2.Establishment and evaluation of severe head injury model for treatment with mild hypothermia

Objective To set up an animal model with severe head injury that is similar to clinical condition and treated with mild hypothermia. Methods A total of 120 SD rats were randomly divided into eight groups (n=15 in each group), ie, before cooling, cooling 6, 12, 24, 48, 72, 96 and 120 hours groups. The model was induced by dropping a object of 60 g from a height of 50 cm onto the epidura and treated with mild hypothermia (32-34℃). Blood pressure and ECG were observed and serum K+, Na+ and Mg 2+levels measured. The change of neuron microstructure in rat cerebral traumatic tissue of every group was observed by transmission electron microscope (TEM). Results Animal mortality increased but serum K+ and Mg 2+levels gradually decreased with the treating time lasting. Serum Na+ showed insignificant change. Bradytachycardia occurred in one rat of 72 hours group, two of 96 hours group and two of 120 hours group. TEM showed that the rat’s encephaledema presented gradual relief trend during mild hypothermia treatment. There was one rat in 96 hours group and one in 120 hours group with diffuse tubuli renales necrosis. Conclusion The established model is in conformity with the characteristics of vital sign, serum electrolyte and cerebral microstructure of clinical severe head injury treated with mild hypothermia.
3.Explortation of the clinical application of total thyroidectomy

Objective:To discuss the method of surgery treatment for nodular goiter,Graves disease,Hashimoto thyroiditis and differentiated thyroid carcinoma. Methods:A retrospective analysis of the clinical records of 76 patients undergoing total thyroidectomy was analyzed for the method of surgery treatment for thyroid diseases from resection extent,operative techniques,intraoperative and postoperative complications. Results:Five patients(78.4%)of all the patients accepted total thyroidectomy in first operation had transient or short-term completely recovered hypocalcemia postoperatively,and there were no cases with injury of recurrent laryngeal nerve. 4 patients(33.3%) had one-sided recurrent laryngeal nerve injury and temporary hypoparathyroidism in recurrent patient,and one of the cases had permanent hypoparathyroidism. Conclusion:Total thyroidectomy could be served as the routine operative method for multinodular goiter,Graves disease,Hashimoto thyroiditis and differentiated thyroid carcinoma.
4.Radiotherapy for residual and recurrent foci after surgery for thyroid gland cancer

Purpose: To evaluate the effect of radiotherapy and to detect the prognostic factors for residual and recurrent foci after surgery of thyroid gland cancers. Materials and Methods: 109 patients with residual or recurrent foci of thyroid cancers after surgery were treated with radiotherapy. Results:The overall 5-,10-, 15-, 20- and 25- year survival rates were 93.6%, 91.7% , 88.7%, 87.6% and 87.6% and the 5-, 10-, 15-, 20- and 25- year survival rates with no evidence of disease were 90.8%, 89.8% ,85.6%, 84.1% and 84.1%, respectively. The clinical stage was the main factor and patient's age at diagnosis, patient's sex, histopathological types and radiation dose were also the prognostic factors. Conclusion:Radiotherapy is beneficial to residual and recurrent foci after surgery of thyroid cancer. The optimum radiation dose is 45~65 Gy.
5.Evaluation on the exploration of choledochus through cystic duct

Objective To study the indication,method and value of choledochal exploration through cystic duct.Methods The cystic duct was cut 0.5~1 cm from choledochus and its edge was raised with a forceps and a bakes dilator was inserted to probe the bill duct.If the dilator can not pass the choledochus,palpation around the point of dilator would help to distinguish the stone tumor or narrow.Results In 51 patients with cholecystitis and cholelithiasis,36 were explorated,of which 6 had the explorative indications,4 were postive(66.6%);in 30 without the indications,3 were positive(10%).The total positvity was 19.4%.Conclusions This method is less traumatic,convenient and able to find choledochal disease accurately.The exploration would be done if the cystic duct is over 0.3 cm in diameter.
6.Investigation of the sarcosaphagous fly in Guizhou Province

Objective To investigate the fly in relation to cadaver in the area of Guizhou. Method According to entomological investigation method, 12 areas were selected for investigation, where specimens were collected from 10th to 20th in February, May, August and November in a year. Results 37433 sarcosaphagous fly specimens were collected in 12 areas in a year, which were classified into 27 species of 17 genera in 5 families.Conclusion Data on the sarcosaphagous fly species, area- and season-related distribution were obtained, which is reported first time in China. and may form a basis for further study on the development of sarcosaphagous fly and provide reference to estimation of possible season and circumstance when death occurred.
7.Transjugular intrahepatic portosystemic shunt by direct transcaval approach: Indications and anatomic foundation

Objective To investigate into the indications and related anatomic foundation of transjugular intrahepatic portosystemic shunt(TIPS) creation by direct transcaval approach in patients with portal hypertension cirrhosis suffering unusual anatomy between the hepatic veins and portal bifurcation; and to evaluate the security, feasibility and clinical significance. Methods Direct transcaval approach TIPS were performed in 65 patients including active variceal bleeding (n=52), intractable ascites (n=12), and as a bridge to liver transplantation (n=1). Results Technical and functional success were achieved in all patients. The success rate was 100% without related complications including the technique and primary patency rate is obvious higher than classical TIPS. Conclusion In patients with unusual anatomy between the hepatic veins and portal bifurcation, and inaccessible or inadequate hepatic veins, transcaval TIPS creation is secure and feasible. The results suggest that the direct transcaval approach offering favorable primary patency because the shunt has a straight line in construction.

Objective To observe the recent effect and influence of Lung Cancer Fang No.1 on immunologic function and serum vessel endothelia growth factor (VEGF), keratoprotein 21-1 (CYFRA21-1) in non-small cell lung cancer patients (NSCLC). Methods One hundred non-small cell lung cancer patients were randomized into 52 cases of treatment group on Lung Cancer Fang No.1 and 48 cases of chemotherapy group. 10.The relationship between ECochG and glycerol test in vertigo patients (Report of 112 cases)

Objective:For further investigating the diagnostic value of the ECochG in Meniere's disease.Method:112 vertigo patients including Meniere's disease, sudden deafness, cervical vertigo and other vertigo dis-ease, had been done ECochG,glycerol test,audiometry and caloric test. The results were compared with those ofnormal group. Result:In Meniere's disease,the positive rate of abnormal-SP/AP in ECochG was 54.69% andthe positive rate of glycerol test was 53.12%. There are no statistics difference ( P >0.05) between this twotests with X analysis. Conclusion :We think that the glycerol test and other relative examination are necessary forthe diagnosis of Meniere's disease while the-SP/AP >0.4 ms in the examination of ECochG.
