Objective To investigate the effect of traditional Chinese medicine on the renal function and quality of life in patients with early diabetic nephropathy (DN).Methods 78 diagnosed as DN and TCM syndrome type belonged to cases of yin deficiency of liver and kidney were included in the study, they were randomly divided into the control group(40 cases)and observation group (38 cases). The two groups underwent conventional treatment of western medicine, the observation group received traditional Chinese medicine. The 24h urine protein counts, blood glucose (FBG,2hPG),lipid column (TC,TG,LDL -C,HDL -C),UAER,ACR,SCR,bun and quality of life of SF-36,adverse reaction were observed. Results The blood glucose, blood pressure,UAER,ACR,SCR,BUN,SF -36 score, urinary protein count of the two groups were improved than before treatment.6 months after treatment, the 24h urine protein of observation group was (1.02 ±0.42)g,which was significantly lower than the control group, the difference was statistically significant (t =2.702,P <0.05).After treatment,UAER,ACR,SCr,BUN of the observation group were (82.09 ±10.74)mg/24h,(84.92 ±9.33)mg/g,(108.43 ±38.35)mmol/L,(6.17 ±2.65)mmol/L, which were better than the control group, the differences were statistically significant (t =8.960,P <0.01;t =15.528,P <0.01;t =11.987,P <0.01;t =10.566,P <0.01).No adverse reaction was observed. Conclusion The combination of Chinese and western medicine in the treatment of early DN has good efficacy and better than the use of western medicine alone,it can improve renal function and is safe and reliable.

Objective To observe the influences of modified constraint-induced movement therapy (mCIMT)on lower-extremity walking ability and femoral artery blood flow among elderly patients with stroke. Methods Totally 67 patients with stroke were randomly divided into mCIMT group(n =35) aged ( 73.2 ± 5.2 ) years and neurodevelopmental treatment (NDT) group ( n =32) as control aged(76.4 ± 3.8) years.Patients in control group exercised by NDT 2 h/time,2 times/d,5 d/week for 6 weeks. Patients in mCIMT group exercised including: up and down exercise,100-120 times/d; movement flatbed exercise for 16-20 min/d; upstairs and downstairs exercise, balance training, standing in a single leg exercise, mandatory exercise time of lower-extremity about 4 h/d,5 d/week for 6 weeks. The patients were assessed for lower-extremities motor function using maximum walking speed (MWS),Berg balance scale (BBS),timed up to go test (TUGT) and Fugl-Meyer(FMA-L) at pre-treatment and post-treatment.The change of femoral artery blood flow velocity and lumen diameter on the affected lower limb were observed by color Doppler. Results There were no differences in the above scores, lumen diameter and blood flow velocity before treatment between the two groups (P> 0.05).After treatment, the scores of MWS (56.68 ± 6.57vs.45.61 ± 5.34),BBS(46.84 ± 4.05vs.29.84 ± 4.05),TUGT ( 14.55 ± 8.25vs.25.35 ± 8.70)were higher in mCIMT group than in NDT control group (t=15.09,17.38,15.25,all P=0.001)while no difference in FMA-L score between the two groups was found (35.24 ± 7.62 vs.31.32 ±3.28,t=19.99,P>0.05).Lumen diameter of femoral artery [(9.05±1.15) mm vs.(8.05±0.68)mm,t=6.72,P=0.001] and blood flow velocity[(92.55±18.25)cm/s vs.(69.35 8.7)cm/s,t=6.83,P=0.001] were increased in mCIMT group as compared with NDT group. Conclusions The mCIMT therapy is better in improving the lower-extremity walking function and blood flow velocity of femoral artery.

Objective:To investigate the risk factors for nosocomial infection in older adult inpatients in some psychiatric hospitals in Ningbo of Zhejiang Province. Methods:The clinical data of 832 inpatients who received treatment in Ningbo Kangning Hospital and the Third Branch of Healthcare Group of Xiangshan Hospital of Traditional Chinese Medicine were retrospectively analyzed. These patients were divided into one infection group ( n = 45) and one non-infection group ( n = 787). The risk factors for nosocomial infection were analyzed. Results:Univariate analysis results showed that there were significant differences in age, disease type, primary pulmonary infection, time of antibiotics use, length of hospital stay, suffering from mental diseases, and use of urinary catheters ( χ2 = 10.721, 18.127, 12.183, 67.127, 56.326, 65.114, 27.153, all P < 0.001). Multivariate analysis showed that age ≥ 75 years, disease type, primary pulmonary infection, time of antibiotics use, length of hospital stay, suffering from mental diseases, and use of urinary catheter were independent risk factors for nosocomial infection in older adult inpatients ( OR = 3.360, 3.777, 5.238, 3.364, 3.939, 4.428, all P < 0.001). Conclusion:Age ≥ 75 years, disease type, primary pulmonary infection, time of antibiotics use, length of hospital stay, suffering from mental diseases, and use of urinary catheters are independent risk factors for nosocomial infection in older adult inpatients in some psychiatric hospitals in Ningbo of Zhejiang Province.
