1.Radical resection of rectal carcinoma by laparoscopic versus conventional laparotomy

Objective To study the feasibility and curative effect of laparoscopic radical resection of rectal carcinoma. Methods Twenty-eight cases undergoing laparoscopic radical resection (laparoscopic group) and 26 cases undergoing open radical resection (open group) were enrolled from January 2004 to December 2007. The following parameters: operation-related situations, postoperative recovery,result of radical resection, and postoperative outcome were compared between the two groups. Results The blood loss during operation in laparoscopic group was less than that in open group [(148.0±26.5) ml vs (396.0±79.6) ml, P<0.01]. The gastrointestinal tract and urination function in laparoseopic group recovered faster than those in open group[the time of diet in taking was (2.8±0.1) d vs (3.9±0.3) d,the time of detaining urethral catheter was(4.2±0.2) d vs (6.0±0.8) d] (P<0.05). The hospital stay was shorter in laparoscopic group than that in open group [(9.8±1.1) d vs(13.2±2.8) d, P<0.01]. The operation time, the number of cleared lymph nodes and complications of laparoscopic between the two groups were no significant difference (P>0.05). There was no significant difference in local recurrent and beyond metastases rates between the two groups. Conclusion Laparoscopic radical resection of rectal carcinoma parallels with open surgery is in safety and effectiveness, it has less traumatic and blood loss and faster in patients recovery.
2.Titer Determination of Main Component in Kitasamycin Tablets and Acetylkitasamycin Dry Suspension by Turbidimetry

OBJECTIVE:To determine the titer of main component in kitasamycin tablets and acetylkitasamycin dry suspension.METHODS:Staphylococcus aureus was used as test organism and which were cultured at (37?0.5) ℃ for about 4~5 hours by adding 1.0%~1.5% kitasamycin and 2.0%~2.5% acetylkitasamycin,then the absorbance was determined at a wavelength of 530 nm.RESULTS:The linear range of kitasamycin was 0.5~4.0 IU?mL-1 and that of acetylkitasamycin was 0.45~6.3 IU?mL-1,and their recovery rates were 101.7%(RSD=1.9%,n=9) and 101.1%(RSD=2.1%,n=9),respectively.CONCLUSION:The established turbidimetry is sensitive,rapid yet with few influencing factors,thus it is applicable for the titer determination of main component in kitasamycin tablets and acetylkitasamycin dry suspension.
3.Clinical characteristics of Baló's concentric sclerosis: analysis of 12 cases

Objective To explore the clinical features, imaging and their diagnosis vaule of Baló's concentric sclerosis (BCS). Methods The clinical manifestation, laboratory examination, neuroimaging characteristics, pathology and follow-up of 12 (2 outpatients and 10 inpatients) cases of BCS were analyzed retrospectively. Results The onset age of 12 patients (male 4, female 8) ranged from 8 to 54 years old (mean 36.8 years). Unconcerned and slowness response were common in the disease onset (7 cases).Other symptoms and signs such as hypophrenia ( 10 cases), limitation of limb or hand movement (8 cases),urinary and fecal incontinence (7 cases), hypo-myodynamia (7 cases), positive pyramidal sign (6 cases)were not rare in course of disease. Eight cases underwent the examination of cerebrospinal fluid myelin basic protein, which increased in 7 cases. Demyelinating change and perivascular inflammatory infiltration were shown in 6 brain biopsies. The involvements of multi-lesions were found in 11 cases. Four patients underwent CT scanning and all the cases showed with hypodense lesions and couldn' t be enhanced. On MRI, all cases appeared round or round-like in shape. Enhanced lesions showed ring-like or half-ting-like shape (8 cases) and concentric ring (3 cases). It's easy to be misdiagnosed as brain tumor (8 cases).Conclusions BCS is a kind of demyelinating disease of subacute or chronic onset accompanied with cognitive disorder being the most common initiating symptoms. It's prone to be misdiagnosed. Diagnosis depends on MRI and corticosteroid treatment is effective. BCS has a good prognosis and presents with benign clinical course.

Objective To study the therapeutic mechanism of Polygonum cuspidatum extract on the gouty arthritis induced by monosodium urate (MSU). Method Acute gouty arthritis rats model was induced by MSU. Twelve hours after modeling,the positive expression area and integral optical density of intercellular adhesion molecule-1 (ICAM-1) and nuclear factor-?B p65 protein (NF-?Bp65) in synovial membrane were observed by immunohistochemical assay. The effect of Polygonum cuspidatum extract on ICAM-1 and NF-?Bp65 was observed. Results There existed less expression of ICAM-1 and NF-?B p65 protein on synovial membrane in the normal group,and their expression was much increased in the model group. Polygonum cuspidatum extract in the dose of 360 mg/kg significantly reduced the positive expression area and integral optical density of ICAM-1 and NF-?Bp65 protein. Conclusion Polygonum cuspidatum extract can inhibit the abnormal expression and activation of ICAM-1 and NF-?Bp65 in synovial tissue,which may be an important therapeutic and preventive mechanism for gouty arthritis.
6.Effect of FTY720 on pancreas islet xenograft rejection

Objective To evaluate the effect of FTY720 on pancreas islet xenograft rejection by setting up the rat-to-mouse islet xenotransplantation model. Methods Rat islets were harvesed by means of pancreatic duct irregution with collagenase and purified by discontinuous density gradient method. Then the islets were transplanted under the kidney capsule of the mouse. The recipients were divided into 3 groups randomly: control group,the mouth was administrated with saline without any immunosuppressant; experiment group 1,the mouth was administrated with FTY720 (1.0 mg/kg) orally from the operation day to day 14 after operation; experiment group 2,the mouth was administrated with combination of FTY720(1.0 mg/kg) with CsA(15 mg/kg) from the operation day to day14 after operation. The xenograft were removed with the kidney at day 3, 5, 7 and 14 after transplantation, and the rejection was analyzed. Results In the control group and experiment group 1, the xenografts were completely destroyed within one week; on day 7, no intact islets could be seen, but numerous lymphocytes infiltration were found. In experiment group 2, many intact islets were still seen under the kidney capsule in day7 and 14 after operation; and infiltrated lymphocytes could hardly or just occasionally be found. Conclusions FTY720 alone can not inhibit the rejection of islet xenotransplantation; FTY720 combination with CsA can inhibited islet xenograft rejection effectively in the rat-to-mouse model.
7.The value of DWI for the diagnosis of ovarian lesions

Objective To investigate the diagnostic value of diffusion-weighted imaging (DWI) in ovarian cystic or solid lesions with different b values.Methods 95 patients,total 105 lesions,including 61 benign lesions (53 cystic,19 solid) and 33 malignant lesions (10cystic,23 solid) with ovarian cystic or solid lesions underwent traditional MRI and DWI.The scan sequence of DWI was SE-EPI,with the b values of 0,500,1 000 s/mm2.The contrast to noise ratio (CNR) was calculated to evaluate the image quality.The apparent diffusion coefficient (ADC) value and exponent apparent diffusion coefficient (eADC) value of ovarian masses were measured.Results With the increase of b values,the signal and CNR of ovarian masses was decreased,and the intensity of cystic component was sharper than that of solid component.There was no signifi cant difference of the ADC values and eADC values between the benign and malignant ovarian cystic masses (P>0.05).But the results had significant difference between the benign and malignant ovarian solid masses (P<0.05).Conclusion Quantitative analysis of ADC and analysis the characteristic of CNR will help the differential diagnosis of some benign and malignant ovarian diseases.

Breast cancer is one of the most common malignancies happened in female patients.The incidence of breast cancer in Chinese females has increased in recently years.Human epidermal growth factor receptor-2 (HER-2) exhibits gene amplification or high expression of receptor protein in about one third of breast cancer patients.HER-2 positive breast cancer patients has poor prognosis,high risk of recurrence and short survival.Trastuzumab is a specific inhibitor of human epidermal growth factor receptor-2 [1],which has been widely used in the treatment of HER-2-positive breast cancer patients,and trastuzumab molecular targeting therapy compared with the traditional chemotherapy,with the advantages of high specificity and low toxicity.It changes the natural disease progression of patients with HER-2-positive breast cancer and prolongs the patient's survival time.This article will review the researches on the four aspects,which including the development and the application of trastuzumab in the treatment of HER-2 positive breast cancer.
9.Adhesive behavior of dual-targeted microbubbles carrying both Sialyl Lewisx and anti-ICAM-1 monoclonal antibody in vitro

Objective To assess the adhesive behavior of dual-targeted microbubbles carrying both Sialyl Lewisx and anti-ICAM-1 monoclonal antibodies in vitro. Methods Selectin-targeted (with Sialyl Lewisx) microbubbles (MB-S),ICAM-1-targeted (with anti-ICAM-1 monoclonal antibodies) microbubbles (MB-Ⅰ),and dual-targeted (with both ligands) microbubbles(MB-D) were prepared by attaching the ligands to the biotinylated lipid-microbubbles via multi-step avidin biotin bridging chemistry. A parallel plate flow chamber combined with a novel automated tracking algorithm,were used to analyze the transient velocities,rolling and firmly adherent numbers of microbubbles at various shear stress (0. 6,2.0 and 4.0 dyn/cm2)over 6 min. Microbubbles detachments were tested by ramping up the shear stress at 30 s intervals. Results At 0.6 dyn/cm2 shear stress, the rolling numbers of MB-S and MB-D were remarkably more than that of MB-I( P<0.05), while at 2.0 and 4.0 dyn/cm2 MB-S performed higher rolling efficiency as compared with either MB-I and MB-D ( P< 0.05). In all flow conditions, the adhesive numbers of MB-D to the targets were obviously greater than those of MB-S and MB-I ( P< 0.05). Half-maximal detachment decreased gradually in MB-I, MB-D and MB-S by turns ( P< 0.05). Conclusions MB-I, MB-S and MB-D have different adhesive behaviors. MB-I exhibites primarily firm adhesion with low rolling efficiency, while MB-S reveales unstable or transient adhesion with high rolling efficiency,and MB-D exhibites firm adhesion with high rolling efficiency. MB-D may be suitable for molecular imaging in high-flow vessels.
10.A clinical comparative study of multiple sclerosis and neuromyelitis optica

Objective To compare the clinical characteristics of multiple sclerosis ( MS) and neuromyelitis optica (NMO) for better diagnosis and differential diagnosis of them. Methods The characteristics of 40 MS and 38 NMO cases were retrospectively studied on clinic manifestations, electroneurophysiology,some laboratory indices, imaging characteristics and so on. Results The ratios of male to female were 1: 1. 35 and 1:4. 43 respectively in patients with MS and NMO, so patients with NMO were more likely to be female as compared with MS ( P < 0. 05 ). The mean onset age was ( 35. 5 ±13. 9 ) years in MS patients and (30. 6 ± 15. 6) years in NMO patients, but no significant difference was found (P>0. 05). The cases of visual acuity ≤0.1 in patients NMO was 13, which of MS was merely 1. The cases of visual acuity less than 0. 5 after treatment in NMO patients was 19, which in MS was only 1. The cases of cognitive impairment in NMO was 3, which of MS was 10. The cases of cerebrospinal fluid oligoclonal bands in MS was 16, which in NMO patients was 9. The lesions of spinal cord shown in MRI of MS patients were typically oval, peripheral and asymmetric, but those in NMO patients extended longitudinally and converged centrally. The mean number of involved vertebral segments in NMO patients was significantly greater than that in MS patients ( 6. 6 vs 2. 2, P < 0. 01). Furthermore, the number of spinal cord lesions in MS patients was alse remarkably greater than that in NMO patients (2. 0vs1.2, P <0. 01). Conclusions NMO may be a distinct clinical entity, which is likely to be differentiated from MS by its tendency to affect women, younger age at onset, and other features clinical manifestations, electroneurophysiology, laboratory parameters, neuroimaging show..
