With the increasing incidence of pelvic fractures, the concept of damage control was applied to the treatment of severe pelvic fractures, and the principle of phased treatment and indications were put for-ward. Meanwhile, the development of damage control surgery, which improves the survival rate and reduces the disability rate of patients, brings the treatments of pelvic fractures to a new stage.

Objective To investigate the diagnosis method of intervertebral disc low back pain and the best treatment clinical efficacy. Methods The imaging data of 132 cases with intervertebral disc low back pain diagnosed by discography for disc imaging was retrospectively analyzed from October 2001 to October 2008. Randomly divided into three groups: group A of pereutaneous laser disc decompression in 63 cases of gasification, group B extraperitoneal anterior disectomy, Cage fusion 21 cases, group C posterior fusion after disectomy arch root 48 cases of screw fixation. Observed in patients with postoperative visual analogue scale (VAS) and Oswestry disability index (ODI) score improvement. Results MRI of lumbar intervertebral disc on T_2-weightod lesions as low signal was important. VAS after treatment in three groups followed by lower values to reduce, group A was (9.7 ± 0.9) points, group B was (7.4 ± 1.2) points, group C was (6.9 ± 1.4) points. There were statistically significance between group A and group B, C (P< 0.05). ODI in order to reduce the value of reducing, group A was (19.4 ± 1.4) points, group B was (18.8 ± 2.9) points, group C was (16.5 ± 2.1) points. There were statistically significance between group A and group B, C (P< 0.05). Conclusions Discography is a reliable method to diagnose intervertebral disc low back pain. Percutaneens laser disc decompression compared with posterior or anterior disectomy and fusion combined with internal fixation has good results in improving pain and function of patients.

PurposeTo investigate the accuracy of18F-FDG PET/CT in diagnosing mediastinal and hilar lymph node metastasis of lung cancer and in guiding surgery. Materials and Methods Seventy-eight pathology-proven lung cancer patients underwent 18F-FDG PET/CT scanning and surgery. Histology was used as gold standard to evaluate the diagnosis of mediastinal and hilar lymph node metastasis.18F-FDG PET/CT was also compared to chest CT.Results The pathological examination confirmed metastasis lymph nodes in 105 out of 231 excised lymph nodes in 18 patients. No lymph node metastasis was found in mediastinum in the other 60 patients with lymph node staging of pN0. Lymph node staging was pN1 in 5 patients, pN2 in 11 patients, and pN3 in 2 patients. In all 78 cases, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of18F-FDG PET/CT were 80.9%, 94.7%, 91.0%, 85.0%, 93.1%; while 61.1%, 71.7%, 69.2%, 39.2%, 86.0% on chest CT. These were statistically different (χ2=4.325,P<0.05).Conclusion PET/CT is superior to plain chest CT examination as an accurate and efficient method in evaluating lymph nodes staging of lung cancer.

Objective To summarize the clinical effects of the treatments of skin necrosis with nickelclad exposure after calcaneal fractures surgery by lateral leg flap. Methods Thirty-five cases of postoperative skin necrosis and nickelclad exposure of calcaneal fractures were repaired by lateral leg flap. Results The flaps was applied successly in 35 cases and followed up from 6 months to 5 years. All cases'wound was primary healing, the texture of the flaps were fine, and the function of the feet was not affected. Conclusion The lateral leg flap is an optimal method for the repair of postoperative skin necrosis and nickelclad expo-sure of calcaneal fractures, which can get satisfactory clinical effects.

Objective To observe the multi-segmental cervical spondylotic myelopathy with simple anterior or posterior joint pre-and post-operative prognosis of spinal cord function improved and the status, explore co-operation after the clinical efficacy and complications. Methods The clinical data of 298 cases of multi-segmental cervical spondylotic myelopathy with anterior or posterior of the simple pre-and post-joint surgery from January 2001 to January 2008 were retrospectively analyzed. The clinical efficacy, titanium anterior cervical decompression and fusion surgery net implanted titanium plate fixation 121 cases, posterior open-door laminoplasty 112 cases, 65 cases of combined surgery before and after. JOA score line of spinal cord function and somatosensory evoked potential, as compared 3 groups after surgical efficacy. Results All patients were followed-up 1- 7 years, averaged (4.7±1.4) years. The anterior cervical decompression and fusion surgery titanium mesh implanted titanium plate fixation to improve the rate was 78.1%, excellent and good rate was 72.7%(88/121). Posterior open-door laminoplasty to improve the rate was 70.6%, excellent and good rate was 66.1% (74/112), there was statistically significant between them (P < 0.05). After anterior surgery, improving rate was 86.7%, excellent and good rate was 83.1% (54/65). Anterior and posterior combined surgery before and after comparison was significant (P < 0.05), regardless of near-term results, long-term effects were better than that of anterior or posterior surgery. Conclusions The spinal cord in the treatment of multi-segmental operation of cervical spondylosis after anterior surgery is obviously superior to the efficacy of anterior or posterior surgery alone. OBJECTIVE: To explore the relationship between the portal hemodynamics or the indices of liver fibrosis and the liver function score in patients with different traditional Chinese medical syndromes of liver cirrhosis. METHODS: One hundred and forty-seven cases of liver cirrhosis with different syndromes were included in the study. Diameters and blood flow velocities of the portal vein and splenic vein were tested by color Doppler sonarography. The indices of liver function (TBIL, ALT, AST, Alb, Glb, PTA) and the indices of hepatic fibrosis (HA, PC-III, LN, VI-C) were tested. RESULTS: The diameters of portal vein in liver cirrhosis patients with different syndromes (internal accumulation of of damp-heat, stagnation of liver-qi, superabundance of dampness due to spleen-asthenia, asthenia of liver and kidney yin, asthenia of spleen and kidney yang, blood stasis) were not significantly different. The blood flow velocities and flow volumes of portal vein in patients with the blood stasis syndrome and the internal accumulation of damp-heat syndrome were significantly different (P < 0.05). The diameter of splenic vein in patients with the internal accumulation of damp-heat syndrome was significantly different to that of the asthenia of liver and kidney yin syndrome, or the blood stasis syndrome, or the asthenia of spleen and kidney yang syndrome, respectively (P < 0.05 or P < 0.01). The blood flow velocity of splenic vein in patients with the blood stasis syndrome or the asthenia of spleen and kidney yang syndrome was significantly different to that of the internal accumulation of damp-heat syndrome, or the stagnation of liver-qi syndrome, or the asthenia of liver and kidney yin syndrome, respectively (P < 0.01). The blood flow volume of splenic vein in patients with the internal accumulation of damp-heat syndrome was significantly different to that of the asthenia of liver and kidney yin syndrome, or the asthenia of spleen and kidney yang syndrome, or the blood stasis syndrome, respectively (P < 0.01). The liver cirrhosis indices HA and PC-III in patients with the asthenia of liver and kidney yin syndrome were significantly different to those of the internal accumulation of damp-heat syndrome (P < 0.05). In Child-Pugh classification, the incidence of Child-Pugh C was the highest in the asthenia of spleen and kidney yang syndrome, while the incidence of Child-Pugh A was the highest in the stagnation of liver qi syndrome. CONCLUSION: The changes of portal vein dynamics in liver cirrhosis patients with the asthenia of liver and kidney yin syndrome, or the asthenia of spleen and kidney yang syndrome, or the blood stasis syndrome are significantly different to those of the internal accumulation of damp-heat syndrome and the stagnation of liver-qi syndrome.

AIM To compare the pharmacokinetics and bioequivalence of between capsule and solution of benzoylmetronidazole. METHODS Ten chinese healthy male volunteers in a randomized 2-way crossover study were given a single oral dose 960 mg capsule(test) and solution (control) respectively. The serum concentrations of metronidazole, one of the metabolites of benzoylmetronidazole, was measured by high performance liquid chromatography. RESULTS The serum concentration-time curves appeared one-compartment open model. The results of the capsule and the solution of benzoylmetronidazole showed that T max (5.10?1.60) h and (3.12?0.90) h; C max (5.32?0.87) mg?L -1 and (6.51?1.25) mg?L -1 ; T 12 (10.56?1.75) h and (10.16?1.65) h; AUC (106.96?19.62) mg?h -1 ?L -1 and (113.59?19.84) mg?h?L -1 . CONCLUTION No significant difference appears in the pharmacokinetic parameters between the two formulations except of T max and C max ( P
