1.Compliance of hand hxgiene among health care workers

Objective To evaluate the intervention efficacy of hand hygiene of health care workers(HCWs)in a hospital before and after the comprehensive interventions were taken. Methods The baseline investigation stage was in April-June,2013 ,intervention was performed from July,2013,post-intervention efficacy evaluation stage was in October-December ,2013. Hand hygiene compliance was observed by investigators ,and questionnaires were filled out. Results After multiple interventions were taken,hand hygiene compliance rate of HCWs increased from 38.02% to 53.86% (χ2= 181.82,P<0.05),post-intervention compliance rates of hand hygiene improved significantly in departments of internal medicine,surgery and intensive care unit (all P<0.05).After intervention,the overall hand hy-giene compliance rate at different work opportunities rose from 41.87% (1 740/4 156)to 53.45% (2 044/3 824)(χ2=107.2,P<0.05);except sterile manipulation,the other 4 manipulation opportunities were all higher than pre-intervention (all P<0.05);HCWs of different professions were all higher than pre-intervention(all P<0.05).Conclusion Active hand hygiene intervention can effectively improve hand hygiene compliance of HCWs.
2.MicroRNAs and solid tumors drug resistance

Drug resistance is major obstacles in the successful treatment of malignant solid tumors. Multiple mechanisms involeve in development of cancer drug resistance. Recent research suggests dysregulation of microRNAs is associated with cancer drug resistance. The profiles of microRNAs in drug resistance cancer cells or tissues are different with sensitivity cells or tissues in various solid tumors. Restoring microRNAs could improve chemosensitivity of cancer cells. MicroRNAs expression profiles may provide a critical link for understanding mechanisms involved in chemoresistance. We can also find a specific marker for screening chemosensitivity patients through identification of the microRNAs patterns of drug resistance cells or tissues.
3.Analysis of vitamins and inorganic elements in the water extracts of Radix Changii

OBJECTIVE: To study the material basis of the tonifying effect of Radix Changii by observing its contents of nutrient elements. METHODS: The contents of vitamins in Radix Changii and inorganic elements in the water extracts of it were tested by high performance liquid chromatography, fluorescence spectrophotography and scanning electron microanalysis. RESULTS: Radix Changii contains fat-soluble vitamins E and K 259.2 and 64.1 microg/kg respectively, and water-soluble vitamins C, B1 and B2 78.5, 0.6 and 0.7 mg/kg respectively. Its decoction contains abundant potassium and calcium ions. CONCLUSION: Radix Changii is rich in elements necessary for human body. It has the material basis for tonification.
4.Diagnosis and treatment of small intestine leiomyosarcoma:an analysis of 10 cases

Objective:To explore the diagnosis and treatment of small intestine leiomyosarcoma. Methods:Ten patients with small intestine leiomyosarcoma in Changhai Hospital from 1993 to 2001 were retrospectively analyzed. Results:Radical resection was performed in 6 cases with the shortest survival time being over 1 year and the longest being 8.5 years (still in good health). One case received extirpation operation with the survival time being over 4 years.Partial resection was performed in 3 cases,and the 3 patients died 1,2,3 years after operation respectively.Conclusion:The small intestine leiomyosarcoma is rare and its diagnosis is difficult due to the lack of clinical feature. Final diagnosis should be based on pathology.The disease is not very sensitive to radiotherapy and chemotherapy,which makes surgical treatment the choice.
5.Experimental Study on Dual-Direction Regulation of Tongxie Yaofang on Gastrointestinal Movement of Mice

Objective To investigate the dual-direction regulation of Tongxie Yaofang on gastrointestinal movement by studying its effect on gastrointestinal movement of mice in various functional states. Methods Adopting diarrhea model induced by rhubarb,constipation model by compound diphenoxylate and normal mice,the effect of different doses of Tongxie Yaofang on loose stool quantity,degree,diameter,diarrhea rate,loose stool incidence rate,loose stool grade,diarrhea index of every group were observed. By esta blishing hyperfunction of small intestinal function in mice by neostigimine,inhibition of small intestinal function in mice by atropine sulfate and small intestinal propellant function in normal mice,the effect of Tongxie Yaofang on intestinal movement of mice were studied. Results Tongxie Yaofang could inhibit gastrointestinal movement of diarrhea model and hyperfunction of small intestinal function in mice by neostigimine. It could promote gastrointestinal movement of constipation model and inhibit small intestinal function in mice by atropine sulfate. But it had no obvious action on natural mice and its intestinal movement. Conclusions Tongxie Yaofang has dual-direction regulation on gastrointestinal movement of mice in various functional states,which has a good agreement with formula-syndrome in traditional Chinese medicine,conforms to treatment based on syndrome differentiation.
6.Application of objective structured clinical examination in assessment of interns of traditional Chinese medicine in anesthesiology department

Objective To investigate the application efficacy objective structured clinical exam-ination (OSCE) in assessment of interns of traditional Chinese medicine in anesthesiology department. Methods Totally 62 interns were examed by OSCE formulated by anesthesiology department before de-partmental rotation. Contents of the exam included preoperative evaluation,endotracheal intubation tech-nique,deep venipuncture,lumbar puncture,case analysis,medical documents writing,etc. Assessment was made by exam results and questionnaire survey. Quantitative data were analyzed by ANOVA and results of questionnaire were analyzed by calculating percentage of each option and describing directly. Results Test results showed that the score of medical document writing was the highest(18.25 ± 1.53),while that of case analysis was the lowest(13.49±1.46)and there was no significant difference in scores of three clinical skills. Questionnaire survey showed the OSCE method is superior to traditional method since it is more scientific,objective and fair. Conclusions OSCE method can scientifically as-sess the clinical practice effect of interns of traditional Chinese medicine in anesthesiology department. Meanwhile, it can find the shortcomings in the teaching, which is helpful in promoting the assessment and improvement of clinical teaching.
7.Progress and ideas of irritable bowel syndrome animal model construction based on combination of disease and syndrome

Establishment of combination of disease and syndrome animal model is an ideal technology platform in TCM research. Currently, many scholars have prepared matching IBS animal models combined typical animal models with etiology, which em-bodies the advantages of syndrome differentiation and promotes the development of combination disease and syndrome animal model. Even though, there still exists some problems about sub-jective evaluation on syndromes, paying more attention on dis-ease and less on syndromes. Therefore, we should deepen the pathogenesis of diseases and TCM syndromes research on animal model replication and improve the method of construction of combination of disease and syndrome animal model through the biological research of combination of disease and syndrome sys-tem. By this way, the idea of preparation animal models will be more mature and the scientific connotation of TCM will be pres-ented more exactly.
8.Expression and clinic significance of IGFBP-2 and IGFBP-6 in colorectal adenomas

Objective To study the insulin‐like growth factor binding protein (IGFBP)‐2 and IGFBP‐6 expression and clini‐cal significance in colorectal adenomas .Methods A collection of Chengde Medical College Hospital from July 2012 to March 2013 after surgical treatment of colorectal cancer confirmed by pathology (colorectal cancer ,CRC) tissue samples of 50 patients ,colorec‐tal adenomas (colorectal adenoma ,CRA) 50 cases ,20 cases of colorectal normal mucosa .Immunohistochemistry and RT‐PCR were used to detect the expression of IGFBP‐2 and IGFBP‐6 protein and mRNA ,combined with clinical and pathological data were statis‐tically analyzed .Results IGFBP‐2 protein positive expression and the amount of mRNA expression in the CRA group compared with normal colorectal mucosa had a rising trend;While compared with CRC group had a tendency to reduce ,and the differences are obvious statistical significance (P<0 .05) ,and IGFBP‐6 protein positive expression in the CRA group compared with normal color‐ectal mucosa had a lower trend;While compared with CRC group amount IGFBP‐6 mRNA expression in the CRA group compared with normal colorectal mucosa had a rising trend;While compared with CRC group had a tendency to reduce ,and the differences were obvious statistical significance (P<0 .05) .In the CRA group ,IGFBP‐2 ,IGFBP‐6 positive expression and the patient′s age , sex ,tumor and the number of parts were no significant statistical difference (P>0 .05) ,but with the degree of hyperplasia had sig‐nificant statistical difference(P<0 .05);In the CRA group ,IGFBP‐2 and IGFBP‐6 expression were negatively related to each other , and the difference was statistically significant (P<0 .01) .Conclusion Colorectal adenomas in normal colorectal mucosa of colorec‐tal cancer to the middle part of the transformation process ,and in its occurrence and development process ,insulin‐like growth factor family (IGFs) and IGFS‐R axis plays an important and irreplaceable role ,so IGFBP‐2 ,IGFBP‐6 may be used as diagnostic colorectal adenomas and early predictors of prognosis ,clinical studies on colorectal adenomas is important .
9.Effect of tumor surgery and postoperative relapse on angiogenesis balance in breast cancer

Objective To observe the serum VEGF/ES ratios before and after operation and at clinical relapse in patients with breast cancer, and to discuss the potential effect of tumor surgery and postoperative relapse on the angiogenesis balance. Methods The serum VEGF and ES levels of 59 cases of breast cancer before and after operation and at clinical relapse were determined using competitive enzyme immunoassays and ELISA, (respectively), and compared with the results of 30 cases of benign breast tumors and 59 cases of normal (controls). Results (1) Preoperatively, the serum ES and VEGF levels of breast cancer patients were (significantly) elevated, as compared to the other 2 groups, and the VEGF/ES ratio was 9.1. Postoperatively, at 3 weeks, VEGF level decreased significantly and ES remained at a high level. The VEGF/ES ratio was (3.3). (2)At the time of clinical relapse, serum VEGF level was followed by marked elevation, and the VEGF/ES ratio increased up to 14.2. Multivariate analyses showed that the postoperative VEGF/ES ratio was an independent factor related to the postoperative recurrence of breast cancer. Conclusions Breast cancer surgery may affect the balance between serum VEGF and ES, and the determination of VEGF/ES ratio can have auxiliary value in the assessment of prognosis of breast cancer patients.
10.Ethical Discussion in the Extraction of Teeth

Extraction of teeth is the most basic and widely used operation of oral and maxillofacial surgery. According to characteristics of extraction of teeth, the author views medical ethics principles which in the diagnoses and treatments process from the aspect of medical ethics, they are,①Doctors must intensify their own quality and self - cultivations and establish the idea of " People First ". ②Doctors must carry on the overall and systematic inspection, make the plan of diagnoses and treatments synthetically toimplement the idea of " People First ". ③Doctors should pay attention to interact with patient, meet different patients' demands and create the harmonious medical environment.
