Objective Investigate the changes of adhesion molecule on myocardium in patients during mitral valve replacement(MVR) operation on beating heart. Methods Patients were divided into two groups. There were 16 patients in control group (aorta crossclamped and middle hypothermic) and 14 patients in experimental group (aorta unclamped and moderate hypothermic). Samples of right atrial were obtained before CPB, 30 min after CPB. Immunohistochemistry were employed to dectect the expression of ICAM-1 and VCAM-1 on cardiac myocytes. Results Comparing with those before CPB, ICAM-1 and VCAM-1 on cardiac myocytes and myocardial vascular endothelial cells increase significantly in both groups(P
2.Quantification of IL-10 gene expression in induced sputum of asthmatic patients by competitive PCR

Objective: To investigate the role of anti-inflammatory cytokine IL-10 in the pathogenesis of asthma.Methods:IL-10 internal standards was constructed by restriction digest.The expression levels of IL-10 mRNA were determined in peripheral blood and induced sputum of 23 asthmatic patients by(RT-PCR) with this competitive internal standards. Results:There was no IL-10 mRNA expression in most of the asthmatics.IL-10 mRNA was detected in some catabatic patients.But IL-10 mRNA was readily detected in 29 normal subjects. Conclusion:Airway inflammation of asthma was related to the expression of IL-10 gene.Asthmatic patients may have difficulty in IL-10 synthesis with transcript.
3.The Distribution of Body Weight Related Index and Relationship with Cardiovascular Diseases in Rural Residents of Yunnan Province

Objective The aim of this study was to investigate the distribution of body weight related index and to explore the relationship between weight related index and cardiovascular diseases in rural residents in Yunnan Province. Methods By random cluster sampling method,rural residents were selected in Yunnan Province. The data were collected by face-to-face questionnaire and medical examination. Results 4070 rural residents in Yunnan participated in this study. The survey results showed that gender,age,education level and ethnicity affected both the distribution of body weight related index and low body weight,overweight,obesity,and central obesity(P < 0.05). Compared with low body weight,non overweight,non obesity and non central obesity group,non low body weight,overweight,obesity and central obesity group had higher prevalence rate of hypertension(P < 0.01). Among central obesity group,the prevalence rate of coronary heart disease was higher than non central obesity group(P < 0.01). Conclusion The distribution of body weight related index in rural residents of Yunnan province showed some special characteristic. The overweight,obesity,and central obesity were related to cardiovascular diseases. Change unhealthy living habits or behavior as well as improving their cultural quality have effects on controlling body weight related index and help to reduce the prevalence rates of overweight,obesity and central obesity which can further reduce the prevalence rates of cardiovascular diseases.
4.Effect of geniposide on proliferation of pancreatic βcells and its mechanisms

Objective To explore the role of geniposide (Gen) in the proliferation of pancreatic βcells and the cell signaling transduction pathway .Methods After INS-1 pancreatic βcell line was treated with indicated concentrations of Gen for 24 hours, the number of viable cells was measured with MTT assay , and the cell proliferation was evaluated with BrdU labeled method .Additionally , Western blotting was used to probe the phosphorylation of Akt 308 and FoxO1 induced by Gen.Results The number of INS-1 cells and BrdU labeled positive cells was increased by Gen .Moreover, the phospho-rylation of Akt308 and FoxO1 was induced , and the phosphorylation of FoxO 1 induced by Gen could be prevented in the presence of LY294002, an inhibitor of PI3K kinase.Conclusion Gen induces the proliferation of INS-1 cells in a dose-dependent manner, and PI3K signal pathway plays an essential role in the phosphorylation of FoxO 1 induced by Gen.
5.Clinical application of high resolution melting analysis in mutation detection of phenylulanine hydroxylase gene

Objective To detect PAH gene mutations in classical PKU patients by HRM analysis. MethodsMutation scanning of PAH gene were performed in 17 classical PKU patients by HRM analysis ( LightScanner), covering the 13 exons and exon-intron boundaries. The HRM results were further confirmed by DNA sequencing, and the sensitivity and specificity of HRM method in PKU diagnosis were also evaluated. In addition, prenatal diagnosis was performed in two fetuses at risk for classical PKU. Results In the 17 patients, two mutations were identified in 16 patients, three mutations were identified in 1 patient.In this subject, a total of 22 different pathogenic mutations : 194V( c. 280A > G), IVS4nt-1 G > A( c. 442-1G > A), R158Q( c. 4736 > A), Q160X( c. 478C > T), W187X( c. 561G > A), E6nt-96A > G( c. 611A >G), G239D( c. 716G > A), R241 C( c. 721C > T), R243Q( c. 728G > A), G247R (c. 739G > C), G247V (c. 740G>T), R261X(c. 781C >T), PR261Q(c. 782G > A), H264R (c. 791A > G), F302fsX39 (c. 904delT), E305K( c. 913G > A), G312V( c. 935G > T), Y356X( c. 1068C > A ), V399V ( c. 1197A >T), R408Q(c. 1223G > A), T418P(c. 1252A > C) , A434D(c. 1301C > A), 3 silent mutations Q232Q (c. 696G > A), V245V(c. 735G > A), L385L(c. 1155C > G), and one single nucleotide polymorphism rs2280615 ( c. 402A > C) were identified, of which 194V ( c. 280A > G), Q160X ( c. 478C > T), H264R (c. 791A > G), G312V( c. 935G > T) and E305K ( c. 913G > A) were novel mutations identified in PAH gene. The prenatal diagnosis results of the two fetuses : one was diagnosed as normal, the other was diagnosed as a carrier. In this study, the sensitivity and specificity for mutation detection by HRM were 100%, and the HRM results were consistent with DNA sequencing results. Conclusions HRM analysis is a simple,accurate, rapid, high-throughput and low-cost genetic analysis approach. It could be applied to mutation scanning of classical PKU of PAH gene and rapid prenatal diagnosis in parents with known mutations.
6.Renal interstitial microvascular density and calcium-sensing receptor expression in children with steroid-resistant nephrotic syndrome

ObjectiveStudy the relationship among CaSR expression, tubulointerstitial damage,metabolic disturbance of calcium and phosphorus and microvascular density around the tubulointerstitium in children with steroid-resistant nephrotic syndrome.Methods36 cases of children with primary nephrotic syndrome were divided into hormone-sensitive group and steroid-resistant group.Semi-quantitative scores for tubulointerstitial pathological evaluation of the extent of damage, automatic biochemical analyzer for the determination of serum calcium (Ca), phosphorus (P) concentration of renal tubular epithelial CaSR expression and microvessel microvascular density around the tubulointerstitium were determined by immunohistochemical assay.ResultsMore severe the tubulointerstitial damage, lower level of serum Ca and higher level of serum P were observed [(2.26 ± 0.15) mmol/L]in children of the steroid-resistant group and the steroid-sensitive group [(1.90 + 0.12) mmol/L, P < 0.05].CaSR expression (4.63 + 0.78) of renal tubular epithelial cells in the steroid- sensitive group was significantly lower than that in the steroid-resistant group (6.56 + 1.22, P < 0.05), but microvascular density was significantly higher in the steroid- sensitive group(2.98 +0.35 vs 2.02 +0.24, P <0.05).When the tubulointerstitial damage was mild, CaSR expression (4.15 +0.58) in renal tubular epithelial cells in the steroid- sensitive group (4.26 ±0.61) was lower than the steroid-resistant group(3.12 ± 0.33; 3.01 ± 0.21), and microvascular density was higher,but the difference was not significant(P >0.05).In the moderate tubulointerstitial damage, CaSR expression in renal tubular epithelial cells in the steroid- sensitive group (5.35 ± 0.64) was significantly lower than the resistant group (7.37 +0.81, P <0.01), and microvascular density was significantly higher than the resistant group (2.81 ±0.16, 2.02 ±0.14, P <0.05).Compared by mild and moderate tubulointerstitial damage in children with the steroid-resistant, CaSR expression (11.46 ± 1.38) in children with severe tubulointerstitial damage was significantly increased, and microvascular density (1.15 ± 0.11) was significantly decreased (all P < 0.01).ConclusionsCaSR expression was increased and microvascular density around the tubulointerstitium was decreased in children with steroid-resistant nephrotic syndrome.Dut to steroid resistance, the cytotoxic of steroid damaged the renal tubular epithelial cells, the metabolic disturbance of calcium and phosphorus and the damage of blood vessel endothelium finally resulted in severe tubulointerstitial damage.
7.Effects of Sevoflurane Postconditioning on Lung Function in Patients Undergoing Cardiac Valve Replacement with Cardiopulmonary Bypass

Objective To investigate the effects of sevoflurane postconditioning on lung function in patients undergo-ing heart valve prosthesis implantation with cardiopulmonary bypass (CPB). Methods Thirty patients, scheduled for heart valve prosthesis implantation, were randomly divided into control group (C, n=15) and sevoflurane postconditioning group (S, n=15). Anesthesia was maintained by intermittent intravenous injection of midazolam, fentanyl and pipecuronium. In group S, 2%sevoflurane was inhaled continuously for 15 min , 2 min before aortic unclamping. Blood samples were taken at 30 min before CPB (T2), 30 min after CPB (T3), 6 h (T4), 12 h (T5) and 24 h (T6) after operation. The values of p(O2), SaO2, airway peak pressure (Ppeak) and airway plateau (Pplat) pressure during the time of mechanical ventilation were recorded. Oxygen index (OI), alveolar arterial oxygen tension difference (AaDO2), dynamic pulmonary compliance (Cdyn) and static pulmonary compli-ance (Cstat) were calculated. Results Compared with group C, the values of SaO2 and Cdyn were significantly higher in group S after 30 min of CPB. The values of OI, p(O2), Cdyn and Cstat were significantly higher in group S than those of group C after 6 h of operation. The value of AaDO2 was significantly lower 12 h and 24 h after operation than that of group C (P<0.05). Conclusion CPB led to the lung injury. The sevoflurane postconditioning has a protective effect on lung function in pa-tients undergoing cardiac valve replacement with CPB.
9.Prevalence of Diabetes and Its Impact on Quality of Life among Jingpo Ethnic Residents in Yunnan Province

Objective To analyze the prevalence of diabetes and its impact on the quality of life (QOL) among Jingpo ethinic residents in Yunnan province.Methods Questionnaire survey and fasting blood glucose (FBG) testing were conducted among 1367 Jingpo residents aged ≥ 35 years selected with random sampling method in Mangshi municipality of Dehong prefecture in Yunnan province.The Medical Outcome Study 36-Item Short-Form Health Survey (SF-36) was used in the survey.Results The prevalence rate of diabetes among the Jingpo residents was 4.1%,and 4.2% for males,4.0% for females.Among Jingpo diabetic patients,mean scores for the domains of physical functioning (PF),role physical (RP),bodily pain (BP),general health (GH),vitality (VT),social function (SF),role emotional (RE),mental health (MH),physical component summary (PCS) and mental component summary (MCS) were (84.64 ±16.81),(53.13 ±45.24),(71.70 ± 24.28),(49.46 ± 21.38),(65.09 ± 11.02),(90.08 ± 14.00),(79.17 ± 39.98),(70.29 ± 13.73),(57.12 ± 9.39) and (55.07 ± 6.58),respectively.The identified diabetic patients had significantly lower QOL scores for the doamins of PF,RP,BP and PCS than their counterparts,respectively (P<0.05).The results of the multiple linear regression analysis showed that older Jingpo residents had lower scores in the domain of PCS and MCS (P<0.01);men has lower scores in the domain of PCS and MCS than women (P<0.01);the higher the level of education,the higher the scores of PCS and MCS (P<0.01);married Jingpo residents had higher scores in domain of PCS than those of unmarried and divorced (P<0.01);the diabetic patients had lower scores of PCS than their counterparts (P<0.01).Conclusion It is necessary to strengthen the health education for the elderly and people with low level of education,and improve the management of patients with diabetes,so as to effectively improve the QOL of Jingpo ethnic minority residents.
10.Analysis on the Influencing Factors of Overdrinking

Objective To provide useful information on reducing risky drinking for the Dept.of Public Health taking a county in Yunnan province as an example to explore the risk factors related to risky drinking.Methods A total of 574 participants identified as Miao,Yi and Han People at or older than 12 years old,who had been living there for 6 months or more were selected and surveyed in 5 townships in the county through stratified sampling.Alcohol consumption was collected using the beverage-specific quantity frequency (BSQF) method and analyzed using binary logistic regression and chi-square test.Results The number of drinkers in the family,undesirable drinking environment among friends and enculturation (β =0.073) could increase the likelihood of risky drinking.Risky drinking pattern was positively associated with age (β =1.006),and negatively associated with gender (β =-2.947) and awareness of risky drinking among rural residents.Conclusion The important measures to control risky drinking behavior in terms of health promotion and education include reducing the number of drinkers in the family,and preventing from undesirable drinking environment among friends and harmful drinking culture.These measures are crucial to prevent early onset of alcohol drinking among teenagers and to reduce risky drinking among middle aged and aged population.
