1.Analysis the Related Factors Associated with Meningioma Peritumoral Edema

ObjectiveTo study the factors associated with meningioma peritumoral edema.MethodsPeritumoral edema of 18 patients was assessed on a semi - quantitative form from preoperative radiography. All the patients' clinical information( tumor size, tumor envelope damage, brain surface vein compression, pathological type), malignancy of tumor tissue, vascular endothelial growth factor(VEGF), immuno-histo-chemisty stain were studied to find out the correlation with peritumoral edema. Results There was no correlation between tumor size, pathological type and brain vein compression with peritumoral edema。 Tumor envelope damage and VEGF immunoreactivity were strongly associated with peritumoral edema. ConclusionThese data suggest that meningioma peritumoral edema may bea result of tumor cell producing VEGF in combination with tumor envelope damage, which lead to the damage of normal blood brain barrier and the enterance of edema-causing chemical factor.
2.A quantitative analysis of the kinematics of reaching by hemiplegic stroke patients using microsensor motion capture

Objective To evaluate the feasibility of using a microsensor motion capture unit (MCU) to assess the reaching characteristics of hemiplegic stroke patients. Methods Twenty-three hemiplegic stroke patients with an average age of (61 ± 11) years and ten normal subjects of matching age were asked to sit on a chair and use the affected arm (for the patients) or the right arm (for the normal subjects) to reach for a cup on a table just in front of the shoulder at arm's length away.Four small sensor boxes which could detect movement and its speed and smoothness in three dimensions were fixed to the subject's spine,upper arm,forearm and hand.The test was repeated four times after two trial runs.The average velocity,peak velocity,degree of joint dispersion,entropy of acceleration,joint coordination and active range of motion ( AROM ) of the shoulder were analyzed.The patients and normal subjects were statistically compared.The results were correlated with Fugl-Meyer upper extremity scores using Spearman correlation analysis. Results All of the measured variables showed significant differences between the patients and normal subjects.In the correlation study,the average velocity,peak velocity,AROM of the shoulder and joint coordination showed significant correlation with the Fugl-Meyer scores.There was,however,no significant correlation between degree of dispersion of the shoulder or elbow joints or the entropy of acceleration ( which represented the quality of movement) and Fugl-Meyer upper extremity scores. Conclusions The results suggest that microsensor MCUs can quantitatively assess the kinematics of reaching among hemiplegic stroke patients.They can provide valuable and objective data about the functional quality of multiple joint movements in three dimensions.
3.Quantitative analysis of the reaching ability of stroke survivors with synchronous surface electromyography

Objective To assess the reaching ability of hemiplegic stroke survivors using a motion capture unit (MCU) combined with surface electromyography (sEMG).Methods Sixteen stroke survivors with hemiplegia formed the experimental group,while healthy counterparts were selected as the control group.Both groups were asked to sit on a chair and reach for a cup on a table in front of their shoulder at arm's length using their affected arms in the experimental group and their right arms in the control group.MCUs were fixed on their spines and arms to obtain kinematic signals,and the sEMG signals of the trapezius,the anterior deltoids,biceps and triceps of the tested limb were recorded.Each subject repeated the test 3 times,and the best result was retained for further analysis.After signal processing,the range of movement of the shoulders and elbows was extracted along with the time used to reach the cup,peak angular velocity,time to peak velocity of the shoulders and elbows,work of the muscles and work ratios of the trapezius/deltoid and biceps/triceps.The upper limb section of the Fugl-Meyer assessment (FMA) was also administered to evaluate the patients' upper limb function.Independent sample rank sum tests compared the patients with the controls in terms of kinematics and sEMG parameters.Spearman analysis was used to explore the correlation between the FMA scores and the kinematics and sEMG characteristics.Results Significant differences in the kinematic and myoelectric indicators were found between the patients and the controls.The average FMA score of the patients was correlated with the peak velocity of the shoulder joint.Moreover,the ROM of the shoulder was closely related to the work of the trapezius,while the time for the shoulder joint to reach peak velocity was closely related to the work ratio of the biceps and triceps.Conclusion An MCU integrated with synchronous sEMG can quantitatively assess the kinematics and kinetics of hemiplegic stroke survivors,at least in reaching.This can provide objective guidance to optimize clinical rehabilitation.
4.The Evaluation of Earlier Diagnosis and Follow-up on Bleeding and Necrotizing Pancreatitis by Ultrasound

Objective To evaluate the value of early diagnosis and follow-up of bleeding and necrotizing pancreatitis by ultrasound.Methods 52 cases with bleeding and necrotizing pancreatitis confirmed by operation and pathology were examined by ultrasound. The pancreas were observed section by section in the blood vessel surrounding the pancreas acted as the mark under reduced the influence of gas in gastro-intestine and using adopting protruding probe with high resolution and compounding scanning technique in multi-posture. Results The accurate rate of diagnosis by ultrasound was 92%(48/52).During the follow-up period, pseudocysts and other complications were found in 9 cases, all cases recovered gradually after operations and treatment of symptoms except 3 cases who died caused by complications such as multiple organ dysfunction. Conclusion Ultrasound can identify bleeding and necrotizing pancreatitis early and also can regard as a significant follow-up method.
5.Comparison of two methods of transurethral surgical treatment for small size prostatic hyperplasia

Objective To compare the effect of transurethral resection of prostate (TURP) and TURP combine with transure-thral incision of the bladder neck(TUIBN) in the treatment of small size prostate hyperplasia .Methods From March 2002 to Octo-ber 2010 ,124 cases of small-size prostate hyperplasia patients were treated .All patients were randomized to undergo TURP or TURP with TUIBN .There were 62 patients in TURP group .Of which ,54 cases were followed up .There were 62 patients in TURP plus TUIBN group .Of which ,50 cases were followed up .The treatment effect of the two different surgical methods was evaluated by comparing international prostate symptom (IPSS) ,maximum flow rate(Qmax) and post voiding residual urine volume(PVR) et al .Results No statistical difference (P>0 .05)was found in age ,medical histories ,prostatic weight ,IPSS ,Qmax ,PVR between the two groups before operation .But significant difference(P<0 .01)was observed in IPSS ,Qmax ,PVR between the two groups after operation .There was no statistical difference(P>0 .05) in resected tissue weight and follow up between the two groups .The inci-dence rate of bladder neck contracture was 4% in TURP plus TUIBN group ,and it was significantly lower than that of TURP group(11% )(P< 0 .01) .Conclusion TURP plus TUIBN was better for the treatment of small size prostatic hyperplasia than TURP .
6.Prevalence of diabetes and prediabetes in northeast Sichuan area

Objective To investigate the prevalence of diabetes and prediabetes in northeast Sichuan area.Methods A total of 8 053 adult residents(over 18 years old)from the hospital who had underwent physical examination were recruited in the study.The concentrations of Fasting plasma glucose(FPG),glycated hemoglobin A1c(HbA1C)and blood lipids were measured.Diabetes and prediabetes were defined according to the 2013 American Diabetes Association criteria.Results The FPG and HbA1C concentra-tions increased with age.The overall prevalence of diabetes and prediabetes were estimated to be 8.5% and 39.6% in the adult pop-ulation in northeast sichuan area respectively.The prevalence of diabetes and prediabetes in men were significantly higher than those in women(10.7% vs.5.7%,41.5% vs.37.2%,P <0.05,respectively).Furthermore,the prevalence of diabetes increased with age,and the prevalence of prediabetes had a trend of first enhancement followed by a decline.Conclusion The prevalence of diabe-tes and prediabetes are relatively high in northeast Sichuan area.

8.The Influence of Merged County-based Medical Community on the Conscientiousness of Physicians in Township Health Centers:the Mediating Role of Emotional Feeling

Objective Based on the affective events theory,explore the influence of the cmerged county-based medical community reform on the conscientiousness of physicians in township health centers and its internal mechanism,and provide references for improving their conscientiousness.Methods Self-designed questionnaire was used to conduct the survey,including the basic personal information,physician's conscientiousness,county-based integrated service delivery networks and efficacy emotional feeling.Select physicians from 23 township health centers of 2 merged county-based medical community in Hubei Province as the investigation objects and use structural equation model to analyze the the influence of compact county-based integrated service delivery networks on the conscientiousness.Results The county-based integrated service delivery networks efficacy had a positive impact on the emotional feeling.Emotional feeling had a positive impact on the conscientiousness.The county-based integrated service delivery networks efficacy on the was conscientiousness 0.044,while the indirect effect was 0.236,and the ratio of indirect effect to the total effect was 84.3%.Conclusions The emotional feeling plays a fully mediating role between county-based integrated service delivery networks efficacy and physician's conscientiousness of township health centers.
