1.Treatment of Senile Intervertebral Disc Herniation with Acupuncture, Tuina and Traction

Objective: To observe the therapeutic effect of treating senile intervertebral disc Herniation with acupuncture, tuina and traction and with physiotherapy and traction. Methods: Altogether 150 cases above 65 years old were randomly allocated into treatment group and control group, each containing 75 cases. Traction, acupuncture and tuina therapy were adopted in the treatment group, while traction and physiotherapy were adopted in the control group. Results: After one-month treatment, the recovery rate of the treatment group were 70.7%, which was significantly better than 53.3% of the control group (P< 0.05). Conclusion:Combination of acupuncture, tuina and traction has very good efficacy in treating senile intervertebral disc Herniation.
2.Relationship of cytokines and cytokine receptors gene polymorphisms with acute rejection in kidney transplantation recipients

Objective To study the relationship of cytokines and cytokine receptors gene polymorphism with acute rejection in kidney transplantation recipients,whose 21 single nucleotide polymorphisms in 5 kinds of cytokines and their receptors were tested with cytokine oligonucleotide array.Methods According to the allele sequences of 21 gene polymorphisms of IL-4,IL-6,IL-10,TNF-?,TGF-?_1 and their receptors,58 oligonucleotide probes were synthesized. A pair of group special primers labeled by the Cy5 were designed and were used in the PCR. The labeled PCR products with Cy5 were hybridized with array. The signals were scanned by a scanner and analyzed by image software. Genomic DNA samples from the peripheral blood lymphocytes of 144 kidney transplant recipients were tested by this array. To study the protective effect and preliminary mechanisms of asiatic acid against oxygen-glucose deprivation/reoxygenation (OGD/R) injury of PC12 cells, Na2S2O4 combined with low glucose induced damage of PC12 cells was served as OGD/R injury model in vitro. MTT method was used to evaluate cell survival. Ultraviolet spectrophotometry was performed to determine lactate dehydrogenase (LDH) leakage, lactic acid (LD) content, intracellular superoxide dismutase (SOD), malonyldialdehyde (MDA), and cellular Caspase-3 activity. Flow cytometry was applied to assay cell apoptosis. Na2S2O4 combined with low glucose induced significant cell survival rate decreasing compared with normal cells. Cell survival rate increasing, LDH leakage alleviating, LD producing inhibiting, SOD activity promotion, MDA content reducing, cell apoptotic rate decreasing and Caspase-3 activity inhibiting were observed when cells were preincubated with different concentration of asiatic acid (10, 1 and 0.1 micromol x L(-1)). Evident protective effect of asiatic acid against OGD/R injured PC12 cells was verified in our experiment, and the possible mechanisms were related to eliminating free radicals and inhibiting cell apoptosis.

Objective To explore the role of intravenous nutrition and enteral nutrition in the treatment of gastroplegia after gastrectomy. Methods The clinical data of 63 cases of gastroplegia after gastrectomy were retrospectively analyzed. Among the 63 cases, 30 were treated by gastrodynamic drugs and intravenous nutrition(intravenous nutrition group), and the other 33 cases were treated by gastrodynamic drugs and enteral nutrition(enteral nutrition group). Gastrointestinal decompression amount and recovery time of gastroplegia between the two groups were compared. 6.Interventional Therapy of Ischemic Necrosis of Femoral Head

Objective To study the curative effect of ischemic necrosis of femoral head by interventional thrombolysis.Methods By applying Seldinger's technique,the catheter was guided via arteria femoralis with super-selection into circumflexa femoris medialis,lataralis,and acetabuli to performed the interventional thrombolysis in 50 cases(60sides)with necrosis of femoral head.Results All patients were followed-up for 6~48 months,combined with angiography,the clinical symptoms and the change of bone.The curative effect was evaluated.The ratio of excellent and better effect was 91.4%,the ratio of the improvement of angiography was 87%.the angiography showed that after treatment,the supply artery of femoral head was increased in number;the stained area of femoral head was extended,the capillaries were incereased and the reflux of vein was remarkably improved.The ratio of the improvement of clinical symptom was 96.6%.90% of the necrotic area of femoral head showed hyperplasia,sclerosis,and reduction of cystic changes.Conclusion The interventional therapy by leading to target vascular with thrombolysis in treating necrosis of femoral head has remarkable therapeutic effect.
7.The observation of therapeutic efficacy of combination of ACEI and ARB on diabetic nephropathy

Objective To investigate the efficacy of monotherapy or combination of ACEI and ARB in treating diabetic nephropathy(DN). Methods 183 patients with DN were randomly allocated into three treatment groups: benazepril,losartan or the combination of these two drugs for 12 weeks.The changes of blood pressure, proteinuria, serum potassium and creatinine, creatinine clearance(Ccr)were observed before and after treatment. Results Both of benazepril and losartan reduced blood pressure and proteinuria efficiently.And their effects were similar.In contrast, the reduction in proteinuria was greater in the combination therapy than in treatments of either drug alone(P<0.05), but their reductions in blood pressure were achieved to the same level(P>0.05).There were not significant differences in serum levels of potassium, creatinine and Ccr before and after treatment(P>0.05). Conclusion A combination of ACEI and ARB in patients with DN produces a better anti-proteinuric effect than either of the monotherapies.This protection is not dependent on changes in blood pressure.
8.Hepatic implanted tumor in rat: superparamagnetic iron oxide enhanced MR imaging

Objective To make a curve of dose grads liver signal of superparamagnetic iron oxide (SPIO) in normal rats, to compare lesion to liver contrast noise ratio (C/N) and to assess the ability on MR imaging before and after SPIO injection. Methods Group one: 72 normal SD rats divided into 18 specimen groups at random, underwent MR enhancement with 0,2,5……280 ?mol Fe/kg SPIO respectively after plain scanning. The signal to noise ratio of liver was measured and a curve of dose grads liver signal was made in order to find an appropriate dose for enhancement. Group two: 38 rats with 43 implanted liver adenocarcinomas underwent T 1 and T 2 weighted MR imaging before and after the injection of SPIO agent. Evaluations included enhancing characteristics of the tumors, counting the number of lesions and statistical analysis of quantitative changes in the signal intensity of both lesions and normal liver. Results On SPIO enhanced T 2 weighted and T 1 weighted MR images, the signal intensity of liver was reduced to peripheral noise on the dose of 10 ?mol Fe/kg and 40 ?mol Fe/kg respectively. The highest contrast to noise ratio (C/N) could be acquired on the dose of 20 ?mol Fe/kg on enhanced T 2 weighted MR images; its C/N almost doubled compared with that of enhanced T 1 weighted and unenhanced T 2 weighted MR images. The C/N of unenhanced T 1 weighted MR images was lowest. The number of lesions detected on enhanced MR imaging was the same as that of the pathology, and enhanced MR imaging was superior to unenhanced MR imaging in tumors smaller than 3 mm, of 36.3% escaped detection on unenhanced MR images. Conclusion Good enhancement effect of SPIO on T 1 and T 2 weighted MR imaging could be acquired at the dose of 20 ?mol Fe/kg, its detecting ability approached that of gross pathology, and the best contrast to noise ratio was found on T 2 weighted enhanced MR image.
9.Adrenocorticotropic hormone-independent macronodular adrenocortical hyperplasia-report of seven cases report and review of literature

Objective To study the characteristics of clinical and laboratory findings in patients with adrenocorticotropic hormone(ACTH)independent macronodular adrenocortical hyperplasia(AIMAH).Methods The clinical and laboratory findings of 7 cases of AIMAH admitted between 1997 and 2005 were reviewed retrospectively.Results 5 of the 7 patients showed typical clinical features of Cushing's syndrome,including moon face,central obesity,hypertension,etc.A part of patients had hypokalemia(43%)and secondary diabetes mellitus(71%).There was lowering of plasma ACTH levels,and an increased cortisol secretion or disturbance in or circadian rhythm.In 6 patients,a low dose or high dose of dexamethasone suppression tests failed to suppress cortisol secretion.CT scan showed bilateral macronodular adrenal hyperplasia.Pituitary MR imaging for pituitary was negative in 6 patients.Unilateral total adrenalectomy was performed in 2 cases.4 cases were treated by bilateral adrenalectomy.One patient died of stroke before operation.5 cases received glucocorticoid replacement therapy after operation.Pathologic examination showed diffuse yellow or brown nodules measuring 0.3 cm~7.0 cm in diameter in all the specimems of 6 patients who undement operation.There was no recurrence in all the patients ofter surgery during the follow-up period.No Nelson syndrome occurred in these 6 patients postoperatively.Conclusion AIMAH is a rare cause of Cushing's syndrome with unique clinical,CT image and pathological findings.Bilateral adrenalectomy in one stage or in stages,or unlateral adremectomy is indicated on the basis of clinical and CT findings.
10.Pancreatic ductal adenocarcinoma and mass-forming focal chronic pancreatitis: water/fat analysis by using chemical shift method

ObjectiveTo study the water/fat ratio of patients with pancreatic ductal adenocarcinoma (PDAC) and mass-forming focal chronic pancreatitis (MFP),and to provided guide for the clinicians.MethodsThirteen patients with PDAC,8 patients with MFP and 20 healthy volunteers were scanned by GE 3.0T MR IDEAL sequence.The signal strength of outcome images was measured; the water/fat ratio analysis was performed.Two kinds of formula were applied,the first was WF1 =SW/SF,the second was WF2 =( SIP + SOP) / ( SIP - SOP).SW was the signal strength of water,SF was the signal strength of fat,and SIP was the signal strength of in-phase,while SOP was the signal strength of opposite phase.ResultsBy using the WF1 formula,the water/fat ratio of normal pancreas,PDAC,MFP was 7.97 ±0.95,9.94 ±1.19,5.08 ±0.49,respectively.By using the WF2 formula,the water/fat ratio of normal pancreas,PDAC,MFP was 11.51 ± 1.62,13.87 ±1.84,5.73 ±0.65,respectively.The difference among the three groups was statistically significant (P < 0.05 ) under the same formula.The value of WF2 was higher than that of WF1,the difference in PDAC groups was also statistically significant ( P <0.05 ).ConclusionsThe water/fat ratio of pancreas among PDAC,MFP and normal pancreas is different.PDAC has the highest water/fat ratio,followed by the normal pancreas; MFP has the lowest ratio.
