1.Effects of low-dose naloxone on morphine analgesia and plasma leveb of opiold peptldes

Objective Low-dose naloxone has been shown to reduce side-effects of morphine while morphine analgesia is not antagonized and may even be enhanced. The purpose of this study was to evaluate the effects of low-dose naloxone infusion (50 ng? kg-1? h-1 ) on morphine analgesia and plasma levels of opioid peptides in patients after abdominal hysterectomy.Methods Forty-two ASA Ⅰ- Ⅱ patients aged 36-50 yrs, weighing 55-67 kg undergoing abdominal hysterectomy under combined spinal-epidural anesthesia were enrolled in this study. Spinal puncture was performed at L2-3 interspace. The patients received intrathecal 0.75% ropivacaine 2.0-2.6 ml. 2% lidocaine was used for epidural injection. The block height was maintained at T8-6 . For postoperative analgesia the patients were randomized to receive either intravenous morphine infusion at 10 ?g?kg-1 ?h-1 (group M, n = 21) or IV morphine infusion (10 ?g?kg-1?h-1)+ naloxone infusion at 50 ng?kg-1?h-1 (group MN, n = 21). Pain was assessed using VAS (0-10) with 0 representing no pain and 10 representing the worst possible pain. Blood samples were taken from peripheral vein before anesthesia (T0), at the end of surgery (T1) and at 6, 24, 48 h (T2,3,4) after operation for determination of plasma levels of ?-endorphin (?-EP), dynorphin A1-13 (Dyn) and leu-enkepholin (L-EK) .Results The patients were comparable with respect to, age, weight, occupation and duration of operation between the two groups. Two patients (1 patient in each group) were excluded from the study because of their refusal to have repeated blood samples taken. The analgesia was significantly better in group MN than that in group M in terms of VAS scores. The incidence of spinal cord injury is becoming higher and higher, of which more than half are cervical spinal injury. The main cause of death in high cervical spinal injury is respiratory function failure. The patients who survived must rely on ventilators to sustain life. In view of the shortcomings of ventilators, many researchers tried to use diaphragm pacing technology instead of ventilator to reconstruct the function of respiratory. This article introduced the application of diaphragm pacing technology in patients with cervical spinal injury.
3.The prevalence of hyperuricemia and its relationship to blood lipids in a population for routine check-up in Nanning, Guangxi province

In 4 602 subjects for routine check-up,blood uric acid,total cholesterol,triglyceride,high density lipoprotein-cholesterol,and low density lipoprotein-cholesterol were determined.Results showed that the overall prevalence of hyperuricemia in Nanning,Guangxi was 19.8% (28.8% in male,9.4% in female).Blood uric acid and lipids in hyperuricemia group were higher than those in normal uric acid group(all P<0.01).Serum uric acid had a positive correlation with total cholesterol,triglyceride,low density lipoprotein-cholesterol,but it was negatively correlated with serum high density lipoprotein-cholesterol.The prevalence of hypercholesterolemia was 30.8%,and that of hypertriglyceridemia was 22.2%.Logistic multi-factor regression analysis showed that men,high total cholesterol,triglyceride,and low density lipoprotein-cholesterol could be independent risk factors for hyperuricemia,and that high density lipoprotein-cholesterol was a protective factor.The prevalence of hyperuricemia in population of Nanning,Guangxi during health examination is high.Hyperuricemia is closely associated with dyslipidemia.Timely intervention of hyperuricemia can reduce the related diseases effectively.
4.Comparative analysis of postoperative atrial fibrillation in esophagus cancer and lung cancer patients

Objective To investigate the incidence, causes, management and prognosis of atrial fibrillation (AF) after operation of esophagus cancer(EC)and lung cancer(LC). Methods The patients of postoperative AF following EC and LC surgery between December 2004 and May 2006 were taken into EC group and LC group. Results AF occurred in 27 patients ( 11.2%) of EC and in 18 patients (5.6%) of LC. The duration of operating and the level of urine catecholamine within the first two days after operation in the EC group were higher than those of the LC group (P0.05). All patients had been cured. Conclusions The EC predominate over the LC in the incidence of AF after operation; the early medicine treatment needs to be advisable to patients, and the patients of LC can be treated with removing causes at first.
5.Teaching practice of basic surgical skills training

Basic surgical skills tralning which belongs to the basic course of surgery, is clini-cal basis for the medical students. The teaching practice of basic surgical skills tralning was explored by the Affiliated Hospital of Inner Mongolia Medical University, the theoretical teaching was combined with skills tralning practice, three kinds of means which contalned the model, animal tissues and or-gans in vitro and animal experiment were used to carry out systemic and standardized tralning. Finally, the basic surgical skills of postgraduate in surgery were improved, and the expected alm of teaching was achieved.
6.Clinical effect of syringopleural shunt of patients with simple syringomyelia

Objective To evaluate the clinical effect of the simple syringomyelia patients with syringopleural shunt (SPS) and syringosubarachnoid shunt (SSS).Methods Twenty-eight patients with syringomyelia were selected as our subjects.Of which,18 patients were operated with SPS and 10 cases were with SSS.The therapy effect was compared between groups.Results All patients were checked with MRI 3 months after the operations and showed that syrinx cavity was significantly narrow of all patients.At early stage (48 h) after surgery 9 cases in SPS and 4 cases in SSS were showed the decrease syrinx cavity.Among patients with SPS,15 cases (83.3%) had the symptoms and signs improved,1 case (5.5%) withno changes,1 case (5.5%) with worse effect,and 1 cases(5.5%) occurred the tethered spinal cord.Meanwhile,among,patients with SSS,8 cases(80.0%) had the symptoms and signs improved,1 case(10.0%) with no changes,and 1 case (10.0%) with worse effect.There were 4 patients with pneumothorax in SPS group after operations,and the lung compression ratios were less than 5%.These cases were not taken any special treatment for the pneumothorax.All patients in two groups had not been infected and pleural effusions.No cases had taken the tube plugging.Conclusion The simple syringomyelia patients with the spine injury should take the cavity shunt.The SPS has some advantages because it can provide the partly negative pressure.But it should be certified by more cases and a long-time follow up.
7.Updates of heterotopic auxiliary liver transplantation with portal vein arterialization

In recent years,liver transplantation donor shortage as one of world medical problems is paid more attention by domestic and overseas scholars.In view of that,heterotopic auxiliary liver transplantation emerges which transplants the whole or some parts of a donor liver outside the original liver position on the condition that some parts or the whole of the original liver were retained.As for the liver transplantation with poor conditions on portal vein,reconstruction of portal vein has become an aporia.Based on this,some scholars put forward the theory——arterialization of portal vein(PVA),namely a method to increase arterial blood supply or replace portal vein blood perfusion of liver by establishing some pathes among artery and portal vein or its branches.The research background and current situation of heterotopic auxiliary liver transplantation with portal vein arterialization,the transplanting position of donor liver,vessel reconstruction,dynamics mechanism after reconstruction and liver regene-ration are summarized in this review.
8.Arthroscopic reconstruction of posterior cruciate ligament with semit endinosus tendon and gracilis tendon

Objective To evaluate the short-term results o f reconstruction of PCL(posterior cruciate liga-ment )with quadruple-stranded semitendi nosus tendon and bi-stranded gracilis tendon and to discuss the key point s of the surgery and postoperative rehab ilitation.Methods 12cases of PCL ruptures were reconstructed arthroscopically with quadruple-stranded semitendi nosus tendon and bidruple-stranded gracilis tendon.Results No patients had symptoms of knee instability.The sa g sign and posterior drawer test were negative in all.According to Lysholm knee function scale,10patients were rated as excellent and 2as good 1year after operation,with an average Lysholm score of 85?4.1.Conclusion Arthroscopic reconstruction of posterior cruciate ligament with quadr uple-stranded semitendinosus tendon and bi-stran ded gracilis tendon can result in lim ited invasion,strong graft and satisfactory clinical results.[

The system is applied to record work attendance on net, by which the management department can check work attendance of all departments of our hospital at any time. The application of the system realizes the management of work attendance on network, enhances work efficiency and avoids trivial manual work of count and collection.
