1.The Mechanisms of Organophosphorus Pesticides-induced Hyperglycemia:a Review of Recent Researches
Journal of Environment and Health 1993;0(03):-
As one of important sorts of pesticides, organophosphorus pesticides(OPPs) are used widely in the world. They can cause serious environmental problems and have some adverse effects on the health of people and animals. They can induce acute neurotoxicity and delayed neurotoxicity, and result in the damages of organs. The objective of this paper is to provide a brief review of the mechanisms involved in the OPPs-induced hyperglycemia.OPPs can influence body glucose homeostasis by several mechanisms including oxidative stress, nitrosative stress, inhibition of cholinesterase, inhibition of paraoxonase, disturbance in metabolism of liver tryptophan and so on.

ObjectiveTo evaluate the therapeutic value of neoadjuvant chemotherapy for patients with stages Ⅱ and ⅢA(Only T3N1M0) breast cancer, and assess the relationship of the tumor size and pathologically complete response.MethodA total of 408 breast cancers patients at stages Ⅱ and ⅢA,were studied, which were divided into neoadjuvant chemotherapy group (group A) and control group (group B).All patients of group A received 2-4 cycles NAC with TE scheme in breast cancer. The impact of neoadjuvant chemotherapy on surgical methods choice, local control rate,5-year overall survival rate and 5-year disease free survival rate were analyzed.ResultsFor patients of stage Ⅱ,the rate of breast conservation significantly increased from 23.6% to 49.1% (P =0.000) and there was no significant difference between groups A and B ( P =0.939,0.858 ) in 5-year overall survival rate and disease free survival rate. For patients of stage ⅢA,the 5-year overall survival rate (59.8% ) and disease free survival rate (51.8%) in group A were greatly higher than that in group B (35.7% and 27.6% ) (P =0.000,0.000).The 3-year local relapse and metastasis rate(7.9% ) was lower than group B (18.4%)(P < 0.05 ).The tumor less than 3.0 cm is easy to achieve pathological complete remission (P =0.001 ).ConclusionsThe neoadjuvant chemotherapy can improve potential rate of breast conservation, reduce the rate of local-regional recurrence rate for the patients with stage Ⅱ and ⅢA breast cancer, and also improve the 5-year survival rate for the patients of stage ⅢA.Patients with clinically complete response or PCR may have higher 5- year overall survival rate and disease free survival rate.Primary tumor size was found to be the significant predictive factor for pathologically complete response to neoadjuvant chemotherapy in patients with breast cancer, particularly, the small tumor is easy to reach pathologically complete response.

Objective To investigate the diagnosis and treatment of pancreatic cystic tumors. Methods The clinical data of 35 patients with pancreatic cystic tumors were retrospectively analyzed. Results The preoperative diagnosis rate was as follows: ultrasound was 28.6% ( 10/35 ), CT 44.4% ( 12/27 ), MRI 70% (7/10). Twenty-eight cases underwent operation and were diagnosed finally by postoperative pathology. Serous cystic neoplasm (SCN) was found in 2 cases, mucinous cystic neoplasm (MCN) in 9 cases, cystic adenocarcinoma in 7 cases, intraductal papillary mucinous neoplasm ( IPMN ) in 3 cases, solid pseudopapillary tumor (SPT) in 7 cases. Twenty-three cases were followed up successfully. The post-operative 5 years survival rate was 62.5% (5/8) in pancreatic cystic adenoma without recurrence. The longest survival time in cystic adenocarcinoma after complete resection was 5 years and 8 months, while in which after incomplete resection was 6 months. The 4 cases of SPT are all alive, and one of them has survived for 4 years as the longest one. Conclusions It is difficult to diagnose the pancreatic cystic neoplasm accurately before operation. CT and MRI may be helpful to make the diagnosis. Surgical resection is the most effective treatment. The enhancement of the communication and cooperation between the related departments, the early diagnosis, and the appropriate operation are needed to improve the diagnostic and therapeutic efficacy.

Objective To summarize the clinical characters, diagnostic and therapeutic strategy of the cured patient with human H5N1 avian influenza pneumonia in mainland of China, and to provide effective experiences for the physicians to diagnose human avian influenza infection. Methods The clinical data of the cured patient with H5N1 avian influenza infection in China was analyzed. Results The patient was treated with short-term symmetrel and ribavirin for antiviral therapy companied with the lowdosage and long-term(4 weeks) glucocorticoid therapy. The CPAP assistant ventilation was used and the airway was kept unobstructed. The antibiotic was used to prevent and control the followed infection. The supportive treatment was applied to protect the organ′s function of the patient. The pathogenetic condition of the patient became better with the hemogram risen and the lungs pathological changes absorbed gradually. The chest CT reexamined at the 7th day of the hospitalized-term suggested that the left lung fibrosis started to be absorbed and it was obviously absorbed at the 16th day.The breath function and all the internal organs functions became better. The patient convalesced and left hospital at the 46th day. Conclusion In the human avian influenza infection, the pathogenetic condition may progress quickly after the pneumonia appears. The lung injury and lung fibrosis as well as the multi-organs dysfunction were emerged in the patient rapidly. The progression of disease can be controlled and the prognosis can be improved by the prompt and correct treatment.

Objective To explore the effect of core stability training (CST) on motor of upper limbs after stroke. Methods 60 patients with stroke were divided into observation group (n=30) and control group (n=30). Both groups accepted routine medicine and motor relearning program, and the observation group accepted CST before training. They were assessed with Simple Test for Evaluating Hand Function (STEF), modified Barthel index (MBI), and the Trunk Control Test (TCT) before and 4 weeks after treatment. Results The scores of STEF,MBI, and TCT improved after treatment in both groups (P<0.01), and improved more in the observation group than in the control group (P<0.01). Conclusion CST may facilitate the motor recovery of upper limbs after stroke.

Objective To study the role of Anopheles anthropophagus in malaria transmission and transmission threshold so as to provide basis for vector surveillance and malaria control strategy. Methods Parasitological and entomological methods were used in the investigation at 5 villages of Xinyang City, Henan Province. Results From July to August, 1999, 74 febrile cases (10\^9% of the total population) were examined. Among them 50 were infected, the incidence in the population of surveyed spots was 7\^4%. Active detection was made in another randomly selected two villages and found that the parasite rate in the inhabitants was 2\^0%, and the positive rate of IFA was 8\^4%. Only vivax malaria was detected. An.anthropophagus and An.sinensis were collected, with An.anthropophagus as the predominant one in human dwellings. The estimated man\|biting rate and the human blood index were 4\^9388 and 0\^7858 respectively. The vectorial capacity of An. anthropophagus was 5\^5296. The critical man\|biting rate of An.anthropophagus was 0\^2407 as calculated by the formula (ma=-rlnP/abP\+n) according to Macdonald′s model. The local man\|biting rate was 20 times higher than that of the critical man\|biting rate. Conclusion The results demonstrated that An.anthropophagus is the principal vector in malaria transmission in the area. The findings imply that the critical man\|biting rate is of practicable importance in vector surveillance.

Objective To explore the effectiveness of self-care intervention for elderly patients with diabetes mellitus in rural community. Methods We randomly selected 100 elderly patients with diabetes from January 2013 to December 2013 in rural community health service station in Yizhou, Guangxi Zhuang Autonomous Region. Individualized intervention of diabetes-related knowledge was delivered to patients. Fasting blood glucose and glycated hemoglobin were compared before and after the intervention. The effectiveness of intervention was investigated by diabetes self-care scale. Results Levels of fasting blood glucose [(5. 91 ± 0. 10)mmol/L] and glycated hemoglobin [(4. 62 ± 0. 14)%] from 100 patients were lower than those before the intervention after 1 year intervention (t=11. 840,12. 379;P<0. 01). After intervention, the total score of diabetes self-care scale was improved to (89. 66 ± 1. 56) from (75. 32 ± 1. 50) (t = -7. 597,P <0. 01). Conclusions Blood glucose can be effectively controlled by helping the rural elderly diabetic patients to learn about self-care.
