1.Clinical effect of 58 patients with severe craniocerebral injury treated by surgery

Objective To investigate the clinical effect of standard large bone flap decompression for severe craniocerebral injury.Methods This study was performed in 58 patients with severe traumatic brain injury,according to different surgical methods,the patients were divided into control group and study group,29 cases in each group.The control group was treated with conventional bone flap craniotomy,and the study group was treated with large bone flap decompression surgery.The clinical effect and complications were compared between the two groups.Results After operation,the patients were followed up for 5 months.The mortality of the study group (10.34%)was significantly lower than that of the control group (27.59%),and the difference was statistically significant (χ2 =3.98,P <0.05).In addition,the good rate of the study group (55.17%)was significantly higher than that of the control group (27.59%),and the difference was significant (χ2 =4.55,P <0.05).The preoperative intracranial pressure had no significant difference between the two groups.While at postoperative 1d,3d and 5d,the intracranial pressure of the study group was significantly lower than that of the control group,and the difference was statistically significant (t =3.55,4.02,6.12,all P <0.05).The scores of GOS and BAI in the study group were significantly higher than those in the control group (t =5.02,4.21,P <0.05 ).The incidence rates of complications such as cerebrospinal fluid leakage,trauma of cerebral infarction,trauma of epilepsy,hydrocephalus,wound infection of the study group were sig-nificantly lower than those of the control group,and the differences were statistically significant (χ2 =4.00,5.02, 456,4.22,3.99,all P <0.05).Conclusion Compared with conventional craniotomy,open standard big bone flap decompression can significantly reduce the mortality rate,decrease intracranial pressure,and improve the quality of life of patients,it has better curative effect and prognosis for patients with severe craniocerebral injury,and is worthy of clinical application.
2.Application effect of three-point small incision in double blepharoplasty

Objective To explore the application effect of three-point small incision in double eyelid plasty. Methods From October 2013 to October 2016,160 patients with double blepharoplasty in the Central Hospital of Linfen were selected and randomly divided into traditional group(treated with traditional surgery) and small incision group(treated with three-point small incision in double eyelid plasty) according to the digital table,with 80 cases in each group. All patients were followed up for half a year. The pain score at postoperative 24h,wound healing time, swelling regression time,satisfaction score and the incidence of complications were compared between the two groups. Results The pain score at postoperative 24h of the small incision group was (2. 75 ± 0. 32)points,which was signifi-cantly lower than (4. 15 ± 0. 41)points of the traditional group,and the swelling subsided time,postoperative incision healing time of the small incision group were (4. 24 ± 0. 41)d,(5. 27 ± 0. 32)d,respectively,which were significantly shorter than those of the traditional group[(6. 12 ± 0. 32) d,(8. 17 ± 0. 45) d],the differences were statistically significant(t=24. 076,32. 331,46. 974,all P <0. 05). After follow -up for half a year,the satisfaction score of patients in the small incision group was ( 89. 75 ± 4. 32 ) points, which was significantly higher than ( 82. 15 ± 4. 10)points of the traditional group,the difference was statistically significant(t=11. 413,P<0. 05). The incidence rate of complications in the small incision group was 0. 00% , which was significantly lower than 7. 50% in the traditional group(χ2=6. 233,P <0. 05). Conclusion The effect of three -point small incision intervention on double eyelid plasty patients is significant,and the satisfaction rate of the patients is higher.
3.Establishment of HPLC method for determination of SN38-PA and SN38 in rat plasma

Objective To develop an HPLC method for the simultaneous determination of 7-ethyl-10-hydroxycamptothecin-10-palmitic acid ester(SN38-PA)and its active metabolite 7-ethyl-10-hydroxycamptothecin(SN38)in rat plasma. Methods The inter standard was 10-hydroxycamptothecin. The protein in plasma was precipitated with methanol after acidification with formic acid. SN38-PA and SN38 were separated on Agilent C18 column(4.6 mm×250 mm,5μm) with gradient elution by using the mobile phase of methanol-0.2% formic acid solution. The flow rate was 1 ml/min. The detection wavelength was set at 372 nm. The column temperature was maintained at 30℃. Results The linear ranges for SN38-PA and SN38 were 0.25-62.5(r=0.9998) and 0.05-12.5 μg/ml (r=0.9997) respectively. The limits of quantification were 0.18 and 0.04 μg/ml, respectively. The average relative recovery of SN38-PA and SN38 were 95.89% and 97.03%. The average absolutely recovery of SN38-PA and SN38 were 99.54% and 99.84%. The RSD for intra-day and inter-day were both less than 3%. Conclusion The method is fast, convenient, accurate and sensitive, so it can be used for determination of SN38-PA and SN38 in vivo.

6.Study on the Quality Control of Rukang Capsule

Objective To formulate the quality standard of Rukang Capsule. Method The TLC of Zhebeimu, Xuanshen, Ruxiang and Tiandong were done separately. The HPLC-ELSD was used to determine the content of Astragaloside in Mongolian Milkretch Root. Result There were the same spots between the controls and samples and an excellent linear relationship for 1.44~4.32 ?g to the content determination of Astragaloside. The average recovery can reach 100.3% with 0.5% of RSD. Conclusion This method is reliable and accurate to application, and can be used for the quality control of the preperation.
7.Relationship between cyclooxygenase-2 and vascular endothelial growth factor with liver sinusoidal capillarization of chronic hepatitis B

Objective To study the relationship between expression of cyclooxygenase-2 (COX-2) and vas-culax endothelial growth factor (VEGF) with liver sinusnidal capillarization (LSC) of chronic hepatitis B (CHB). Methods We studied liver biopsies from 200 patients with CHB COX-2 ,VEGF immunohistochemical stain were ob-served to accomplish relationship between expression of COX-2,VEGF and LSC. Results LSC occupy above 80% in the group. There were manifestation in mild-LSC (focal) , middle-LSC (sheet-shape) and severe-LSC (widespread). Electron microscope shown the laceration in the endothelium of sinuses and formation of basal lamina and budding for-mation lumen of blood vessel and fat-storing cell convert myofibroblast. Expression of COX-2, VEGF, Co-Ⅳ and retic-ulum, collagen and elastic fibers with mild or severe in LSC is manifest locking relate. Conclusion Increased ex-pression of COX-2 ,VEGF in liver tissue of CHB may facilitate LSC and hepatic fibrosis.
8.?-adrenoceptor desensitization and changes of calcium transient in rat ventricular myocytes following chronic hypoxia

AIM: The present study was to investigate the time course of ?-adrenoceptor desensitization and changes in the calcium transient in rat ventricular myocytes following chronic hypoxia. METHODS: With the spectrofluorometric method, the intracellular calcium( i) transient and its response to ?-adrenoceptor stimulation were determined in the single right ventricular myocytes, loaded with Fura-2.RESULTS: After 2-3 weeks of chronic hypoxia, the amplitudes of electrically induced i transient and caffeine-induced i transient started to decrease and the duration of i transient prolonged. The enhanced electrically induced i transient evoked by isoprotrenol was also decreased. After 3 or 4 weeks of chronic hypoxia, these changes aggravated gradually. After 8 weeks of chronic hypoxia, the changes of all these parameters were convalescent, meanwhile there was no significant difference compared with that of 4 weeks group. CONCLUSIONS: After 2-4 weeks of chronic hypoxia, the ?-adrenoceptor desensitization occurs, the underlying mechanism is related to the decreased function of L-type of calcium channel, ryanodine receptor-operated calcium channel and calcium ATPase, which is responsible for the decreased cardiac functionl. At 8 weeks of hypoxia, the heart is in adaptation and compensatory process.
9.A Clinical and Experimental Study on the Use of Decoction of Jie Du Hua Yu Tang in the Treatment of Psoriasis

Objectives To study the clinical effects and therapeutic mechanisms of the application of Chinese traditional medicine,the Decoction of Jie Du Hua Yu Tang,in the treatment of psoriasis.Meth-ods The clinical effects were observed in50patients with psoriasis vulgaris,to whom the Decoction of Jie Du Hua Yu Tang was given.The mice were used as experimental models whose tail scale epidermis was nat-urally lacking granular layer,similar to psoriatic epidermis.The mice were fed with Jie Du Hua Yu Tang and the effects on the formation of granular layer in their tail scale epidermis were noticed.Results The clinical cure rate was42%and the total response rate was90%with the Jie Du Hua Yu Tang.There was a signifi-cantly strong promoting effect on the formation of granular layer in mouse tail scale epidermis with the use of Jie Du Hua Yu Tang,in comparison with the control group.Conclusions The Decoction of Jie Du Hua Yu Tang is effective for the treatment of psoriasis vulgaris.The decoction could promote the formation of granu-lar layer in mouse tail scale epidermis.The therapeutic mechanism of the decoction may be related to the inhibition of proliferation of epidermal cells.
10.Correlation between overexpression of matrix metalloproteinase-9 with acute graft injury after small-for-size liver transplantation

Objective Portal hypertension and ischemia/reperfusion (I/R) have been implicated in small-for-size liver graft dysfunction. Matrix metalloproteinases-2 (MMP-2) and MMP-9 are critically involved in hepatic I/R injury. The goal of this study was to investigate the role of MMP-2 and MMP-9 in acute small-for-size graft injury. Methods 108 rats were divided into three groups:100 % (full-size), 50 % (half-size) and 25 % (quarter-size) liver transplantation groups. Blood and liver samples were collected to assess liver function, hepatic malondialdehyde (MDA) content, tissue myeloperoxidase (MPO) activity and histological changes. ELISA, real-time PCR, gelatin zymography, and immunohistochemistry were used to determine the expression pattern of MMP-2 and MMP-9 in liver grafts. Results The expression levels of MMP-9 were significantly higher in quarter-size and half-size grafts than those in full-size liver grafts 6, 12, and 24 h after reperfusioa And theelevated levels of MMP-9 were related to graft size inversely. However, MMP-2 was expressed and remained in all groups invariably. MMP-9 overexpression was accompanied by extensive liver I/R injury, as evidenced by significant increases in hepatic microscopic damage scores, MDA content,MPO activity and liver function levels. Furthermore, MMP-9 was found mainly to locate around periportal area. The presence of the active form of MMP-9 was significantly higher in small-for-size grafts, which was correlated with sinusoidal dilatation, congestion and hemorrhage. Conclusion These results support critical function of MMP-9 in acute small-for-size liver graft injury. Moreover,portal hypertension may be a crucial trigger for expression and activation of MMP-9.
