1.Initially identification of core interventions to pediatric nursing based on Nursing Interventions Classification

Objective To identify the core interventions preliminarily performed by pediatric nurses for hospitalized children. Methods Based on Nursing Interventions Classification (NIC), the data of survey related to the factors of representation and frequency derived from the definition of Core Nursing Interventions were collected from 59 pediatric senior nurses above the 3rd clinical ladder, working in one of Shanghai university affiliated and tertiary children hospitals. The data was analyzed by L type of Metric Diagram. The intervention with product of two factors above 12 was selected as the core intervention. Results A total of 109 interventions were selected as core interventions on pediatric nursing from 497 interventions of NIC. The top 3 domains in quantity of interventions were physiological complex domain (36 items), physiological basic domain (26 items) and health system domain (19 items). The domain with the highest score (16.79 ± 3.42) was safety domain, followed by physiological complex domain (16.19 ± 2.98), while domain with the lowest score (13.62±2.05) was family domain. Conclusions The initial and systematic pediatric nursing core interventions were identified based on NIC and definition of core intervention. Part of core interventions reflects the characteristics of pediatric nursing practice, but more is requested to reflect the pediatric-specific care related to family-center and age-appropriated concepts. Some of inefficient interventions remind nursing administers further reflect thinking, especially on human resource management.
2.Ethical dilemma and coping strategies of nurses involving in caring dying patients

Objective To explore nurses' experiences of ethics dilemma and coping strategies involed in caring dying patients. Methods Interviews were conducted with 25 nurses from four different levels of hospitals in Shanghai. Results The ethical dilemmas that participants encountered with were derived from different standpoint and ethics decision-making between "relieve pain" and "extend life span","life value" and "insufficient medical resources","agent execution for patients' rights of being informed consent" and"medical intervention rights". Nurses' coping strategies included patiently communicating with family members to achieve agreement,failing to change family's decision then passively accepting it by self-adjustment,and following hospital rules and regulation or relying on team decision-making. Conclusion The study analyzed and discussed the reasons behind ethical dilemmas,as well as providing some suggestions on how to improve the coping abilities for nurse staffs. It can provide reference and basis for ethical decision-making.
3.Research in application of two kinds of pressure ulcer assessment scale in children patients

Objective To evaluate and compare the predictive validity of the Braden Q and Braden pressure ulcer risk assessment scales in the acute pediatric patients, and explore the cutoff score of the scales. Methods We adopted the prospective cohort study design.The settings included three PICU of three different children's hospitals. Two nurses were appointed as the data collectors, and they were in charge of both the scoring and the skin assessment separately. Results We collected 145 samples. There were 9 patients who developed the pressure ulcers, and the incidence rate was 6.2%. The cutoff score of Braden Q and Braden scale was 17 and 14. The AUC of the ROC curve of the two scales was 0.481 and 0.398. Conclusions The Braden Q scale is more suitable for the pediatric patients. Further study is neededed to improve the scale.
4.An oligonucleotide microarray approach for clarithromycin-resistance Helicobacter pylori detection

Objective To develop an oligonucleotide array to detect single nucleotide mutations in 23S rRNA gene.Methods Primers and probes targeting A2142G.A2143G and C2182T mutations in 23S rRNA gene were designed tp develop an oligonucleotide array.Samples were performed by an asymmetric PCR and the PCR products were hybridized with the specific DNA microarray chips.Non fluorescence-labeled PCR products were cloned into T vectors.The results of oligonucleofide array were confirmed by direct DNA sequencing and evaluated by minimal inhibitory concentration (MIC).Results The results obtained from oligonucleotide microarray were identical to those of direct sequencing.In 54 Helicobacter pylori samples,oligonucleotide microarray indicated that no A-to-C transition at 2142 was found,and the mutant rate of A2143G was 11.11 % (6/54),the mutant rate of C2182T was 12.96% (7/54).A2143C,A2143T,C2182A and C2182G mutations were not found.The other specimens were wild-type.All the above results were the same as that of MIC tests.Conclusions The oligonucleofide microarray is a reliable and accurate genotyping assay for clarithromycin-resistance of Helieobaeter pylofi.It is high-throughput screening method for gastric mucosa and improve the application of strategy for personalized therapy.
5.Using event-related potentials to explore language processing

Objective To investigate the use of component N400 of the event-related potentials (ERPs) elicited by Chinese idioms ending with matching and nonmatching words to explore the cognitive patterns and processing mechanisms elicited by Chinese characters. Methods Sixty-two right-handed healthy adults (age 19-50 years) , were presented with Chinese idioms ending with words with the same pronunciation but different form and meaning, words with similar forms but different pronunciation and meaning, words with same meaning but different pronunciation and form, and words with different pronunciation, form and meaning. A Guangzhou Runjie WJ-1 instrument was used to record component N400 of the potentials evoked. Results N400 showed a midline distribution , and could be detected under electrodes Fz, Cz and Pz. Significant differences were found in N400 latencies and amplitudes between matching and nonmatching ending-words. Compared with idioms with matching ending-words, N400 latencies were prolonged and amplitudes increased in nonmatching cases. The observed changes in N400 latencies and amplitudes were related to stimulus types, and the longest latency and largest amplitude were e-licited by ending-words with different pronunciation, form and meaning. No gender difference in N400 latency or amplitude was observed. Conclusions Compared with Western phonetic words, Chinese ideographic words are more flexible stimuli for N400 research, offering 3 dimensions for change: pronunciation, form and meaning. The N400 features elicited by matching and nonmatching ending words in Chinese idioms are mainly determined by the meaning of the word.
6.Expression of COX-2 and ERCC1 in gastric cancer and their clinical significance

Objective To investigate DNA excision repair cross-complementing gene-1 (ERCC1) and cyclooxygenase-2 (COX-2) expression levels in gastric cancer (GC) and evaluate the correlation between two proteins. Methods Immunohistochemistry was performed to detect the expression of ERCC1 and COX-2 in 239 GC cases, and analyzed the association between ERCC1 and COX-2 expression levels and clinicopathologic parameters. Results ERCC1 and COX-2 expression levels were significantly correlated with lymph node metastasis (P <0.05), while not associated with tumor size, location, depth of invasion and tumor differentiation (P>0.05). The positive expression rate of COX-2 in GC patients with HP infection was 86.5%, higher than those without HP infection. No correlation was found between two proteins. Conclusion Both ERCC1 and COX-2 were related with tumor progression, which may serve as valuable markers for determining biological behavior in GC. However , they did not show positive interaction.
7.The instant effect of upper extremity functional electrical stimulation on stroke patients

Objective To study the immediate effect of applying a portable functional electrical stimulation device (PUEFESD) to the upper extremities of stroke patients.Methods Twenty stroke survivors were selected.Pairs of electrodes of the PUEFESD were applied on the dorsal and palm sides of the affected forearm and thenar to stimulate the extensors and flexors as well as the thenar muscles sequentially according to a programmed protocol.Functional evaluation was conducted before and immediately after the stimulation.All participants were assessed with regard to their motor function and performance in the activities of daily living using an upper extremity function test (UEFT),the upper extremity motor function portion of the Fugal-Meyer assessment (FMA),and rating with the modified Barthel index (MBI).A visual analogue scale (VAS) was used to evaluate pain in the affected upper extremity.Results The patients' average FMA score was 19.4 ± 7.1 before the stimulation and 30.4 ± 9.6 after.The corresponding UEFT scores were 3.6 ± 3.7 and 26.9 ± 5.9,and the average MBI scores were 53.8 ± 26.2 and 65.8 ± 17.7.All of these increases were statistically significant.The average VAS score,however,did not change significantly.Conclusions Functional electric stimulation can quickly improve upper extremity function and the ADL ability of stroke patients,but pain is not relieved significantly.

9.Clinical research on blood glucose fluctuations in peritoneal dialysis patients with end stage diabetic nephropathy attaining glycated hemoglobin standard

Objective To investigate the characteristics of blood glucose fluctuation of continuous ambulatory peritoneal dialysis (CAPD) patients with end stage diabetic nephropathy(ESDN) attaining glycated hemoglobin standard. Methods The study recruited 17 patients with type 2 diabetes attaining glycated hemoglobin standard, and used continuous glucose monitoring system (CGMS) to monitor glycemic variation for 72 h. General information was collected and biochemical indexes were determined. Results The mean amplitude glycemic excur-sions (MAGE), standard deviation ,mean blood glucose levels, maximum of blood sugar, and the proportion of greater than 13.9 mmol/L in peritoneal dialysis patients were (8.36 ± 4.44) mmol/L, (3.38±1.08) mmol/L, (9.88±1.92) mmol/L, (17.95±13.11)%, which were significantly higher than those in normal. The mean amplitude glycemic excur-sions (MAGE), standard deviation ,mean blood glucose levels, maximum of blood sugar, and the proportion of greater than 13. 9 mmol/L in daytime were (8.25± 3.71) mmol/L, (2.83±0.89) mmol/L, (11.32±2.54) mmol/L, (16.61±3.86) mmol/L, (28.45±19.56)%, which were significantly higher than those in nighttime: (4.20±2.67) mmol/L, (1.34±0.89) mmol/L, (7.02±1.92) mmol/L, (9.61±2.77) mmol/L, (5.31±1.28)%, all P<0.05. The minimum and the proportion of less than 3.9 mmol/L between the two groups had no significant difference ( P>0.05). Besides, biochemical glycosylated hemoglobin was less than the calculated from CGMS: (5.88± 0.73)%vs. (7.85±1.20)%, t=4.76, P<0.01. Conclusions Peritoneal dialysis patients with ESDN have an increased glycemic fluctuation and a unsatisfied glycemic control, which is worse in daytime. Glycosylated hemoglobin is undervalued. Glycosylated hemoglobin should not be simply used on hemodialysis patients with ESDN to evaluate whether they have a good glycemic control. CGMS can better describe their blood sugar condition.
10.Clinical observation of combined treatment of Chinese medicine and western medicine on treatment of 156 patients with chronic lower limb ulcer

Objective To explore clinical effect of combined treatment of Chinese medicine and western medicine on treatment of chronic lower limb ulcer.Methods 156 patients with lower limb ulcer were treated with combined treatment of Chinese medicine and western medicine,and wound healing rate and symptoms were observed .Results After treatment ,curative rate was 19.87% and overall response rate was 88.46%.The average of wound healing rate was (51.24± 12.38)% and the average time of wound healing rate was (11.45±6.52)days.Conclusion Com-bined treatment of Chinese medicine and western medicine has a remarkable curative effect in treat-ment of patients with chronic lower limb ulcer.
