN, and significant rise in LF, HF, LF/HF, IMT and mean number of carotid plaques in abnormal PWV subgroup, P<0.05 or <0.01;Pearson correlation analysis indicated that in EH patients, PWV and IMT were significant inversely correlated with SDANN (r=-0.713,-0.699, P<0.01 both), significant positively correlated with LF, HF, LF/HF and carotid plaques (r=0.506~0.935, P<0.01 all), and PWV was significant positively correlated with IMT (r=0.883, P=0.001).Conclusion: In EH patients, SDANN, LF, HF, LF/HF of HRV are significantly correlated with arteriosclerotic extent.Besides lowering blood pressure , HRV etc.related with artery pathological changs should also be treated.
5.Effect of BMP-2 and -3 overexpression on osteogenic and chondrogenic differentiation of prenatal mouse intervertebral disc cells in vitro.

OBJECTIVETo investigate the effect of recombinant adenovirus-mediated bone morphogenetic protein (BMP)-2 and -3 over expressions on chondrogenesis and osteogenesis of prenatal mouse intervertebral disc cells and provide experimental evidences for the application of BMPs in the therapy of disc diseases.
METHODSThe prenatal mouse intervertebral disc cells were infected with a recombinant adenovirus expressing BMP-2 and BMP-3 for 5-7 days, and the expressions of collagen type I (Col I), collagen type II (Col II), aggrecan, osteocalcin, osteoprotegerin and osteopontin mRNAs were detected with RT-PCR. The expression of cartilage matrix was evaluated with toluidine blue staining, and alkaline phosphatase (ALP) activity was detected with ALP reading and ALP staining.
RESULTSBMP-2 and -3 overexpression did not enhance chondrogenesis and osteogenesis of annulus fibrosus (AF) cells or cause significant increases in the expressions of Col I, Col II or aggrecan mRNA in nucleus pulposus (NP) cells. Adenovirus-mediated overexpression of BMP-2 and BMP-3, however, promoted osteogenesis of NP cells and significantly increased the expression of osteocalcin mRNA; the overexpression of BMP-2, but not BMP-3, enhanced the mRNA expressions of osteoprotegerin and osteopontin. Toluidine blue staining demonstrated that BMP-2 and BMP-3 overexpression did not obviously affect the secretion of cartilage matrix. In NP cells, BMP-2 and -3 overexpression significantly enhanced ALP activity, which was not observed in AF cells.
CONCLUSIONAdenovirus-mediated BMP-2 and -3 overexpression can promote the osteogenic differentiation of NP cells but can not affect osteogenesis of AF cells or chondrogenesis of NP cells.

6.p27 gene methylation and clinicopathologic features of colorectal carcinoma

Objective To investigate the relationship between p27 gene methylation and pathology of colorectal carcinoma. Methods p27 gene methylation promotor region and p27 protein expression were detected respectively by methylation specificity polymerase chain reaction and immunohistochemical staining SP in 106 cases of colorectal carcinoma and each adjacent normal mucous membrane tissue and 22 cases of colorectal adenoma tissue. Results The positive expression rate of p27 gene methylation was statistically different in colorectal carcinoma tissue compared with normal mucous membrane and colorectal adenoma tissue (P<0.05). Their positive expression rate were 59.4% (63/106), 18.2% (4/22) and 3.8%(4/106) respectively in colorectal carcinoma tissue,colorectal adenoma and normal mucous membrane tissue (P < 0. 05). p27 gene methylation in poorly differentiated group was significantly higher than that in welldifferentiated group (48.0% vs. 24. 7%, P <0. 05), in Dukes-A + B stage group was significantly lower than that in Dukes C + D stage group(20. 0% vs. 41.2%, P < 0. 05 ), and it was higher in lymph nodes metastases group than that in lymph nodes negative group(41.5% vs. 23. 1%, P <0. 05), that in positive serosa infiltration group was higher than negative serosa infiltration group(32. 5% vs. 24. 1%, P > 0. 05 ).Conclusions Methylated p27 gene protein expression in colorectal carcinoma was significantly higher than normal mucous membrane and colorectal adenoma tissue. The methylation rate of p27 gene in colorectal carcinoma was significantly associated with tumor differentiation, invasive depth, Dukes stage, lymph node metastasis.
7.The diagnostic value of six-slice coronal reformation in patients with acute midepigastric pain

Objective To evaluate the diagnostic value of six-slice coronal reformations in patients with acute midepigastric pain. Methods A total of 974 patients with acute midepigastric pain were included in this study and divided into group A(coronal reformation)and group B(non-coronal reformation).For group A,reconstructed coronal and oblique-coronal images were acquired.A comprehensive diagnosis was made based on coronal reformations and axial planes.Anatomical nomenclature was adopted,including kidney-ureter plane,abdominal aorta plane,superior mesenteric artery plane,ascending colon-appendix plane,stomach-cholecyst plane and colon-small intestine plane.For group B,the diagnosis was made based on axial planes.Finally,a comprehensive analysis was made,missed cases in these two groups were counted and compared,and statistical analysis was performed using the SPSS software(version SPSS V17).Results For group A,the missed diagnosis was made in 12 cases(1.23%)and it was 53 cases(6.58%)for group B.There was a statistically significant difference between two groups(P<0.05).Conclusion Combined with axial planes or oblique coronal reformations,six-sclice coronal reformation can reduce the the rate of missed diagnosis of acute midepigastric pain.
8.Correlation of serum visfatin and resistin levels with cardiometabolic index and bone mineral density in elderly patients with essential hypertension complicated with osteoporosis

Objective:To investigate the correlation of serum visfatin and resistin levels with cardiometabolic index (CMI) and bone mineral density (BMD) in elderly patients with essential hypertension (EH) complicated with osteoporosis (OP) .Methods:From Feb. 2021 to Feb. 2023, 120 elderly patients with EH complicated with OP in Department of Cardiovascular Medicine, Hangzhou Hospital of Traditional Chinese Medicine were chosen (the EH complicated with OP group), and 50 healthy physical examination subjects (the NC group) and 50 EH complicated with low bone mass (the EH complicated with low bone mass group) were chosen as the control. Venous blood samples were collected from all patients, serum visfatin and resistin levels were measured, and CMI and BMD were tested. Statistical methods were used to analyze the data.Results:In EH patients with decreased bone mass or OP, the level of serum visfatin was often low, but resistin was elevated, which showed significant differences compared with that in the NC group ( F visfatin = 26.02, F resistin = 9.50, P < 0.001). The CMI in NC group was lower than that in the other two groups ( F = 52.54, P < 0.001). In terms of bone mass, BMD in NC group was higher than that in the other two groups, especially in the key parts of lumbar spine ( F=21.44, P < 0.001), femoral neck ( F=10.54, P < 0.001), Chinese triangle ( F=12.30, P < 0.001) and femoral trochanter ( F=4.87, P < 0.001), etc. BMD in EH complicated with low bone mass was also higher than that in EH complicated with OP group, including lumbar spine ( t=4.60, P < 0.001), femoral neck ( t=2.32, P=0.022), Chinese triangle ( t=4.58, P < 0.001) and femoral trochoid ( t=4.33, P < 0.001). In addition, visfatin was positively correlated with bone mass ( P < 0.05), lumbar spine ( r=0.36, P < 0.001), femoral neck ( r=0.38, P < 0.001), Chinese triangle ( r=0.28, P=0.020) and femoral trochoid ( r=0.37, P < 0.001), while resistin was negatively correlated with bone mass, lumbar spine ( r=-0.40, P < 0.001), femoral neck ( r=-0.50, P < 0.001), and femoral trochanter ( r=-0.40, P < 0.001). However, there was no significant correlation between CMI and visfatin ( r=0.06, P > 0.05) and resistin ( r=0.11, P > 0.05). Logistic analysis showed that visfatin level was a risk factor of OP in EH patients ( P < 0.001), and resistin was a protective factor ( P = 0.008) . Conclusion:Serum visfatin and resistin levels in elderly EH patients complicated with OP are correlated with BMD, suggesting that they may play an important role in disease development and treatment.
9.The value of blood coagulation function in early stage of acute renal fail-ure

Objective To study the changes of blood coagulation function in early stage of acute renal failure patients, and illustrate its significance. Methods From October 2011 through January 2013, 80 patients with acute renal failure in the Nephrology Department and Pediatrics Department of Yuebei People’s Hospital were enrolled in this study,50 health people in the same period as control group,then compare the differences in creatinine, blood urea nitrogen, blood platelet(PLT), activated partial thromboplastin time(APTT), prothrombin time(PT), thrombin time(TT), plas-ma fibrinogen (FIB) between the two groups. Results The FIB levels and PLT count in observation group were higher than those of controls group (P<0.05), but APTT,PT,TT showed no statistically significant difference (P>0.05). Con-clusion Hypercoagulabale state could occur in patients with acute renal failure in early stage, abnormal clotting may be participates in occurrence of acute renal failure.
10.Safety and clinical efficacy of TIPS with various stents for treatment of cirrhosis with esophageal gastric varices bleeding.

OBJECTIVETo assess the safety and clinical efficacy of transjugular intrahepatic portosystemic shunt (TIPS) with various stents for treating patients with cirrhosis and esophageal gastric varices bleeding.
METHODSOne hundred and five patients were stratified according to stent type: bare stent group, covered stent-grafts group, combined stents group. Rates of success, shunt insufficiency, rebleeding, patient survival, and major complications were observed. The shunt insufficiency rate, rebleeding rate, and survival rate were calculated by the life tables method, the Kaplan-Meier analytical curve, and the log-rank test; a p-value less than 0.05 was considered statistically significant.
RESULTSThe overall success rate of all TIPS for treating the esophageal gastric varices bleeding was 100%. The overall shunt insufficiency rates at 6-, 12-and 24-months post-TIPS were 8%, 9% and 16%, rebleeding rates were 2%, 6% and 17%, and survival rates were 100%, 97% and 94%. The shunt insufficiency rate was 26% in the bare stent group, 14% in the covered stent-grafis group, and 5% in the combined stents group (x2=1.00, P=0.61). The rebleeding rate was 33% in the bare stent group, 7% in the covered stent-grafts group, and 3%in the combined stents group (x2=1.69, P=0.43). The survival rate was 92% in the bare stent group, 93% in the covered stent-grafts group, and 100% in the combined stents group (x2=1.91, P=0.39). The shunt insufficiency rates were higher in patients with splenectomy than in those without splenectomy (30% vs.14%; x2=4.15, P=0.04). The intraperitoneal hemorrhage rates in the covered stent-grafis group and the combined stents group were significantly lower than that in the bare stent group (0% vs 0% vs 13%; x2=8.88, P=0.01).
CONCLUSIONSTIPS with an 8 mm stent effectively treated and prevented esophageal gastric varices bleeding in patients with cirrhosis. Intraperitoneal hemorrhaging caused by TIPS was significantly decreased in the covered stent-grafts group and combined stents group,which represented an improvement in safety of this treatment. However, the influence of covered stent-grafis and combined stents towards the clinical efficacy of TIPS needs further study.
