1.CHEMOTHERAPY THROUGH SUPER-SELECTED INTERNAL ILIAC ARTERY INCUBATION ON 36 CASES OF ADVANCED MALIGNANT TUMORS IN GYNECOLOGY

Objective To study the effects of super-selected internal iliac artery incubation in the treatment of advanced gynecological malignant tumors. Methods 36 cases of advanced gynecological malignant tumors (16 ovarian carcinoma, 13 cervical carcinoma, 3 vaginal carcinoma, 2 endometrial carcinoma, 1 choriocarcinoma and 1 leiomyosarcoma of uterus) were treated with this method for 48 times. 31 of these 48 times were embolized in bilateral iliac arteries, 13,2 and 1 cases the inferior mesenteric artery, the hepatic proper artery and the ovarian artery were embolized respectively. Results 4 cases showed complete response, 23 showed partial response, no change in 5 cases and in the rest 4 cases the disease was still in progress. The main side effects were slight gastro-intestinal reactions and embolism symptoms but no incubation complication occurred. Conclusion The super-selected artery of incubation will give high local density of chemotherapeutic drugs and low concentration in general circulation. The effects are better in cervical, endometrial and vaginal carcinomas than in ovarian carcinoma.
2.Effect of lentivirus mediated HER2-shRNA on chemosensitivity of human ovarian cancer SKOV3 cells

Objective To investigate the lentivirus-mediated RNA interference(RNAi) reduced human epidermal growth factor 2 (HER2) gene expression in ovarian cancer chemosensitivity.Methods Specifically targeted short hairpin RNA HER2 gene ( shRNA) lentiviral expression vector and negative control sequences lentiviral vector were constructed.stable expression of HER2-shRNA in SKOV3 cell line ( KD) and negative control cell line (NC) were established.The uninfected SKOV3cell lineserved as blank control(CON)group, sensitivity to changes in three kinds of cells to cisplatin chemotherapy in the HER2 gene expression was detected by RT-PCR and Western blot technique, CCK-8 assay CON and KD cells strains were established xenograft model in nude mice in each group to detect the expression of the HER2 protein, as well as by cisplatin chemotherapy tumor size. Results Expression results showed that a stable cell line HER2-shRNA and negative control cell lines had been successfully establish by fluorescence and flow cytometry.RT-PCR results showed KD cells relative HER2 mRNA expression levels compared with the NC group, the difference was statistically significant (P<0.01), whereas no significant difference when compared with the CON group, HER2 mRNA KD Group inhibition rate was (95.5 ± 0.12)%.After transduction specific HER2-shRNA ovarian cancer tumor cells, HER2 protein expression was significantly reduced, growth had slowed down significantly after cisplatin treatment ( P<0.01 ) .Conclusion Lentivirus-mediated HER2-shRNA can effectively inhibit the expression of the HER2 gene, increase chemosensitivity to cisplatin in ovarian cancer.
3.Effects of CYP2C19 genetic polymorphism on the pharmacokinetics and pharmacodynamics of omeprazole in Chinese volunteers

4.Early histopathological changes in explosive wounds at canine limbs following seawater immersion

Objective To observe early histopathological changes in explosive wounds at canine limbs after seawater inmersion.Methods Forty adult dogs,weighing 10 to 15 kg each,were assigned into 2 equal groups.The left hind limb of each dog in both groups was injured by a man-made explosion.The explosive wound was first washed by normal sodium.The 20 injured limbs in the experimental group (EG)were immersed in seawater for one hour while those in the control group (CG) were only exposed naturally for one hour without seawater immersion.All the wounds were covered with sterile dressing without suture.The pus and soft tissue at the wound were taken for pathological examination,bacterial culture and drug sensitivity test on day 3.The time of wound healing was recorded.Tissue sections were taken from the wounds for pathological examination at 4 and 8 weeks.Results Fifteen wounds (75%) were infected in the EG,significantly more than the 8 wounds infected (40%) in the CG( P < 0.05).The wounds healed in a mean time of 38.4 days in the EG,significantly longer than the mean time for wound healing (23.1 days) in the CG ( P < 0.05).In the EG,Vibrio infection caused more serious tissue necrosis and inflammatory reaction than Bacillus and coccus did.At 4 and 8 weeks,tissue necrosis and inflammatory reaction in the EG were worse than those in the CG.Conclusion Seawater immersion can lead to increased infective tissue necrosis and inflammatory reaction in an explosive wound,as well as longer time for wound healing.
5.The effects of acetylcysteine on proliferation and collagen synthesis of human lung fibroblast

Objective To study the effects of N-acetylcysteine (NAC) on the proliferation, apoptosis and collagen synthesis of human lung fibroblast (HLF). Methods HLF was primarily cultured in complete medium of DMEM/F12. Different concentrations of NAC (5,10,20,40mmol/L) were administrated. Cell proliferations were tested by methylthiazolyltetrazolium (MTT) ,apoptosis and cell cycle were detected with Flow cytometer and mRNA and 40 mmoi/L )after 24 hours,the apoptosis rates were (34.38±5.80)%, (37.72±3.10)%, (44.05± 4.52) % and (59.18±5.24) % ) respectively, significantly higher than that of the controls (3.92±1.24) % pression of type Ⅰ procollagen in HLF was decreased significantly after administration of NAC at 5, 10,20, 40 mmol/L respectively (P < 0.01 ). Conclusion Administration of NAC induces apoptosis and directly inhibites the proliferation and the collagen synthesis of HLF.
6.Application of Left Posterior Lateral Thoracotomy in Reoperation of Mitral Valve

Objective To explore the application of left posterior lateral thoracotomy in the reoperation of mitral vavle. Methods Between April 1995 and April 2003, 11 cases of postoperative recurred mitral valve diseases received reoperation through a left posterior lateral thoracotomy under cardiopulmonary bypass via descending aorta for artery line and main pulmonary artery cannulation for venous line. Results There were no operative and post-operative deaths. The mean CPB time was 138 min. Blood loss of postoperation was less than 300ml in each patient. Conclusion Left posterior lateral thoracotomy in the reoperation of postoperative recurred mitral valve diseases had a minimal adhesiotomy. Extracorporeal circulation via descending aorta and main pulmonary artery cannulation proved easy to do and reliable. This operative approach was simple and safe.

8.Causality assessment of 112 patients with drug-induced liver injury

Objective To evaluate the diagnostic accuracy of drug-induced liver injury over a 10-year period . Methods The 112 cases of drug-induced liver injury were retrospectively studied. Inter-national consensus criteria were applied to assess the causality of suspected drug hepatotoxicity,in which the chronological criteria and elimination of other causes were analysed. Results Among 1127 adult inpatients with acute hepatitis over 10-year period, 112 patients(10%) were recorded as drug-induced liver injury. Based on the causality assessment of 112 cases, either incompatible time to apparent onset of the reaction or incompatible course of the reaction was found in 60 cases(53.6%) and 20 cases(17.9%), respectively. The presence of other possible causes for the reaction was found in 34 patients (37.5%). Therefore, 26 cases(23.2%) of liver injury were related to the incriminated drug, 25 cases(22.3%) unrelated,61(54.5%) were considered to indeterminate cause. Conclusions The international consensus criteria were helpful to identify drug-related or unrelated liver injury, but more than half of reported hepatic adverse drug reactions are still in suspected condition. The current diagnostic methods for drug-induced liver injury are needed to be revised in clinical practice.
9.In Vitro Antibacterial Activity of Meropenem in Gram-negative bacilli

OBJECTIVE To investigate in vitro antibacterial activity of meropenem in Gram-negative bacilli isolated from clinical specimens.METHODS Gram-negative bacilli were isolated from various clinical specimens in our hosptial from Jan 2008 to Dec 2008.The identification test was determined by VITEK-32 Full Automated Microbiology Analyzer,the antimicrobiol susceptibility was detected by K-B method.RESULTS The resistance rate of Enterobacteriaceae to meropenem was extremly low.The resistance rates of Escherichia coli,Klebsiella pneumoniae and Enterobacter cloacae to meropenem were 0,27.6% and 4.2%.The resistance rates of Stenotrophomonas maltophilia,Burkholderia cepacia,Chryseobacterium meningosepticum,Pseudomonas aeruginosa and Acinetobacter baumannii to meropenem were 100.00%,48.81%,94.12%,21.85% and 31.72% respectively.CONCLUSIONS Meropenem has high in vitro antimicrobial activity to Enterobacteriaceae.but generally low to non-fermentative Gram-negative bacilli.
10.Diagnosis and Treatment of Airway Complication After Heart-lung Transplantation

Objective To sum up the diagnosis and treatment of airway complication after heart-lung transplantation. Methods One patient who occurred dyspnea, cough and wheeze at the 200th day after heart-lung transplantation, was diagnosed suffering from anastomosis stenosis. Comprehensive multimodality approach such as rigid bronchoscopic debridement and laser photoresection were performed. Eventually, this case was surgically managed with sleeve resection of stenotic bronchial segment. Results Surgical operation by sleeve resection of stenotic segment was successful, and the patient's symptoms disappeared. Till now the patient has been living more than 14 months after transplant, and CT scanning and fibrous bronchoscopy examination showed that the anastomasis healed well. Conclusion Airway complication's early recognition and proper management could improve the patients symptoms and survival rate.
