Methylenetetrahydrofolate reductase(MTHFR),a key enzyme in methionine-folic acid metabolism,participates in the synthesis of thymidine monophosphate(TMP) and purine as an indirect donator of methylium.It plays an important role in maintaining the normal level of homocysteine and DNA repair and replication.The MTHFR gene mutation decreases the activity of enzymes,increases the homocysteine level of blood plasma,and induces abnormal DNA methylation and various diseases.This review focuses on the structure and function of the MTHFR gene,its polymorphism and the relationship between its polymorphism and congenital heart disease.

OBJECTIVE: To summarize the cause and mechanic of inflammation reaction induced by medical biomaterials in the body.DATA SOURCE: A computer-based online search of Pubmed database was undertaken to identify the relevant articles published from January 2004 to December 2008 in English with the key words of "biomaterials, inflammation, mechanism".Meanwhile, with Chinese articles were retrieved from CNKI database between January 2005 and December 2008 with the key words of "biomaterials, inflammation, mechanism".DATA SELECTION: Articles concerning the cause and mechanic of inflammation reaction induced by medical biomaterials were included. Articles addressing physical and chemical features and mechanics of medical biomaterials were excluded.MAIN OUTCOME MEASREUS:①After the material implants in vivo, inflammatory cell and material adhering.②Cell factor expression.RESULTS: After initially examined 70 literatures were obtained. According to inclusion and exclusion criteria, the reasons and the mechanism related to the medical biological material in host in vivo for causing the inflammation were analyzed. Along with the tissue engineering development, various new high-polymer medicine biological materials with good biological compatibility,the biological activity and the biodegradation absorption function are emerging unceasingly. The biomedicine material was used to repair or substitute damage organization and the organ, and to cause function recovery. The infection was still the serious complication following the medical biological material was implanted. The mechanism of biomedicine material-induced infection is: complement in blood adhere the biological material surface, which causes collection and adherence of integrin-mediated leukocytes, release of cytokines and growth factors from adherent monocytes/macrophages, resulting in the occurrence of biomedicine material-related infection. To prevent the medical biological material-related inflammation, we should first embark from the material itself, and seek for some to respond the small medical material to the host inflammation.CONCLUSION: The success of material implantation depends on the medical biological material relevant inflammation. The production of biomaterial-related inflammation is action results of inflammatory cell, inflammation factor, complement as well as enzyme, oxygen free radical. Material surface microstructure, chemistry and dielectric and so on immediately influence inflammatory cells on material response.

Thirty-two patients with comminuted proximal humerus fractures(Neer Ⅲ or Ⅳ) were admitted to Department of Orthopedics,First Hospital of Jilin University from March 2001 to April 2006.The patients consisted of 19 males and 13 females,aged 19-70 years,including 15 of 19-49 years and 17 of 50-70 years.All patients underwent treatment of cloverleaf plate-screw fixation.During the follow-up for 8 months,union time and scores from Constant & Murley functional scale were similar between male and female groups.Compared with patients aged 50 years,and the scores from Constant & Murley functional scale were significantly lower(P 50 years old is relatively longer with unfavorable functional recovery in the shoulder joint.

Gefitinib and erlotinib play important roles, which as the representative of the epidermal growth factor receptor tyrosine kinase inhibitors for non-small cell lung cancer (NSCLC).However, preclinical and clinical studies find that many patients exist primary or acquired drug resistance to this drug, which limits the use of molecular targeted therapy drugs.At present, there are many studies focus on delaying or reversing drug resistance and developing new targets, which provide more potential for the molecular targeted therapy of NSCIC.

Objective To determine platelet count (PLT),mean platelet volume (MPV),and hemodynamic changes in patients with cir-rhotic portal hypertension (CPH)post splenectomy plus pericardial devascularization (SPPD),and to assess the surgical effect on coagula-tion function.Methods A retrospective analysis was performed on 83 CPH patients undergoing SPPD in our hospital from January 2008 to December 2012.Results Compared with preoperative levels,postoperative portal venous pressure decreased,blood flow was reduced,and portal vein diameter was significantly reduced;additionally,postoperative hepatic artery diameter was increased,and hepatic artery blood flow increased.Blood alanine aminotransferase,albumin,total protein,and fibrinogen levels,thrombin time,and MPV did not significantly change 30 days postoperatively versus 7 days preoperatively(P>0.05 );the corresponding total bilirubin,activated partial thromboplastin time and prothrombin time relatively declined(P<0.05),whereas PLT and prothrombin time activity increased(P<0.05).There was a significant increase in PLT (P<0.05)but no significant change in MPV 30 days postoperatively versus 7 days preoperatively (P>0.05). Conclusion SPPD can significantly improve liver functional reserve and coagulation function in patients with CPH.

Objective To find out the occurrence patterns of medical disputes over prosthodontic treatments through surveys and analyses so as to provide basis for the scientific prevention of such occurrences.Methods A statistical analysis was made of the causes of 92 medical disputes over prosthodontic treatments that occurred in the past three years,the data were processed using self-made tables,and the occurrence rates of the disputes were calculated according to their causes.Results The disputes over prosthodontic treatments in the past three years accounted for 22% of the total number of medical disputes over prosthodontic treatments.The cause leading to the most disputes involved medical quality,followed successively by service management,service manners,personal factors on the part of the patients,and charges.Conclusion There are certain patterns for the occurrence of medical disputes over prosthodontic treatments,which can be avoided to some extent if appropriate preventive measures are taken.

Objective To compare the effect of the new and traditional anastomotic modes on the life quality of the patients with esophageal cancer. Methods 400 cases of the patients with esophageal cancer were randomly divided into A and B group. All the patients received the anastomosis of the no imbedding with 3 rows suture through the pathway of thoracotomy allied with left neck,anteriolateral surface of right thorax and the epigastrium middle line. In group A the patients received the anastomosis with esophageal arc amputation and gastric half-arc amputation. In group B the patients received the traditional anastomosis with esophageal horizontal amputation. After operation the amount of taking foods, the width of anastomotic stoma, the amount of barium reflux, pH value of esophagorrhea, 24 hours pH value of esophagorrfea and intraesophageal pressure were measured in both the two groups patients. Results 13 cases of anastomotic fistula happened in all the patients, amounted to 3.25%. 5 cases had infection in the chest with 3 cases dead (1 from Group A, 2 from group B).All obove indices showed remarkable difference between the two groups of the patients except intraesophageal pressure. Conclusion The new anastomotic mode can improve the life quality of the patients with esophageal cancer much more than the traditional anastomotic mode.

BACKGROUND: Multidrug resistance (MDR) makes the treatment of tumor more difficult. Many factors contribute to the happening of MDR. The combination of two or more kinds of reverse reagent at the same time may be more effective to MDR.OBJECTIVE: To investigate the reversal effect of Tanshinone Microemulsion on MDR of human leukemia cell line (K562) and ADM resistant cell line (K562/ADM) cell line and the mechanism of it.DESIGN: An observational controlled experiment.SETTING: First Affiliated Hospital's Central Laboratory of Jilin University.MATERIALS: K562 and K562/ADM (Tianjin Blood Research Institute of CAS). Tanshinone and Tanshinone Microemulsion (Pharmaceutical College of Jilin University). ADM(Pfizer pharmaceutical corp). P-gp-PE fluorescent antibody and Bcl-2-FITC fluorescent antibody (Immunotech Biotech corp).DMSO(Beijing Chemical Plant).METHODS: The experiment was carried out in the Central Laboratory of First Hospital ffiliated to Jilin University from March 2003 to January 2004. ① K562 was cultured in liquid medium(RMPI1640) which contains 100 g/L calf serum, 100 U/mL penicillin and streptomycin. The culture tank's environment was kept at 37 ℃, with saturated humidity and 5%CO2. K562/ADM was cultured in the same environment as above, but ADM was added to the RMPI1640. ② The reverse effect of Tanshinone Microemulsion (Tanshinone and Microemulsion as control) on K562/ADM cells' MDR is observed. ③Tanshinone Microemulsion,Tanshinone and Microemulsion's MDR reverse fold to K562 cell line was detected by the method of MTT. ④ ADM of different concentrations (10, 20, 40, 60 mg/L)were added to grouped cells of exponential proliferation period. The intracellular drug concentration was observed by detecting fluorescent density.⑤ Expression rate of P-gp, Bcl-2 and apoptosis ratio of K562 and K562/ADM added with Tanshinone Microemulsion, Tanshinone and Microemulsion cell line were observed by the method of Flow Cytometry.MAIN OUTCOME MEASURES: ①Tanshinone Microemulsion, Tanshinone and Microemulsion's MDR reverse fold to K562 cell line. ②Tanshinone Microemulsion, Tanshinone and Microemulsion's effect to K562 cell's intracellular contration.③Tanshinone Microemulsion, Tanshinone and Microemulsion's influence to K562 cell's expression rate of P-gp, Bcl-2 and apoptosis ratio.RESULTS: ①Tanshinone Microemulsion, Tanshinone and Microemulsion could decrease the MDR of the K562/ADM. The effect of Tanshinone Microemulsion were significantly higher than Tanshinone and Microemulsion(P ＜ 0.05 ).②The intracellular concentration of ADM can be improved by Tanshinone Microemulsion, Microemulsion and Tanshinone. The ADM's concentration of K562/ADM treated by Tanshinone Microemulsion is highest and the difference is significant(P＜ 0.05). ③Tanshinone Microemulsion, Tanshinone and Microemulsion could reduce the expression level of P-gp and Bcl-2 in K562/ADM, and increase the apoptosis percentage of tumor cell. Tanshinone Microemulsion has the most significant effect on it,andincreases the apoptosis percentage of tumor cell with ADM (P ＜ 0.05 ).CONCLUSION: Tanshinone Microemulsion (0.5 mg/L) can increase the intracellular concentration of ADM, which is consist with the conclusion of down regulation of the expressions of P-gp and Bcl-2. Reversal effect of Tanshinone Microemulsion is more effective than that of Tanshinone and Microemulsion.

Objective To focus on the complication and manipulation of the ventricle-peritoneal(V-P) shunt operation.Methods The former clinical datas of the V-P operation were reviewed .Results Death rate was 13 percent,infect rate was 13 percent and re-operation rate was 18 percent.Conclusion The rate of the V-P operation complications is so high that avoiding the complications is important to improve success.
