1.Effects of Different Equilibrium Amino Acid on Nutrition Metabolism after Surgery and Prognosis of Severe Abdominal Trauma Patients

OBJECTIVE:To compare the effects of equilibrium amino acid and it rich in branched chain amino acid on nutrition metabolism after surgery and prognosis of severe abdominal trauma patients. METHODS:A total of 60 severe abdominal trauma patients selected from our hospital during Sept. 2016-Jun. 2017 were divided into group A and B according to block randomization,with 30 cases in each group. Both groups were all given iso nitrogenous and iso caloric parenteral nutrition support on the basis of surgery and routine treatment after surgery;group A was additionally given 8.5% Compound amino acid injection (18AA-Ⅱ)1.2 g/(kg·d)for 12 h each day at least;group B was additionally given 10% Compound amino acid injection(20AA) 1.2 g/(kg·d)for 12 h each day at least. Both groups were treated for 7 d. The levels of nitrogen balance and serum protein(total protein,albumin,prealbumin and transferrin),plasma amino spectrum were observed in 2 groups before and after treatment. Hospitalization time,survival rate,the occurrence of complication and ADR were compared between 2 groups. RESULTS:Two patients of group A and one patient of group B were discharged,and totally of 57 patients completed the study. Before treatment, there was no statistical significance in the levels of nitrogen balance,serum protein or plasma amino spectrum (P>0.05). After treatment,the cumulative nitrogen balance of group B was significantly better than that of group A. The level of nitrogen balance was increased significantly in observation group 3-7 d after treatment and control group 5-7 d after treatment. Since 4 d after treatment,the level of nitrogen balance in group B was significantly better than control group,with statistical significance (P<0.05). The levels of albumin,prealbumin and transferring in group A were decreased significantly,and were significantly lower than group B, with statistical significance (P<0.05). The levels of glycine and threonine in 2 groups were increased significantly. The level of arginine in group B was decreased significantly, while the levels of valine and branched chainaimino acid were increased significantly;those indexes of group B were significantly lower or higher than those of group A respectively,with statistical significance(P<0.05). The incidence of gastrointestinal complications and disruption of wound,total incidence of complications in group B were significantly lower than group A,with statistical significance(P<0.05). There was no statistical significance in hospitalization time,survival rate or the incidence of infection between 2 groups (P>0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Compared to equilibrium amino acid,equilibrium amino acid rich in branched chain amino acid can further improve and maintain the nitrogen balance,the levels of serum protein and plasma amino acid of severe abdominal trauma patients after surgery with low incidence of complication.
2. Effect of home-based cardiac tele-rehabilitation on quality of life, disease burden status,cardiac function and exercise endurance of patients after percutaneous coronary intervention

Objective:
To explore the effect of home-based cardiac tele-rehabilitation on quality of life, disease burden status,cardiac function and exercise endurance of patients after percutaneous coronary intervention (PCI).
Methods:
A total of 106 patients after PCI from January to December in 2018 in Heart Disease Center of Shiyan People′s Hospital, Hubei Province were randomly divided into observation group and control group according to the order of discharge, there were 53 cases in each group. The control group received routine rehabilitation care, and the observation group received home-based cardiac tele-rehabilitation on the basis of routine nursing. 6 minutes walking test(6MWT), anerobic threshold, oxygen peak and left ventricular ejection fraction(LVEF) in two groups were measured at 3 months after the intervention, the SF-12 scale and Family Burden Scale of Disease (FBS) scale were also scored.
Results:
After 3 months of intervention, the 6MWT, anerobic threshold, oxygen peak, physiological total score and psychological total score of SF-12 scale, and FBS score was (596.26±79.35) m, (14.65±1.82) ml·kg-1·min-1, (25.86±3.46) ml·kg-1·min-1, (47.46±9.86) points, (50.68±9.82) points, (18.94±6.61) points in the observation group, and (532.65±76.57) m, (12.95±1.87) ml·kg-1·min-1, (21.95±3.28 ) ml·kg-1·min-1, (43.28±8.21) points, (48.26±9.69) points, ( 21.75±7.18) points in the control group. The difference between the two groups was statistically significant (t value was -0.317-6.034, all P<0.05).
Conclusions
Home-based cardiac tele-rehabilitation can improve the quality of life, exercise endurance and can help to reduce the disease burden status on patients after PCI.
3.Effect of Rotundic acid on proliferation,migration and invasion of human lung adenocarcinoma cells

Objective :
To investigate the effect of Rotundic acid (RA) on proliferation,migration and invasion a- bility of human lung adenocarcinoma cells as well as its possible mechanisms.
Methods :
Human lung adenocarci- noma A549 and PC9 cells were divided into control group,blank control group,solvent group and 20,40,60,80 μmol / L RA groups.CCK-8 assay and scratch assay were used to detect the proliferation and horizontal migration of human lung adenocarcinoma cells.Transwell migration assay and Transwell invasion assay were used to detect the longitudinal migration and invasion ability of A549 and PC9 cells in each group.The protein expression levels of ja- nus kinase 2 (JAK2) and signal transducer and activator of transcription 3 ( STAT3) in the supernatants of A549 and PC9 cells were detected by ELISA.The mRNA expression levels of JAK2 and STAT3 were detected by RT- PCR. Statistical analysis was made on the differences among groups in each index.
Results :
After RA treatment on human lung adenocarcinoma cells ,compared with the control group ,the proliferation activity of A549 and PC9 cells in the experimental groups decreased (P<0. 05) ,the number of cells crossing polycarbonate membrane and matrix glue decreased (P<0. 05) ,the expression of JAK2 and STAT3 proteins in cell supernatant decreased (P < 0. 05) ,and the mRNA expression of JAK2 and STAT3 decreased (P<0. 05) .The decrease of the above indices was concentration-dependent and had statistical significance (P<0. 05) .Compared with the control group,the pro- liferation activity of A549 and PC9 cells in the solvent group showed no significant difference.
Conclusion
RA may inhibit the proliferation,migration and invasion of human lung adenocarcinoma A549 and PC9 cells in vitro, possibly through the inhibition of JAK2 / STAT3 pathway.
4.Characteristics and management mechanism of imported malaria from overseas during the malaria elimination phase in Wuhan City

Objective To analyze the characteristics of imported malaria epidemic from overseas in Wuhan, to explore the management mechanism of on-site cases, and to accumulate experience for the treatment of imported malaria in large cities after malaria elimination. Methods The epidemiological data on imported malaria from abroad during the period of malaria elimination (2010-2019) in Wuhan were collected. The gender, age and severe illness-related factors of the cases were analyzed. Based on the characteristics of the epidemic and the current situation of prevention and control, the content and experience of the “Municipal-District 24-7” case mechanism were discussed. Results The medical resources in Wuhan were the best in the central region, resulting in a large number of imported malaria cases, with a total of 474 cases reported from 2010 to 2019 (40.79% of the total number of cases in Hubei Province), including 359 cases of falciparum malaria, 36 severe cases and one death (the death rate was 0.28%). The patients were mainly young and middle-aged men aged 20 to 49 years old (97.26%). There were many referral cases (40.30%), and there was no seasonal clustering of cases reported. The undiagnosed proportion at the first visit was 44.85%, and the time of attack-diagnosis was 4 days or more in 61.00% of cases. The occurrence of severe cases was related to unconfirmed diagnosis at the first visit (χ2=35.46, P<0.001) and attack-diagnosis time (Z=-6.49, P<0.001). Conclusion Imported malaria occurs frequently in Wuhan, mainly falciparum malaria. However, “Municipal-District 24-7” case mechanism has effectively curbed the occurrence of severe and death cases and provided valuable experience for case management in similar cities in China.
