Ischemic preadaptation and ischemic postconditioning are the self-protection mechanism of the body, can reduce blood anoxic injury of brain tissue, inhibit nerve cell apoptosis, and reduce infarction area.However, due to the time of occurrence of ischemic disease is unpredictable, ischemic postconditioning research to guide clinical work more operational and practical, and thus become the focus of research. Apoptosis of programmed cell death is widespread in the process of biological life and has a close relationship with jak2/stat3/bcl-2 signaling pathway, PI3K/AKT signaling pathways and apoptotic molecules.microRNA as a class of highly conserved non-coding small RNA, its transcriptional expression involved in regulating cell value, development and apoptosis in a series of life activities.
2.Retrospective analysis on pathological diagnosis of primary pulmonary cryptococcosis in 32 immunocompetent persons

Objective To understand clinical characteristics of primary pulmonary cryptococcosis (PPC),so as to provide reference for clinical diagnosis and treatment.Methods Clinical data of 32 patients who were confirmed PPC by pathological diagnosis in a hospital from January 2008 to June 2014 were analyzed retrospectively. Results All 32 cases were community-acquired infection ,26 male and 6 female (age between 17 and 62 years old, the average age was (35.53 ± 11 .29).Among 32 patients,8 had underlying diseases,2 were parturients,4 were carpenters,5 were pigeon keepers,3 were seafood transport drivers,and 10 lived in humid environment.Imaging findings:Solitary or multiple nodules and cluster shape(n=21),lobar consolidation(n=2),diffuse patchy shadow on bilateral lung (n=5),pulmonary cavity(n=3),and diffuse and mixed lesions (n=1).Pathological confirmation:di-agnosis through percutaneous lung biopsy(n=23),thoracoscopic surgery(n=7),and thoracotomy(n=2).Progno-sis:30 were cured,and 2 had marked effect.Conclusion PPC is commonly occurs in young and middle-aged immu-nocompetent persons,the onset is occult,clinical manifestations and imaging features lack specificity,can be easily misdiagnosed or omitted diagnosis,diagnosis is difficult,lack rapid diagnostic method in clinical practice,invasive pathological biopsy can be used as the basis of diagnosis;there is a controversy on therapy,adverse reaction of flu-conazol is mild,and has good therapeutic effect.
3.Phloroglucinol effectively alleviates abdominal pain in acute gastroenteritis

The aim of the study was to evaluate the efficacy and safety of phloroglucinol in treatment of abdominal pain caused by acute gastroenteritis.Three hundred patients with acute abdominal pain were randomly assigned to receive 80 mg intravenous infusion of phloroglucinol injection ( treatment group,n =150 ) or 10 mg anisodamine injection (control group,n =150).The pain intensity was estimated by the VAS scores before and after the injection,meanwhile adverse reactions were also recorded.The results showed that the pain intensity was relieved significantly after 1h of the injection in both groups(P <0.01).The effective rate of phloroglucinol was 84.1% (126/150),while that of anisodamine was 80.0% (120/150).The adverse reactions of treatment group were lower than that of control group( all P <0.01).The results indicate that phloroglucinol can effectively relieve abdominal pain caused by acute gastroenteritis with safety.
4.The clinical and social factors associated with functional disturbance of acute hemiparetic stroke patients

Objective To investigate the prognostic factors of life and functional outcome of the first hemiparetic stroke patients. Methods One hundred and eighteen stroke subjects were registered prospectively. The Barthel index (BI) , Rankin scale (RS) , Mortricity index(MI) , Mini-mental state examination (MMSE) , Montgomery-Asberg depression scale (MADRS) and a scale of general state and risk factors were used to evaluate at the 48th hour, the 15th day and the 90th day after stroke. Results The patients' performance, as demonstrated by their scores with all the evaluation instruments, changed significantly at all the time points of evaluation after stroke (P <0.05). There was no significant difference between the performance at the 48th hour and the 15th day after stroke ( P > 0.05 ). But at the 90th day after stroke, the activity of daily living performance and the depression status recovered significantly (P < 0.01 ). Logistic regression analysis showed that, such factors as pneumonia, urinary incontinence within 48th hour and deep sensation disturbance might adversely influence patients' activity of daily living performance at the 90th day after stroke; the muscle strength of upper extremities at the 48th hour, and MMSE scores at the 15th day after stroke acted as the protective factors. Conclusions The stroke patients improved significantly with regard to their clinical and functional manifestations when evaluated 90 days after stroke onset. The main factors influencing the activity of daily living performance 90 days after stroke onset included deep sensation disturbance,pneumonia, urinary incontinence and muscles strength of upper extremities at 48th hour, and MMSE scores at the 15th days after onset.
5.LPS preconditioning relieves chronic liver injury induced by carbon tetrachloride

AIM:To investigate the effect of lipopolysaccharides(LPS)preconditioning on CCl4-induced liver injury and the change of LPS signal transduction.METHODS:The male Wistar rats were divided randomly into liver-injury group,which were injected with CCl4 5 mL/kg first,three days later were injected 0.3 mL 40% CCl4 and 60% olive oil. Animals in LPS preconditioning group were injected with LPS 0.5 mg/kg before the day CCl4 was given. Rats received high fat diet were as liver injury group,and normal control group received normal diet. The lymphocytes infiltrated in the liver tissue were counted. The endotoxin and ALT level in rat plasma,TNF-? content and expressions of TLR4,p38,p-p38,I??,NF-?? in the rat livers were also determined.RESULTS:The lymphocytes in liver slice and ALT level of the plasma in LPS preconditioning group were lower significantly than those in the liver injury group,and the expressions of TLR4,p-p38,NF-?? in the liver were the same. In contrast,the expression of I?? was higher.CONCLUSION:LPS preconditioning relieves obviously CCl4-induced chronic liver injury. The mechanism may be associated with change of signal transduction of LPS,which results in decrease of pre-inflammatory cytokines.
6.Effects of Yixinshu Capsule on Cardiac Function and Plasma Levels of Ang Ⅱ and TNF-? in Patients with Chronic Cardiac Failure

OBJECTIVE:To evaluate the curative efficacy of Yixinshu capsule(YXSC)for patients with chronic heart failure(CHF)and its effects on plasma levels of angiotrnsionⅡ(Ang Ⅱ)and tumor necrosis factor-alpha(TNF-?).MET-HODS:60 patients with CHF were randomly assigned to either treatment group(n=30)or control group(n=30):the control group received conventional symptomatic treatment with western medicines including anti-infection therapy,relieving airway obstruction,continuous low-flow oxygen inhalation,cardiac and diuretic therapy,vasodilator,correcting acid-base imbalance and electrolyte disturbances etc;the treatment group received YXSC in combination with the above conventional symptomatic treatment.Plasma levels of AngⅡ and TNF-? in two groups before and after treatment were detected;and the cardiac function and the indexes of echocardiography(LVEF,A/E)of all cases were recorded in detail before and after treatment.Meanwhile,10 healthy subjects served as normal control.RESULTS:After treatment,plasma levels of AngⅡ and TNF-? in both group decreased remarkably(P
7.A qualitative study on the psychological experiences of the patients with end-stage cardiopathy

Objective To describe the psychological experience of the patients with end-stage cardiopathy in China. Methods Phenomenology method was used in the study. Fifteen end stage cardiopathy patients received a non-structured interview. Results Four themes were extracted including heavy psychological burden about the disease and the selection of treatment options; complex psychological responses to families and relatives; wishing of social support;lose heart to personal future and prospects. Conclusions The patients on the end stage cardiopathy have complicated and intensive psychological responses. Medical staff should focus on the emotional of these patients, and provide effective strategies to promote patients'physical and psychological rehabilitation.
8.Effect of Comprehensive Rehabilitation on Acute Cerebral Infarction

Objective To observe the effect of comprehensive rehabilitation on the motor function and activity of daily living in patients with acute cerebral infarction. Methods 110 patients with acute cerebral infarction were randomly divided into rehabilitation group and control group. The patients in control group were treated with medicine, while the patients in rehabilitation group were treated with comprehensive therapy, including medicine, physical therapy and neuromuscular electrical stimulation. They were assessed with the simplified Fugl-Meyer assessment (FMA), modified Barthel index (MBI), and the clinical defect of neurol function before and 4 weeks after treatment. Results The scores of FMA and MBI improved significantly in both groups after treatment, and improved more in rehabilitation group than in control group (P<0.05). The effective rate was 75% in the rehabilitation group, and 56% in the control group (P<0.05). Conclusion Comprehensive rehabilitation can promote the recovery of motor function and activity of daily living in patients with acute cerebral infarction.
9.Effect of Early Rehabilitation Training on Serum Brain-derived Neurotrophic Factor Expression and Motor Function in Patients with Acute Cerebral Infarction

Objective To observe the effects of early rehabilitation training on serum brain-derived neurotrophic factor (BDNF) expressionand motor function in patients with acute cerebral infarction. Methods 48 patients with acute cerebral infarction were randomly dividedinto rehabilitation group (n=24) and control group (n=24). The control group accepted routine medication, while the rehabilitation group acceptedearly rehabilitation training in addition. They were assessed with Fugl-Meyer Assessment (FMA), and the expression of BDNF in serumwas detected before and after treatment. Results The expression of BDNF and the score of FMA increased significantly after treatmentin both groups (P<0.05), but increased more in the rehabilitation group than in the control group (P<0.05). Conclusion Early rehabilitationtraining can promote the expression of serum BDNF and recovery of motor function in patients with acute cerebral infarction.
10.Relationship between esophageal squamous cell carcinoma with lymph node metastasis and serum proteome fingerprint

Objective To analyze the alterations of serum protein in ESCC,compare alterations of serum protein with and without LM. Methods Serum samples were collected from 64 ESCC patients before operation and 60 cases with gender and age-matched healthy controls,special serum protein or peptide spectra was determined by SELDI-TOF-MS measurement after treating the sample onto weak cation exchange (WCX2) protein chip for each case. The serum protein profiles were compared by Biomarker Wizard Software between the ESCC patients and healthy controls, and among ESCC patients stratified according to gender, age, location of tumor, size of tumor, infiltration and with or without LM. Results (1)120 protein peaks were detected at the molecular range of 0 to 50000 in comparing of ESCC patients and healthy controls. 31 significantly different peaks were found between ESCC patients and healthy controls (P <0.05), 10 peaks were selected(P<0.01). (2) One significantly different protein peak (m/z 4174) was detected between T1 and T3, T4 (P<0.05). (3) There were three significantly different protein peaks (m/z 3970,4174 and 4277) between with LM and without LM (P<0.05).The peak (m/z 4174) was shared by two groups above. (4) No significant different protein was found when patients stratified according to gender, age, location of tumor and size of tumor. Conclusion Significant difference exists in serum proteins between ESCC patients and healthy controls. There are statistical difference exists in serum proteins between T1 and T3, T4, with LM and without LM. This difference is less than between ESCC patients and healthy controls. Some commonness is existed in serum protein fingerprint for patients with serious infiltration and with LM.
