1.Progress of research on relationship between gene p16 and lung cancer

p16 tumor suppressor gene plays an important role in early diagnosis for lung cancer. The occurrence of lung cancer is closed associated with p16 gene aberrant methylation in promotor, seconed exon homozygous deletion, gene point mutation , lose of protein and mRNA expression. Also p16 is related to gene therapy and drug target therapy. This review will summarize the relationship between p16 and lung cancer.
2.Advancement of perioperative application of lornoxicam

Lornoxicam is a new non-steroidal anti-inflammatory drug,it can inhibit the activity of cyclooxygenases and the immunologic injury afteroperation.Its efficacies on preoperative,operative period and postoperative analgesia are similar to those of opioid.Lornoxicam can decrease the dosage of opioid drugs and has the good qualities of short half-life,few adverse effects and satisfactory tolerance.Lornoxicam can provide rapid and stable analgesic effect in perioperative phase.
3.A clinical study of serum lipid metabolism and immunoregulation of patients with primary nephritic syndrome

Objective To explore the serum lipid metabolism and immunoregulation of patients with primary nephritic syndrome (PNS) .Methods 50 PNS patients were served as the test group and 50 healthy people the control group .Automatic biochemical analyzer was utilized to detect their serum low density lipid-cholesterol (LDL-C ) ,high density lipid-cholesterol (HDL-C ) , triglyceride(TG) ,total cholesterol(TC) ,apolipoprotein(Apo)A1 ,ApoB ,and lipoprotein (a) .Immunization rate nephelometry was employed to measure their serum IgG ,IgA ,IgM ,complement C3 ,complement C4 ,CD3+ ,CD4+ ,CD8+ and CD4/CD8 .Results Compared with the control group ,serum levels of TC ,TG ,LDL-C ,ApoB ,ApoA1 ,IgM and CD4/CD8 of patients in the test group were significantly higher ,while those of HDL-C ,IgG ,complement C3 ,complement C4 ,CD3+ ,CD4+ ,CD8+ were obviously lower , with both statistically significant differences (P<0 .05) .However ,the differences of serum lipoprotein (a) and IgA between the two groups was not statistical significant(P>0 .05) .Conclusion Serum lipid level of PNS patients is higher than healthy people ,and considerable loss of Ig and complements and T cell subsets disproportion results in humoral and cellular immune dysfunction .
4.Clinical observation and prevention of antibiotic associated diarrhea

Objective To investigate the characteristics,the incidence, relative factors of antibiotic associated diarrhea (AAD),and prevent and control it. Methods Retrospective analysis was performed in 78 patients with AAD. Results Prevalence ratio of AAD was 9.3%, the risk of AAD related to the kinds of using mix antibiotics and period of treatment, using many meddle measures for medical and age.The antibiotics causing AAD were extending penicillin or third cephalosporins, penicillin, carbapenemases, second cephalosporins.Conclusions Rational use of antibiotic,control use of antibiotic and reduce the state of consciousness are the key measures to prevent and control the AAD.
5.The improvement of ABCD2 scoring accuracy for predicting cerebral infarction after transient ischemic attack

Objective To investigate the application of improved ABCD2 scoring for predicting the onset of cerebral infarction within 7 days after transient ischemic attack (TIA).Methods A total of 133 patients with TIA adnitted from July 2014 to December 2015 were enrolled in this study.The ABCD2 scoring and ABCD2 combined with carotid ultrasound (CU) scoring were used to predict the risk of cerebral infarction occurred within 7 days after TIA.The univariate analysis and multivariate logistic regression analysis were performed to estimate the contribution of clinical risk factors to triggering the cerebral infarction within 7 days after TIA.Receiver operating characteristic curve (ROC) was used to evaluate the predictive value of two different kinds of scoring process in early cerebral infarction after TIA.Results Of 133 patients with TIA,35 (26.3%) suffered from cerebral infarction within 7 days after TIA.Univariate analysis showed cerebral infarction occurred within 7days after TIA was closely associated with BP≥ 140/90 mmHg,unilateral weakness,speech impairment,TIA duration ≥ 10 minutes,past history of hypertension,diabetes mellitus,carotid plaque,and carotid stenosis.Multivariate logistic regression analysis showed that unilateral weakness (OR =3.52,95% CI:1.76-12.34),TIA duration ≥ 10 minutes (OR =2.45,95% CI:1.06-9.27),diabetes mellitus (OR =3.37,95% CI:1.27-10.94),past history of hypertension (OR =4.15,95% CI:1.71-13.34),carotid plaque (OR =6.32,95% CI:2.46-19.40),and carotid stenosis (OR =12.73,95% CI:2.67-44.35) were significantly correlated with early onset of cerebral infarction after TIA (all P < 0.05).The ROC analysis revealed the ABCD2-CU scoring (AUC =0.802,95% CI:0.717-0.888,P =0.000) had a larger area under curve compared to ABCD2 scoring (AUC =0.614,95% CI:0.511-0.717,P =0.036).Conclusions The ABCD2-CU scoring was more accurate in predicting the imminent risk of cerebral infarction in the patients with TIA compared to ABCD2 scoring.And ABCD2 scoring combined with carotid ultrasound could improve the accuracy for predicting the risk of cerebral infarction occurred within 7 days after TIA.
6.The clinical significance of nitric oxide and endothelin-1 in exhaled breath condensate and serum of patients with acute lung injury/acute respiratory distress syndrome

Objective To observe the level of the nitric oxide (NO) and endothelin-1 (ET-1) in the exhaled breath condensate(EBC)and serum of the patients with ALI /ARDS, and investigate its clinical significance. Methods The study group included 52 mechanical ventilation patients with ALI/ARDS in ICU , which were divided into the survival and death group, while 30 healthy volunteers were recruited as healthy control. EBC samples of the healthy control and the study group on the 1st day and 5st day were collected by EcoScreen condenser with the synchronous collection of the venous blood. The concentrations of NO and ET-1 in the EBC and serum were measured by EIA. Results The levels of NO and ET-1 in EBC and serum of the patients with ALI /ARDS were all significantly higher than those of the healthy control. After treatment , the levels of NO and ET-1 in EBC and serum of the patients all decreased significantly compared with before treatment. After treatment , The levels of NO in EBC and serum of the survival group were significantly lower than those of the death group. After treatment , the levels of ET-1 in serum of the survival group was significantly lower than that of the death group. Conclusions Detecting the levels of NO and ET-1 in the EBC and serum can reflect oxidative stress , inflammatory reaction and endothelial injury in lung of patients with ALI/ARDS.
7.Clinical significance on detection of exhaled breath condensate biomarkers in lung cancer

Exhaled breath condensate(EBC) analysis,as a new technology of studying the respiratory biochemical components in recent years,has the advantages of non-invasiveness,simple collection and good repeatability,etc.The detection of exhaled breath condensate biomarkers plays an important role in screening,early diagnosis,disease monitoring,efficacy and prognosis assessment,following up of lung cancer.
8.Prognostic value of pre-operative NLR, d-NLR, PLR and LMR for predicting clinical outcome in surgical colorectal cancer patients

Objective:To evaluate the impact and predictive value of neutrophil-to-lymphocyte ratio( NLR) ,derived neutrophil-to-lymphocyte ratio( d-NLR) ,platelet-to-lymphocyte ratio( PLR) and lymphocyte-to-monocyte ratio( LMR) on the prognosis of patients with colorectal cancer with curative resection.Methods: Retrospective analysis of 205 cases of clinical data and returning data after patients with colorectal cancer surgery,ROC curve was applied to determine thresholds for four biomarkers and their prognostic values were assessed using Kaplan-Meier curve,univariate and multivariate COX regression models of patients with NLR,D-NLR,PLR and LMR and other clinicopathological factors and the relationship between the 5-year recurrent-free survival( RFS).Results:According to the ROC curve,LMR could not be used as a potential diagnostic biomarker for subsequently analysis,for its AUC was less than 0.50, The optimal cut-off values were 3.12 with the highest specificity and sensitivity ( 75% and 73.1%) for NLR, while in the univariate analysis show,NLR,D-NLR and PLR were all risk factors of poor recurrence-free survival ( P<0.05 ) , the multivariate regression analysis,NLR≥3.12 and colorectal cancer staging were independent risk factors of prognosis of colorectal cancer.Conclusion:NLR≥3.12 can provide improved accuracy for predicting clinical outcomes in surgical CRC patients under surgery resection.
9.Clinical value of prealbumin and C-reactive protein in children with bacterial community acquired pneumonia

Objective To investigate the clinical value of prealbumin (PA) and C-reactive protein (CRP) level for the diagnosis of children patients with bacterial community acquired pneumonia (CAP).Methods Seventy-two children with bacterial CAP (CAP group) between July 2012 and December 2013 were enrolled in the study.Fifty healthy children were selected as healthy control group.The level of serum PA and CRP were measured by flow cytometry and turbidimetric immunoassay respectively.The results were compared.Results Before treatment,the level of CRP in CAP group was higher than that in healthy control group,and the level of PA in CAP group was lower than that in healthy control group,there was significant difference (P < 0.01).The level of white blood cell count and the rate of neutrophilic granulocyte in CAP group was higher than that in healthy control group,but there was no significant difference(P > 0.05).After treatment,the level of CRP in CAP group was decreased and the level of PA was increased,there was significant difference (P < 0.05).Pearson analysis showed that the level of CRP in CAP patients had negative relationship with the level of PA (r =-0.613,P < 0.01).Conclusion Detection of serum PA and CRP contributes to clinical diagnosis of children bacterial CAP and evaluation of anti-infection effect.
10.Effects of propofol on the expression of ketamine-induced HSP 70 gene in the rat posterior cingulate cortex

Objective To investigate the effects of propofol on ketamine-induced HSP 70 mRNA and protein expression in the rat posterior cingulate cortex and to explore the possibility of using propofol to prevent or treat ketamine-induced psychotomimetic effects and neuronal damage. Methods Thirty male Wistar rats weighing 250-300 g were randomly divided into 5 groups with 6 animals in each group: group 1 received normal saline intraperitoneally ip (NS); group 2 received ketamine 100 mg? kg-1 ip (K); group 3 received propofol 100 mg ? kg-1 ip (P); group 4 received propofol 50 mg?kg-1 + ketamine 100 mg?kg-1 ip (P1 K) and group 5 received propofol 100 mg?kg + ketamine 100 mg?kg-1 ip (P2K) . In group 4 and 5 the interval between propofol and ketamine administration was 15 min. Twenty-four hours after ketamine and/or propofol administration, the animals were decapitated and brain was removed. HSP 70 mRNA expression in the posterior cingulate cortex was detected by using semi-quantitative RT-PCR technique; HSP 70 protein expression in posterior cingulated cortex was determined by immuno-histochemical method. Results The levels of HSP 70 mRNA and HSP 70 protein expression were significantly different among the 5 groups. Ketamine induced marked HSP 70 mRNA and HSP 70 protein expression in the posterior cingulated cortex. Propofol itself did not induce HSP 70 gene expression in this brain region. Propofol significantly inhibited ketamine-induced HSP 70 mRNA and HSP 70 protein expression in the posterior cingulate cortex in a dose-dependent manner. Conclusions Propofol pretreatment can significantly inhibit ketamine-induced HSP 70 mRNA and protein expression in the posterior cingulated cortex. It may be one of the mechanisms of inhibition of ketamine-induced psychotomimetic effect and neuronal damage by propofol.
