1.Stiffness changes in short-segment spine after destabilizing by using the Distraction Reduction Fixation System

BACKGROUND:Self-made Distraction Reduction Fixation System is characterized by Distraction Reduction Fixation functions,which can completely fuse to the design of steel plate,and by no need to provide many surgical instruments,by reduction of bleeding and trauma.OBJECTIVE:To test changes in the stiffness of porcine short spinal segments in integrity and following destabilizing and internal fixation of the Distraction Reduction Fixation System.DESIGN,TIME AND SETTING:The control observation experiment was performed at the Laboratory of Biodynamics,Institute of Onhopedics,General Hospital of Chinese PLA from December 2004 to December 2005.MATERIALS:Twelve fresh porcine lumbar spines(T11-S2 segments)were bought from a market.Upper and lower segments were crossed and inserted with Kirschner wire to make porcine spine samples of the L1-7 segments.METHODS:Stiffness of intact samples was recorded during forward-bending,backward-bending,lateral-bending and axial rotation.Intervertebral discs,facet ioints,anterior and posterior longitudinal ligaments were removed to loose the spine.Stifrness of each motion was measured.Stiffness tests were repeated again after internal fixation on short segments using Distraction Reduction Fixation System.MAIN OUTCOME MEASURES:Stiffness in the intact,unstable and internal fixed spines during each motion.RESULTS:The stiffness iu unstable porcine spine were significantly lower than those in intact porcine spine(P<0.05),but the stiffness after internal fixation was significantly higher than in unstable porcine spine(P<0.05).No significant difference was detected in stifiness during forward-bending,lateral-bending and axial rotation(P>0.05)except that it was significantly higher during backward.bending after internal fixation compared with the intaet spine(P<0.05).CONCLUSION:Biodynamics confirms that the stiffness in short-segment fixation porcine after using Distraction Reduction Fixation System is markedly increased than in unstable porcine spine,except during backward-bending,which is comparable with that in intact porcine spine.

3.Characteristics of Epstein-Barr virus-positive diffuse large B-cell lymphoma and the treatment progress in the era of new drugs

Epstein-Barr virus (EBV) -positive diffuse large B-cell lymphoma, not otherwise specified (DLBCL-NOS), an aggressive B-cell lymphoma associated with chronic EBV infection, is an entity included in 2016 World Health Organization classification of lymphoid neoplasms. EBV-coding RNA (EBER) is expressed in the nucleus of these tumor cells. EBV -positive DLBCL can be mostly found in the elderly who have poor immunochemotherapy effect and short overall survival time, and this poor prognosis is inconsistent with international prognostic index (IPI) stratification. CD30 and programmed death 1/programmed death ligand 1 are expected to be the potential prognostic indicators and therapeutic targets. This paper reviews the relationship between EBV-positive DLBCL-NOS and EBV infection, clinicopathological characteristics, prognostic evaluation factors and treatment in the era of new drugs.
4.Application of poly-D-lysine in assisting suspension cells transfection and colony formation experiments

Objective:To investigate the application of poly-D-lysine (PDL) in assisting suspension cells transfection and colony formation experiments.Methods:The human-derived diffuse large B-cell lymphoma (DLBCL) cell line SU-DHL-4 was selected. The cytotoxicity of different concentrations (0.01, 0.1, 1.0, 10.0 mg/ml) of PDL solution on SU-DHL-4 cells was assessed by using the CCK-8 assay, and cell viability was calculated. SU-DHL-4 cells with normal morphology in logarithmic growth phase were seeded into 96-well plates coated and uncoated with PDL. Cells were then infected with viral suspensions at a multiplicity of infection (MOI) of 50 or 100, and cell morphology was observed. A stable SU-DHL-4 cell model transfected with the luciferase gene was constructed by using lentiviral infection method. The infected SU-DHL-4 cells were divided into the control group (infected with control virus fluid) and the experimental group (infected with virus fluid carrying luciferase gene). The transfected cells were continuously cultured in RPMI 1640 complete medium containing puromycin. A dual-luciferase gene reporter assay kit was used to detect fluorescence intensity. Colony formation experiments with suspension cells were conducted by using PDL-coated dishes. The cells were cultured in RPMI 1640 complete medium containing 0.1% dimethyl sulfoxide (DMSO) (the control group), 0.1 μmol/L etoposide (group A), and 0.2 μmol/L etoposide (group B), and colony formation was observed and colony formation rate was calculated.Results:After 48 h of seeding SU-DHL-4 cells into 96-well plates coated with 0.01, 0.1, 1.0, and 10.0 mg/ml PDL, cell viability was (98.1±1.6)%, (97.1±0.7)%, (91.7±1.5)%, and (83.3±2.0)%, respectively, compared to (100.0±2.7)% in the control group (not adding PDL solution). There were no statistically significant differences between 0.01 and 0.1 mg/ml PDL-coated groups and the control group (both P > 0.05); there were statistically significant differences between 1.0 and 10.0 mg/ml PDL-coated groups and the control group (both P < 0.001). There were statistically significant differences between 1.0 mg/ml and 10.0 mg/ml PDL-coated groups ( t = 5.80, P = 0.004). Therefore, 0.1 mg/ml PDL-coated dishes, which showed no obvious toxicity to SU-DHL-4 cell growth, were used in subsequent experiments. SU-DHL-4 cells seeded in uncoated dishes grew in suspension, while SU-DHL-4 cells seeded in 0.1 mg/ml PDL-coated dishes adhered to the dish bottom and exhibited a monolayer growth pattern. SU-DHL-4 cells seeded in uncoated 96-well plates and infected with viral suspensions at MOI 50 or 100 failed to transfect. However, when SU-DHL-4 cells were seeded in PDL-coated 96-well plates, the originally suspended cells adhered to the dish bottom and exhibited a monolayer growth pattern, and gentle shaking did not dislodge the cells, which maintained intact and smooth cell membrane structures. After puromycin selection, the fluorescence intensity of SU-DHL-4 cells in the experimental group was 106 times that of cells in the control group (26 903±248 vs. 252±11, t = 186.10, P < 0.001), indicating successful transfection. Compared to the control group, colony formation was reduced and smaller in group A, and nearly no colony formation was observed in group B. The colony formation rates in the control group, group A, and group B were (100±6)%, (48±5)%, and 0, respectively, and the differences were statistically significant between groups A and the control group, between group B and the control group ( t = 11.13 for group A, t = 28.53 for group B, both P < 0.001). Conclusions:PDL-coated dishes at a concentration of 0.1 mg/ml can increase the transfection efficiency of suspension cells and assist in colony formation experiments with suspension cells.
5. Analysis of current situation of pediatric medical resources allocation in Beijing from 2013 to 2017

Objective:
To find out the present pediatrician allocation and the burden of diagnosis and treatment in Beijing, so as to provide evidence for effective allocation of pediatricians.
Methods:
Data and indicators were selected from Beijing Health Comprehensive Statistical Information Platform, including Manpower Basic Information Survey of Beijing Health Institutions, Annual Report of Medical Institutions, Beijing Statistical Data on Health Work(Compendium), as well as Beijing Health and Family Planning Statistical Yearbook. The study dynamically analyzed the number of pediatricians, that of pediatricians per 1 000 children, that of outpatients and emergency patients per pediatrician and the number of discharges per pediatrician from 2013 to 2017. Descriptive analysis was carried out on the data.
Results:
In 2017, the proportion of pediatricians accounted for 2.78% of the total number of doctors in the city, the number of pediatric emergencies accounted for 6.47% of the total outpatients and emergencies in the city, and the number of hospitalizations accounted for 5.70% of the total discharge in the city.From 2013 to 2017, the number of outpatients and emergency patients per pediatrician increased by 40.58%, and the number of discharged patients increased by 36.70%. The proportion of hospitalization at tertiary hospitals increased from 80.2% to 85.1%, and that in primary institutions decreased from 6.3% to 1.7%.
Conclusions
The study found such setbacks in the allocation of pediatric medical resources in Beijing, as insufficient pediatricians, unreasonable allocation and excessive burden on pediatricians. We should strengthen the planning and construction of pediatric human resources, improve the training mode of pediatricians, improve the salary level of pediatricians, and ease the burden on pediatricians, so as to provide high-quality medical services for children.
6.Analysis of the Nature,Flavor and Combination Characteristics of the Two-herb Compound Formulas inTreatise on Cold Damage and Miscellaneous Diseases (《伤寒杂病论》)

Based on the natures and flavors of herbal medicinals recorded in Shen Nong's Classic of the Materia Medica (《神农本草经》); Miscellaneous Records of Famous Physicians (《名医别录》), this study analyzed the characte-ristics of the natures, flavors and combination of medicinals of the two-herb compound formulas in Treatise on Cold Damage and Miscellaneous Diseases (《伤寒杂病论》).Finally, 31 compound formulas were included, and it was found that the nature and flavor of the herbs in these two-herb compound formulas are closely related to the functions of the compound formulas, such as the common pairing of the acrid and the sweet herbs to warm yang and transform qi, the acrid and the bitter herbs in pairs to regulate and harmonize cold and heat, the sweet and the bitter in pairs to remove dampness and clear heat, the acrid and the acrid in pairs to arrest vomiting and direct qi downward, and the sour and the sweet in pairs to slow the urgent and relieve pain. Regardless of the deficiency or excess nature of the disease, the corresponding two-herb compound formulas often aim to reinforce healthy qi while eliminating pathogenic factors, with some formulas showcasing a unique correspondence between the disease pattern and the symptoms addressed.
