2.Changes of cytokines in aqueous humor of patients with proliferative diabetic retinopathy after intravitreal bevacizumab and their correlation analysis

Objective To study the changes of cytokines in aqueous humor of patients with proliferative diabetic retinopathy (PDR)after intravitreal bevacizumab (IVB) and the correlation of these cytokines.Methods Fifty-six patients(63 eyes) with severe PDR underwent pars piana vitrectomy (PPV) with IVB pretreatment in our hospital from April,2014 to February,2015 were collected.All the patients received IVB therapy from 3 days to 7 days before PPV.Aqueous humor samples were collected at the time of IVB and PPV treatment.The concentrations of vascular endothelial growth factor (VEGF),interleukin-6 (IL-6),interleukin-8 (IL-8) and monocyte chemotactic protein-1 (MCP-1) were measured by cytoinetric bead array (CBA).The changes of these cyto-kines after IVB were analyzed by using Wilcoxon rank sum test for paired data.The correlation between these cytokines was analyzed by using Spearman rank test.Results From 3 days to 7 days after IVB,the concentration of VEGF in aqueous humor was (6.57 ± 12.29)pg · mL-1,lower than the pre-operation of (278.71 ±340.20)pg · mL-1,the difference was statistically significant (P < 0.05).The concentration of IL-8 was (385.60 ± 1099.47) pg · mL-1,higher than pre-operation of(298.87 ± 1005.79) pg ·mL-1,the difference was statistically significant (P < 0.05).IL-6 and MCP-1 were increased after IVB,but the difference was not statistically significant (all P > 0.05).VEGF,IL-6,IL-8 and MCP-1 had an obviously relationship between each two factors(all P < 0.05).Conclusion Angiogenesis in patients with PDR are regulated by a variety of cytokines,and targeted reduction of VEGF may compensatory increase the concentration of other inflammatory cytokines.
3.EFFECTS OF DIETARY SUPPLEMENTATION OF ANTIOXIDANTS ON zf-9 mRNA EXPRESSION IN RAT HEPATIC STELLATE CELLS DURING ACUTE LIVER DAMAGE

Objective:To investigate the effect of vitamin E, selenium and quercetin on the expression of zf-9 mRNA of hepatic stellate cells (HSC) during early acute liver injury in rats. Methods:90 healthy SD rats were randomly divided into four groups: normal control, pathological control, VE 20 mg/(kg bw﹒d) + Se 16 ?g/(kgbw﹒d) supplement, and quercetin 50 mg/(kg bw﹒d) supplement through intragastric way for 15 d respectively. The normal control group was injected with normal saline, the others were given intraperitonal CCl4 once to induce acute liver injury model. The serum levels of MDA, SOD, ALT and AST were determined at 3 h, 6 h, 12 h and 24 h after CCl4 injection respectively. The pronase-collagenase perfusion of liver was used to isolate HSC, and the expression of zf-9 mRNA was examined by RT-PCR. Results: Serum levels of MDA were lower in VE+Se supplement group and quercetin supplement group than pathological group. Supplementation with VE+Se and quercetin could decrease the level of ALT and AST and down-regulate the expression of zf-9 mRNA. Conclusion:Dietary supplementation of VE,Se and quercetin could effectively down-regulate the expression of zf-9 mRNA in HSC during early acute liver injury in rats.
4.Preliminary study on appendicular skeletal muscle mass, muscular strength and function in the elderly

Objective To investigate the status of appendicular skeletal muscle mass,muscular strength and changes of muscle function in the elderly in Shanghai.Methods A total of 402 participants were recruited from physical examination centers in our hospital.Appendicular skeletal muscle mass (ASM) was measured by bioelectrical impedance analysis (BtA).The ASM index (ASM/height2) was calculated.Muscle function was measured by testing gait speed (m/s).Muscle strength was measured by handgrip strength (HGS) using an electronic hand dynamometer.Results The ASM index significantly declined with age in both men and women (F=2.133 and 3.124,both P=0.000).For women,there was a significant difference in HGS only between the youngest age group (aged 60~69 years) and the oldest age group (aged≥80 years)(P=0.005).For men,HGS was significantly different among all age groups (all P<0.05).There was no significant difference in gait speed between 60-69 years group and 70-79 years group in both genders (both P>0.05),while the significant differences in gait speed were observed between the 70-79 years old groups and ≥80year old group in both genders (both P<0.05).Conclusions Appendicular skeletal muscle mass,strength and function are decreased with ageing in the elderly.The muscle strength decreases faster in the elderly man than in the elderly woman,and muscle function decreases significantly in both genders after 80 years old.
5.Comparison between transsylvian-transinsular approach and transcortical approach for hematoma evacuation in hypertensive basal ganglia hemorrhage

Objective To compare the efficacy of hematoma evacuation between transsylvian-transinsular approach and transcortical approach in hypertensive basal ganglia hemorrhage.Methods The patients with hypertensive basal ganglia hemorrhage who underwent hematoma evacuation via transsylviantransinsular approach and transcortical approach were enrolled retrospectively.Demographics and baseline data,as well as the outcome (the modified Rankin scale 0-3 as good outcome and ≥4 as poor outcome) and mortality at 3 months were compared in both groups.Results A total of 68 patients with hypertensive cerebral hemorrhage (40 cases via transsylvian-transinsular approach and 28 via transcortical approach) were enrolled.There were no significant differences in the demographics and baseline data between the two groups (all P> 0.05).The good outcome rates in the transsylvian-transinsular approach and transcortical approach at 3 months after surgery were 52.50% (21/40) and 21.43% (6/28),respectively.The former is significantly higher than the latter (x2 =6.642; P=0.01); the mortalities in the transsylvian-transinsular approach and transcortical approach were 2.50% (1/40) and 21.43% (6/28),respectively.The former is significantly lower than the latter (Fisher's exact test,P=0.017).Conclusions The clinical efficacy of hypertensive basal ganglia hemorrhage via transsylvian-transinsular approach is better than the transcortical approach.
6.Effects of different doses of dexmedetomidine on perioperative inflammatory responses in patients undergoing one-lung ventilation

Objective To investigate the effects of different doses of dexmedetomidine on perioperative inflammatory responses in patients undergoing one-lung ventilation (OLV).Methods Thirty-six ASA T or Ⅱ patients (aged 43-72 years and weighing 50-78 kg) scheduled for esophagectomy were randomly divided into three groups (n =12 each):control group (group C),low dose dexmedetomidine group (group D1) and high dose dexmedetomidine group (group D2).Dexmedetomidine 1 μg/kg was infused intravenously 10 minutes before anesthesia induction,then infused at a rate of 0.2 μg· kg-1 · h-1 (group D1) or 0.5 μg· kg-1· h-1 (group D2) until 30 minutes before the end of operation.Group C received the equal volume of normal saline.Blood samples were collected before anesthesia induction (T0),immediately before OLV (T1),30 minutes after OLV (T2),90 minutes after OLV (T3),30 minutes after lung inflation (T4) and 2 hours after operation (T5) for monitoring serumlevels of tumor necrosis factor-alpha (TNF-α) and interleukin-8 (IL-8).Results Compared with T0,serum levels of TNF-α and IL-8 significantly increased at T3 and T5 in all the three groups (P < 0.05).Compared with group C,serum levels of TNF-α and IL-8 significantly decreased at T3 and T5 in group D2 (P < 0.05).There was no significant difference in the indexes mentioned above between group C and group D1 (P > 0.05).Conclusion Dexmedetomidine 1 μg/kg given before anesthesia induction and then infused at the rate of 0.5 μg· kg-1 ·h-1 during operation can reduce inflammatory responses in patients undergoing OLV.
7.Effect of isocaloric meals of conjee, steamed bread, and cooked rice with different glycemic indices on glucose level in type 2 diabetes

To study the effects of conjee and cooked rice on postprandial glucose and plasma insulin levels in type 2 diatetes,and to help diabetic patients select reasonably food.41 diabetes were divided into cooked rice group ( group A),conjee with steamed bread group ( group B),and oatmeal group ( group C ).At 1 h after meal,the values of postprandial plasma glucose (PPG) was significantly lower in group C than those in group A and group B [ ( 11.17± 2.30 vs 12.88 ± 1.29,13.29 ± 1.97 ) mmol/L,P < 0.05 ].At 2 h after meal,the value of PPG was significantly lower in group C than in group A [ ( 8.88 ± 2.66 vs 10.87 ± 1.63 ) mmol/L,P <0.05 ].At 1 h and 2 h after meal,there was no significant difference between the value of PPG in goup A and group B ( P>0.05 ).At 1 h after meal,the value of plasma insulin was significantly lower in group C than those in group B [ (46.02 ± 26.32 vs 88.56 ± 68.75 )μU/ml,P <0.05 ],and there was a littler higher in group B than group A ( P>0.05 ).At 2 h after meal,there was no statistical difference of plasma insulin among group A,B,C [ ( 57.10 ± 33.56,62.26 ± 24.42,54.16 ± 41.35 )μU/ml,P>0.05 ) ].Isocaloric oat food is potentially beneficial in sustaining blood glucose status and decreasing insulin secretion.It is the ideal choice for type 2 diabetes.Meanwhile,there were no statistical differences in PPG and insulin levels between the individuals taking conjee with steam bread and cooked rice.
8.Correlation of serum uric acid and early renal damage in patients with type 2 diabetes mellitus

The association of serum uric acid ( SUA ) and urine albumin-to-creatinine ratio ( ACR ) in patients with type 2 diabetes mellitus was explored. The ACR abnormal rate was higher in hyperuricemia group( P<0.01 ). ACR level was positively correlated with SUA( P<0.01 ). The general linear model showed that SUA was a risk factor in ACR. Patients with hyperuricemia and type 2 diabetes mellitus were apt to have abnormal ACR and develop early renal damage.
9.Effects of low-glycemic index meal replacement on management of patients with type 2 diabetes mellitus

A total of 127 type 2 diabetic patients were divided into low glycemic index meal replacements (intervention) group and standard food-based diet (reference) group in an experiment for 12 weeks.The results showed that fasting plasma glucose,postprandial 2 h plasma glucose,fasting insulin,and homeostasis model assessment insulin resistance(HOMA-IR) in the intervention group decreased significantly after 12 weeks trial ( P<0.05 or P<0.01 ).However,there were no significant changes in lipid profile and HbA1C in intervention group.In addition,percentage of body fatty,visceral fatty area,and waist-hip ratio also decreased in intervention group( all P<0.01 ).Superoxide dismutase and glutathione levels increased significantly in intervention group by the end of trial (both P<0.01 ),while malondialdehyde was decreased (P<0.01 ).There were no significant changes in the aforementioned indices in the reference group.Weight,body mass index,and waist circumferences were decreased in both groups,but without significant difference between the two groups.
10.Association of HbA1c level with nutritional status in community-based patients with type 2 diabetes

Objective To explore the possible correlation between HbA1c level and nutritional status in community.based patients with type 2 diabetes.Methods A totaI of 219 type 2 diabetes patients were assigned into 2 groups:one with HbAIc<6.5%(n:108)and HbA1f≥6.5%(n=111).Metabolic parameters,food components.and nutritional status were compared between 2 groups.Results (1)49.32% of the participants attained HbA1c<6.5%.(2)HbA1c level was positively correlated with fasting plasma glucose,postprandial plasma glucose,and homeostasis assessment for insulin resistance(HOMA-IR)(r were 0.56,0.49,and 0.20,respectively,P<0.05 or P<0.01),but negatively correlated with high-density lipoprotein-cholesterol(HDL-C)(r=0.16,P<0.05).(3)Linear regression analysis showed that energy,carbohydrate,protein,and fat were the independent risk factors of HbA1c(all P<0.05).(4)Patients with HbA1c<6.5%consumed more fruits.The intake of pure energy-providing foods and protein-,fat-,or saturated fatty acid-rich foods were more frequent in patients with HbA1c≥6.5%(P<0.05).(5)The linear regression revealed that HbA1c level were decreased 0.36%(P<0.10)or 0.46%(P<0.01)by intake of more fruits,roughage and beans,and HbA1c levels were also decreased 0.42%(P<0.05)or 0.37%(P<0.10)by intake of less meat or oils.Conclusions In communitybased patients with type 2 diabetes mellitus,the incidence of HbA1c<6.5% remains low,There exists great difference in nutritional status between the groups with high and low HbA1c levels.The impact of diet OB HbA1c level is great.It's necessary to emphasize the importance of diet therapy far better diabetes control.
