2.Clinical analysis on 826 cases of vitiligo

We retrospectively reviewed the clinical data of 826 cases with vitiligo treated in Beijing Hospital from 1998 to 2008. Most of patients present vitiligo in early age;20. 6% patients had positive family history. There were 44. 12 %, 41.18% and 10. 00 % of patients with vitiligo in their first, second and third degree relatives, respectively. Patients with family history had an earlier onset than those without a family history ( t = 1.97, P < 0. 05 ). Vitiligo vulgaris was found in 91.40 % patients. The most common affected site was head and face (31.5%) ;8. 1% of the patients was associated with other autoimmune diseases, including thyroid diseases, diabetes. There were more females than males in patients aged below 14. The child patients were more likely to present with Koebner' sign and halo nevus than the elder ones (P<0.05).
3.A clinical analysis of the predisposing factors for toenail onychomycosis in the elderly

Objective To evaluate the susceptibility to toenail fungal infection (FI) in the elderly. Methods Two hundred patients with toenail FI aged 60 years or older from 2005 to 2007 in our hospital were collected in our study, including one hundred elderly patients with toenail fungal infection (case group) and one hundred elderly patients without toenail fungal infection (control group). The clinical data of the patients in two groups were analysed. Results In case group, diabetes mellitus was the most common complication, accounting for 24% (24/100), while there were only 6 cases with diabetes mellitus (6%) in control group (P<0. 05). In case versus control group, there were 31 cases vs 12 cases with lower extremity varicose vein (P<0. 05), and 36 cases vs 19 cases with foot deformity, 11 cases vs 3 cases with toenail onychomychosis following trauma of their toes (P<0.05) ,32 cases vs 12 cases with history of wearing tight shoes (P<0. 05). In case group, 12 of 47 female patients used to wear high-heel shoes. Conclusions Toenail trauma, foot malformation, lower extremity varicose vein and diabetes mellitus are predisposing factors for toenail onychomycosis in the elderly.
4.Effect of montelukast on the secretion of mucus protein in human bronchial epithelial cells

ObjectiveTo investigate the effect of montelukast on the secretion of mucus protein in lipopolysaccharide (LPS) induced human bronchial epithelial cells.MethodsPrimary human bronchial epithelial cells were isolated and identiifed in vitro. LPS (1μg/mL) was used to induce cell inlfammatory response. Montelukast (50 μmol/L, 20μmol/L, 10μmol/L) was used as intervention. The concertration of mucin-5 subtype AC (MUC5AC) in cell supernatants was measured by ELISA. The expression levels of MUC5AC mRNA and protein were determined by RT-PCR and Western-blot. DCFH-DA lfuorescent probe was used to detect reactive oxygen species (ROS). To further elucidate the mechanism, NF-κB (p65)、IκBα、ERK1/2 phosphorylation be-fore and after montelukast intervention were determined by Western-blot.ResultsMontelukast decreases the expression levels of MUC5AC mRNA and protein in a dose-dependent manner in LPS induced human bronchial epithelial cells. Meanwhile, mon-teluskast suppresses ROS generation and NF-κB (p65)、IκBα、ERK1/2 phosphorylation.Conclusions In response to LPS in-duced inlfammation, montelukast decreases the expression level of MUC5AC in vitro, which may be related to NF-κB and ERK activation.
5.The expression of LIGHT and HVEM in T lymphocytes post allo-HSCT and its relation to the development of GVHD

Objective To explore the relationship between the expression of LIGHT/HVEM in patients after allo-HSCT and the development of GVHD. Methods The expression of LIGHT and HVEM in T lymphocytes was detected by FACS Calibur. Results All patients achieved engraftment and hematopoiesis reconstitution, aGVHD occurred in 9 of 26 patients (34.6%) with grade Ⅲ-Ⅳ aGVHD in 3 cases. Seven cases developed cGVHD (26.9%). LIGHT was not expressed in T lymphocytes from healthy donors and patients without GVHD, while constitutive expression of HVEM was detected. When aGVHD occurred, the expression levels of LIGHT in T lymphocytes were significantly increased, while those of HVEM decreased. After GVHD was controlled, the expression levels of those co-stimulators went back to normal On the day 15 after transplantation, the expression of LIGHT in T lymphocytes of patients with aGVHD later was significantly higher, while that of HVEM lower than in those without aGVHD. The patients with aGVHD of grades Ⅲ-Ⅳ demonstrated higher LIGHT expression than in those with aGVHD of grades Ⅰ-Ⅱ. Conclusion The expression of LIGHT and HVEM might be involved in the development of GVHD after allo-HSCT and could be used as one of the useful indicators in predicting aGVHD.
6.Clinical efficacy of ALA-PDT in the treatment of condyloma acuminatum

Objective To evaluate the clinical efficacy of 5-aminolevulinic acid-induced photodynamic therapy (ALA-PDT) in the treatment of condyloma acuminatum. Methods Thirty-six cases of condyloma acuminatum were treated with ALA-PDT. Results After the treatment, 33 (91.67%) patients were cured, 35 (97.22%) improved. Relapse occurred in 3 (8.33%) patients. Side effects were noted in 2 (5.56%) patients. Conclusions ALA-PDT is a relatively simple, effective and safe treatment with a lower recurrence rate for condyloma acuminatum, especially for small skin lesions or lesions at specific area. However, the high expense required for multiple treatments with ALA-PDT may restrict its application.
7.Correlation of personality characteristics and quality-of-life in patients with chronic urticaria

WHO quality-of-life scales (WHOQOL-BREF) and Eysenck personality questionnaire (EPQ) were used for evaluating the quality-of-life and personality characteristics of 60 outpatients with chronic urticaria.According to statistical correlation analysis, a positive correlation existed between education level and psychological field (P < 0.05).And there were negative correlations between N scale, P scale and psychological field (P < 0.05, P < 0.01).The incidence and incurability of chronic urticaria was more correlated with anxiety and depression.The patients with chronic urticaria were more likely to have a worse quality-of-life than normal population.
8.Clinical analysis of 11 cases of vulvovaginal-gingival syndrome

Objective To investigate clinical features and therapeutic protocols of vulvovaginal-gingival syndrome(VVGS). Methods Clinical data were collected from 11 female patients with VVGS and analyzed. Results Among the 11 patients, the mean and median ages at a definite diagnosis of VVGS were both 46 years, and the average disease duration was 4 years. Painful erosive erythema was found on the vaginal vestibule in 7 patients, on the gingiva in 10, as well as on the bilateral buccal mucosa in 5. Additionally, 8 and 4 patients reported dyspareunia and vaginal hemorrhage, respectively. Seven patients were treated with low-dose oral methylprednisolone combined with hydroxychloroquine or colchicine, and the other 4 were treated with oral hydroxychloroquine alone. Furthermore, all of the 11 patients applied topical tacrolimus 0.1%ointment. Finally, 8 patients were cured or markedly improved. No obvious adverse reactions were observed in any of the patients. Conclusions VVGS mostly occurs in middle-aged women, with the vaginal vestibule and gingiva as predilection sites of painful erythema and dyspareunia as common symptoms. Patients have good responses to methylprednisolone combined with hydroxychloroquine, and hydroxychloroquine alone also shows good efficacy in patients with mild VVGS. Tacrolimus 0.1%ointment can be one of drugs for topical maintenance treatment of VVGS.
9.Correlation between preoperative hidden blood loss and nutritional status in elderly patients with intertrochanteric fracture

BACKGROUND:The emergence of a large number of hidden blood loss during perioperative period of intertrochanteric fracture in the elderly not only increases the risk of perioperative period and complications, but also affects the postoperative recovery of joint function. At present, there is no relevant report about nutritional status and the hidden blood loss before surgery in and outside China.
OBJECTIVE:To identify the effect of nutritional status on preoperative hidden blood loss in elderly patients with intertrochanteric fracture.
METHODS:183 elderly patients with fresh and initial femoral intertrochanteric fracture were included. Laboratory serological examinations on admission and preoperation were completed. By mini nutritional assessment, patients were randomly divided into normal-nourishment group, malnourishment at risk group, and malnourishment group. The original blood volume and preoperative hidden blood loss were calculated depending on height, weight, hematocrit on admission and preoperation. According to the proportion of mean preoperative hidden blood loss on the original blood volume, patients were divided into low and high hidden blood loss groups. We compared preoperative hidden blood loss, and their proportion on the original blood volume and the preoperative incidence of high hidden blood loss, and analyzed the correlations between preoperative high hidden blood loss and preoperative nutritional status.
RESULTS AND CONCLUSION:(1) The nutritional status of elderly intertrochanteric fracture patients measured by mini nutritional assessment score was that the number of patients was 48 cases (26%) in normal-nourishment group, 64 cases (35%) in the malnourishment at risk group, and 71 cases (39%) in the malnourishment group. There were no obvious differences in the preoperative complications between any two groups (P>0.05). (2) Thirty-eight cases affected high hidden blood loss. The mean preoperative hidden blood loss was 260.43 mL. The proportion of preoperative hidden blood loss to the original blood volume was 6%. (3) The preoperative hidden blood loss, their proportion on the original blood volume and the incidence of high hidden blood loss were significantly higher in the malnourishment at risk group and malnourishment group than in the normal-nourishment group. Paired comparison showed significant differences (P<0.05). (4) Results confirmed that preoperative hidden blood loss, their proportion on the original blood volume and the incidence of high hidden blood loss gradual y increased with deterioration of nutritional status. The nutritional status is an important factor influencing the occurrence of preoperative hidden blood loss, and can be used as an important index for judging the high hidden blood loss and prognosis in elderly patients with intertrochanteric fracture.
10.The relationship between coagulation/anticoagulation imbalance and oxidative stress in patients with chronic obstructive pulmonary disease

Objective To explore the relationship between coagulation/anticoagulation imbalance and oxidative stress in the patients with chronic obstructive pulmonary disease during acute exacerbation (AECOPD)before and after treatment.Methods Plasma tissue factor(TF)and tissue factor pathway inhibitor(TFPl)activity was detected by chromogenic assay in 28 AECOPD patients before and after treatment as well as in 30 healthy controls.The total antioxidative capacity(TAC),malondialdehyde (MDA)and gtutathione peroxidase(GSH-PX)in plasma were measured in both groups.Results The levels of plasma TF and TFPI,and their ratio(TF/TFPI)in AECOPD patients before treatment were significantly higher than those after treatment(all P<0.0 1),the latter were still higher than those in the healthy persons(all P<0.01).The levels of the TAC and GSH-PX in plasma in AECOPD patients before treatment were significantly lower than those after treatment(all P<0.01),the latter were still lower than those in the healthy persons(all P<0.01).The plasma MDA in AECOPD patients before treatment was significantly higher than that after treatment(P<0.0 1),which was still higher than that in the healthy persons(P<0.05).There were negative correlations between TF/TFPI ratio and TAC(r=-0.518.P<0.01),GSH-PX(r=-0.454,P<0.05),PaO2(r=-0.511,P<0.01)respectively and a positive correlation between TF/TFPI ratio and the percentage of neutrophils(r=0.379,P<0.05)in AECOPD patients before treatment.There still were negative correlations between TF/TFPI ratio and TAC (r=-0.420,P<0.05),FEV1% to predicted(r=-0.480,P<0.05)respectively,and a positive correlation between TF/TFPI ratio and MDA(r=0.45 1,P<0.05)in AECOPD patients after treatment.Conclusions There existed coagulation/anticoagulation imbalance and oxidation/antioxidation imbalance before and after treatment in AECOPD patients and their relationship was explored.
