1.Individual Nursing for Persistent Headache after Interventional Embolization for Intracranial Aneurysm

Objective To explore the effect of individual nursing care on persistent headache in patients with intracranial aneurysm who accepted interventional embolization. Methods 100 patients with intracranial aneurysms were divided into observation group (n=50) and control group (n=50). The routine headache nursing care was taken in the control group and the individual nursing care was applied in the observation group. They were assessed with Visual Analogue Scale (VAS). Results The score of VAS reduced more in the observation group than in the control group (P<0.05), and the headache endurance and the satisfaction for nursing care were significantly better in the observation group than in the control group (P<0.05). Conclusion The individual nursing may relieve the pain, and improve the endurance of headache and the satisfaction for nursing care.
2.Application of clinical nursing pathway in clinical teaching for nursing practice students in neurosurgery department

Objective To explore clinical nursing pathway(CNP) teaching method for nursing practice students.Methods Nursing practice students ( n =120 ) were randomly divided into two groups:experimental group and control group.CNP teaching method was applied in experimental group while traditional teaching method was used in control group.The test results of theory,operational skill,comprehensive quality,patient's satisfaction degree and nursing mistake between the two groups were compared.Results The results of theory,operational skill,comprehensive quality,patient's satisfaction degree in experimental group were obviously better than those in control group(P <0.05),the result of nursing mistake in experimental group was greatly decreased compared with that in control group(P < 0.05).Conclusion CNP teaching method is superior to traditional teaching method and it greatly improve the teaching quality.
3.Hypertension,Diabetes Mellitus and the Cognitive Disorder in Cerebrovascular Patients:915 Cases Investigate

Objective To investigate the incidence of the cognitive disorder in cerebrovascular patients and to its risk factors.Methods Cerebrovascular patients were surveyed with Mini-Mental State Examination(MMSE)Chinese-version in county or above hospitals among 23 provinces(cities).Results 915 cerebrovascular patients were surveyed and 281 patients(30.71%)were of cognitive disorder.And the incidence of the patients with hypertension or/and diabetes mellitus was significantly higher than the others(P<0.01).Conclusion The incidence of cognitive disorder is high in cerebrovascular patients,and the hypertension and diabetes mellitus are both the risk factors.
4.Cloning and Identification of the Gene Fragments of Paragonimus westermani

Objective To screen and identify the recombinants from the cDNA library of the adult Paragonimus west-ermani (PwA) for immunodiagnosis and immunoprophylaxis. Methods PwA cDNA library was screened with the PwA antigen immunized rabbit sera(IRS) pre-absorbed by the extract of E. coli XLl-Blue. The recombinants from positive clones were amplified by PCR, sequenced and cut off by KpnI/BarnHI and, then sub-cloned into pRESETB vector. The fusion protein was expressed,analysed by SDS-PAGE and identified by Western blotting with immune rabbit serum against worm antigen of Paragonimus westerrnani. Results The inserted cDNA fragment from the positive clone Pw-2 was about 800 bp, which contained an open reading frame(ORF) encoding Pw pre-procathepsin L belonging to cysteinase family. Expression product of Pw-2 was a fusion protein of 32 kDa, which can be recognized by immune rabbit serum against worm antigen of Paragonimus 晈esterrnani. Conclusion A recombinant plasmid Pw-2 encodes Pw pre-procathepsin L is constructed.
5.Hemodynamic responses of esmolol to nasotracheal intubation with fiberbronchoscope

Objective To evaluate the hemodynamic responses of esmolol to nasotracheal intubation with fiberbronchoscope(FOB). Methods Thirty-five ASAⅠorⅡpatients undergone stomatology and otorhinolaryngology surgery were randomly divided into fiberoptic nasotracheal intubation esmolol group (esmolol group) and fiberoptic nasotracheal intubation group (control group). The intravenous administration of esmolol 1mg?kg-1 was performed 2 min before tracheal intubation in esmolol group. Noninvasive SBP,DBP,MBP,HR and SpO2 were recorded before and after anesthetic induction,at intubation and 1,2,3,4,5 min after intubation. Objective To analyze the relationship between pillow height and cervical intervertebral disc stress in lateral decubitus position of the human body using three-dimensional finite element method,and to provide the basis for appropriate pillow selection.Methods A cervical spine CT scan was performed on a normal male volunteer.The medical 3D image reconstruction software (Mimics),reverse engineering software (Geomagic Studio) and finite element software (MSC.Patran) were collectively used to establish three-dimensional finite element models of the whole cervical spine under three lateral decubitus conditions,includes pillow height of 10 cm (cervical spine leftward flexion),17 cm (cervical spine neutral),and 25 cm (cervical spine rightward flexion).Moreover,the finite element analysis and calculation of the three models were conducted.Results The Von-Mises stress,maximal principal stress and maximum shear stress of the model in cervical spine neutral (pillow height=17 cm) were all significantly lower than those of the cervical spine lateral model.Conclusions When the cervical coronal plane is in neutral position,the cervical discs can achieve the best stress distribution state.In lateral decubitus position of the human body,a appropriate pillow height should make the cervical vertebrae in neutral position.
7.Comparison of dosimetry and toxicities between postoperative fixed-field intensity-modulated radiotherapy and image-guided radiation therapy/volumetric modulated arc therapy for cervical cancer

Objective To compare the dosimetry and toxicities between postoperative fixed-field intensity-modulated radiotherapy (FF-IMRT) and image-guided radiation therapy/volumetric modulated arc therapy (IGRT-VMAT) for cervical cancer.Methods A total of seventy patients with stage I b-Ⅱa postoperative cervical cancer who had high risk factors,were divided into FF-IMRT (FF-IMRT group,n =35)and IGRT-VMAT (IGRT-VMAT group,n =35),to compare the difference of target dose and adverse reaction between the two groups.Results In the IGRT-VMAT group,the interfractional setup errors in the x,y,and z axes were (0.25±0.14) cm,(0.26±0.16) cm,and (0.24±0.18) cm,respectively;the intrafractional setup errors in the x,y,and z axes were (0.1±0.09) cm,(0.12±0.09) cm,and (0.11±0.09) cm,respectively;the margins in the x,y,and z axes were 0.75 cm,0.84 cm,and 0.78 cm,respectively.Under the same dosimetric conditions,the IGRT-VMAT group was superior to the FF-IMRT group in terms of conformity index,treatment time,and number of monitor units (P=0.000).The Dmean and volume receiving high-dose irradiation for the bladder,rectum,and small intestine were significantly lower in the IGRT-VMAT group than in the FF-IMRT group (P=0.000).Compared with the FF-IMRT group,the IGRT-VMAT group had a significantly reduced incidence of acute and chronic gastrointestinal,urinary,and hematologic toxicities (P< 0.05).Conclusions IGRT-VMAT can correct setup error online,shorten the treatment time,reduce the dose to organs at risk,and alleviate acute and chronic toxicities,and is especially suitable for patients with postoperative small bowel position changes.
8.Prognostic analysis and comparison of the efficacy and complication of different radio-therapy techniques for postoperative early cervical squamous cell carcinoma

Objective:To explore the factors affecting the postoperative prognosis of early cervical squamous cell carcinoma and inves-tigate the different radiotherapy techniques. Methods:A total of 100 postoperative patients of early cervical squamous cell carcinoma treated in Affiliated Tumor Hospital of Xinjiang Medical University and Affiliated Tumor Hospital of Sun Yat-sen University from March to September 2013 were enrolled in our study. The prognostic factors for these patients were analyzed. After being subjected to differ-ent postoperative radiotherapy techniques, the efficacy and complications of the techniques were assessed for patients, including 50 cases of conventional radiotherapy (CRT) and 50 cases of image-guided radiotherapy (IGRT). Results:The 3-year overall survival rate was 89%, and the 3-year disease-free survival rate was 78%. The 3-year overall survival rates of the CRT and IGRT groups were 78.57%and 89.06%, respectively. The difference between these rates was statistically significant (P=0.034). The 3-year survival rates of the CRT and IGRT groups were 66.67%and 87.36%, respectively. The difference was statistically significant (P=0.042). The incidence of ear-ly and late complications in the CRT group was higher than that in IGRT group, and the difference of P<0.05 was statistically significant. Multivariate analysis showed that postoperative radiotherapy, parametrial invasion, lymph node metastasis, lymphovascular space in-vasion, perineural invasion, and deep stromal invasion were the independent risk factors for the prognosis of patients with early stage cervical squamous cell carcinoma. Conclusion:Early prognosis of cervical squamous cell carcinoma results from the interaction of mul-tiple factors. The 3-year survival rate of the IGRT group was significantly better than that of the CRT group in the early stage of cervical squamous cell carcinoma. IGRT alleviates acute and chronic toxicities and helps improve the quality of life of patients.
