1.Clinical observation of fiudarabine, cytarabine and granulocytecolony-stimulating factor regime in the treatment of children with refractory and relapsed acute leukemia

Objective To observe the clinical efficacy and adverse reaction of the combination of fiudarabine,cytarabine and granulocytecolony-stimulating factor (G-CSF) (FLAG regime) therapy for refractory and relapsed acute leukemia in children. Methods From 2004 to date, a total of 9 patients with relapsed and refractory acute leukemia patients in our hospital accepted the treatment, in 9 cases 8 cases were AML,1cases were ALL; in 9 cases 5 cases were refractory acute leukemia, 4 cases were recurrent acute leukemia.Results Among the 9 cases,6 cases with 1 cycles of chemotherapy achieved complete remission (CR),CR rate was 66.7 % (6/9); partial remission (PR) rate was 22.2 % (2/9),total efficiency (CR+PR) was 88.9 %.In 6 CR patients 2 underwent hematopoietic stem cell transplantation, are disease-free survival; this regimen' s main adverse reactions were infection,bone marrow depression and gastrointestinal reaction.Conclusion The remission rate of FLAG regimen in the treatment of children with refractory and relapsed acute leukemia is relatively high, adverse reactions were tolerable; the FLAG program is a choice for the treatment of children with refractory and relapsed acute leukemia,which provides the opportunity for subsequent hematopoietic stem cell transplantation.
2.Effects of bone marrow stromal cells and antibody against late antigen-4 combined with Ara-C on proliferation and apoptosis of HL-60 cells

Objective To explore the bone marrow stromal cells,anti-late antigen-4 (VLA-4) antibody (aVLA-4),cytarabine (Ara-C) on the proliferation and apoptosis of leukemia HL-60 cells.Methods The experiment was divided into five groups:HL-60 cells were cultured alone (control group),HL-60 cells and stromal cells group (stromal cells group),HL-60 cells + stromal cells + aVLA-4 (antibody group),HL-60 cells + stromal cells + Ara-C group (drug group),HL-60 cells + stromal cells + aVLA-4 + Ara-C group (antibody +drug group).Cell proliferation or inhibition rate was detected by CCK-8 method,the HL-60 cells apoptosis was detected by flow cytometry.The expression of anti-apoptotic gene bcl-2 in HL-60 cells was determined by Western blot.Results After 24 h and 48 h,treatment,the number of the stromal cells group HL-60 cells were higher than that of the control group with significant difference cultured [(7.2±0.3)×1O5/ml vs (5.3±0.4)×105/ml,(8.4±0.2)×105/ml vs (6.8±0.3)×105/m1,P < 0.001],while the HL-60 cell proliferation inhibition rate [(24.3±2.1) %,(37.0±2.6) %,(65.6±3.8) %] and apoptosis rate [(5.7±0.6) %,(8.0±0.5) %,(10.4±0.9) %,(16.5±0.7) %] of antibody group,drug group,antibody + drug group were higher than the control group with a difference of statistically significant (P < 0.05),and the increase of antibody + drug group was most obvious.With the decreasing of the bcl-2 protein expression,which was most the decrease of antibody + drug group was most obvious.Conclusion Bone marrow stromal cells can stimulate the proliferation of leukemia cells,aVLA-4 interference the interaction between stromal cells and leukemia cells can enhance the chemosensitivity of leukemia cells to Ara-C.
3.Case control study of the correlation between obesity and colorectal adenoma

Objective To investigate the association between body mass index(BMI),waist circumference(WC),waist hip ratio(WHR) and the risk of colorectal adenoma,which was considered as a precancerous lesion.Methods Subjects aging from 25 to 88 years old who underwent colonoscopy at Tongji Hospital from December 2006 to December 2007 were selected and assigned into the adenoma group( n =250) and the control group(n=289) according to the findings of the colonoscopy.The body height,weight,waist and hip circumference of every subject were measured respectively.The logistic multi-factors regression analysis was applied to analyze the data.Results When obesity was determined by BMI or WC,the risk of adenorrm in pure obesity group and abdominal adiposity group was 2.48(95%CI = 1.19~5.20,P<0.05) and 1.75(95%CI=1.15~2.66,P<0.01 ),respectively.The corresponding value in male was 4.10(95%CI = 1.26~13.31,P<0.05) and 1.70(95%CI = 1.00 -2.88,P<0.05).The risk of advanced and non-advanced adenoma in pure obesity was 2.71(95%CI=1.01~7.29,P<0.05 ) and 2.39(95%CI=1.05~5.47,P<0.05) ; the risk of non-advanced adenonm in abdominal adiposity group was 2.03(95%CI=1.25~3.28,P<0.01),but no significant difference in risk of advanced adenoma was detected.When obesity was determined by WHR,no significant difference was found in any regarding.Conclusion Obesity and abdominal adiposity are associated with the risk of colorectal adenoma,beth advanced and non-advanced,which is more obvious in male.
4. Determination of Sevoflurane, Isoflurane and Enflurane in the air of workplace by gas chromatography

Objective:
To establish a solvent desorption gas chromatographic method for determination of Sevoflurane, Isoflurane and Enflurane in the air of the Workplace.
Methods:
Sevoflurane, Isoflurane and Enflurane were collected with activated carbon tube and desorbed with dichloromethane, separated with DB-1 capillary columns, and then detected with flame ionization detector.
Results:
The linearity ranges were 1.9-304.8 μg/ml for Sevoflurane, 2.1-300.4 μg/ml for Isoflurane and 1.7-305.2 μg/ml for Enflurane, The correlation coefficient was both >0.999. Their limits of detection were 0.6 μg/ml, 0.6 μg/ml and 0.5 μg/ml, and Their limits of quatification were 1.9 μg/ml, 2.1 μg/ml and 1.7 μg/ml, and their minimum detectable concentrations were 0.1、0.2 and 0.1 mg/m3 per 4.5 L of air. 7.Meta-analysis of the Effects of National Essential Medicine System on the Utilization Rate of Hormone Drugs in Primary Medical Institutions

OBJECTIVE:To evaluate the effects of the implementation of National Essential Medicine System on the utilization rate of hormone drugs in primary medical institutions in China systematically.METHODS:Retrieved from CJFD,Wanfang database and VIP,the literatures about the use of hormone drugs were collected before and after the implementation of National Essential Medicine System.Meta-analysis was performed by using Stata 13.0 software after utilization rate dam extraction and quality evaluation with Cochrane system evaluator manual 5.1.0.RESULTS:A total of 20 literatures were included.Results of Meta-analysis showed that utilization rate of hormone drugs after the implementation of National Essential Medicine System was significantly lower than before implementation,with statistical significance [RD =-0.03,95 % CI (-0.05,-0.02),P< 0.001].Results of subgroup Meta-analysis of utilization rate of hormone drugs in different areas before and after the implementation of National Essential Medicine System showed that the utilization rate of hormone drugs in eastern areas was significantly lower after the implementation of National Essential Medicine System than before [RD=-0.06,95% CI(0.09,-0.03),P=0.001] with statistical significance.There was no statistical significance in the utilization rate of hormone drugs in middle area [RD=-0.02,95 % CI(0.06,0.02),P=0.235] or western area [RD=-0.01,95% CI (-0.02,0),P=0.122] before and after the implementation of National Essential Medicine System.CONCLUSIONS:The implementation of National Essential Medicine System reduces the utilization of hormone drugs in primary medical institutions.The effect of the eastern area is more obvious than in middle and western area.
8. Direct determination of manganese in urine by Graphite Furnace Atomic Absorption Spectrometry

Objective:
To establish a method for the determination of manganese in urine by graphite furnace atomic absorption spectrometry (AAS) without the use of matrix modifier.
Methods:
The urine samples were 5 times diluted with 1% nitric acid then directly determined by AAS. Zeeman was used for background correction.
Results:
The linear range for determination of manganese in urine was 5~60 μg/L (urine) . The correlation coefficient was greater than 0.995 with the detection limit of 1.5 μg/L and with the lower limit of quantification of 5.0 μg/L. The relative standard deviations (RSDs) of within-run precision was between 1.1%~4.3%, the RSDs of between-run precision was between 3.3%~7.0%. The average recovery was 102.6%. The samples can be stored for 14 days at room temperature, 4℃, -8 ℃ and -35 ℃.
Conclusion
The method is feasible for determination of manganese in urine.
9.Efficacy observation of massage nursing intervention on children patients with bronchial pneumonia

Objective To study the clinical efficacy of massage nursing intervention on children patients with bronchial pneumonia .Methods Totals of 80 children patients with bronchial pneumonia during the period from May 2012 to May 2013 were randomly divided into the control group ( routine nursing ) and the observation group ( massage nursing intervention ) . The clinical efficacy , hospitalization time , cost of hospitalization , and nursing satisfaction in the two groups were observed and compared .Results Compared with the control group, the effective rate was significantly increased (95.0% vs 80.0%), the hospitalization time [(6.5 ±1.5) vs (8.5 ±2.0)d] and cost of hospitalization [(1 900 ±100) vs (2 650 ±150)yuan] were significantly reduced , and the nursing satisfaction degree was obviously increased in the observation group ( great satisfaction:32 vs 24;satisfaction:6 vs 8;dissatisfaction:2 vs 8), and the differences were statistically significant (χ2 =4.11,t=2.21,3.47,Z=4.11,respectively;P<0.05).Conclusions For the children patients with bronchial pneumonia , the massage nursing intervention can significantly increase the clinical efficacy , promote early recovery .
10.A novel double heterozygote of HBB c.219T>A;220G>T: gene diagnosis and pedigree analysis.

OBJECTIVETo identify a novel hemoglobinopathy applied by direct sequencing and clone sequencing.
METHODSEDTA anticoagulated blood of proband and his parents were analyzed by hematology analyzers and Capillarys hemoglobin electrophoresis (CE). Then thalassemia genetypes were screened by gap-PCR and reverse dot blot (RDB). Proband was suspected with abnormal hemoglobin combine alpha beta compound thalassemia. The mutation of beta-globin was identified by direct sequencing and clone sequencing.
RESULTSHb analysis showed that probands Hb A2 variant was eluted in Z (C) zone and his father's in Z (A2) zone on CE,and proband's mother elevated HbA2 of 4.6%. Screened by RDB, the proband was CD71-72(+A) homozygote and showed the mismatch with his parents. Through direct sequencing and clone sequencing, we deduced that our proband inherited the mutations of HBB c.[219T>A;220G>T] from his father and inherited the Southeast-Asian deletion and HBB c.216-217insA from his mother.
CONCLUSIONA novel double heterozygote of HBB c.[219T>A; 220G>T] was identified in south China. This mutation enriches the beta-thalassemia gene mutation spectrum in Chinese population.
