1.Surgical treatment of benign prostatic hyperplasia in elderly patients

Objective To investigate the clinical efficacy of surgery treatment of benign prostatic hyperplasia in elderly patients.Methods 60 elderly patients with prostatic hyperplasia aged 80 years and over were admitted in our hospital from January 2005 to January 2009.56 cases were treated with transurethral plasmakinetic resection of benign prostatic hyperplasia,and 4 cases were treated with suprapubic prostatic hyperplasia enucleation.IPSS score,residual urine volume and maximum urinary flow rate (Qmax) in all patients were compared between pre-and post-surgery.Results 60 cases of patients were operated successfully without significant intraoperative capsular perforation,ureteral orifice and urethral sphincter injury.IPSS scores and residualurine volume were significantly less after surgery than before surgery [(20.3 ± 3.7) vs.(11.5 ± 2.1),(85.1 ± 13.0) ml vs.(37.7±7.9) ml,t=16.02,24.14,both P<0.05].The maximum flow rate Qmax was significantly higher after surgery than before surgery[(8.7 ± 2.1) ml/s vs.(14.9 ± 3.2) ml/s,t =12.55,P<0.05].All patients were followed up for 6-12 months.No incontinence and recurrence of benign prostatic hyperplasia were found.Conclusions Transurethral plasmakinetic resection for treatment of benign prostatic hyperplasia is safe and effective in elderly patients,with a low recurrence rate,and is worthy of promoting.
2.Follow-up Study on Psychological Health Status and Glucose in Elderly Patients with Newly Ddiagnosed Type 2 Diabetes for One Year

Objective To investigate the characteristic of psychological health status and blood glucose in elderly patients with newly diagnosed type 2 diabetes for one year. Methods 48 patients in elderly with newly diagnosed type 2 diabetes were followed up on their blood glucose level and psychological status for one year. Results At newly diagnosed phase, fasting plasma glucose, 2 h postprandial plasma glucose and HbA1C were (9.48?2.67) mmol/L, (13.49?4.79) mmol/L and (8.89 ?2.74)%, but after 3months, those descended to (7.35?2.47) mmol/L, (10.01?3.14) mmol/L and (6.75?2.14)%, respectively. Compared with newly diagnosed phase, psychological happiness and living satisfaction improved; while the negative emotion, including depression, loneliness, hypochondriasis and anxiety, decreased consistent with the drop of blood glucose level after 3 months. Following 6 and 12 months, blood glucose and psychological heath status were stabilized on the whole. Conclusion Psychological status deteriorated seriously at newly diagnosed phase than in 3, 6 and 12 months, when is important phase for psychological intervention. Moreover, optimal glycemic control can promote psychological heath.
3.Study on the function and mechanism of Th17/Treg imbalance on mucosal remodeling of ECRSwNP.

Chronic rhinosinusitis (CRS) can be controlled by a combination of conservative treatment and surgical procedures. However, there is one group of CRS endotypes, refractory CRS, such as eosinophilic chronic rhinosinusitis with nasal polyp (ECRSwNP), for which the current treatment strategies of anti-inflammatory and/or antibiotic therapy or surgical removal of the lesion to improve sinus drainage are less effective. This lack of treatment efficacy highlights the need for further fundamental and clinical research to improve or restore nasal mucosal function of CRS. The role of the immune response in the pathogenesis of mucosal remodeling, which is a key problem in refractory CRS, is of particular current interest. ECRSwNP characterized by Th17/Treg imbalance in- duced by IL-6 has a different mucosal remodeling pattern. This review intends to illustrate the role of the imbalance between Th17 (T helper 17) and Treg (regulatory T) cells in the mucosal remodeling of ECRSwNP and to suggest a novel therapeutic target on treating ECRSwNP.

Hepatocellular carcinoma is a common malignant tumor, which has a poor prognosis and a high mortality rate. Under the influence of respiratory motion, the position error of liver cancer is relatively high. The image-guided radiotherapy is a combination of radiotherapy and imaging device, which can detect and correct the error caused by the movement of the organs and the position of the pendulum to improve the accuracy of radiotherapy.
5.Tryptophan hydroxylase 2 gene polymorphisms and poststroke anxiety disorders

Objective To investigate the association of two functional polymorphisms of tryptophan hydroxylase 2(THP2) gene,rs4570625 and rs4565946,with poststroke anxiety disorders.Methods Totally 112 poststroke anxiety patients and 246 non-anxious stroke controls were included and completed Hamilton Anxiety Rating Scale.DNA was extracted from blood and genotyped for the two polymorphisms of THP2 gene by polymorphism chain reaction.Results The G allele of rs4570625 was associated with the increased risk of poststroke anxiety.Both the GG genotype and G allele were observed to be significantly higher in female case than in control.No significant difference in genotype and allele firequencies of the rs4565946 was found among the groups.Patients with the G-C haplotype had significantly increased the risk of poststroke anxiety compared to controls.Conclusions Our findings suggest that these ftmctional polymorphisms in TPH2 gene may be involved in development of poststroke anxiety.
6.Propylene glycol mannate sulfate down-regulated the expression of adhesion molecules(ICAM-1,VCAM-1)in kidney of diabetic rats

Objective To observe the effects of propylene glycol mannate sulfate(PGMS) on the expression of ICAM-1 and VCAM-1 in kidney of diabetic rats, and explore the protective effect of PGMS on kidney. Methods Diabetes was induced by injecting STZ. Then the rats were randomly divided into four groups: normal control group, diabetic modal group, treatment group with high-dose and low-dose of PGMS. 8 weeks later, the blood glucose concentrations, HbA1c, the ratio of kidney weight to body weight, urinary albumin excretion rate (UAE ) in 24 hours were examined. The ICAM-1 and VCAM-1 located were detected by immunohistochemistry, while the expression of ICAM-1 and VCAM-1 were detected by Western blot method and the levels of ICAM-1 and VCAM-1 mRNA by RT-PCR.Results PGMS could reduce kidney hypertrophy and lower urinary albumin excretion rate (UAE)of 24-hour significantly. Immunohistochemical study with light microscopy demonstrated that the levels, both ICAM-1 and VCAM-1 in the kidney of test groups decreased obviously. PGMS down-regulated the mRNA and protein levels of ICAM-1 and VCAM-1 significantly . A positive correlation between expression of ICAM-1,VCAM-1 and kidney lesion was observed. Conclusion: PGMS decreased the expression of ICAM-1 and VCAM-1 in diabetic rats.
7.Apoptosis of implanted tumor of human primary gastric cancer cells in nude mice induced by resveratrol

AIM: To investigate the molecular mechanism of the apoptosis of implanted tumor of human primary gastric cancer cells in nude mice induced by resveratrol. METHODS: Human primary gastric cancer cells were planted into nude mice to establish the cancer model. Resveratrol at different doses were injected near the carcinoma on the nude mice. After treatment, transmission electron microscope and TUNEL staining method were used to detect the apoptosis of implanted tumor cells. Immunohistochemical staining and RT-PCR were used to detect the expression of apoptosis-related genes bcl-2 and bax in implanted tumor. RESULTS: Resveratrol significantly inhibited carcinoma growth when it was injected near the carcinoma. The apoptotic cells in implanted tumor induced by resveratrol were detected by transmission electron microscope and TUNEL staining, immunohistochemical staining and RT-PCR showed resveratrol inhibited bcl-2 expression and increased bax expression in human primary gastric cancer cells. CONCLUSION: Resveratrol inhibits implanted tumor of human primary gastric cancer cells in nude mice through inducing apoptosis. This apoptosis may be mediated by down-regulation of bcl-2 expression and up-regulation of bax expression.

OBJECTIVE To construct siRNA eukaryotic expression vector targeting protein kinase CK2?and to investigate its inhibitory effect on invasion of the HEp-2 cell line in human laryngeal carcinoma. METHODS siRNA expression vector psiRNA-hH1neo-CK2 targeting protein kinase CK2?was constructed by gene recombination,and then was transfected into the HEp-2 cells by lipofectamine methods. Protein kinase CK2?mRNA and protein of the transfected cells were detected by reverse transcription polymerase chain reaction(RT-PCR) and Western blot,respectively. The invasion of the transfected cells was measured by Boyden chamber.SP method was used to examine the expressions of MMP2 and TIMP2 protein of the transfected HEp-2 cells. RESULTS Protein kinase CK2?siRNA expression vector was successfully constructed by gene recombination. OBJECTIVE:
To explore the expression of interleukin-25 (IL-25) in chronic rhinosinusitis with nasal polyps (CRSwNP) and its potential significance in pathogenesis.
METHOD:
IL-25 expression in blood was detected by enzyme-linked immunosorbent assay (ELISA). IL-25 expression in tissue was detected by immunohistochemistry (LSAB method) from polyps (68 CRSwNP patients) and 55 inferior turbinate mucosa from patients with deviation of nasal septum served as control. Complete blood count and HE staining of blood and tissue eosinophil infiltration degree.
RESULT:
IL-25 expression in CRSwNP group were significantly higher than the control group, the difference was statistically significant (P < 0.01). IL-25 expression in local organizations was positively correlated with the number of eosinophil infiltration in CRSwNP group (r = 0.679, P < 0.01).
CONCLUSION
The expression of IL-25 in CRSwNP patients mutually reinforcing and might increase eosinophil infiltration and play an important role in the development of CRSwNP.

10.Impact of allergic airway diseases on definition of eosinophilic nasal polyps' inflammatory subtypes

OBJECTIVE To compare the clinical characteristics between eosinophilic and noneosinophilic CRSwNP and identify the predictors of eosinophilic CRSwNP. The impact of comorbidity (allergic rhinitis-AR, bronchial asthma-BA) on the predictors was also studied. METHODS Histologic characteristics of surgical samples were analyzed in 182 CRSwNP patients, who were classified into eosinophilic CRSwNP (ECRSwNP) group and noneosinophilic CRSwNP (non-ECRSwNP) group. Factors associated with ECRSwNP were selected by correlation analyses. The optimal cutoff points of the predictors were determined by a receiver operating characteristic curve. Based upon the different comorbidity, four groups were divided to study the impact of comorbidity on the optimal cutoff points of the predictors. RESULTS Patients with ECRSwNP had significant higher rate of revision FESS, significant higher concurrent rate of allergic rhinitis and bronchial asthma than non-ECRSwNP patients (all P<0.05). There were significant differences in Ratio of Lund-Mackay scores, peripheral blood eosinophil percentage and serum total IgE between ECRSwNP group and non-ECRSwNP group except the peripheral blood neutrophilic percentage (U value were 1028.00, 1143.50, 800.00 respectively, all P<0.05). Peripheral blood eosinophilic percentage had the highest significant correlation with degree of infiltration of eosinophils in nasal polyps (r =0.538, 0.568, both P<0.05) and there also existed significant correlation between degree of infiltration of eosinophils in nasal polyps and Lun-Mackay score (r =0.419, P<0.05). Comorbidity of AR and BA could impact the peripheral blood eosinophil percentage. According to the different combination of comorbidity, the range of the optimal cutoff points of peripheral blood eosinophil percentage was 3.05%-4.8%. CONCLUSION ECRSwNP and non-ECRSwNP displayed significant differences in certain clinical features. Comorbidity of AR and BA could increase the peripheral blood eosinophil percentage, that may impact the diagnosis of the subtype of ECRSwNP.
