1.Terbinafine versus itraconazole for treatment of dermatophyte onychomycosis: a meta-analysis

AIM: To access the difference of the efficacy between terbinafine and itraconazole in the treatment of dermatophyte onychomycosis. METHODS: The Medline, Science Direct On Site (SDOS), and Springer database were searched in detail on the data of the mycological cure rates of the two antifungal agents for treatment of dermaphyte onychomycosis occourring in patients aged from 18 a to 60 a with the published double blind randomized clinical trials and then pooled. The odds ratio (OR) and its 95 % confidence interval (CI) were calculated. RESULTS: Six treatises of double blind randomized clinical trials were selected for this analysis according to the screening criteria. The mycological cure rate of continuous terbinafine 250 mg per day was higher than that of either therapeutic effect of itraconazole pulse 400 mg per day (OR = 5.01, 95 % CI (3.42 - 7.33)) or continuous itraconazole 200 mg per day (OR = 2.58, 95 % CI (1.91 - 3.49)) . CONCLUSION: Terbinafine is more effective than itraconazole in the treatment of dermatophyte onychomycosis.
2.Advances in research on relation of ubiquitin-proteasome pathway to malignancy

Ubiquitin-proteasome pathway is an important mechanism regulating many processes of cellular biology,and also a potential target for abnormal regulation associated with malignancy. This pathway may up-regulate or down-regulate the expression of some tumor-inhibitory genes, transcriptional factors and cyclins,and alter the generation of MHC-I-restricting antigen peptides through the activity of specific proteasome, and consequently,participates in the genesis and progression of malignancy.
3.Gemcitabine arterial infusion chemotherapy versus intravenous chemotherapy for patients with advanced pancreatic cancer: a comparative study

Objective To compare the clinical efficacy of gemcitabine arterial infusion chemotherapy with intravenous chemotherapy in the management of patients with advanced pancreatic cancer and to evaluate the efficacy and safety of gemcitabine arterial infusion chemotherapy. Methods 43 patients with advanced pancreatic cancer were included in this study, of whom 21 patients received arterial infusion chemotherapy (Group A) and the other 22 were treated by intravenous chemotherapy (Group B), gemcitabine combined with 5-FU chemotherapy was administrated in both groups. The main outcomes were clinical benefit response (CBR), tumor response rate and toxicity. Results Compared with Group B, there was a significant improvement of CBR in group A (81% vs 50%, P =0.033) ; there was also significant improvement of pain control in group A (76.2% vs 45.5%, P =0.039). There was no significant difference in the tumor response rate between two groups (33.3% vs 22.7%, P =0.498). No significant increase of side effects was observed in both groups. Conclusions In the management of advanced pancreatic cancer, the arterial infusion method may be more favorable than intravenous approach in improving clinical benefits with mild toxicity and well tolerability.
4.Testicular breeding sperm function and p63 expression in androgen receptor knockout mice

Objective To study the testicular breeding function and p63 expression in seminiferous tubules in androgen receptor(AR)knockout mice.Methods Eight AR knockout mice selected by Cre-lox and 8 Wistar male mice were studied.Testicular breeding function was determined by Makler score and p63 was detected by immunohistochemistry. Results Seminiferous tubules inner diameter deflated in AR knockout group than in Wistar group(60.0±1.5 μm vs 92.05±2.0μm,P<0.01),peri-tube membrane thickened(4.0±0.7 μm vs 2.85=0.9 μm,P<0.01),the number of cell layer decreased(2.0±0.3 vs 4.0±0.2,P<0.01),generative eell maturation arrested(3.0±1.2 score vs 5.05±0.5 score,P<0.01),Makler score was lower(8.0±0.3 vs 16.0±0.5,P<0.01).p63 expression was lower in AR knockout group than in Wistar group and the expression was located at the stage of spermatogenous cell and spermatocyte.Conclusion Testicular breeding sperm function is damaged in AR knockout mice,lower expression of p63 mainly affects early differentiation in generative cells.
5.Changes of metabolic index of diabetes before and after Ramadan

Objective To investigate changes of metabolism of glucose and lipoids in diabetes before and after Ramadan. Methods A total of 65 Muslim fasting diabetic patients, with complete data, were investigated about their diet and administrated medicines before and after the fasting and occurrence of hypoglycemia during the fasting. Indexes of WEIGHT, WAISTLINE, BMI, FPG, 2h-PG, HbA1c, TG, TC, HDL-C, and LDL-C were evaluated by questionnaire, physical examination and laboratory tests. Results During Ramadan, fasting time was prolonged (P<0.01), intake of lipoids and protein were increased obviously (P<0.01) and a significant increase of 2 h-PG, HbA1c (P=0.0149, 0.0175) was detected after Ramadan in s fasting group. No significant difference was found in WEIGHT, WAISTLINE, BMI, TG, TC, HDL-C, and LDL-C. Blood glucose was improved significantly in the control group than the fasting group. Conclusion Life style was changed obviously leading to unsatisfactory controlling of blood glucose in Muslim diabetic patients during Ramadan.
6.Effect of enriched environment on cognitive function induced by sepsis-associated encephalopathy and NPAS4/BDNF related mechanisms

Objective To observe the effects of enriched environment on cognitive function in mice with sepsis-associated encephalopathy and to study the neuron PAS domain protein 4 (NPAS4)/brain deprive neurotrophic factor (BDNF)related mechanisms.Methods Sixty adult male mice were divided randomly into three groups:sham operation with standard environment group (group SS,n =12),cecal ligation and puncture with standard environment group (group CS,n =24),cecal ligation and puncture with enriched environment group (group CE,n =24).All mice were reared in standard environment or enriched environment for 28 days.The fear condition test was conducted on day 29 af-ter operation in mice.The change of NPAS4 and BDNF,the density of dendritic spine were detected by western blot or golgi staining.Results Compared with group SS,the context freezing time, NPAS4 and BDNF expression and the density of dendritic spine in hippocampus decreased significantly in group CS (P < 0.05).Compared with group CS,the context freezing time,NPAS4 and BDNF expression and the density of dendritic spine in hippocampus increased significantly in group CE (P <0.05).No significant difference was observed in the conditional freezing time among three groups.Conclusion Enriched environment can obviously improve cognitive function impairment induced by sepsis-associated encephalopathy,which may be related with up-regulated expression of NPAS4/BDNF,and promoted synaptic plasticity.
7.Fatigue life analysis of coronary stent by finite element analysis

Objective After the implantation,coronary stent is expected at least to keep its integrity and maintain the predicated function for over 10 years or 4e8 cycles under the pulsatile loading conditions,so the fatigue property of the stent should be evaluated.Method The finite method was used to analyze the stress distribution of different phases and evaluate the fatigue life according to Goodman criteria,meanwhile,the accelerated fatigue experiment was also performed.Results It can be concluded that the dangerous points are all but located in the lateral inner surface of stent curvature.Conclusions The results prove that the fatigue property can be simulated through the finite element analysis,which can provide the theoretical guidance for the stent design.
8.The influence of precise liver resection techniques on intestinal permeability in the diseases needing liver resection

Objective To evaluate the influence of precise liver resection techniques on intestinal permeability in the diseases needing liver resection.Methods Eighty-seven patients needing liver resection were randomized to receive the precise liver resection (n =58,PLR group) or the conventional liver resection (n =29,CLR group).D-lactate and and endotoxin in abdominal fluid were detected in all the patients.The abdominal fluid bacteria cultures were performed.Results The postoperative hospitalization time,the needed time of blood routine,temperature,and oppetite resuming were significantly shorter in PLR group than those in CLR group (t ≥7.36,P < 0.01) ; The postoperative abdominal effusion was significantly less in PLR group than that of CLR group(t ≥ 14.17,P <0.01).The abdominal fluid concentrations of D-lactate and endotoxin in operation or at 1d after operation were significantly higher than those at 5d after operation in both groups(t ≥10.41,P <0.01).Those parameters decreased significantly at 2d after operation,returned to the normal level at 3d after operation in PLR group,and those parameters were significantly lower in PLR group than those in CLR group at the same time phase after postoperative 2 or 3 days (t ≥9.23,P <0.01) ; Those parameters began to drop at 3d after operation,returned to the normal level at 5d after operation in CLR group.The positive rate of abdominal fluid bacteria cultures was significantly lower in PLR group than that in CLR group(13/29) (x2 =23.51,P < 0.01).Conclusions The precise liver resection techniques had an important influence on intestinal permeability in the diseases needing liver resection.
9.The research about the correlation of colorectal adenocarcinoma gene fbxl 20 and E-cadherin

Objective To study the relationship between the fbxl 20 gene、E-cadherin gene and the clinocopathologic features ,respectively , and the correlation about the fbxl 20 and the set gene .Methods The mRNA expression level of fbxl 20 and E-cadherin gene in 50 pairs of human colorectal adenocarcinoma tissues and matched normal tissues were detected by RT -PCR.Results The mRNA expression level of fbxl 20 gene in tumor tissues were significantly increased than that in normal tissues (0.479 ±0.141 vs.0.296 ±0.121,P=0.001),while the E-cadherin were decreased (0.440 ±0.026 vs.0.741 ±0.059,P=0.000).There was no found on the correlation between the mRNA expression level of fbxl 20 and the E-cadherin gene in the 50 tumor tissues(r=-0.165,P=0.251).However,the E-cadherin expressed level was negatively correlated with the fbxl 20 expressed level in the lower differentiation degree、in the high differentiation degree and the Duke′s D tumor tissues(r=-0.600,P=0.008;r=-0.784,P=0.017;r=-0.643,P=0.032).Conclusions The decreased mRNA expression level of E -cadherin in tumor tissues are largely due to the increased fbxl 20 gene expressed level ,which is related with the high mobility and invasion ability of the advanced tumor .
