Objective: study on recovery action of Buflomedil Hydrochloride to peripheral nerve crush lesion in rats. Methods: Sciatic nerve crush lesion model were established in rats,The conduction velocity of sciatie nerve and the morphological changes were observed by light and electron. Results:At the end of 4 weeks, Buflomedil Hydrochloride can obviously recover the sciatic crash lesion in rats. Conclusion:Buflomedil Hydrochloride shows recovery effect on the peripheral crush lesion.
2.Current status and progress of preoperative pulmonary function evaluation in patients with lung cancer

With the diversification in surgical population and the progress of surgical techniques, the current clinical application of pre-operative pulmonary function assessment methods can hardly be qualified for accurately assessing whether lung cancer patients can tolerate surgical treatment and surgery-related risk. This paper focuses on the advantages and weaknesses of the commonly-used pul-monary function assessment methods, with the literature review of the clinical application status and progress of pulmonary function currently. We aim to achieve a reasonable evaluation of preoperative cardio-pulmonary function assessment, and sequentially reduce the risk and occurrence of surgical-related complications.
3.Bedside closestool improve constipation and anxiety symptoms for patients with acute intracerebral hemorrhage

Objective To investigate the effect of defecation modes on constipation and anxiety in patients with acute intracerebral hemorrhage.Methods Sixty three conscious patients with acute hintracerebral hemorrhage were recruited in this study.Twenty five patients chose bedside closestool for defecation according their own intention and 38 chose in bed defecation.The incidence of constipation was documented and the severity of anxiety was evaluated with Zung self-rating anxiety scale (SAS) at day 7,14,21 and 28 after admission in two groups.Results Patients using bedside closestool for defecation had significantly lower rate of constipation between day 14-28 after admission (x2 =63.0,P < 0.01) and lower SAS scores at day14,21 and 28 after admission (t =6.7,7.7 and 9.8,all P <0.01) than those with in bed defecation.Conclusions The bedside mode of defecation may improve constipation and anxiety in patients with acute intracerebral hemorrhage.
4.Diagnosis and Typing of Ureterocysts by Ultrasonography

Objective To explore the utility of the diagnosis and typing to the ureterocysts using ultrasonography.Methods Based on the difference of anatomical location and appearance of cysts.the ureterocysts were divided into 4 types as fllowing:⑴Type of inside bladder:ureterocysts completely location in the inside of bladder.⑵Type of outside bladder:ureterocysts completely location in the outside of bladder.⑶Type of mixture:a part of cysts location in the inside of balader and other part localed in the outsider.⑷Type of ectopia:rare,cysts location in the neck of bladder or urethra usually complicated whith double kidney and abnormal location in the end of the ureter.Results UItrasonographic manifestations of 15 cases for ureterocele were reported.There were 3 cases of bilateral ureterocele and 18 cysts with a diameter 0 4cm~6 0cm. In which ,Type of inside bladder had 8 cases with 9cysts altogether .Type of outside bladdder had 2cases with 2 cysts only,Type of mixture had 5cases with 7 cysts;Type of ectopia had no one in this report .All cases were confirmed by ureteropyelography,in which,6 cases confirmed by surgical pathological examinations.Conclusion Ultrasonography has a high clinical value in the diagnosis and typing of ureterocele.
5.Effects of calcium channel blocker on the c-fos expression and nerve function following sciatic nerve injury in rats

Objective To study the effects of calaium channel blocker(CCB) on c-fos expression and nervous function following peripheral nerve injury.Methods Sciatic nerve was crushed with pincers to make the models of sciatic nerve crush injury in rats.The model rats were administered with Flunarizine 1 mg/kg(low dose group),2 mg/kg(high dose group) and normal saline 10 ml/kg(model group) via intraperitoneal injection.The c-fos positive cells at 1st and 4th week and nerve conduction velocity of sciatic nerve,distance between right behind toes were examined at 4th week respectively by immunohistochemical,behavior and electrophysiologic techniques.The results were compared with normal rats.Results(1) The c-fos positive cells were higher significantly in model and Flunarizine low dose groups compared with normal control group(all P0.05).Conclusion The c-fos expression following peripheral nerve injury can be reduced by CCB,which has the protective effect on the damaged peripheral nerve function.
6.Clinical observation of blind needle synovial biopsy

Objective To investigate the optimal technology of blind needle synovial biopsy.Methotis Blind needle synovial biopsy was performed on 81 knees with pain and swelling.Twenty patients with rheumatoid arthritis (RA) received uhrasonographic (US) examination before biopsies.Results Synovium were obtained in all patients and the successful rate was (71±21)%,while the area of synovium was (1.8±0.8)mm2.The time for accomplishing the procedure was (26±6) min and the depth of trocar insertion Was (3.1±0.7) cm.The successful rate in patients with RA (80±6)% was much higher than that in non-RA patients (54±10)%.The successful rate with US guidance (85±5)% was much higher than that without US guidance (78±6)%.The positive predictive value of synovium evaluation by naked eye Was 95.0%,while the negative predictive value was 81.1%.Conclusion Blind needle synovial biopsy should be spread because it is simple and safe to perfonn and it can obtain sufficient synovium for all purposes.
7.Treating constipation in pregnancy with testa triticum tricum purif: A multicenter study

Objective To assess the clinical efficacy and safety of testa triticum tricum purif (trade name: fiberform) in treating constipation during pregnancy. Methods With multicenter,prospective and self-controlled study method, in four domestic hospitals 140 pregnant ladies with constipation were enrolled according to recruiting criteria. The patients were treated by orally taking Festa Triticum Tricum Purif 3. 5 g every time, twice a day. Before and after taking medicine,constipation symptoms and the characters of feces were observed and quantized with score to identity its efficacy and side effects. Results After Testa Triticum Tricum treatment, dyporsia was relieved and the characters of the feces were improved significantly. Taking Testa Triticum Tricum for 7 days and 14 days, the effective rate was 90.0% and 93.6% respectively. Before and after taking medicine,the difference in constipation symptoms was statistical significant. No side effects were observed during Testa Triticum Tricum Purif treatment. Conclusions Fiberform is effective and safe for functional constipation treatment.
8.Application of three-dimensional thoracoscope system in minimally invasive thoracic surgery

Background and purpose:With the development of the three-dimensional (3D) science, minimally invasive technology is going into the 3D period. While 3D laparoscope system has been widely accepted by surgeons because of its better visual effect and safer operations compared to traditional laparoscope system, this study aimed to evaluate the feasibility of 3D thoracoscope system in minimally invasive surgery for the treatment of thoracic diseases. Methods: A total of 96 cases of thoracic diseases were accomplished with KARL STORZ 3D thoracoscope during Mar. 2014 to Oct. 2014, including 33 cases of lobectomy, 2 cases of segmentectomy, and 10 cases of wedge resection of pulmonary tumor, 27 cases of mediastinal tumor, 20 cases of esophageal tumor and 4 case of esophageal achalasia. The data of operative time, bleeding volume, postoperative chest tube drainage and hospital time and postoperative complications were counted. Results: All the operations were completed successfully, without conversion to open surgery. All the operations cost 30 to 237 min. The operation of local resection lasted 30 to 120 min, with an average of 52 min, the operation of pulmonary lobectomy lasted 63 to 122 min, with an average of 75 min;the operation of mediastinal tumor resection lasted 35 to 125 min, with an average of 77 min;and the operation of esophageal diseases lasted 57 to 237 min, with an average of 189 min. The bleeding volumes were 2 to 85 mL, 15 to 72 mL, 30 to 186 mL and with the average of 50 mL, 47 mL, 118 mL in pulmonary, mediastinal and esophageal operations respectively. The time of postoperative chest tube drainage of each surgery was 1 to 5 days after pulmonary operations, 1 to 3 days after mediastinal operations, and 2 to 6 days after esophageal operations. The postoperative hospital time of pulmonary operation was 2 to 10 days, with an average of 6.3 days;the time of mediastinal operation was 3 to 6 days, with an average of 4.2 days;and the time of esophageal operation was 4 to 19 days, with an average of 13.3 days. No complications and tumor recurrence or metastasis were observed during the followed 3 months. Conclusion: 3D thoracoscope system not only preserves the minimally invasive advantage of video-assisted thoracoscopic surgery (VATS), it can also provide high-definition and stereoscopic vision and better sense of depth which facilitate the operation more precise and safer, thus operation time becomes shorter. Besides, 3D system possesses the advantage of natural vision similar to open surgery, thus it has an easier and shorter learning curve.
9.Establishment and preliminary clinical application of serum bile acid spectrum analysis by ultra high

ObjectiveTo establish an ultra high performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method for detecting the bile acid expression in serum and to explore the relationship between the bile acid spectrum and the obesity.Methods After pre-treatment through solid phase extraction,serum samples were chromatographed by gradient elution on an UPLC HSS T3 column,and then mass spectrometric analysis of bile acid was performed by multiple reaction monitoring ( MRM ) mode with negative electrospray ionization (ESI).And its methodology performance, including specificity,linearity,sensitivity,imprecision and recovery,were verified according to the guideline of State Food and Drug Administration (SFDA).Furthermore,serum samples from 10 simple obesity subjects and 10 healthy controls were analyzed with this establishedmethod, and ranksum test was used for statistical analysis.Results This established UPLC-MS/MS method could simultaneously quantify 14 bile acid subgroups in serum sample,its analytical linear range was 10 - 20 000 nmol/L.For different bile acid subgroups,the limit of detection (LOD) was 0.02 - 7.90 nmol/L,limit of quantitation (LOQ) was 0.07 -44.20 nmol/L; and its within-day coefficient of variation (CV) was 0.35% - 12.41%,between-day CV was 1.11% - 13.04% ; the relative recovery ratio of this method was 89.8% - 114.6%.Some differenceswere observed in chromatogram between obesity and control subjects,and both free bile acid and conjugated bile acid concentrations in obesity subjects [ 0.49 ( 0.45 - 1.90 ),1.44 ( 0.84 - 3.72 ) μmol/L ] were lower than them in control subjects [0.98(0.53 -3.06),1.99(0.67 -2.88) μmol/L],but the difference was not significant ( Z =- 0.958,- 0.801,P > 0.05 ).ConclusionsThis established UPLC-MS/MS method can efficiently differentiate and quantify 14 bile acid subgroups,and is characterized with broad analytical measurement range,good analytical sensitivity and precision.This method can be applied for the free and conjugated bile acid analysis in clinical specimens.
10.Effect of Methycobal on Neuron Apoptosis of Caspase-3 mRNA Expression in Rat Brain Tissue After Cerebral Ischemia Reperfusion

Objective To investigate the effects of methycobal on the expression of Caspase-3 in brain tissue after cerebral ischemia reperfusion in rats. Methods Rats were randomly divided into sham-operation group, model control group, nimodipine group and low-dose methycobal group, high-dose methycobal group(n=30 in each group).Rats in the sham-operation group and model control group were administered intragastrically with 0.9% sodium chloride solution, rats in the nimodipine group were treated with 1 mg . kg-1 . d-1 of nimodipine, rats in the low- and high-dose of methycobal groups were given 50 and 100 μg.kg-1 .d-1 of methycobal, respectively. The rat model of cerebral ischemia reperfusion was established by middle cerebral artery occlusion with suture method for 3 h.Neurological deficit scores were evaluated 24 h after reperfusion.The apoptosis of perifocal cortex cells was detected by TUNEL method and the expression of Caspase-3 was analyzed by RT-PCR 6, 12 and 24 h after reperfusion. Results Neurological deficit scores in model control group, nimodipine group, low-dose methycobal group and high-dose methycobal group were 2.70±0.52, 1.30±0.51, 2.20±0.75 and 1.30±0.81, respectively.Compared with model control group, neurological deficit scores were significantly different in the nimodipine group, low-dose methycobal group and high-dose methycobal group(P<0.05 or P<0.01).There were no significant differences between the high-dose methycobal group and nimodipine group ( P>0. 05 ) . There was a significant difference between the high-dose methycobal group and low-dose methycobal group( P<0. 05 ) . The results of apoptosis by TUNEL were as follows: compared with model control group, the apoptosis decreased obvsiouly in the nimodipine group, low-dose methycobal group, and high-dose methycobal group at each time point.There was significant difference between the high-dose methycobal group and nimodipine group at the end of the 24th hours (P<0.01).Compared with low-dose methycobal group, there were significant differences in the high-dose methycobal group at the end of 6th, 12th and 24th hours(P<0.01 or P<0.05).The results of RT-PCR were as follows: there was expression of caspase-3 mRNA in the perifocal cortex of all groups, with weak expression in the sham-operation group.Compared with the sham-operation group, the expression of caspase-3 mRNA was increased significantly in the model control group(P<0.01).The expression of caspase-3 mRNA was reduced significantly in the nimodipine group, the low-dose methycobal group and high-dose methycobal group as compared with model control group at each time point( P<0.05 or P<0.01) , but it was not significantly different in the low-dose methycobal group and high-dose methycobal group as compared with that of the nimodipine group(P>0.05).There were significant differences between the high-dose methycobal group and low-dose methycobal group at the end of 24 h(P<0.05). Conclusion Methycobal can protect the brain cells from injury after cerebral ischemia reperfusion by adjusting the expression of Caspase-3m RNA, and the high-dose methycobal is more effective.
