1.Clinical Analysis of 56 Preliminary Diabetic Patients with Unprovoked Ketosis

Objective To investigate the clinical characteristics and classification of diabetic patients with unprovoked ketoacidosis.Methods According to body mass index(BMI),56 patients were divided into 3 groups: low body weight group(LBW,BMI25 kg/m2,n=20).Clinical characteristics,including age,gender,the course of the disease,positive rate of glutamic acid decarboxylase antibody(GAD-Ab) and the function of islet beta-cell,were compared between these three group.Results There were no significant differences in some clinical features(glycemia,and ketosis status) at beginning of disease between 3 groups.The level of serum TG and Insulin was higher in OBW group than that in other groups.Only 3 patients were found GAD-Ab positive in LBW group.Conclusion The clinical and immunological characteristics of OBW and LBW patients were quite different,some of the obese patients should be classified into type2 diabetes,and some into idiopathic type1 diabetes.
2.Laryngeal polypus at the ventricular band symptomized firstly by hemoptysis: a case report.

Published reports about polyps of larynx at the ventricular bands are extremely rare. The first symptom of this case presents recurrent hemoptysis. The video laryngoscope demonstrated that a smooth-faced and hyperemic polypoid lesion was found in the anterior part of the right false vocal cord, close to laryngeal surface of epiglottis. Postoperative pathological report revealed that the mass was a vascular polyp.

3.Primary malignant lymphoma of the breast: imaging characteristics and correlation with histopathologic findings

Objective To evaluate the mammographic characteristics of primary lymphoma of the breast and to correlate the imaging features with the histopathologic findings.Methods We retrospectively reviewed records in 27 patients with histologically proven primary malignant lymphoma of the breast.Mmammography was performed before surgery in 14 cases.The clinical, pathologic, and imaging features were retrospectively reviewed and correlated in these 14 patients.All patients were female, with the mean age of 41.5 years (range 28-56).The lesion was in the right breast in 7 patients, the left in 6, and bilateral in 1.Results Histology revealed 13 Non-Hodgkin lymphoma (NHL) and 1 Hodgkin lymphoma (HL).In primary lymphoma, mammography revealed a unilateral solitary mass in 9 cases, unilateral multiple masses in 2 cases, bilateral multiple masses in 1 case, and unilateral diffusely increased opacity with skin thickening in 2 cases.The diameter of the masses ranged from 0.7 cm to 5.0 cm (mean 2.6 cm). Of the total 18 masses, the margins of the masses were well defined in 13, and partially defined in 5.Neither calcifications nor spiculations nor adjacent skin retraction were seen.Of the 13 NHL, histologic examination showed diffuse NHL in 12 cases and follicular NHL in 1.No mammographic features were identified that could helped distinguish follicular from diffuse NHL.Conclusion Primary lymphoma of the breast usually presents with clinical and mammographic findings mimicking a carcinoma or benign tumor.Although the clinical and imaging characteristics may suggest the possibility of breast NHL, none of the findings are pathognomonic.The final diagnosis depends on histopathology.
4.Knowledge, attitudes and practices of preconception and prenatal care in Tianjin obstetrics and gynecology personnels

Objective To better understand the knowledge, attitudes, practice (KAP) and relative demand of precon?ception and prenatal care (PAPC) in medical staffs of obstetrics and gynecology department in Tianjin. Methods Using ran?dom, stratified and clustered sampling approach, we selected 382 obstetrics and gynecology personnels in Tianjin. A self-ad?ministered questionnaire on PAPC KAP was employed to survey their general characteristics, their awareness of PAPC knowledge, their attitudes to PAPC, their service capabilities of PAPC and their request of continuing education. Results The respondents were lacking of KAP in Calcium and Iron supplementation, alternative preconception examination and emerging prenatal examination but present good KAP in folic acid supplemen, necessary prenatal items and classic prenatal examination. Doctors′average knowledge score was higher than nurses′. The scores in primary health care institutions was significantly higher than that in tertiary and secondary medical institutions. Young and junior medical respondents showed higher awareness than older and senior ones. Most (91.21%) respondents believe that PAPC is necessary. But 86.54%of re?spondents are not confident enough to provide comprehensive PAPC services. There were 63.74%, 54.67%, 38.74% and 32.42%of the respondents who could provide special PAPC advisory of adverse pregnancy history, thyroid dysfunction, re?productive infections and diabetes respectively. The most urgent training contents include PAPC knowledge in presence of complications or risk factors, advances in prenatal diagnosis, nutrition during pregnancy and consulting skills. Conclusion It′s necessary to intervent obstetrics and gynecology personnels to improve their cognitive level to PAPC, as well as to amelio?rate relative attitudes and behavior so they can provide more scientific and comprehensive PAPC service.
5.Proteomics Analysis of Component in Serum with Anti-asthma Activity Derived from Rats Treated by Acupuncture

Objective: To identify the differential proteins in the serum with anti-asthma activity of acupuncture to pinpoint the substance basis of anti-asthma activity of acupuncture by proteomics method. Methods: Two-dimensional electrophoresis of protein was employed to analyze the differential proteins in the serum with anti-asthma activity of acupuncture, and identify the differential proteins with mass spectrum technique. Results: In adrenalectomy (ADX)-asthma acupuncture group and asthma acupuncture group, two-dimensional electrophoresis showed that there was an obvious fuscous point (MW=44.1kD, PI=5.1), which was 60S ribosomal protein L13, or hypothetical protein FLJ32191 analyzed by mass spectrum technique, different from other groups. In ADX-asthma acupuncture group, there were two obviously fuscous points (MW=39.SkD, PI=4.9 and MW=39.SkD, PI=5.0), which were zinc finger protein 208, and thyroid hormone receptor-associated protein analyzed by mass spectrum technique, and two special proteins (MW=17.3kD, PI<4.5 and 16.9kD, PI<4.5), which were Cyclophilin A and zinc finger protein 91 analyzed by mass spectrum technique, different from other groups. Conclusion: There are multi-target effects regulating the whole body in acupuncture treating asthma relating to immunoloregulation, gene expression, and protein synthesization. The effects of acupuncture on the response proteins need further research.
6.Treatment of tubeless transurethral ureteroscopic 2 μm laser vaporesection for ureter cyst in pediatrics (33 cases)

Objective To explore the feasibility of tubeless 2 μm laser vaporesection in treating pediatric ureter cysts by ureteroscopy.MethodsClinical data of 33 ureter cysts patients who received tubeless 2 μm laser vaporesections by ureteroscopy were reviewed. The median age of patients was 4 years with a range from 1 to 7 years. The operations were carried out by RevoLix 2 μm laser through ureteroscopy without ureter stents and catheters indwelling.ResultsAll operations were successfully performed. And no serious complications occurred after the operations.ConclusionsTubeless transurethral 2 μm laser treatment by ureteroscopy was a superior micro-invasive surgery method for pediatrics with ureter cysts, with advantages of little blood loss, high safety, convenient operation and infrequent complications.
7.Value of plasma miR-193a-5p level on diagnosis and treatment evaluation in acute myeloid leukemia

Objective To explore the value of the plasma miR-193a-5p level on diagnosis and monitoring the response to treatment in acute myeloid leukemia (AML). Methods Peripheral blood samples were obtained from AML patients enrolled in hematology department of the Second Hospital of Shanxi Medical University from July 2015 to December 2015, including 30 de novo AML patients, 9 patients in complete remission (CR) and 6 patients in relapse. Peripheral blood samples from 15 healthy people were randomly choosed as the health control group. Plasma miR-193a-5p expression levels were detected by using quantitative real-time reverse transcription polymerase chain reaction (qRT-PCR). Results The plasma miR-193a-5p relative expression level of AML patients group [0.465 6 (0.103 1-5.000 2)] was obviously lower than that of health control group [0.766 1 (0.052 1-3.134 4)] (U= 122, P= 0.018 7). The plasma miR-193a-5p relative expression levels of de novo group and relapse AML group were significantly lower than those of CR group and health control group (P<0.05), and there was no significant difference between the CR group and health control group (U= 56, P= 0.511 9). No significant correlation was found between the plasma miR-193a-5p level and age, gender, blast percentage of the bone marrow, peripheral blood leukocyte count, platelet count, CD34+cells'percentage and so on. Conclusion The decreased plasma miR-193a-5p expression level can be served as a new and noninvasive biomarker for the evaluation of diagnosis and treatment in AML.
8.Effectiveness of antiretroviral therapy for the first time on HIV/AIDS patients in Jiangyin

Objective To analyze the effect of HIV/AIDS patients receiving antiretroviral therapy (ART) for the first time in Jiangyin, and to provide a reference for further improvement of Jiangyin's AIDS antiretroviral treatment. Methods The historical cards and related information in the treatment management database of Jiangyin City's cases who received ART for the first time from 2005 to 2019 were collected and statistically analyzed. The changes in viral load and CD4+ T lymphocytes (CD4 cells) before and after treatment were compared. Results Among 652 patients receiving ART, 507 cases (77.76%) were successful in virological treatment. The median natural change rate of annual average CD4 cell count was 90.8 cells/μL/year (χ2=37.915, P<0.05; H=9.781, P<0.05). There were statistically significant differences in virological treatment and immune recovery between different age groups (χ2=10.713, P<0.05; H =10.394, P<0.05) and different baseline CD4 count layers. The results showed that age and baseline CD4 value were the influencing factors of treatment effect. Conclusion Age and baseline CD4 value can affect the effect of ART treatment. The older the age and the lower the baseline CD4 value, the worse the virological efficacy and the recovery effect of CD4 cells. It is suggested that the infected patients should be involved in ART in time, which is conducive to shorten the time of initial treatment and further improve the effect of antiviral treatment.
9.Mediating effect of psychological resilience in breast cancer patients on social support and fertility anxiety

Objective:To study the status and influencing factors of fertility anxiety in breast cancer patients, and analyze the relationship between social support and fertility anxiety and the mediating role of psychological resilience.Methods:From January to May, 2020, 120 young breast cancer patients were investigated by the Cancer Reproductive Anxiety Scale, the Perceived Social Support Scale, and the Self-Resilience Scale and analyze the relationship of psychological resilience, social support with fertility anxiety, as well as the mediating role of psychological resilience among them.Results:Social support, psychological resilience were significantly negatively related to fertility anxiety and all dimensions ( r value was -0.428 to -0.126, P<0.05).Social support had negative predicting effect on fertility anxiety ( β value was -0.386, P<0.001). Psychological resilience of breast cancer patients had a partial mediating role between social support and fertility anxiety, accounting for the 35.8% of the total effect. Conclusions:Psychological resilience plays an important role in alleviating the fertility anxiety of breast cancer patients. Enhancing the psychological elasticity of breast cancer patients is an effective way to alleviate the anxiety of breast cancer patients.
