1.Effect of Continuous Passive Motion on Hand Spasm and Hand Function of Hemiplegic Patients

Objective To observe the effects of hand function training with continuous passive motion (CPM) on hand spasm and function of the hemiplegic patients.Methods 60 hemiplegic patients after stroke or brain trauma and with hand spasm were randomly divided into the treatment group and control group with 30 cases in each group. The cases in the treatment group were treated mainly with the method of CPM together with the routine rehabilitation procedures, while the those in the control group were only treated with the routine rehabilitation procedures. The changes of hand spasm and function of two groups were observed.Results After four weeks' treatment, the efficacy of the treatment group was obviously better than that of the control group ( P<0.05).Conclusion The rehabilitation training is an effective method for treatment of hemiplegia, and CPM is an important auxiliary method in the treatment. These two Methods combined together can shorten the course of treatment, and strengthen cure efficacy.
2.Diagnostic value of MDCT and MRI in ovarian thecofibroma

Objective To evaluate the MDCT and MRI findings of the ovarian thecofibroma.Methods 15 patients with 16 lesions of ovarian thecofibroma confirmed by pathology after surgery were analyzed retrospectively.MDCT was performed in 1 2 patients and MRI in 8.Results 16 lesions were all unilateral,of which 1 patient had 2 lesions on the same side.10 lesions were located in the right side of ovary,while 6 ones were in the left side.The shape of more lesions was round or oval,and that of huge lesions was lobulated. 14 lesions were well-defined.These tumors were solid in 4 lesions,mixed in 9,and cystic in 3.The largest diameter of tumors ranged from 20 to 220 mm with a mean value of (89±58.29)mm.Plain MDCT showed the substantial part of tumors with iso-den-sity or slight lower density with the mean CT value of (40±8)HU.There was no or slight enhancement in comparison with the mean CT values of (49±9)HU on enhanced MDCT.5 lesions with isointensity and 3 with slight hypointensity on T1-weighted ima-ges,and all the tumors with heterogeneous signal on T2-weighted images were found.6 lesions appeared isointensity with obscure nebulous high signal on T2-weighted.The capsules of 5 lesions showed incomplete ring with lower signal.There was high signal ring outside of tumor capsule on T2-weighted in 5 lesions.All of 8 lesions were enhanced slightly,and the cystic portion was lower signal on T1-weighted images and high signal on T2-weighted images.Among all patients,ascites was found in 10 cases (66.7%)and other kinds of tumors of ovarian or uterus in 13 cases (86.7%).Conclusion MDCT and MRI findings of the ovarian thecofibroma have cer-tain characteristics,which is helpful for the diagnosis and differential diagnosis.
3.Clinical Analysis of 40 Cases of Idiopathic Thrombocytopenic Purpura with Pregnancy

Objective To evaluate the diagnosis and the optimal management of idiopathic thrombocytopenic purpura(ITP) with pregnancy and the related high risk factors with neonatal passive immune thrombocytopenia(PIT) occurring in infants born to mothers with ITP. Methods Medical records of 40 pregnant women with ITP and their neonatal outcomes were reviewed retrospectively from 1992.2 to 2001.8 in our hospital. Results The incidence of pregnancy with ITP was 3.4‰. The maternal complications included pregnancy-induced hypertension syndrome (17.5%), postpartum hemorrhage (15%), preterm labor (12.5%), gestational diabetes(7.5%) and FGR (7.5% ). One mother died at 36 weeks of gestational age. There were 13 cases delivered by vagina and 27 cases delivered by cesarean section. Nine neonates(28.1%)had neonatal PIT, Among them three neonates (9.3%) had severe PIT, One neonate(2.5%) had an intracranial hemorrhage(ICH) and two perinatal death (5%)occured in 40 neonates. No significant correlation was found between neonatal platelet counts and maternal platelet counts, maternal PAIgG values and maternal treatments respectively. Conclusion The prognosis of mothers and neonates in pregnancy with ITP are better. The incidence of severe fetal or neonatal PIT is low and not related to maternal platelet counts, maternal treatments and PAIgG values.
4.Modification with methoxy polyethylene glycol to grafts alleviates acute graft versus host disease severity in mice following haploidentical bone marrow transplantation

BACKGROUND: Methoxy polyethylene glycol (mPEG) is a kind of amphotedc compound with no immunogenicity that has been used to modify various proteins covalently and to prepare versatile blood types. If mPEG modification blocks the activation of T cells in grafts, graft versus host disease (GVHD) reaction probably becomes less serious and transplantation may become successful. OBJECTIVE: To construct haploidentical bone marrow transplantation mudne model and to observe the effects of mPEG modification to grafts on acute GVHD following haploidentical bone marrow transplantation, DESIGN, TIME AND SETTING: A randomized paired design experiment was performed at the Laboratory Animal Center and Pediatric Laboratory of the General Hospital of Chinese PLA between March and November 2003. MATERIALS: Twenty 8-10 week old male BALB/cH-2d mice served as donors and sixty 8-10 week old female CBe F1H-2d/b mice served as recipients, mPEG was provided by Sigma, USA. METHODS: Mixture of donor bone marrow and spleen cells was routinely prepared. After irradiated with 60Co γ ray at a total dose of 8.0 Gy, recipient mice were randomly divided into 3 groups, with 20 rats per group: irradiation control, non-modification, and mPEG modification. Within 12 hours after irradiation, the irradiation control group was injected with 0.5 mL RPMI-1640 culture medium via caudal vein, the non-modification group was administered with 0.5 mL non-modified mixture of bone marrow and spleen cells via caudal vein, and mice from the group were given 0.5 mL mPEG-modified mixture of bone marrow and spleen cells via caudal vein. MAIN OUTCOME MEASURES: After transplantation, hematopoietic recovery, survival rate, acute GVHD and chromosomal karyotype were studied and compared with controls. RESULTS: All mice from the irradiation control groups died within 2 weeks. The 30-day survival rate was significantly higher in the mPEG modification group than in the non-modffication group (75% vs. 40%, x2 = 5.01, P= 0.025). Histopathological examinations of skin, liver and intestine showed typical signs of acute GVHD. The mPEG modification group exhibited less severe pathological presentation and lower Thomas grading than the non-modification group. Cheimedsm examination revealed complete donor-type implantation in living recipient mice at 75 days after transplantation. CONCLUSION: mPEG modification to grafts can greatly alleviate acute GVHD and enhance the survival rate of mice after haploidentical bone marrow transplantation.
5.Reference jitter values in voluntarily activated extensor digitorum communis muscle recorded with concentric needle electrode

Objective To validate the technique and establish the reference jitter values in voluntarily activated extensor digitorum communis using a concentric needle electrode (CNE) in healthy Chinese adults.Methods From January to August 2013,forty-two Chinese subjects from healthy examination center of our hospital were prospectively studied,including 20 males and 22 females.Routine electromyogram was tested and jitter was recorded with CNE in all subjects.The jitter values of action potentials pairs of muscle fibers were expressed as the mean consecutive difference (MCD) after 20 analyzed potential pairs.Results The mean MCD of 42 subjects was (23.0 ± 3.1) μs ranging from 17 to 32 μs.The mean jitter value of all 840 potential pairs was (22.8 ± 7.5) μs ranging from 8 to 54 μs.Upper 95% confidence limits for mean MCD and individual MCD were 29.2 μs and 37.8 μs,respectively.The mean value of MCD of 20 males and 22 females were (23.2 ± 2.8) μs and (22.8 ± 3.4) μs,respectively.There was no statistic difference between genders in MCD (t =0.44,P =0.66).There was no correlation between age and MCD (r =0.11,P =0.48).The mean value of mean interpotential interval was (802 ± 139) μs ranging from 541 to 1 160 μs.Conclusion The present study confirms the suitability of jitter analysis with CNE,which can serve as an objective and valuable method for testing the function of neuromuscular conjunctions.
6.Treatment of Vertebroarterial Vertigo of Cervica Vertebra Disease with Self-made Jingxuan Decoction

Objective To observe the clinical effect of self-made Jingxuan decoction in treating vertebroarterial vertigo of cervical vertebra disease. Methods Sixty five patients were randomly recurited into a control group (30 cases) and a treatment group (35 cases). The contol group received safflor yellow and sodium chloride injection, deproteinised calf blood serum injection, flunarizini, and metroclopramidum; While the treatment group received self-made Jingxuan decoction besides the treatments in the control group. 7 days constituted a course of treatment. The curative effects were assessed after two courses of treatment. Results The effective rate was 94.29% in the treatment group and 76.67% in the control group, showing significant difference (P<0.05). Conclusion Self-made Jingxuan decoction is an effective drug for vertebroarterial vertebroarterial vertigo of cervical vertebra disease.
7.Content Determination of Dracorhodin in Shujin Jiegu Tablets by HPLC

Objective To control the quality of Shujin Jiegu tablets by determining the content of dracorhodin in the preparation by HPLC.Method With dracorhodin as index,the content of dracorhodin in the preparation was determined by HPLC.Results Dracorhodin showed a good linear relationship in the range of 1.53~7.66 ?g,r =0.999 9.The average recovery was 98.64%,RSD=1.12%.Conclusion This method is simple,rapid,repeatable,and applicable to the quality control of the preparation.
8.Value of absent or reversed diastolic flow in predicting hepatic artery thrombosis after liver transplantation

Objective To probe the value of absent or reversed diastolic flow in hepatic artery shown by Doppler sonography of recently transplanted liver in predicting subsequent hepatic artery thrombosis.Methods The records of 54 liver transplantations performed were retrospectively reviewed.The first Doppler study of every case was done within 24 h after transplantation and subsequently reviewed.Findings were categoried into one of three patterns: forward flow,no flow and reversed flow throughout diastole.Meantime,the flow status of portal vein of every case was studied by Doppler sonography.Results Of 54 liver transplants, 12 had aberrant diastolic flow in the hepatic artery after liver transplantation.Four had reversed flow and 8 showed no flow in diastole.In this group of 12 transplants,hepatic artery thrombosis was not developed.Conclusions Reversed or absent diastolic flow in hepatic artery of a recently transplanted liver has no correlation with subsequent hepatic artery thrombosis.
9.Askin Tumors in Children

Objective To analyze the clinicopathological features and enhance the knowledge of Askin tumor in children. Methods The clinicopathological features of 4 cases of children with Askin tumor were described. Results Clinically, the tumor was often seen in elder child patients with painful thoracopulmonary masses.Histopathologically, the nest-like or lobular structures composed of small round-to-oval cells were surrounded by sparse fibrovascular stroma. Among tumor cells there were no or few resticular fibers,however in the surrounding of tumor masses the fibers were found. PAS staining in the tumor in a part of cases was positive. The positive rates of CD99 and Syn detected by immunohistochemistry were high. Neurosecretory granules can be observed by electron microscopy. Two patients received both chemotherapy and radiotherapy three patients died of metastatic tumor of lung and brain and chemotherapy complication in 24,25 and 5 months, respectively. Conclusions Askin tumor is a highly malignant neurosecretory tumor in thoracopulmonary region, which often be misdiagnosed as neuroblastoma, lymphoma, embryonal rhabdomyo sarcoma, tuberculosis and lung abscess, and requires a synthetical treatment plan consisting of pre-and postoperative chemotherapy, radiotherapy and surgical resection. The treatment methods of Askin tumor needs to be further studied.
10.Malignant fibrous histocytoma of the kidney and bladder

Objective To present 3 cases of MFH of the urinary system. Methods Two cases of renal MFH and one case of bladder MFH were retrospectively analysed. Results The diagnosis was established on pathological examination and immunohistochemistry. Two patients died 4~7 months affer operation , while the other patient has been well for 30 months. Conclusions It is difficult to differentiate it from primary renal and bladder neoplasm.Immunohistochemistry is helpful to distinguish it from carcinoma,renal sarcoma and renal parental cell carcinoma.
