1.Stenting for patients with high-risk and complicated carotid artery stenosis

Objective To investigate the intervention methods of patients with high-risk and complicated carotid artery stenosis. Methods Twenty-five patients with high-risk and complicated carotid artery stenosis were treated with stenting During the procedure, they used the individualized therapeutic programs, including the implantation of temporary cardiac pacing, ev3 filter, combination of predilation and postdilation, guiding catheter (coaxial technique), staged angioplasty and stenting, and multiple stent implantations. Results The technical success rate of the 25 patients was 100%. The symptoms disappeared or improved in 21 patients (84%). The intraoperative dislodgement of emboli occurred in 1 patient. Transient ischemic attack occurred in 1 patient. No patients died. Conclusions Treatment with stenting in patients with high-risk and complicated carotid artery stenosis is safe. The preoperative evaluation of risks, the use of intraoperative individualized treatment programs, and the postoperative standardized management are the preconditions for ensuring efficacy.
2.Emergency operation remedy for acute stroke induced by internal carotid artery thrombosis

Objective To evaluate the feasibility and efficacy of emergency operation in treating acute stroke induced by internal carotid artery thrombosis.Methods Two patients with acute stroke with internal carotid artery thrombosis were treated with internal carotid artery incision,thrombus extraction,carotid endarterectomy and angioplasty.Results The cerebral ischemia symptoms of the patients were obviously improved.One patient showed complete rehabilitation,and the other patient could take self-care.No thrombosis and restenosis in internal carotid artery were formed during one-to nine-month follow-up.Conclusion Emergency operation in acute stroke with thrombosis occlusion in internal carotid artery is safe and feasible.

4.An analysis of relevant factors for depressive symptoms in residents aged 60 and over in Shimen Erlu Community of Shanghai

Objective It aims to investigate prevalence of depressive symptoms among the elderly in community and to understand their related social factors. Methods Four neighborhood committees were randomly selected with cluster sampling as survey sites. Enrolled residents at age of 60 to 89 years were evaluated with a general background inventory and a geriatric depression scale(GDS). All the data were input to a microcomputer and were analyzed for its relevant factors with SPSS13. 0 software by multivariate logistic regression model. Results Totally, 1731 elderly people in the community were interviewed and rated with the inventory and scale mentioned above, and depressive symptoms were found in 297 of them, with prevalence of 17. 2 percent. Multivariate logistic regression analysis showed that there were seven factors associated to their depressive symptoms in the elderly, including family expenditure, housing condition, medical condition, familial support, hobbies in spare-time, ability to take care of oneself and participation to social activities. Conclusions A few social factors, such as living alone, insufficient income for family expenditure, disability of self care-taking, lack of hobby in spare-time, less participation to social activities, lack of familial support and severe chronic diseases by serf-evaluation, are all associated with depressive symptoms in the elderly.

ObjectiveTo valuate the clinical effect of maintence of arteriovenous fistula by Songling Xuemaikang capsule.Methods40 cases were obtained from patients with chronic renal failure had received arterio-venous fistula operation,24 cases were observed with the combination treatment and 16 cases were served as control group,Hemorheology and difference value of vein diameter before and after treatment,blood flow of native arteivenous fistula and primary using time were observed.ResultsThe improvement degree of high shear rate and low shear rate were significantly as compared with that in the control group (P =0.000,P =0.001 ),while the blood flow after fistula maturation and ifference value of vein diameter were better than that in the control group( P =0.0356),primary using time between two groups were no significant differences ( P =0.056).ConclusionSonglinxuemaikang capsule was an effective drug for prevention high blood caogulation state,increasing blood flow of native arteivenous fistula,and enlargment vein diameter after arterio-venous fistula operation,which had certain value in clinical application.
6.Effect of sodium hyaluronateon on vascular endothelial growth factor mRNA expression of the synovium in rabbits with osteoarthritis

BACKGROUND:Pathogenesis of osteoarthritis is poorly understood,however,studies have demonstrated that vascular endothelial growth factor(VEGF)Is involved in the progression of osteoarthritis.OBJECTIVE:To detect the changes of VEGF mRNA expression of synovium in rabbit osteoarthritis and to evaluate the effect of sodium hyaluronate on its expression.METHODS:Twenty four white rabbits were divided into the normal control,physiologic saline,and sodium hyaluronate groups.The unilateral anterior cruciate ligament transection(ACLT)was performed in the physiologic saline and sodium hyaluronate groups.At weeks 4 after operation,rabbits in the physiologic saline group were injected 0.3 mL physiologic saline and in the sodium hyaluronate group received 10 g/L sodium hyaluronate injection,once per week for 5 successive weeks.All the animals were sacrificed at week 10 after operation.The cartilage changes on the medial femoral condyles were graded separately VEGF express Jon of synovium was detected by using teal time polymerase chain reaction(RT-PCR).RESULTS AND CONCLUSION:The macroscopic score showed that the cartilage degeneration in the physiologic sailne group was significantly more severe than that of the normal control and sodium hyaluronate groups(P<0 05)The expression of VEGF mRNA was obviously decreased in the physiologic saline group than that of the normal control group(P<0 05).of which was increased in thesodium hyaluronate group,but still smaller than the normal control group(P<0 05).The results demonstrated that the decreased VEGF expression in synovium may involved in the progression of osteoarthritis,and sodium hyaluronate has protective effect on articular cartilage by up-regulating the VEGF expression.
7.Preliminary experimental study of in vivo endothelialization of viable homograft valve

Objective To determine an optimal method of in vivo endothelialization when homograft valve (HV) leaflets are treated with different methods. Methods Twenty five fresh HV leaflets were divided into five groups randomly. The leaflets of GroupⅠ were fresh HV ones without any treatment; GroupⅡwere cryopreserved in liquid nitrogen for 1 month; Group Ⅲ was treated by DOA; GroupⅣ was treated by 25 g?L -1 glutaraldehyde; Group Ⅴ was first treated by hypotonic/hypertonic, then with an enzyme-based solution. Each HV leaflet was trimmed to 2 mm one which was cultured with lymphocyte from the recipient. In order to compare the antigenicity of HV leaflets, 3H-TdR incorporation was observed after tissue-cell coculture and stimulous index (SI) was calculated. The HV leaflets of different groups were implanted into the sheep's femorofemoropopliteal for three months; then the following procedures were done. Immunohistochemistry of Ⅷ factor correlative antigen was used to identify the cells property; observation of growth condition of EC and superficial morphosis of HV leaflets was made with scanning election microscope (SEM). Results Multiple comparison showed that SI of Groups Ⅱ, Ⅲ, Ⅳ, and Ⅴ differed significantly from Group Ⅰ(P0.05), but Group Ⅲ, Ⅳ, and Ⅴ all had significant differences from GroupⅡ (P
8.Simultaneous Determination of Schaftoside, Isoschaftoside, Deoxyelephantopin and 4, 5-Dicaffeoylquinic Acid in Shennongcha Granules by HPLC

Objective:To establish an HPLC method for determining four constituents ( schaftoside, isoschaftoside, deoxyelephan-topin and 4,5-dicaffeoylquinic acid) in Shennongcha granules. Methods:An HPLC method was performed on a Hypersil C18 column (200 mm × 4. 6 mm,5 μm) with the mobile phase of acetonitrile as the phase A and 0. 025 mol·L-1 phosphoric acid solution as the phase B with gradient elution. The flow rate was 1. 3 ml·min-1 . The detection wavelength was set at 270 nm for schaftoside and isos-chaftoside, 208 nm for deoxyelephantopin and 327 nm for 4, 5-dicaffeoylquinic acid. The column temperature was room temperature. Results:The calibration curve was linear over the concentration range of 5. 850-117. 000 μg·ml-1 for schaftoside, 4. 650-93. 000 μg ·ml-1 for isoschaftoside, 4. 160-83. 200 μg · ml-1 for deoxyelephantopin and 5. 470-109. 400 μg · ml-1 for 4, 5-dicaffeoylquinic acid. The correlation coefficient of all curves was more than 0.999. The average recoverywas 97.70% (RSD=1.40%), 96.87%(RSD=1.13%), 97.53%(RSD =1.69%) and 99.29%(RSD =1.01%) (n =6) , respectively. Conclusion: The developed HPLC method is simple,accurate,and can be used in the content determination of Shennongcha granules.
9.Study on the Quality Standard for Xiaozhong Tablets

Objective To establish the quality standard for Xiaozhong Tablets (Radix Angelicae Sinensis, Pheretima, etc.). Methods TLC was performed to Radix Angelicae Sinensis, Pheretima, Ursolic acid. The content of Aconitine was identified by TLC. HPLC was used to determine Strychnine content. Results The characteristic for identification by TLC was distinct and hightly specific. The quantification method of Strychnine had the linear range of 0.52~1.56 ?g (r =0.9998). The average recovery was 99.9% (RSD=1.77%, n =5). Conclusion The methods of identification and quantification were accurate, realizable and reproducible. It can be used effectively for the quality control of Xiaozhong Tablets.
