[Objective]To explore the clinical outcome of anterior cervical decompression for severe cervical spondylotic myelopathy with ossification of posterior longitudinal ligament. [Method]Thirty-three patients with severe cervical spondylotic myelopathy and continuous OPLL underwent posterior longitude ligament resection and floating in anterior cervical decompression.The intervertabral space were stabilized by autoilum graft or titanium cage and fixed with windows plates. [Result]All patients were followed up for 8 to 45 months,mean 22 months.Preoperative JOA scores were 6.7,and the postoperative JOA scores were 10.1 and 10.7(evaluated in 3 months and 12 months after surgery),the mean amelioration rate was 72.7% and 78.8%.Of these 33 patients,no severe complications such as cord or vertebral artery injury occurred. [Conclusion]Posterior longitude ligament resection and floating in anterior cervical decompression is a safe and effective treatment for severe cervical spondylotic myelopathy with ossification of posterior longitudinal ligament.

Objective To assess the relationship between polymorphism of XRCC1 Arg194Trp Arg399Gln and ESCC TNM stage and regional lymph node metastasis. Methods PCR and PCR-RFLP were used to detect polymorphism of XRCC1 Arg194Trp and Arg399Gin. The results were analyzed and compared to TNM staging and regional lymph node metastasis. Results The XRCC1 194Arg/Trp and 399Arg/Arg wildtype in ESCC with metastasis is 32.3%, which is higher than 194Arg/Arg (20.0%, P=0.01). There was no remarkable difference between IIb-IV stage ESCC with lymph node metastasis and 0-IIa stage without metastasis. Conclusion The ESCC patients with 194 variability and 399 wildtype had high metastasis rate, XRCC1 Arg194Trp and Arg399Gln had relationship with ESCC TNM stage and lymph node metastasis. Both of them affected to prognosis of ESCC.

Objective To investigate the significance of the mutation of c-kit gene in the development,(clinical) pathology and prognosis of gastrointestinal stromal tumors(GISTs).Methods The technique of(PCR-SSCP) was used to detect ckit gene mutation in 106 cases of GIST,and 57 of the cases were(followed-up).Results The c-kit gene mutation rate in the whole group was 45.3%(48/106),and(3.4)%(2/41) in benign GIST,and 70.8%(46/65) in malignant GIST.The 1-,3 and 5-year survival rates and median survival time of the mutation positive group were all markedlly lower than those of the(mutation) negative group.A comparison of the parameters that can differentiate the benign GISTs from malignant ones such as size,histological necrosis,aggressive grade,nucleus mitotic count and nuclear discrepancy,showed significant difference between the of postive and negative gene mutation(P

ObjectiveTo determine the relationship between the expression of bax and bcl-2 gene in mucous adenocarcinoma of rectum and patient′s prognosis.MethodsImmunohistochemical method was used to evaluate monoclone antibody of bax and bcl-2 protein in 38 cases of mucous adenocarcinoma of the rectum. ResultsThe positive-staining rate of bax was 77.8% in Dukes′ A and B stage compared to 45.0% in Dukes′ C stage( ? 2=4.260,P=0.039).The intensity of positive-staining of bax over bcl-2 was significantly correlated with tumor stage(P=0.04) and the survival (P=0.001).ConclusionThe expression rate of bax and bcl-2 gene was very high in mucous adenocarcinoma of large bowel, and it might have a prognosis implications in patients with colonic mucous adenocarcinoma.

Objective The Application of Circular Mapping during Circumferential Pulmonary Vein Ablation (CPVA) for Atrial Fibrillation Guided by Ensite/NavX System. Methods To evaluate the effect of combined circular mapping on the success rate of CPVA. 61 consecutive cases (40 males and 21 females) were included in the study. 50 cases with paroxysmal atrial fibrillation (PAF) and 11 cases with chronic atrial fibrillation (CAF) underwent CPVA guided by the Ensite/NavX system. A circular electrode catheter was used to map the pulmonary vein (PV) potentials. Pulmonary vein isolation was taken as the endpoint for ablation. Results All the 61 cases underwent the procedure successfully. LPVs were isolated in 34 cases and 35 cases underwent single RPVs isolation. Circumferential ablation of both LPVs and RPVs were carried out in 23 cases. Atrial fibrillation was terminated when delivering radiofrequency energy in 16 cases with PAF and 5 cases with CAF. The termination sites were located near the anterior or posterior roof of the superior PVs in 19 cases and at the mid-portion of the posterior wall from RSPV in 1 case. Atrial fibrillation was terminated when after comple to LPV ablation in 1 case. During the follow-up of 6?2 months, 42 PAF cases and 5 of 11 CAF cases were atrial fibrillation free. The total success rate was 77%. Complications: Pericardium effusion was found in 1 case and disappeared after pericardium puncture and drainage. 1 case developed hemothorax and was cured after thorax puncture and drainage. Conclusion The application of circular mapping during circumferential pulmonary vein ablation guided by Ensite/NavX System contributes to improve the success rate of atrial fibrillation ablation.

Along with the increasing medical image data,it is imperative to set up an effective system about medical image retrieval.Essentially,the image classification is crucial for medical image retrieval.As a distribution pattern of image gray scale,texture is an important character.Wavelet multi-scale decomposition is essentially multi-channel filtering and its multi-resolution analysis structure is identical with human visual system.So the extraction of texture feature under different resolutions after multi-band wavelets transform is of great benefit to image recognition and image retrieval.Consequently,this paper designs an image classification method based on eight-band wavelet.This method solves the key technology in the medical image retrieval,and it gains very high classification rate.

Objective To explore the effects of the vagus and its transmitter on cerebrogenic multiple organ dysfunction syndrome induced by cerebral hemorrhage(CH) in rats. Methods 80 wistar rats were randomly divided into sham-operative group (n=10),subdiaphringmatic vagotomy (SDV) group (n=10),CH group (n=20), CH+SDV group (n=20) and CH+SIV group (n=20).Rat CH model was made by stereotaxis injection of collagenase and heparin sodium in caudate nucleus. The success full CMODS rat model was characterized by the abnormality of hepatic and renal function,myocardium enzyme, blood routine and vital sign of rat chalked standard. The rats of CH+SDV group were practiced SDV operation 4 weeks before the model was made. CH+SIV group were put electrostimulation of vagus after the mode was made for 6 times. The stimulus continued 20 min with 4 h interval. All rats were executed 24 h after CH. The pathological change of main organs (liver, kidney, lung, small intestine),blood routine were tested. Myocardium enzyme,hepatic and renal function were examinied. Meanwhile, the expression of FOS,CHAT and VIP in the medullary visceral zone of the rats were examined with immunohistochemistry,compared with the sham-operative group and SOV gruop.
