1.Application of compound coptis chinensis humidified fluid in oxygen administration

Objective The compound coptis chinensis humidified fluid was applied in oxygen administration,to observe the value of reducing the pulmanary hospital infection rate. Methods 30 patients given continuous oxygen administration were chosen and randomly divided into two groups according the hospital number.Cross-over design method(In the first stage,using normal saline as humidified fluid in group A,compound Coptis chinensis liquid as humidified fluid in group B.In the second stage,compound Coptis chinensis liquid as humidified fluid in group A,normal saline as humidified fluid in group B.)was used,and different clinical application effects were compared between compound Coptis chinensis liquid and normal saline. Results There were statistical difference between Compound Coptis chinensis liquid and normal saline as humidified fluid on the number of bacteria(F=73.701,P<0.01),the rate of qualified rate(x2=52.50,P<0.01).There was no difference between the two stages(F=0.984,P>0.05)Conclusion Compound Coptis chinensis liquid could reduce the rate of contamination of humidified fluid,prevent the nosocomial pneumonia.
2.Allergic rhinitis nasal mucosa exposured to allergen histopathologic and the microvascular remodeling of SD rats

OBJECTIVE To study the histopathologic and microvascular changes in allergic rhinitis nasal mucosa of SD rats following persistent exposure to allergen. METHODS A total of 60 heathy SD rats were divided into two groups randomly: the persistence group(n=30)and the control group(n=30). The animals of the persistence group were exposed to allergen persistently after modeling of 7 weeks with OVA provocation in nasal mucosa for 16 weeks. At 8, 12, 16 weeks after OVA provocation each group were sacrificed at each time point. The animals of control group were not provocated and were sacrificed at the same time point. Their nasal mucosa of 5 animals from each group were observed under optical microscope and the other 5 animals were used to made microvascular corrosion casts to observe microvascular changes under scanning electron microscope. RESULTS In the persistence group, the early histopathological transformation was acute inflammatory reaction, nasal mucosa tissues congestion, partial nasal mucosa shedding. The inflammation cells infiltrated into the submucosa. The acute inflammatory reaction declined as the time went by. Then the nasal mucosa repaired step by step. Damaged epithelium repaired in a certain extent with fibration, but the restoration result of epithelial are untidiness, goblet cell volume is increased and metaplasia submucosal blood vessels increased, Through scanning electron microscope, we found the capillary of nasal mucosa expanded and congested, the vascular endothelial cell swelled. As time went by, the capillary hyperplasia and the microvascular endothelial cell analosis even microvascular wall anomalism. CONCLUSION Inflammatory injury at early stage after allergen provocation, the mucosa hyperblastosis and friber restoration in mucosa tissue and microvascular networks reconstitution at later stage form a histopathological basis of dysfunction after persistent exposure to allergen.
3.Expression of matrix metalloproteinase-8 in human breast cancer and its clinical significance

Objective To investigate the expression of matrix metalloproteinase-8 (MMP-8) in human breast cancers and its relationship with the clinicopathological parameters,evaluate the relevance of MMP-8 levels to metastases in breast cancer.Methods Human specimens were obtained from 50 patients with invasive ductal carcinomas,and 20 normal breast glands were enrolled.Intact clinical parameters were aviliable for all patients.The protein expression of MMP-8 was determined by immunohistocbemistry technology SP.Results The rate of MMP-8 positive breast cancer was significantly higher than that in normal breast glands [68 % (34/50) vs 15 % (3/20),P < 0.05].The expression of MMP-8 showed a negative correlation with breast cancer staging.The expression of MMP-8 in stage Ⅰ was higher than that in stage Ⅱ and Ⅲ/Ⅳ[94 % (15/16),57 % (13/23),55 % (6/11),P < 0.05].Lymph node negative patients had a higher MMP-8 expression than patients with lymph node metastases [84 % (21/25) vs 50 % (13/25),P < 0.05].Conclusion MMP-8 expression is associated with human breast cancer metastases and might be involved in inhibiting metastases.
4.Maxillary third molar with three canals in mesiobuccal root:A case report

This case report presents an extracted maxillary third molar,which have 3 separate roots with 5 canals,the canal configuration in mesiobuccal root are 3 separate orifices and canals,1 apical foramen.The clinician should be aware of this anatomical variation,avoid the missed canals.
5.Mitochondrial permeability transition pore and myocardial ischemia-reperfusion injury

Ischemia-reperfusion of the heart leads to the opening of mitochondrial permeability transition pore(MPTP).Transient opening and subsequent closure of the MPTP may lead to the release of cytochrome c and other proapoptotic molecules.If unrestrained,this will lead to the loss of ionic homeostasis and initiate the cascade of necrotic cell death.Direct or indirect inhibition of MPTP provides protection against ischemia-reperfusion injury.Recent researches have shown that transient low-conductance pore opening also provides protection against reperfusion injury.This review gives summarization of the components,molecular mechanism of MPTP,permeability transition in myocardial ischemia-reperfusion and the relationship between MPTP and ischemic preconditioning.This may lead to better protective strategies against myocardial ischemia-reperfusion injury in the future.
6.Thinking on the configuration and clinical application of hybrid operating room in China

Hybrid operating room is used and spread in China with the development of medical equipment and the integration of medical technology. As a new equipment and advanced technology, it faced some questions and challenges on its construction and development. We analyzed the definition, classification, construction and use which combined the development history and clinical application, and proposed some suggestions on how to strengthen the development and construction of hybrid operating room.
7.Treatment of pronation external rotation ankle injuries

Objective To investigate the treatment of pronation external rotation ankle injuries and to evaluate its outcomes. Methods 58 cases of severe ankle injuries of pronation external rotation type who had been hospitalized between October, 2001 and October, 2003 were retrospectively analyzed. They were 42 males and 16 females, with an average age of 32.4 years old (16 to 61 years old). According to Lauge Hansen classification, 39 cases were grouped as grade Ⅲ, while other 19 cases as grade Ⅳ. Most cases were surgically managed, and routine examinations were done. Results The average follow up period was 16.4 months (from 10 to 36 months). The satisfactory outcome was 89.7%. 2 cases of osteoarthritis were observed. Conclusions In treatment of pronation external rotation ankle injuries, the key points are to correct the fibular shortening and external rotation and to take great care of technical details.
8.Clinical and radiological diagnoses of aseptic loosening of prosthetic components after total hip arthroplasty

The aseptic loosening of prosthetic components after the total hip arthroplasty (THA) is a possible clinic problem which needs resolving by orthopedic surgeons. Timely clinical and radiological examinations and ana lyses of the signs and progression of the loosening after THA will play a critical role in prevention and treatment to the aseptic loosening of prosthetic components. The article puts forward the principle for correct evaluation of the aseptic loosening after it reviews and analyzes the existing clinical and radiological examination methods of detecting the loosening. 9.Intervention model of smoking cessation and its implementation

Over the years,the sociological and medical researchers have worked together on smoking cessation intervention model and conducted extensive programs to improve the confidence of smokers,to overcome the withdrawal symptoms and other difficulties in quitting,and to improve the success rate of smoking cessation.This paper reviews the major intervention models of smoking cessation and their implementation in recent years,including psychological intervention,behavior intervention,drug intervention and combined intervention;and also some intervention methods including acupuncture therapy,nicotine vaccine and others.The systematic implementation of smoking cessation model facilitates the success of smoking cessation.

Ventricular arrhythmias closely correlates with stress.Not only individuals with organic heart diseases have a high risk of ventricular arrhythmias,healthy individuals are also susceptible to ventricular arrhythmias during stress.Electrophysiological characteristics of myocardium changes apparently during stress and electrical unstability of myocardium increases,which makes reentry and early afterdepolarization easy to happen.
