1.Advance of resistance mechanisms of echinocandins against Candida and antifungal activity in vitro

Although echinocandins,a new generation of antifungal drugs,shows good fungicidal activity against distinct species of Candida,with the extending usage,the activity of echinocandins is gradually to decline,which is caused by different resistance mechanisms.Also,a number of factors may influence its results of susceptibility in vitro (e.g.,human serum,culture temperature in vitro,the pH of the culture medium,and others).Paradoxical effect of echinocandins has its own characteristics and need a further study.In China,there are a few reports about the instances of drug resistance because of the limited clinic application.The study on the echinocandins in prevalence rate of drug resistance and antifungal activity under different status has important clinical significance.
2.Clinical observation of closed thoracic drainage by minimal invasive in treating spontaneous pneumothorax

Objective To investigate the clinical curative effect of closed thoracic drainage by minimal invasive with needle catheter in treating spontaneous pneumothorax.Methods Seventy-six patients with spontaneous pneumothorax were divided into two groups randomly,the control group was treated by traditional thick tube closed thoracic drainage,the experimental group was treated by the needle catheter minimally invasive thoracic closed drainage.Compared clinical curative effect,pulmonary reexpansion time,the wound healing time,the degree of pain and the occurrence of complication between two groups.Results The effective rate of experimental group and control group was 91.9% (34/37) and 92.3% (36/39),the pulmonary reexpansion time of experimental group and control group was (5.38 ± 2.21) d and (5.29 ± 2.27) d,there was no significant difference (P > 0.05).The pain scores during draining and wound thealing time of experimental group and control group was (1.32 ±0.92) scores,(2.93 ± 1.14) scores,(2.13 ± 1.40) d and (5.32 ± 1.78) d,there was significant difference (P < 0.01).Complication such as subcutaneous emphysema,dislodgment of the catheter and wound infection in experimental group was lower than that in control group.Conclusions The effect of needle catheter minimally invasive thoracic closed drainage in spontaneous pneumothorax is better,simple and less invasive.The wound healing time is short,and little pain.It is worthy of promotion.

4.Syphilis seroconversion among drug addicts during methadone maintenance treatment in Maanshan city,Anhui province: incidence and risk factors

Objective To evaluate the incidence of and risk factors for syphilis seroconversion amomg drug addicts during methadone maintenance treatment (MMT).Methods A retrospective study was performed on drug addicts receiving MMT at MMT clinics in Maanshan,who were seronegative for syphilis before the treatment and followed up by serological tests for syphilis for more than two years.The incidence of and risk factors for syphilis seroconversion during the two years of follow up were analyzed in all the subjects.Results A total of 161 drug addicts were included in this study,of whom,15 experienced seroconversion for syphilis with the average seroconversion rate being 4.66 per 100 person-years.The seroconversion tended to occurr in the early stage of MMT.Both Chi-square analysis and multivariate logistic regression analysis showed that syphilis seroconversion was independently associated with nonuse of condoms during the last sexual behavior before receiving MMT.Conclusions Interventions should be undertaken as early as possible to promote safe sexual behaviors among drug addicts at methadone clinics,and to promote condom use is especially important for blocking sexual transmission of diseases.
5.Research progress on promotion strategy of nurses′ professional identity

In this review, we have briefly described the concept, status quo and relevant variables of worldwide nurses′ professional identity. Moreover, there has been a detailed summary of promotion strategies on foreign nurses. The introduction mentioned above aims at exploring more effective and comprehensive promotion strategy, which might provide the theoretical basis for Chinese nursing identity studies.
6.A study on pre-injection test with mini-dose contrast medium in contrast-enhanced magnetic resonance angiography of vertebral artery

Objective To evaluate mini-dose pre-injection test in the use of contrast-enhanced magnetic resonance angiography (CEMRA), and to inspect the possibility of contrast medium peak-time prediction by age, body weight and heart rate.Methods The data from mini-dose pre-injection test of contrast medium before vertebral artery CEMRA were retrospectively reviewed in 55 patients. The linear correlation and regression of the data including age, body weight, heart rate, and the reaching-time, peak-value-time, duration and peak-value-signal of contrast medium was performed by using SPSS software.Results The age (n=55, =62 years old, M=59 years old), body weight (n=55, = 63 kg), heart rate (n=40, =73 beats per minute), peak-value-time (n=55,=17.5 seconds), peak signal intensity (n=55,=472), and duration of contrast (n=49,=10.35 seconds)were analyzed. No statistically significant correlation existed between peak-value-time of contrast medium and the age (r=0.231, t=1.728, P=0.090), body weight (r=0.118, t=0.865, P=0.392), and heart rate (r= -0.046, t=-0.284, P=0.776). The peak-value-time correlated negatively with peak signal intensity (r=-0.322, t=-2.56, P=0.016)and positively with duration of contrast (r=0.658, t=5.99, P=0.000). The peak signal intensity was negatively correlated with body weight(r=-0.356, t=-2.77, P=0.008). The linear regression analysis show b=-0.284, t=-2.285, P=0.026 between peak-value-signal and peak-value-time, b=-0.322, t=2.590, P=0.012 between peak-value-signal and body weight.Conclusion Mini-dose pre-injection test was more helpful to adjust the rate of contrast medium injection and determine the time delay during scanning. But the prediction of contrast peak-time based on age, body weight and heart rate was unreliable.
7.Effect of adaptive statistical iterative reconstruction algorithm on the imaging quality in low-dose spectral CT scanning of the liver

Objective To investigate the value of the adaptive statistical iterative reconstruction (ASIR) algorithm for reducing the radiation dose and optimizing the image quality in the low-dose spectral CT scanning in GSl (Gemstone spectral imaging) of the liver.Methods A total of 60 patients who underwent hepatic spectral CT scanning in GSI were enrolled in this study.The patients were randomly divided into two groups according to priority with 30 cases per group.Low-dose spectral CT scanning was used for group A, and images were reconstructed by ASIR 0 and 50% , marked as A1 and A2.Group B was scanned with conventional dose of spectral CT, and images were reconstructed by Filtered back projection (FBP).Effective doses (E) for each group were calculated.Image quality was assessed by two radiologists, and the radiation doses were compared between groups A and B.Results All image quality of each group were good enough for clinical diagnosis.E for group A and B were (3.2 ±0.2) and (5.8 ± 0.2) mSv, respectively.There was statistical difference with image noise between group A and B(Z =-6.784,P < 0.05).The image noise, SNR and CNR had statistical differences between group A and B (F =24.013, 15.646, 8.285, P <0.05).Compared with group A1, the image noise was lower, and the SNR and CNR were higher in groups A2 and B(P < 0.05).There were no statistical differences of image noise, SNR and CNR between groups A2 and B (P > 0.05).There were no statistical differences of the image quality score between groups A1, A2 and B (F =102.38,105.768, P < 0.05).Conclusions ASIR combined with low-dose spectral CT scanning was helpful to reduce radiation dose and could obtain better image quality in hepatic CT examination.
8.Clinical application of the Contoura carbon fiber belly board in patients with rectal cancer undergoing postoperative radiotherapy

Objective:To evaluate the protection of small bowel and bladder by Contoura carbon fiber belly board in patients with rectal cancer undergoing postoperative radiotherapy and the position deviation during radiotherapy. Methods: This study enrolled 45 consecutive patients with rectal cancer who had undergone prior surgery. Twenty patients who applied the belly board were defined as group A, and the other 25 patients were defined as group B. All the patients received threE-dimensional conformal radiation therapy(3D-CRT), and the prescriptive radiation dosage of 95% of planning target volume (PTV) was 50 Gy/25 times. Patients in group A underwent two sets of CT scans as follows: group A1, prone alone; group A2, prone with the use of the belly board. The radiation dosage and radiated volume in PTV, small bowel, and bladder were observed by dosE-volume histograms. Ten patients were selected randomly from group A and group B, respectively. Their position deviation during radiotherapy was measured by double exposure field verification system. The acute radiation reactions of all patients were observed and recorded during radiotherapy. Results: No significant difference was found in the total PTV and total volume of small bowel and bladder and mean irradiation dosage to PTV between groups A1 and A2. The mean irradiation dosage to the small bowel and bladder, the volume of small bowel irradiated at 10%-100% dose levels, and the volume of bladder irradiated at 30%-100% dose levels, were significantly decreased in group A2. The difference was significant. Compared with group B, the right-left and superior-inferior position deviations were significantly reduced in group A. The difference was significant (P<0.05). The anterior-posterior position deviation was also reduced, but the difference was not significant (P=0.705). The incidence of grade 3 or more adverse reactions were 15% (3/20) in group A and no patient broke off or stopped treatment; the incidence of grade 3 or more adverse reactions was 24% (6/25) in group B, and two patients broke off the treatment because of severe adverse reaction. One patient terminated the treatment. Conclusions: The Contoura carbon fiber belly board can reduce the irradiation dosage and volume of small bowel and bladder in patients with rectal cancer undergoing postoperative radiotherapy. The position deviation has better reproducibility and the acute radiation reactions are tolerable. It is worthy of application in clinic.
9.Correlation of serum total bile acid levels with liver tissue inflammation grade in chronic liver diseases

Objective To investigate the correlation of serum total bile acid (TBA) levels with the inflammation grades of liver tissue in chronic liver diseases.Methods Cyclophorase assay was used to detect the serum TBA levels in 172 patients with various chronic liver diseases,and the inflammation grades of liver tissue were determined by liver biopsy.The correlation between serum TBA levels and the inflammation grades of liver tissue was evaluated using SPSS 12.0 software.Results Serum TBA level was positively correlated with the inflammation grade of liver tissue ( r =0.275,P < 0.01 ).The inflammation grade reached G2 when serum TBA was 20 μmol/L.Conclusion Serum TBA level may be useful for evaluating the inflammation grade of liver tissue in chronic liver diseases.
10. Research progress and prospect of sense of coherence theory in nursing field

Now in this paper, the concept of psychological alignment and its development in the field of nursing were summarized, summarizes the content of the psychological theory of alignment in nursing, dimension and measurement tools, this paper analyzes the related variables. However, there is a lack of research on the longitudinal study and intervention strategy of psychological consensus in nursing field at home and abroad, which needs further empirical research.
