1.Effects of Xuanyinning Recipe on invasion of SPC-A-1 cells and pathomorphological changes of peritoneum in mice inoculated with sarcoma 180

OBJECTIVE: To observe the effects of Xuanyinning Recipe (XYNR) in inhibiting SPC-A-1 cellular infiltration and on the pathomorphological changes of peritoneum in mice inoculated with sarcoma 180 (S(180)). METHODS: On the bases of isolated culture of mouse peritoneal mesothelial cells, we adjusted and added the human lung adenocarcinoma cell line SPC-A-1 into the double-layer culture medium to observe the number of clones formed. We also took out the peritoneum from the mice administered with three different dosages of XYNR and observed its pathomorphological changes with transmission electron microscope. RESULTS: In the in vitro experiment, the number of clones of SPC-A-1 in culture medium with XYNR (50 microg/ml) decreased distinctly. In the in vivo experiment, it was observed that, in the peritoneum from the XYNR-treated mice inoculated with S(180), the mesothelial cells arranged more and more regularly with the increasing of the dosage of XYNR, while the mesothelial cells in the peritoneum of the mice in the control group necrosed and arranged loosely. CONCLUSION: XYNR can inhibit the invasion of SPC-A-1 cells. It also can improve the loose arrangement of the peritoneal mesothelial cells in mice inoculated with S(180), so as to inhibit the malignant effusion.
2.Expression of tumor stem cell markers Lgr5 and CD44 in esophageal carcinoma and its clinical significance

Objective To analyze the expressions of tumor stem cell markers Lgr5 and CD44 in esophageal carcinoma and its clinical pathological significance.Methods A total of 108 cases of patients with esophageal cancer, 35 cases of stage Ⅱ, 40 cases of stage Ⅲ, 4 cases of stage Ⅳ. After tumor specimens were stained, the expressions of Lgr5 and CD44 in esophageal cancer tissue were detected and its relationship with the clinical features were analyzed.Results There were relations between expressions of Lgr5 and CD44 in esophageal cancer tissue and clinical staging, the later the stageing, the higher expression rate of Lgr5 and CD44, but there was no significant differece with Chi-square test. The Spearman correlation analysis showed there was no correlation between Lgr5 or CD44 and esophageal cancer condition. Conclusion CD44 and Lgr5 are highly expressed in esophageal cancer tissue, but the correlation between CD44 and Lgr5 expression and esophageal cancer condition is uncertain.
3.Clinical observation of caffeic acid for the treatment of leukopenia and thrombocytopenia induced by imatinib in patients with chronic myeloid leukemia

Objective To investigate the curative effect and adverse reaction of caffeic acid (CFA) for leukopenia and thrombocytopenia caused by imatinib in chronic myeloid leukemia (CML) patients.Methods The effect and adverse reactions of CFA for leukopenia or thrombocytopenia induced by imatinib in 42 CML patients were observed.The CFA was admitted by 0.2 g/time,3 times/d,oral.Results 21 cases of 42 CML patients with thrombocytopenia achieved remarkable curative effect after CFA treatment,13 achieved good effect and 8 had no effect.The total effective rate was 81.0 % (34/42).While 15 of 28 leukopenia CML patients achieved remarkable curative effect,8 achieved good effect and 5 had no effect.The total effective rate was 82.1% (23/28).After CFA therapy for 2 weeks,both platelet and white blood cells count were elevated compared with before treatment,the differences were statistically significant (t =2.015,P =0.023,t =1.913,P =0.035).The average onset time of CFA for leukopenia and thrombocytopenia was 2 weeks and no treatment-related adverse reaction was found.Conclusion CFA is an effective and safe auxiliary drug for treatment of imatinib-induce leukopenia and thrombocytopenia in patients with CML.
4.Pedicle screw placement fixation through the injured vertebra in the repair of thoracolumbar fracture:3-year follow-up

BACKGROUND:Many previous retrospective smal-sample studies analyzed the short-term effects using pedicle screw placement to repair thoracolumbar fracture. Conclusions are relatively thin. Simultaneously, screw placement in the injured vertebra can further stabilize the vertebral body, and reduce screw and rod breakage and loss of correction. OBJECTIVE:To evaluate the long-term effects of pedicle screw placement fixation through the injured vertebra in the repair of thoracolumbar fracture. METHODS: From February 2010 to February 2012, 36 patients with thoracolumbar fracture, who were treated in the Department of Orthopedics, Guangzhou Panyu District Second People’s Hospital, were selected. They were treated by vertebral pedicle screw placement fixation. Height ratio of preoperative injured vertebral leading edge was averagely 36%. Kyphosis angle (Cobb angle) was 25°. Sagittal diameter of the center of the vertebral canal was 60%. Nerve function was classified according to Frankel classification: grade A, 2 cases; grade B, 6 cases; grade C, 10 cases; grade D, 9 cases; grade E, 9 cases. The height ratio of injured vertebra, kyphosis angle (Cobb angle), sagittal diameter of the center of the vertebral canal and Frankel classification of nerve function were compared before and after treatment. RESULTS AND CONCLUSION: The postoperative folow-up was 3-3.5 years. Pain was obviously aleviated postoperatively in 36 patients. Average height ratio of injured vertebra was 95%. Kyphosis angle (Cobb angle) was 3.5°. Sagittal diameter of the center of the vertebral canal was 96%. Significant difference was found in the above indexes before and after treatment (P < 0.05). Postoperative neurologic recovery: grade A in 1 case, grade B in 2 cases, grade C in 3 cases, grade D in 4 cases, and grade E in 26 cases. Postoperative fractures were reset. Complications such as breakage, loosing or correction loss were not found. These findings suggest that pedicle screw placement fixation through the injured vertebra in the repair of thoracolumbar fracture can make a satisfactory reduction, enhance the pressure stability of the spine, reduce the internal fixation of fracture and vertebral height loss, and have good long-term effects, indicating that the implant and host have good biocompatibility.
5.The clinical research for the effects of uterine resection and uterine arterial embolization on the function of ovary

Objective To investigate the effects of uterine resection and uterine arterial embolization on the function of ovary. Methods The serum concentrations of estrin(E 2), progestogen(P), follicule stimulating hormone(FSH), luteotropic hormone(LH) and prolactin(PRL) were measured in 15 patients with uterine resection and 22 patients with uterine arterial embolization; including 23 patients with uterine leiomyoma and 29 normal women. We compared the results of the five hormones in each group. Results The serum concentrations of E 2 and P in the group of uterine resection were lower than the normal control group, there was a significant difference between them. Compared the serum concentrations of E 2 and P in the group of uterine arterial embolization and normal control group, there was no significant difference. The serum concentration of PRL in the group of uterine leiomyoma was higher than the normal control group with significant difference. When the uterus was resected or uterine artery was embolished, the serum concentration of PRL decreased remarkably. Conclusions Uterine resection has effects on the function of ovary, but uterine arterial embolization would not. PRL may be one of the factors causing the formation of uterine leiomyoma.
7.Total flavonoid extracted from Sophara japonica buds with assistant of laccase from endophytic Fusarium sp. C-8

Objective To study the application of extracellular laccase from endophytic fungi to total flavonoid extraction. Methods The extracellular laccase vitality of an endophytic Fusarium sp.C-8 from Artemisa annua reached 36.1 U/mL when the fungus was cultivated in revised PDA medium with initial pH 7.0 at 20℃ on a rotary shaker incubator for 6 d. The crude laccase from the cultural medium was applied for extraction of flavonoid from the buds of Sophara japonica. The optimization on the ratio of dry buds to crude enzyme liquid,temperature,time,and pH value was carried out during the process of laccase-assistant treatment. Results After incubation with 40∶1 of crude enzyme (pH 7.0) at 20℃ to dry buds for 1 h,the extraction rate of total flavonoids was 11.4 %,a more increase of 28.7 % than that in regular extraction process. Conclusion The results present a practical method of laccase-assistant extraction process on total flavones in S. japonica buds.

Objective To explore the characteristics of auditory brainstem response(ABR) in children with otitis media with effusion(OME) before and after myringotomy with grommet insertion and the significance of ABR in diagnosing OME.Methods ABR were recorded in 50 cases(100 ears) before receiving myringotomy with grommet insertion for OME,30 cases(60 ears) underwent ABR monitoring after surgery again.For comparison,50 age-matched healthy children(100 ears) were also tested with ABR.Furthermore,30 cases were divided into two groups according to the viscidity of tympanic cavity secretion and the response threshold of wave Ⅴ were compared between the two groups.Results The response threshold of wave Ⅴand the peak latency of wave Ⅰwere normal in 13 percent of 50 cases(100 ears) before surgery.The rate of missed diagnosis was 13%.Among 50 cases,41% had normal response threshold for wave Ⅴ,52% had slight abnormal responses of thresholds of wave Ⅴ and 7% had medium response threshold of wave Ⅴ.The peak latency of wavs Ⅰwas normal in 19% OME children,but it prolonged in 72% OME children and absent in 9% OME children.The prolonged peak latencies(waves Ⅰ,Ⅲ andⅤ) and eleveted response thresholds of wave Ⅴ and shorted transmission time between waves(Ⅰ-Ⅲ andⅠ-Ⅴ) significant when compared to the control group.After surgery,the response thresholds of wave Ⅴand the peak latency of wave Ⅰwere normal in 46.7% children,the response threshold of wave Ⅴ were normal in 70.5% and slight abnormal in 29.5%.Occuring(elicit) rate of wave Ⅰwas 100% and peak latency of wave Ⅰwas normal in 50.2% chilren.The differrence of peak latencies(waves Ⅰ,Ⅲ andⅤ) and the response threshold of wave Ⅴ before surgery
9.Clinical study of high frequency oscillatory ventilation combined thoracic close drainage for the treatment of 22 cases of neonatal pneumothorax

Objective To explore the clinical effect and safety of high frequency oscillatory ventilation (HFOV) combined thoracic close drainage for the treatment of mechanical ventilated neonates with pneumothorax.Methods Twenty-two neonates were enrolled in this study,who needed mechanical ventilation diagnosed with neonatal pneumothorax and received treatment of HFOV combined thoracic close drainage from Jan.2012 and Jun.2014 in the People's Hospital of Dongguan.We recorded blood gas analysis and parameters of breathing machine before using HFOV and 2,12,24,48 hours after using HFOV respectively.Results There were significant differences between HFOV before use (0) and 2,12,24,48 hours after using HFOV in terms of blood pH value,arterial partial pressure of oxygen(PaO2),partial pressure of arterial carbon dioxide (PaCO2),oxygenation index (OI),the inspired oxygen concentration (FiO2),mean airway pressure (MAP) (F =6.606,17.760,8.387,17.242,25.185; P < 0.05).Compared with before using HFOV combined thoracic close drainage,PaO2 was significantly increased from (51.25 ± 13.16) mmHg to (62.60 ± 15.95) mmHg.PaCO2 was significantly dropped from (63.57 ± 13.81) mmHg to (54.02 ± 11.58) mmHg and OI was dropped sharply from (16.57 ± 9.09) to (11.28 ± 4.67) at 2 hours after using HFOV combined thoracic close drainage (P < 0.05).FiO2 significantly decreased from (0.76 ± 0.15) to (0.60 ± 0.13),as well as MAP from (9.91 ± 1.44) cm H2 Oto (8.50 ± 1.68) cm H2O.Furthermore,pH was significantly improved from (7.24 ± 0.15) to (7.34 ±0.10) at 12 hours later(P <0.05).PH,PaO2,PaCO2 were roughly back to normal at 48 hours after treatment.The main complications were intraventricular hemorrhage (2 cases),subarachnoid hemorrhage (2 cases),ventilator associated pneumonia (1 case) and pulmonary hemorrhage (1 case) (not during the time of HFOV treatment).Conclusion HFOV combined thoracic close drainage for the treatment of neonates with pneumothorax is safety and effective methods.
10.Diagnosis and treatment of pancreatic tuberculosis

Pancreatic tuberculosis (PTB) is a rare,chronic,specific and infectious disease which is generally secondary to tuberculosis at the common sites of pancreas,and it has a high misdiagnosis rate due to the hidden onset and nonspecific symptoms of PTB.A patient with PTB was admitted to the First Affiliated Hospital of Zhengzhou University in June 2014.Before operation,the space-occupying lesions of the head of pancreas were detected by preoperative imaging examination,and the patient was regarded as with pancreatic cancer.Intraoperative exploration showed cystic duct involvement,and the granulomatous inflammation was detected by rapid pathological examination using frozen section technique,after that the patient received granuloma resection+cholecystectomy according to suspected PTB.The diagnosis of PTB was confirmed by postoperative pathological examination,and the patient received liver-protective and anti-tuberculosis treatments after discharge.
